필라테스 운동 프로그램이 만성 소뇌경색 환자의 균형능력과 일상생활활동 수행능력에 미치는 영향: 단일 사례연구 (Affects of Pilates Exercise Program on Balance Ability and Activities of Daily Living(ADL) Performance Ability for Patient With Chronic Cerebellum Infarction)

  • 목적 : 본 연구의 목적은 필라테스 운동 프로그램이 만성 소뇌경색 환자의 균형능력과 일상생활활동 수행능력에 미치는 영향에 대해 알아보고자 단일 사례연구를 실시하였다. 연구방법 : 본 연구의 대상자는 대구에 소재한 K병원에서 입원치료를 받고 있는 62세 여자 소뇌경색 환자였다. 중재 전 후 균형능력을 비교하기 위해 버그 균형 척도(Berg Balance Scale; BBS)를 시행하였고, 일상생활활동 수행능력을 비교하기 위해 수정된 바델 지수(Modified Barthel Index; MBI)를 시행하였다. 결과 : 대상자의 균형능력은 중재 전 19점에서 중재 후 29점으로 향상되었음을 알 수 있었고, 일상생활활동 수행능력 또한 중재 전 74점에서 중재 후 87점으로 증가 되었다. 결론 : 본 연구 결과 필라테스 운동 프로그램이 만성 소뇌경색 환자의 균형능력과 일상생활활동 수행능력을 증진시키는데 효과가 있음을 알 수 있었다. 이에 기존의 일반적인 작업치료에 필라테스 운동 프로그램을 적절히 활용한다면 환자의 기능 회복에 더 많은 도움이 되리라 기대된다.

동의보감 현훈, 운동실조의 내용을 중심으로 살펴본 소뇌경색 환자 치료경과 3례 (Case Study of Cerebellar Infarction Base on Oriental Medical Diagnosis of Dongeuibogam)

  • The cerebellum processes input from other areas of the brain, spinal cord and sensory receptors to provide precise timing for coordinated, smooth movements of the skeletal muscular system. A stroke affecting the cerebellum may cause vertigo, nausea, balance and coordination problems. The papers on central vertigo caused by cerebellar infarction were reported by Kim, Ku, Ko, Choi, and Han. Their cases were classed as Heo hoon, Dam hoon, and Sangchopungyeul yeul dam, and they treated with Jaeumkunbi-tang, Taeksa-tang, Cheonghunwhadam-tang gami, Cheonginwhadam-tang, Hachulbosim-tang. In this paper, three patients who suffered from vertigo were diagnosed as cerebellar infarction. They felt too giddy to walk without any help. After Oriental medical treatment based on Dongeuibogam, their sequelae of cerebellar infarction - vertigo, nausea, ataxia - had improved dramatically. So we report these cases, and suggest Oriental medical treatment based on Dongeuibogam have effect on cerebellar infarction. But more clinical case reports should be further examined.

Comparative study on Hsp25 expression in Mongolian gerbil and mouse cerebellum

  • The term 'heat shock protein (Hsps)' was derived from the fact that these proteins were initially discovered to be induced by hyperthermic conditions. In response to a range of stressful stimuli, including hyperthermia, immobilization, UV radiation, amino acid analogues, arsenite, various chemicals, and drugs the mammalian brain demonstrates a rapid and intense induction of the heat shock protein. Moreover, Hsps were expressed on the various pathological conditions including trauma, focal or global ischemia, hypoxia, infarction, infections, starvation, and anoxia. Especially, Hsp25 has a protective activity, facilitated by the ability of the protein to decrease the intracellular levels of reactive oxygen species (ROS) as well as its chaperone activity, which favors the degradation of oxidized proteins. Recently, it has clearly demonstrated that Hsp25 is constitutively expressed in the adult mouse cerebellum by parasagittal bands of purkinje cells in three distinct regions, the central zone (lobule VI-VII) and nodular zone (lobule IX-X), and paraflocculus. The Mongolian gerbil has been introduced into stroke study model because of its unique brain vasculature. There are no significant connections between the basilarvertebral system and the carotid system. This anatomy feature renders the mongolian gerbil susceptible to forebrain ischemia-induced seizure. The present study is designed to examine the pattern of Hsp25 expression in the cerebellum of this animal in comparison with that in mouse.

서산 지역에서의 중풍에 대한 임상적 고찰 (A Clinical Study on Stroke patients(CVA) in Seosan province)

  • Objective : The purpose of this study is about stroke patients in Seosan. Method : The subjects of this study were 45 patients who were admitted to Hanseo University Oriental Hospital because of stroke. Each patients was diagnosed with Brain CT, sasang constitutional analysis. Results : 1. The rates in CVA was 58% on cerebral infarction, and 42% on cerebral hemorrhage. 2. The sites of cerebral infarction were Basal ganglia, MCA, Internal. External capsule. Thalamus, ect. Sites of cerebral hemorrhage were Thalamus, Basal ganglia, Cerebellum. 3. The ratio of left and right hemiplegia in cerebral infarction was 1:1.6, and in cerebral hemorrhage it was 1:2. 4. The most chief complaints in cerebral infarction, were hemiplegia, dysarthria, facial palsy, headache. In cerebral hemorrhage, their were hemiplegia, dysarthria, headache, dizziness, and facial palsy. 5. Classification of human corporal constitution in cerebral hemorrhage, the most was Taeumin, Soyangin, Soeumin. And in cerebral infarction, the most was Soyangin, Taeumin, Soyumin. 6. The ratio between male and female was 1.25:2 in cerebral hemorrhage, 2.5:5 in cerebral infarction. 7. The most prevalent age groups in cerebral hemorrhage was fifties to sixties. and in cerebral infarction was fifties to sixties. 8. The most common preceding disease in cerebral hemorrhage was HTN, DM. 9. The recurrence rate of cerebral hemorrhage was 16%, and cerebral infarction was 8%. 10. The ratio of recovery in cerebral hemorrhage was 84%, in cerebral infarction 58%. Conclusion : From this study, in cerebral hemorrhage most patients were Taeumin, in their fifties to sixties. And in cerebral infarction most patients were Soyangin, in their sixties to eighties. In both stroke patients, there were more female than male patients.

Delayed Brain Infarction due to Bilateral Vertebral Artery Occlusion Which Occurred 5 Days after Cervical Trauma

  • Vertebral artery (VA) injuries usually accompany cervical trauma. Although these injuries are commonly asymptomatic, some result in vertebrobasilar infarction. The symptoms of VA occlusion have been reported to usually manifest within 24 hours after trauma. The symptoms of bilateral VA occlusions seem to be more severe and seem to occur with shorter latencies than those of unilateral occlusions. A 48-year-old man had a C3-4 fracture-dislocation with spinal cord compression that resulted from a traffic accident. After surgery, his initial quadriparesis gradually improved. However, he complained of sudden headache and dizziness on the 5th postoperative day. His motor weakness was abruptly aggravated. Radiologic evaluation revealed an infarction in the occipital lobe and cerebellum. Cerebral angiography revealed complete bilateral VA occlusion. We administered anticoagulation therapy. After 6 months, his weakness had only partially improved. This case demonstrates that delayed infarction due to bilateral VA occlusion can occur at latencies as long as 5 days. Thus, we recommend that patients with cervical traumas that may be accompanied by bilateral VA occlusion should be closely observed for longer than 5 days.

척추기저동맥박리에 의한 소뇌경색 환자 한방치험 1례 (A Case Report of a Patient with Cerebellar Infarction with Vertebrobasilar Artery Dissection)

  • Objective: This study is a report on a case of cerebellar infarction with vertebrobasilar artery dissection which was improved by Korean medicine. Methods: A 63-year-old man diagnosed with cerebellar infarction with vertebrobasilar artery dissection was admitted to hospital for 86 days and treated with Korean medicine (acupuncture and herbal medicine) and rehabilitation treatment. Clinical symptoms were assessed with a Modified Barthel index, functional independent measurement, Berg balance scale, manual muscle test, and a visual analogue scale. Result: After treatment, the clinical symptoms were improved, and the evaluation index scores (modified Barthel index, functional independent measurement, Berg balance scale) increased. Conclusion: Korean medicine may be a meaningful treatment for patients with cerebellar infarction with vertebrobasilar artery dissection.

시상, 중뇌, 소뇌경색으로 인한 안검하수, 안구운동장애를 호소하는 환자의 한방 치험 1례 (A Case Report of a Thalamus, Midbrain, and Cerebellum Infarction Patient Suffering from Blepharoptosis and Ocular Motility Disorders Treated by Korean Medicine)

  • Objectives: This is a case report about the effect of Korean medicine on blepharoptosis and ocular motility disorders that occur with thalamus, midbrain, and cerebellum infarctions. Methods: The patient was treated using Korean medicine such as acupuncture, moxibustion, and herbal medicine (Boyanghwano-tang-gamibang). The change of blepharoptosis was evaluated by measurement of palpebral fissure width (PFW), marginal reflex distance-1 (MRD1), and marginal reflex distance-2 (MRD2). The change inocular motility disorders was evaluated by comparison using photographs of the extraocular movements of the patient. We used the Numeric Rating Scale-11 (NRS-11) of diplopia and dizziness to evaluate the effectiveness of treatment. Results: After treatment, blepharoptosis and ocular motility disorders were improved. The NRS-11 score of dizziness decreased from 10 to 7, and diplopia disappeared after Korean medicine treatment. Conclusions: According to this study, Korean medicine can be effective for treating blepharoptosis and ocular motility disorders in thalamus, midbrain, and cerebellum infarctions.

양측성 선조-담창-치상액 석회증의 전산화단층촬영과 자기공명영상 소견 : 증례보고 (Bilateral Striopallidodentate Salcinosis on CT and MRI : Case Report)

  • Bilateral striopallidodentate calcinosis, popularly referred to as Fahr's disease, is a disorder radiologically characterized by bilateral calcifications of the basal ganglia, thalami, dentate nuclei of the cerebellum, and the white matter of the cerebral hemisphere without serum calcium-phosphorus metabolism and related endocrinologic abnormalities. Intracranial calcifications are easily visible as high-density on CT. On magnetic resonance images, the calcifications exhibit different signal intensities. The differences in signal intensity are thought to be related to the stage of the disease, differences in calcium metabolism, and the volume of the calcium deposit. Based on literature review, I report the case of a 63 year man with bilateral symmetrical calcification in the basal ganglia, dentate nuclei of the cerebellum, and the white matter of the cerebral hemisphere who present a 5 year history of progressive dysarthria associated with left thalamic infarction.

현훈과 돌발성 난청을 주소로 하는 뇌교경색 (Vertigo and Sudden Hearing Loss Caused by Pontine Infarction)

  • Anterior inferior cerebellar artery(AICA) is a branch of the basilar artery which supplies the bloods for ventrolateral cerebellum and the lateral tegmentum of the lower two-thirds of pons. AICA occlusion can cause such symptoms as acute-onset unilateral deafness, vertigo, facial weakness and ataxia. A case of sudden hearing loss, facial palsy and vertigo caused by AICA territory infarction recently presented itself. Korean Oriental and Western medical therapy were applied in combination. Facial palsy and vertigo improved, but hearing loss did not improve. This case is reported with a brief overview of related literatures.

  • Tremor is an most common abnormal movements disease which is to-and-fro shaking. It is caused by problems -with the nerves supplying certain muscles. It can affect the whole body or just certain areas. Parkinson's disease, Brain disease like injury of Frontal lobe, Cerebellum and Brainstem, drug and alcohol caused this symptom. It could be occured by physiological and Essential cause. In this paper, authors report three cases who showed tremor after Cerebral infarction. And we suggest that 'Ssanghwa-Tang' should be helpful for tremor patients.
