편마비 환자에서의 견관절 아탈구 진단에 유용한 촬영법 (Radiological Projection for Diagnosis of Shoulder Subluxation in Patients with Post-Stroke Hemiplegia)

  • 연구목적 : 편마비 환자에 있어 견부 아탈구 진단에 유용한 촬영법을 알아보고, 편마비 상태를 고려한 촬영방법을 알아보고자 하였다. 연구방법 : 뇌졸중으로 인한 편마비 환자 중 마비측 견관절 부위의 견봉 끝과 상완골두 사이가 1 수지폭 이상인 33명의 환자를 대상으로 견관절 전후방향촬영은 누운 자세와 앉은 자세의 영상을 비교하였고, 흉곽을 통한 상완골 측방향 촬영은 앉은 자세에서 건측과 환측의 영상을 비교하였다. 축방향 촬영은 누운 자세에서 건측과 환측을 비교하였고, 상완골두의 중심, 관절와의 중심, 견봉의 최하외측단의 3가지 기준점으로 거리를 측정하여 평균비교 분석(t-test)을 시행하였다. 연구결과 : 대상자를 누운 자세에서 앉히고 견관절 자연지위를 취한 후 견관절 아탈구가 되기까지의 시간은 평균 123초였다. 견관절 전후 방향 촬영에서 상완골두의 중앙점과 견봉의 최하외점과의 거리가 누운 자세에서는 평균 $49.90{\pm}13.6\;mm$이었고, 앉은 자세에서는 $60.72{\pm}16.3\;mm$이로 유의한 차이가 있었다. 흉곽을 통한 측방향 촬영에서는 환측 $35.92{\pm}6.2\;mm$, 건측 $28.76{\pm}5.4\;mm$으로 유의한 차이가 있었다. 누운 자세의 견관절 축방향 촬영에서는 건측과 환측 각각 $23.01{\pm}9.0\;mm$, $22.45{\pm}8.2\;mm$로 유의한 차이가 없었다. 결론 : 견관절 아탈구 진단을 위한 견관절 촬영은 촬영 전 수지폭 검사를 통해 아탈구가 진행됨을 확인한 후 촬영을 하여야 진단적 가치가 있다. 이를 위해 앉은 자세에서 견관절 자연지위로 120초 지난 후 촬영을 하여야 한다. 또한 견관절 전후방향 영상과 흉곽을 통한 측방향 촬영은 아탈구를 평가하는데 유효하였고, 견관절 축방향에서는 아탈구의 차이가 없으므로 진단적 가치가 떨어진다고 볼 수 있다.

저소득층 장애인 의료비에 대한 정부부담금 추계 (A Short-Term Projection of the Government Budget in Medical Expenditures using for the Low-income Handicapped)

  • This study was conducted to estimate the future government budget in medical expenditures using for the low-income handicapped, because medical expenditures to the low-income handicapped is escalating in these days. It became a big problem not only to the central-government but also to the district-government because they have to subsidize a part of co-payment. This study was designed to project the future government budget using structural model. For the short-term projection, the structural model is stronger than the regression model. The data used for this study were the population projection data based on National Census Data(2000) of the National Statistical Office, the data of Ministry of Health & Welfare, and the data of National Health Insurance Corporation from November 2m to June 2001. The results of the study are summarized as follows: The future government budget in medical expenditures using to the low-income handicapped will be 15-18 billion Won in the year 2003, 16-23 billion Won in 2004, 18-30 billion Won in 2005, 19-38 billion Won in 2006 and 21-49 billion Won in 2007. It is predicted that they would be increasing rapidly. Therefore, the government budget in medical expenditures using for the low-income handicapped must be enlarged.

In recent years general variational inequalities and their algorithmic have assumed a central role in the theory of variational methods. This two-step system for nonlinear variational inequalities offers a great promise and more new challenges to the existing theory of general variational inequalities in terms of applications to problems arising from other closely related fields, such as complementarity problems, control and optimizations, and mathematical programming.

Comparison to Cone Models for Halo Coronal Mass Ejections

  • Halo coronal mass ejections (HCMEs) are mainly responsible for the most severe geomagnetic storms. To minimize the projection effect of the HCMEs observed by coronagraphs, several cone models have been suggested. These models allow us to determine the geometrical and kinematic parameters of HCMEs : radial speed, source location, angular width, and the angle between the central axis of the cone and the plane of the sky. In this study, we compare these parameters form two representative cone models (the ice-cream cone model and the asymmetric cone model) using well-observed HCMEs from 2001 to 2002. And we obtain the root mean square error (rms error) between observed projection speeds and calculated projection speeds for both cone models. It is found that the average rms speed error (89 km/s) of the asymmetric cone model is a little smaller than that (107 km/s) of the ice-cream cone models, implying that the radial speeds from both models are reasonably estimated. We also find that the radial speeds obtained from two models are similar to each other with the correlation coefficient of about 0.8.

악관절장애환자에 있어서 하악과두의 골변화양상에 관한 방사선학적 연구 (RADIOGRAPHIC STUDY ON THE BONY CHANGES OF MANDIBULAR CONDYLE HEAD IN TEMPOROMANDIBULAR DISORDER PATIENTS)

  • The author has studied radiographic bony cnages of mandibular condyle head in temporomandibular disorder patients using Oblique lateral transcranial projection, Orthopantomography, and Tomography. The bony change types and the frequencies of occurrence and the incidences of bony changes in three different radiographic techniques were examined. The coincidences of bony change types between the Oblique lateral transcranial projection and the lateral part of Tomogram, the Orthopantomogram and the medial part of Tomogram were also examined. The results were as follows: 1. The mean age of patients was 31.7 years and under 40 years were 24 patients, women werw 27 patients, men were 4patients. 2. The observable case of bony changes in all three radiographic techniques were 19 cases (50%) of 38 cases and the observable cases of bony changes in only Tomography were 5 cases(13.2%) 3. The most frequent radiographic bony change type was osteophyte and next orders were flattening, erosion, concavity. 4. The positional incidences of bony change in Tomogram were 31 cases in lateral part and 27 cases in central part. 5. The coincidence of bony change types between the Oblique lateral transcranial projection and the lateral part of Tomogram was 80%, and the coincidence between the Orthopantomogram and the medial part or Tomogram was 76.0%.

합곡과 대장의 중추신경로와의 연계성에 관한 연구 (Studies of the Central Neural Pathways to the Hapgok(LI4) and Large Intestine)

  • The aim of this study is to identify central neural pathway of neurons following the projection to the large intestine and Hapgok(LI4) which is Won acupoint of the large intestine meridian of hand-yangmyeong. In this experiment, Bartha's strain of pseudorabies virus was used to trace central localization of neurons related with large intestine and acupoint(LI4) which has been known to be able to regulate intestinal function. The animals were divided into 3 groups: group 1, injected into the large intestine; group 2, injected into the acupoint(LI4); group 3, injected into the acupoint(LI4) after severing the radial, ulnar, median nerve. After four days survival of rats, PRV labeled neurons were identified in the spinal cord and brain by immunohistochemical method. First-order PRV labeled neurons following the projection to large intestine, acupoint(LI4) and acupoint(LI4) after cutting nerve were found in the cervical, thoracic, lumbar and sacral spinal cord. Commonly labeled neurons were labeled in the lumbosacral spinal cord and thoracic spinal cord. They were found in lamina V- X, intermediomedial nucleus and dorsal column area. The area of sensory neurons projecting was L5-S2 spinal ganglia and T12-L1 spinal ganglia, respectively. In the brainstem, the neurons were labeled most evidently and consistently in the nucleus tractus solitarius, area postrema, dorsal motor nucleus of vagus nerve, reticular nucleus, raphe nuclei(obscurus, magnus and pallidus), C3 adrenalin cells, parapyramidal area(lateral paragigantocellular nucleus), locus coeruleus, subcoeruleus nucleus, A5 cell group, periaqueductal gray matter. In the diencephalon, PRV labeled neurons were marked mostly in the arcuate nucleus and median eminence. These results suggest that overlapped CNS locations are related with autonomic nuclei which regulate the functions of large intestine-related organs and it was revealed by tracing PRV labeled neurons projecting large intestine and related acupoint(LI4).

Design of Torsion-typed Smooth Picture Actuator for DLP Projection TV

  • Smooth picture module is operated by vibration to tilt the light from the DMD (digital micro mirror device) of DLP projection TV, which makes the screen of the TV smoother and DMD chip cost lower. To satisfy the vibration characteristics of smooth picture module, it is designed by optimizing moment of inertia, spring constant and damping coefficient, using structural and fluid dynamic simulation that showed a good agreement with experimental data. To reduce the material cost and moment of inertia, engineering plastic is used and the reliability is estimated. A VCM (voice coil motor) type actuator for smooth picture has to satisfy performance requirements such as higher driving force, lower power consumption, and lower cost. The initial design and optimization for VCM was performed using FE analysis. It allowed us to optimize the design of magnetic circuit of the proposed actuator to obtain higher force while maintaining a lower cost.

Government and Derivation in Korean Phonology

  • This paper proposes a derivational account of tensing and neutralization of obstruents in Korean within the theory of Government Phonology (GP) (Kaye, Lowenstamm and Vergnaud 1990, henceforth KLV; Park 1996). We begin by outling the relevant tensing and neutralization data in Korean. We point out several problems that need to be addressed in any account of these data. We then set out the central notions of GP, pointing out how adherence to the requirement that government relations remain constant throughout a derivation under the Projection Principle prevents a GP account of tensing and neutralization in Korean, which requires government relations to switch between lexical and phonetic representations. To address this problem, we propose abandoning the Projection Principle, extending lexical representations in GP along the lines of the Markedness Theory approach (Michaels 1989), and adopting the economy principles for derivation of the Minimalist approach (Chomsky 1993; Chomsky & Lasnik 1991). finally, we summarize the analysis of obstruent phenomena in Korean within GP extended in these ways.

GIS/LIS와 수치지도용 국가평면좌표계에 관한 연구 (The National Grid Systems for Digital Mapping and GIS/LIS)

  • 국가평면좌표계는 GIS/LIS의 구축에 있어 가장 기본적인 요소이며 위치정보를 나타내는데 기준이 되고 있다. 우리나라의 국가기본도에서는 1910년대에 동경원점을 기준으로 하는 3개의 평면좌표계를 채택하고 Gauss-Schreiber 투영법을 적용한 바 있으며 그 후 Gauss-Kruger 투영법으로 변경되었다 본 연구에서는 토지조사사업에서 구축된 기존의 지도좌표계의 계산체계와 투영법에 있어서 불명확함으로 인하여 내재되어 있는 구조적인 문제점을 분석하고 10.405"에 대한 처리방안을 제시하고자 하였으며, GIS/LIS에서 공간데이터의 관리와 GPS의 활용에 적합한 새로운 수치지도 좌표계의 도입을 위하여 외국의 사례를 분석하고 검토하였다. 새로운 지도좌표계는 한반도 전역을 포괄할 수 있도록 경도 $127^\circ\;30'$ 을 중앙자오선으로 하고 TM 투영법에 의한 단일좌표계로 설정하는 방안이 제시되었으며, 지구중심좌표계에 의한 GRS80타원체를 채택할 것을 제안하였다.안하였다.

우리나라의 고령화 현상과 베이비붐 세대의 인구추계 (The Ageing Society of Korea and the Population Estimate)

  • 일반적으로 베이비붐 세대는 국가의 사회, 경제적 발전에 큰 영향을 미치는 것으로 알려져 있고, 이는 우리나라도 마찬가지로 경제성장에 있어 베이비붐 세대의 역할을 제외할 수 없다는 것을 의미한다. 우리나라 베이비붐 세대의 은퇴시기가 가까운 미래에 도래할 것이고, 이는 심각한 사회문제인 저출산 문제와 맞물려서 이들에 대한 노후 복지문제 또한 중요한 문제로 부각되고 있다. 이러한 문제들을 대비하기 위한 많은 정책들을 개발하는데 있어 통계청에서 작성하는 장래인구추계와 평균수명 예측결과들은 가장 근본이 되는 자료일 것이다. 본 논문에서는 통계청에서 제공하는 우리나라 사망관련 자료 및 인구 자료를 검토하고, 통계청과 선행 연구결과에서 제시한 예측평균수명을 비교하여 평균수명 증가에 미치는 베이비붐 세대의 영향력을 분석하였다. 그리고 최근 연구에서 전망한 사망률을 바탕으로 미래의 인구를 제시하였고, 이와 더불어 베이비붐 세대의 인구규모 변화를 전망하였다. 연구 결과로서 통계청의 예측결과와 본 연구결과의 비교가 제시되었다.
