구안괘사의 원인(原因)에 대(對)한 문헌적(文獻的) 고찰(考察) (Reference research for the cause of facial nerve paralysis)

  • From the reference research, the results obtained were as follows. 1. Until the "Song" dynasty, the predominant cause of facial nerve paralysis was the attack of Pathogenic Wind to "the Stomach Channel of Foot Yangming, (St.C.); and "the Small Intestine Channel of Hand Taiyang, (S.I.C.). They recognized the facial paralysis as an aspect of palsy. 2. In the period of Jin-Yuan(金元), the predominant cause was described as "Xuexu"(the deficiency of blood) and phlegm. They recognized that the facial palsy was a palsy. However, they also acceded to the possibility that there could be other explanations. 3. In the period of "Ming & Qing", there were numerous kinds of causes. For example, the following were identified as attacking the Meridian: the Pathogenic Cold; Pathogenic Heat; "Xinxu"(the deficiency in the heart); Fire and Heat combined as a pathogenic factor; "Pixu"(the deficiency in the spleen); and, "Xinxu"(the deficiency of blood). 4. In the past, Koreans have explained the facial paralysis according to the Chinese theories mentioned. However, recently there has been an emergence of another Chinese theory; whereby, facial paralysis is classified into causes and symptoms, and then medical treatment is applied accordingly. 5. From the occident medical perspective, the facial paralysis is categorized into two causes. The first is called central facial nerve paralysis and the second is called peripheral facial nerve paralysis. The latter is mainly caused by Bell's palsy, Herpez zoster oticus, and trauma.

The Characteristics of Supramammillary Cells Projecting to the Hippocampus in Stress Response in the Rat

  • The hypothalamus-pituitary-adrenocortex (HPA) axis is the central mediator of the stress response. The supramammillary (SuM) region is relatively unique among the hypothalamic structures in that it sends a large, direct projection to the hippocampal formation. It has been shown that mild stress could activate the SuM cells that project to the hippocampus. However, the role of these cell populations in modulating the stress response is not known. The present study examined the effect of stress on different populations of SuM cells that project to the hippocampus by injecting the fluorescent retrograde tracer, fluorogold (FG), into the hippocampus and utilizing the immunohistochemistry of choline acetyltransferase (ChAT), corticotrophin releasing factor (CRF), serotonin (5-HT), glutamate decarboxylase (GAD), tyrosine hydroxylase (TH) and NADPH-d reactivity. Immobilization (IMO) stress (2 hr) produced an increase in the expression of ChAT- immunoreactivity, and tended to increase in CRF, 5-HT, GAD, TH-immunoreactivity and nitric oxide (NO)-reactivity in the SuM cells. Fifty-three percent of 5-HT, 31% of ChAT and 56% of CRF cells were double stained with retrograde cells from the hippocampus. By contrast, a few retrogradely labeled cells projecting to the hippocampus were immunoreactive for dopamine, ${\gamma}$-aminobutyric acid (GABA) and NO. These results suggest that the SuM region contains distinct cell populations that differentially respond to stress. In addition, the findings suggest that serotonergic, cholinergic and corticotropin releasing cells projecting to the hippocampus within the SuM nucleus may play an important role in modulating stress-related behaviors.

수양명대장경(手陽明大腸經)과 관련(關聯)된 경혈(經穴)과 안면신경(顔面神經)의 표식영역(標識領域)에 관한 연구(硏究) (Central Localization of Facial Nerve and L14 related to the Large Intestine Meridian)

  • The purpose of this morphological study was to investigate the relationship to facial nerve and LI4 related to the large intestine meridian. The common locations of the spinal cord and brain projecting to the LI4 and facial nerve were observed fallowing injection of transsynaptic neurotropic virus, pseudorabis virus(PRV), into the LI4 and facial nerve of the rat. After survival times of 96 hours following injection of PRV, the rats were perfused, and their spinal cord and brain were frozen sectioned(30${\mu}m$). These sections were stained by PRV immunohistochemical staining method, and observed with light microscope The results were as follows: 1. The PRV labeled spinal cord segments projecting to the LI4 and facial nerve were founded in cervical, thoracic, lumbar and sacral segments. Dense labeled areas of each spinal cord segment were founded in lamina IV, V, X, lateral spinal nucleus, intermediolateral nucleus and dorsal nucleus. 2. The PRV labeled medulla oblongata projecting to the LI4 and facial nerve were founded in the A1 noradrenalin cells/C1 adrenalin cells/caudoventrolateral reticular nucleus, rostroventrolateral reticular nucleus, medullary reticular nucleus, nucleus tractus solitarius, raphe obscurus nucleus, raphe pallidus nucleus, raphe magnus nucleus, gigantocellular nucleus, lateral paragigantocellular nucleus, and spinal trigeminal nucleus.

Bupleurum falcatum Prevents Depression and Anxiety-Like Behaviors in Rats Exposed to Repeated Restraint Stress

  • Previous studies have demonstrated that repeated restraint stress in rodents produces increases in depression and anxiety-like behaviors and alters the expression of corticotrophin-releasing factor (CRF) in the hypothalamus. The current study focused on the impact of Bupleurum falcatum (BF) extract administration on repeated restraint stress-induced behavioral responses using the forced swimming test (FST) and elevated plus maze (EPM) test. Immunohistochemical examinations of tyrosine hydroxylase (TH) expression in rat brain were also conducted. Male rats received daily doses of 20, 50, or 100 mg/kg (i.p.) BF extract for 15 days, 30 min prior to restraint stress (4 h/day). Hypothalamic-pituitary-adrenal axis activation in response to repeated restraint stress was confirmed base on serum corticosterone levels and CRF expression in the hypothalamus. Animals that were pre-treated with BF extract displayed significantly reduced immobility in the FST and increased open-arm exploration in the EPM test in comparison with controls. BF also blocked the increase in TH expression in the locus coeruleus of treated rats that experienced restraint stress. Together, these results demonstrate that BF extract administration prior to restraint stress significantly reduces depression and anxiety-like behaviors, possibly through central adrenergic mechanisms, and they suggest a role for BF extract in the treatment of depression and anxiety disorders.

대장(大腸)과 관련(關聯)된 경혈(經穴)들의 신경해부학적(神經解剖學的) 연구(硏究) (Neuroanatomical Studies on the Acupoints Related to the Large Intestine)

  • The purpose of this morphological studies was to investigate the relation between the meridian, acupoints and viscera using neuroanatomical tracers. The common locations of the spinal ganglia, sympathetic chain ganglia, spinal cord and brain projecting to the large intestine meridian were observed following injection of transganglionic tracer, WGA-HRP and transsynaptic neurotropic virus, pseudorabies virus(PRV), Bartha strain(Ba) and PRV-Ba-Gal (Galactosidase)) into the the large intestine(cecum, colon and rectum), ST37 and LI4. After survival times of 96 hours following injection into the thirty rats with WGA-HRP, PRV-Ba and PRV-Ba-Gal. They were perfused, and their spinal ganglia, sympathetic chain ganglia, spinal cord and brain were frozen sectioned($30{\mu}m$). These sections were stained by HRP and X-gal histochemical and PRV immunohistochemical staining method, and observed with a light microscope. The results were as follows : 1. WGA-HRP labeled neurons innervating the large intestine were observed bilaterally within the T13-L4 sympathetic chain ganglia, and T9-11 spinal ganglia. WGA-HRP labeled neurons innervating ST37 were observed within the L3-5 sympathetic chain ganglia, and L2-4 spinal ganglia. WGA-HRP labeled neurons innervating LI4 were observed in the middle cervical ganglion and stellate ganglion, and C5-8 spinal ganglia. 2. In spinal cord, PRV-Ba labeled neurons projecting to the large intestine, ST37 and LI4 were found in thoracic, lumbar and sacral spinal segments. Densely labeled areas of each spinal cord segment were founded in lamina N, V, VII(intermediolateral nucleus), Ⅸ, X and dorsal nucleus. 3. In medulla oblongata, PRV-Ba and PRV-Ba-Gal labeled neurons projecting to the large intestine, ST37 and LI4 were commonly found in the A1 noradrenalin cells/C1 adrenalin cells/caudoventrolateral reticular nucleus, dorsal motor nucleus of vagus nerve, nucleus tractus solitarius, raphe obscurus nucleus, raphe pallidus nucleus, raphe magnus nucleus and gigantocellular nucleus. 4. In pons, PRV-Ba and PRV-Ba-Gal labeled neurons were commonly found in locus coeruleus, Kolliker-Fuse nucieus and A5 cell group. 5. In midbrain, PRV-Ba and PRV-Ba-Gal labeled neurons were commonly found in central gray matter. 6. In diencephalon, PRV-Ba and PRV-Ba-Gal labeled neurons were commonly found in paraventricular hypothalamic nucleus. These results suggest that PRV-Ba and PRV-Ba-Gal labeled common areas projecting to the large intestine may be correlated to that of the large intestine meridian, ST37 and LI4. Especially, These morphological results provide that interrelationship of meridian-acupoints -viscera may be related to the central autonomic pathways.

지역사회 기반 소아 청소년 알레르기 환자의 보완대체요법 이용 실태조사 (The Use of Complementary and Alternative Medicine in Parents of Children and Adolescents with Allergic Diseases: Community-based Survey)

  • Objectives The purpose of this study is to see the prevalence and the patterns of the use of complementary and alternative medicine (CAM) in children and adolescent patients with allergies. Methods We analyzed data on 547 children and adolescents (age from 0-18 years old) chosen from all regions throughout the country with allergic diseases, including atopic dermatitis, allergic rhinitis, asthma and allergic urticaria. We used multiple logistic regression modeling to predict CAM use based on predictor variables. Results The previous 12-months prevalence of CAM usage in overall was 70.7% (atopic dermatitis, 78.1%; allergic rhinitis, 52.9%; asthma, 70.3%; allergic urticaria, 86.3%). Central and southern regions displayed significantly lower rate of using CAM compare to the northern region, and CAM was less likely to be used for the allergic rhinitis patients than the atopic dermatitis patients. The most commonly used CAM type was natural products (62.2%). Top five of the most frequently used CAM modalities were softener water, vitamin, red ginseng, wood bathing and aloe oil. One of the main reasons for trying CAM was from the 'hope for a more effective outcomes in additional to the conventional medicine' (43.9%). The subjective effectiveness of CAM was found to be excellent in 74.0% of the patients, and 70.3% of the parents were willing to recommend CAM therapies to the others. Conclusions CAM is used widely to treat allergic diseases in children and adolescents in Korea. Korean medical doctors should actively discuss the use of CAM with the patients and provide information on the effectiveness and safety of CAM as guide in making choice for usage of CAM.

요통에 관한 침구임상 진료지침 프로토콜 개발을 위한 전자우편 설문조사 (E-mail Survey for Developing Clinical Guideline Protocol on Acupuncture Treatment for Low Back Pain)

  • Objectives: This survey was done in order to find out how Korean medical doctors derive pattern identification for acupuncture prescriptions in treating low back pain in real clinical practice. Methods : The survey questionnaire was developed by the committee of experts who major in acupuncture & moxibustion or statistics for acupuncture clinical trial protocol development. The questionnaires were distributed via e-mail to 75 members of Korean Acupuncture & moxibustion society from March 26th to April 14th in 2009. 57 members completed answers, and the computerized data were analyzed by SPSS 17.0 statistical program. Results : 1.57 Korean medical doctors selected meridian pattern identification based on the course of the meridians(44.6%), visceral pattern identification(32.1%), pattern identification based on cause of disease(14.3%) as the most commonly used pattern identification methods for acupuncture prescription when treating low back pain patients in real clinical practice. 2. In meridian pattern identification based on the course of the meridians, gallbladder meridian of hypochondriac region(13.0%), bladder meridian of lateral low back region(11.2%), governer vessel of central low back region(11.7%) were selected 3. In visceral pattern identification, yang deficiency of kidney(20.2%), deficiency of kidney(19.3%), liver(16.7%), yin deficiency of kidney(14.0%), violence qi of kidney(8.8%), small intestine(7.9%) were selected. Conclusions : In our e-mail survey, Korean medical doctors answered that Meridian Pattern Identification based on the course of the meridians is the most often used diagnosis method. Visceral pattern identification, pattern identification based on cause of disease, meridian pattern identification based on symptom and pattern identification based on qi-blood-yin-yang theory in order of frequency used, were selected for low back pain diagnosis in real clinical practice.

Berberine alleviates symptoms of anxiety by enhancing dopamine expression in rats with post-traumatic stress disorder

  • Post-traumatic stress disorder (PTSD) is a trauma-induced psychiatric disorder characterized by impaired fear extermination, hyperarousal, anxiety, depression, and amnesic symptoms that may involve the release of monoamines in the fear circuit. The present study measured several anxiety-related behavioral responses to examine the effects of berberine (BER) on symptoms of anxiety in rats after single prolonged stress (SPS) exposure, and to determine if BER reversed the dopamine (DA) dysfunction. Rats received BER (10, 20, or 30 mg/kg, intraperitoneally, once daily) for 14 days after SPS exposure. BER administration significantly increased the time spent in the open arms and reduced grooming behavior during the elevated plus maze test, and increased the time spent in the central zone and the number of central zone crossings in the open field test. BER restored neurochemical abnormalities and the SPS-induced decrease in DA tissue levels in the hippocampus and striatum. The increased DA concentration during BER treatment may partly be attributed to mRNA expression of tyrosine hydroxylase and the DA transporter in the hippocampus, while BER exerted no significant effects on vesicular monoamine transporter mRNA expression in the hippocampus of rats with PTSD. These results suggest that BER had anxiolytic-like effects on behavioral and biochemical measures associated with anxiety. These findings support a role for reduced anxiety altered DAergic transmission and reduced anxiety in rats with PTSD. Thus, BER may be a useful agent to treat or alleviate psychiatric disorders like those observed in patients with PTSD.

Pseudorabies virus를 이용한 췌장과 삼음교(三陰交)(Sp6), 음릉천(陰陵泉)(Sp9)에서 투사되는 중추 신경로에 관한 형태학적 연구 (Morphological Studies of the Central Neural Pathways to the Pancreas, Sanyinjiao(Sp6) and Yinlingquan(Sp9) using Pseudorabies Virus)

  • Objectives : The purpose of this morphological studies was to investigate the relations between Sanyinjiao(Sp6), Yinlingquan(Sp9) and pancreas of rats using peudorabies virus(PRV). Methods : We observed labeled neurons following the injection of PRV, Bartha strain, into the Sanyinjiao(Sp6), Yinlingquan(Sp9) and pancreas of rats. After survival times of 4 days following the injection of PRV, the rats were perfused, and their spinal ganglia, spinal cord and brain stem were frozen sectioned($35{\mu}m$). These sections were strained by PRV immunohistchemical staining methods and observed with light microscope. Results : The results were as follows. 1. In the spinal ganglia, the overlap areas of PRV labeled neurons projecting to Sanyinjiao(Sp6), Yinlingquan(Sp9) and pancreas were observed in T10-13 dorsal root ganglia. 2. In the spinal cord, the overlap areas of PRV labeled neurons projecting to Sanyinjiao(Sp6), Yinlingquan(Sp9) and pancreas were lamina I, IV, V, VII, IX, X, intermediolateral nucleus(IML), intermediomedial nucleus(IMM) in thoracic segments. In lumbar segments, the overlap areas of PRV labeled neuron were lamina I, IV, V, VI, IX, X and IMM. In sacral segments, the overlap areas of PRV labeled neuron were lamina I, IV, V, VI, VII, IX, X. 3. In the brain, the overlap areas of PRV labeled neurons projecting to Sanyinjiao(Sp6), Yinlingquan(Sp9) and pancreas were area postrema, nucleus tractus solitarius, caudoventrolateral reticular nu., medullary reticular nu., lateral paragigantocellular nu., C3 adrenalin cells, gigantocellular nu., raphe pallidus nu., raphe obscurus nu., ambiguus nu., raphe magnus nu., pontine reticular formation, A5 cell group, subcoeruleus nu., locus coeruleus, Barringnton's nu., $K{\ddot{o}}lliker$-Fuse nu., dorsal raphe nu., Edinger-Westphal nu., central gray matter, perifornical nu., dorsomedial hypothalamic nu., arcuate nu., lateral hypothalamic nu., paraventricular hypothalamic nu., hindlimb area. Conclusions : In conclusion, these results suggest that the interrelationship of meridian(spleen meridian), acupoints(Sp6 and Sp9) and viscera(pancreas) may be related the central autonomic centers.

The Protective Effect of Black Ginseng Against Transient Focal Ischemia-induced Neuronal Damage in Rats

  • Black ginseng (BG) has been widely used as herbal treatment for improving physiological function. In order to investigate the neuroprotective action of this herbal medicine, we examined the influence of BG on the learning and memory of rats using the Morris water maze, and we studied the effects of BG on the central cholinergic system and neural nitric oxide synthesis in the hippocampus of rats with neuronal and cognitive impairment. After middle cerebral artery occlusion was applied for 2h, the rats were administered BG (100 or 400 $mgkg^{-1}$, p.o.) daily for 2 weeks, followed by training and performance of the Morris water maze test. The rats with ischemic insults showed impaired learning and memory on the tasks. Treatment with BG produced improvement in the escape latency to find the platform. Further, the BG groups showed a reduced loss of cholinergic immunoreactivity and nicotinamide adenine dinucleotide phosphate-diaphorase (NADPH-d)-positive neurons in the hippocampus compared to that of the ISC group. These results demonstrated that BG has a protective effect against ischemia-induced neuronal and cognitive impairment. Our results suggest that BG might be useful for the treatment of vascular dementia.
