Methodology and Case Analysis of the Erroneously High Counts in Automated Hematology Analyzer(XE-2100) using WBC (자동 혈구분석기(XE-2100)를 이용한 WBC 과다측정의 확인방법 및 사례 분석)

  • During an 8 month period from January to August 2007, complete blood count (CBC) samples were taken from various wards and from the outpatient department at Samsung Medical Center. In order to determine whether or not the total white blood cells were over counted, results were obtained from both the 4Diff/channel and the white blood cell channel from the automated blood analysis equipment from S company for comparison. Among patients who were on long term treatments, the number of cases determined to be over counted by comparing the WBC counts during this 8 month period was 25. Clinical chemistry tests were also conducted on the same day on the 25 samples taken. 68% of the patients showed to exceed normal range of aspartate transaminase (AST) and alanine aminotransferase (ALT) indicating abnormal liver function, and the total bilirubin range were also in excess in 60% of the total samples taken. Further clinical information which was obtained from the patients showed 98% of the patients were administered with cefepime which is the 4th generation cephalosporin at the time when the WEC were over counted. It is assumed that a multiple of factors investigated caused the over count of the WEC rather than a single factor.

Antibiotics in third molar surgery, justifiable or not?

  • Objectives : The purpose of this study was to evaluate the significance of antibiotics in reducing postoperative infection rates and other complications following third molar surgery. Patients and methods : Two groups of patients underwent surgical extraction of third molars. The antibiotics group, n=21, received a third generation cephalosporin antibiotic for 5 days, starting from the day of surgery. The non-antibiotics group, n=26, didn't receive any antibiotics and only received analgesics to control postoperative pain. Body temperature and hematologic findings including WBC, neutrophils, lymphocytes and monocytes counts were compared between the two groups at three intervals, preoperatively, 24 hours and 7-10 days postoperatively. Pain and swelling during the follow up period were also recorded in both groups and compared in the second part of the study. Results : In the first part of the study, comparison of body temperature, CBC components (except WBCs) showed no significant difference between the two groups during the follow up period. All parameters were within the normal range at all intervals, which indicated absence of infection. In the second part, 38% of patients in the antibiotic group, compared to 54% of the non-antibiotics group, had one or more complications during the follow up period. However, three patients from the antibiotic group compared to one from the nonantibiotics group reported having a swelling of some degree. Conclusion : Based on our objective parameters (body temperature and CBC components), both groups showed no signs of infection during the follow up period. However, the results related to pain and swelling were less conclusive, probably due to small number of patients included in the study. Accordingly, we are unable to provide definite recommendations on antibiotics use in third molar surgery.

Dissemination of CTX-M Type Extended-Spectrum β-Lactamases Among Klebsiella pneumoniae Clinical isolates in Chungcheong Province (충청지역의 임상검체에서 분리된 폐렴막대균에 CTX-M형 Extended-Spectrum β-lactamases 확산)

  • The emergence and dissemination of extended-spectrum ${\beta}$-lactamse (ESBL) producing Klebsiella pneumoniae isolates make it more difficult to treatment of bacterial infections. In our study, we detected ESBL genes and investigated antimicrobial susceptibility of K. pneumoniae isolates in Chungcheong province. In addition, clonality among the isolates was analyzed by repetitive element sequence (REP)-PCR. Twenty-one of 102 K. pneumoniae isolates produced CTX-M-14 and/or CTX-M-15 and showed high level (over 70%) resistance to third cephalosporins. CTX-M type ESBL producing K. pneumoniae strains isolated in our study showed diverse clonality and some of the isolates have been disseminated in the community. Enhancing infection control will be needed to prevent dissemination of the K. pneumoniae isolates. In addition, for more effective control of resistant bacteria it is considered necessary to monitor the database constructed through convergence of biological investigation and statistical analysis of antimicrobial resistance genes.

A Case of Buccal Cellulitis Caused by Haemophilus influenzae Type b in an Immunocompetent Child (면역 기능이 정상인 소아에서 발생한 b형 Haemophilus influenzae에 의한 협부 봉와직염(Buccal Cellulitis) 1례)

  • Buccal cellulitis which presents with high fever and a swelling of the cheek with violaceous hue in young children is most often caused by H. influenzae. Bacteremia is common in buccal cellulitis caused by H. influenzae, and a culture of cerebrospinal fluid should be obtained because meningitis may be present despite the lack of meningeal irritation signs. Although buccal cellulitis is considered to be one of the important manifestations of H. influenzae infection, only two cases have been reported in Korea yet. We experienced a case of buccal cellulitis with H. influenzae bacteremia in an immunocompetent girl of 18-month-old. She was presented with high fever followed by rapidly progressive swelling and tenderness of both cheeks with violaceous hue in four hours. The blood culture revealed H. influenzae type b. There was no concurrent otitis media, sinusitis, or meningitis and no portal of entry was identified. Fever subsided two days after starting intravenous cefotaxime. Intravenous cefotaxime was subsequently changed to oral cefixime, and antibiotics were administered for a total of two weeks. We report this case with a review of related literature.

The in vitro Drug Resistance of Escherichia coli Isolated from Piglets, Calve, Lamb and Goats with Diarrhoea (대장균 설사중에 이환된 소, 돼지, 양에서 분리한 대장균의 약제감수성)

  • 대장균 설사중에 걸린 어린 돼지, 송아지, 어린 양에서 분리한 대장균 126주의 항생제와 화학요법제(15종)에 대한 감수성을 disc diffusion technique로 조사한 성적은 다음과 같다. 1. 어린 돼지에서 분리한 62주의 대장균은 gentamicin(GM)에 100%, colistin(CL)에 96.8%, kanamycin(KM)에 93.5%, neomycin(NM)에는 91.9%가 감수성을 가지고 있었으나 ampicillin(AM), erythromycin(EM), lincomycin (LM), novobiocin (NB), penicillin (PC), streptomycin(SM), tetracycline(TC), sulfaisodimidin(SU)에는 내성을 가지고 있었다. chloramphenicol(CP), sulfamethoxazole-trimethoprim(SXT), cephalosporin(CE)에는 각각 75.8%, 64.5%, 50%가 감수성이 있었다. 2. 송아지에서 분리한 32주의 대장균은 GM에 100%, CL에는 87.5%, SXT에는 66.7%가 감수성이 있었으나 CP와 KM에는 각각 40.6%, SU에는 56.2% NM에는 62.5%, SM에는 87.5%가 내성을 가지고 있었다. EM, LM, NB, PC에는 전혀 감수성이 없었으며 AM, SM, TC에도 고도의 내성을 가지고 있었다. 3. 어린 양에서 분리한 32주의 대장균은 GM과CL에 100%, CP에 96.9%, KM과 NM에 90.6%가 감수성이 있었다. SM과 SU에도 71.9%나 감수성이 있었으나 CM, EM, LM, PC, TC에는 대부분 내성을 가지고 있었다. AM에는 21.9%가 감수성이 있었다. 4. 2종류 이상의 약제에 내성을 가진 대장균의 AM, CE, CP, CL, GM, KM, NM. SM, TC, SU등 10종의 약제에 대한 multiple drug resistance pattern(MDRP)을 조사한 바 돼지 유래 약제내성 대장균의 MDRP는 18가지였으며 이중 가장 빈도가 높은 것은 AM, CE, SM, TC, SU 내성형으로 전체의 24.2%나 되었다. 송아지 유래 약제내성 대장균의 MDRP는 17가지였으며, AM, CE, CP, KM, NM, SM, TC, SU 내성형이 28.1%로 가장 빈도가 높았다. 반면에 어린 양에서 분리한 대장균의 MDRP는 9가지였으며 AM, CE, SU의 3약제 내성형이 40.6%로 가장 많았다.

Bioequivalence of Cefaclor Capsules Following Single Dose Administration to Healthy Male Volunteers (세파클러 캅셀제에 대한 생물학적 동등성 평가)

  • Cefaclor is a second generation cephalosporin antibiotic that shows a potent antibacterial activity against both Gram-positive and Gram-negative bacteria, when it is orally administered. Due to its patent expiration, a number of generic drugs have been marketed, but not yet elucidated to ensure therapeutic equivalence. In this study, cefaclor capsules manufactured by Chong Kun Dang were bioequivallently assessed by comparing with $Ceclor^{TM}$ introduced originally by Daewoong Lilly. A total of 16 healthy male volunteers were evaluated in a randomized crossover manner with a 2-week washout period. Concentrations of cefaclor in plasma were measured upto 6 hours following a single oral administration of two capsules (500 mg of cefaclor) by high-performance liquid chromatography with UV detection. Although the plasma concentration at 6 hours was not detected, the computed half-life of cefaclor was approximately 0.5 hours. The area under the concentration-vs-time curve from 0 to 4 hours $(AUC_{0-4h})$ was calculated by the trapezoidal summation method. The differences in mean values of $AUC_{0-4h}$, peak plasma concentration $(C_{max})$, and time to peak concentration $(T_{max})$ between the two products were $4.63\%,\;1.84\%,\;and\3.28\%$, respectively. The least significant differences at $\alpha4= 0.05 for $AUC_{0-4h},\;C_{max},\;and\;T_{max}\;were\;6,53\%,\;4.05\%,\;and\;6.47\%$, respectively. In conclusion, the test drug was bioequivalent with the reference drug.

Analysis of Initial Choice Antibiotics Efficacy in Diabetic Foot Infection (당뇨병성 족부 감염 환자의 초기 선택 항생제 효율성)

  • Purpose: Foot infections are common complications in patients with diabetes. The patients are usually immune-compromised; therefore the pathogens could be resistant to narrow spectrum antibiotics. Those drugs, however, are categorized as specially managed antibiotics, and access are difficult without confirming of the pathogens. Our aim was to analyze the common pathogens in diabetic foot infection and figure out the proper antibiotics. Materials and Methods: We studied 68 patients treated with diabetic foot infection. The pathogens which caused the infection and their sensitivity to initial antibiotics were analyzed. We also investigated the change of the antibiotics after the confirming of the culture result and average time to get the result. Results: Among the 68 patients, 56 (82%) received cephalosporin and beta-lactam antibiotics. Only 12 (18%) who were confirmed the drug resistant pathogens from previous culture, were treated with broad spectrum antibiotics such as vancomycin and tazoperan. Average culture study time was 6 days. Methicillin-resistant staphylococcus aureus (MRSA) was cultured in 19 patients (28%), Methicillin-resistant coagulase negative staphylococcus (MRCNS) in 11 patietns (17%), pseudomonas in 11 patients (17%). Total 44 (65%) including 3 of other antibiotics resistant pathogen needed broad spectrum antibiotics. Thirty two patients (47%) were resistant to initial antibiotics.irt follow up culture, 2 MRSA and 2 MRCNS were found. The antibiotics resistant pathogens were confirmed in 48 (71%) patients at last. Conclusion: Diabetic patients with foot infection need proper antibiotics from initial treatment. The proper broad spectrum antibiotics should assigned to the patients from the first time without the confirming of the culture results.

Clinical Significance of Extended-spectrum β-lactamase-producing Bacteria in First Pediatric Febrile Urinary Tract Infections and Differences between Age Groups

  • Purpose: Extended-spectrum ${\beta}$-lactamase-producing bacteria-induced urinary tract infections are increasing and require more potent antibiotics such as carbapenems. We evaluated the clinical significance of extended-spectrum ${\beta}$-lactamase -urinary tract infection in children younger than 5 years to select proper antibiotics and determine prognostic factors. Differences were compared between age groups. Methods: We retrospectively studied 288 patients with their first febrile urinary tract infection when they were younger than 5 years. Patients were divided into extended-spectrum ${\beta}$-lactamase-positive and extended-spectrum ${\beta}$-lactamasenegative urinary tract infection groups. Clinical characteristics and outcomes were compared between the groups; an infant group was separately analyzed (onset age younger than 3 months). Results: Extended-spectrum ${\beta}$-lactamase urinary tract infection occurred in 11 % patients who had more frequent previous hospitalization (P=0.02) and higher recurrence rate (P=0.045). During the antimicrobial susceptibility test, the extended-spectrum ${\beta}$-lactamase-positive urinary tract infection group showed resistance to third-generation cephalosporins; however, 98% patients responded clinically. In the infant group, extended-spectrum ${\beta}$-lactamase-positive urinary tract infection occurred in 13% patients and was associated with a longer pre-onset hospitalization history (P=0.002), higher C-reactive protein level (P=0.04), and higher recurrence rate (P=0.02) than that in the older group. Conclusion: Extended-spectrum ${\beta}$-lactamase urinary tract infection requires more attention because of its higher recurrence rate. The antimicrobial susceptibility test demonstrated resistance to third-generation cephalosporins, but they can be used as first-line empirical antibiotics because of their high clinical response rate. Aminoglycosides can be second-line antibiotics before starting carbapenems when third-generation cephalosporins do not show bactericidal effects for extended-spectrum ${\beta}$-lactamase urinary tract infection.

Characterization of Drug-Resistant Salmonella enterica Serotype Typhimurium by Antibiograms, Plasmids, Integrons, Resistance Genes, and PFGE

  • Forty-seven Salmonella Typhimurium (33 zoonotic, 14 clinical) strains were tested for antimicrobial resistance using the standard disk diffusion method. The presence of relevant resistance genes and class 1 integrons were investigated by using PCR. Pulsed-field gel electrophoresis (PFGE) and plasmid profiling were carried out to determine the genomic diversity of Salmonella Typhimurium. Approximately 57.4% of the S. Typhimurium strains were multidrug resistant (MDR) and showed high resistance rates to tetracycline (70.2%), sulfonamides (57.4%), streptomycin (53.1%), ampicillin (29.7%), nalidixic acid (27.6%), kanamycin (23.4%), chloramphenicol (21.2%), and trimethoprim (19.1%). Resistance towards cephalosporins was noted for cephalothin (27.6%), cephradine (21.2%), amoxicillin clavulanic acid (17.0%), and cephalexin (17.0%). Resistance genes, $bla_{TEM}$, strA, aadA, sul1, sul2, tetA, tetB, and tetC, were detected among the drug-resistant strains. Thirtythree strains (70.2%) carried class 1 integrons, which were grouped in 9 different profiles. DNA sequencing identified sat, aadA, pse-1, and dfrA genes in variable regions on class 1 integrons. Thirty-five strains (74.4%) were subtyped to 22 different plasmid profiles, each with 1-6 plasmids (2.0 to 95 kb). PFGE subtyped the 47 strains into 39 profiles. In conclusion, high rates of multidrug resistance were found among the Malaysian Salmonella Typhimurium strains. The emergence of multidrug-resistant Salmonella Typhimurium to cephalosporin antibiotics was also observed. The strains were very diverse and no persistent clone was observed. The emergence of MDR Salmonella Typhimurium is a worldwide problem, and this report provides information for the better understanding of the prevalence and epidemiology of MDR S. Typhimurium in Malaysia.

Isolation and Identification of Lactic Acid Bacteria Inhibiting Gastro-intestinal Pathogenic Bacteria of Domestic Animal. (가축 소화기 병원성 세균을 저해하는 유산균의 분리 및 동정)

  • To isolate probiotic lactic acid bacteria having superior inhibitory activities against animal gastro-intestinal pathogenic bacteria such as Salmonella gallinarum, Staphylococcus aureus and Escherichia coli, 130 strains were initially isolated from the small intestines of Korean native chickens and 7 lactic acid bacteria were finally selected. By using API CHL kit and 16S rRNA sequencing method, the selected lactic acid bacteria were found to be belonged to genus Lactobacillus except BD14 identified as Pediococcus pentosaceus. Especially, Lactobacillus pentosus K34 showed the highest resistancy to both of HCl and bile salt, as well as the highest inhibitory activities against S. gallinarum, S. aureus and E. coli. All the selected strains were sensitive to various antibiotics such as neomycin, erythromycin, cephalosporin, amoxicillin/clavulanic acid, ampicillin, oxytetracycline, but resistant to ciprofloxacin. All the selected strains except BL strain were resistant to colistin and streptomycin, and BD14, BD16, K34 strains were resistant to gentamicin.
