A COMPARISON OF THE SHAPING ABILITY OF FOUR ROTARY NICKEL-TITANIUM FILES IN SIMULATED ROOT CANALS (엔진구동형 NiTi 파일의 근관성형효과 비교)

  • The purpose of this study was to compare the root canal shaping ability of 4 rotary NiTi instruments in simulated root canals. For the preparation of thirty two curved root canals, Mtwo instruments using "single length"technique, and Profile, ProTaper Universal, and K3 using crown-down technique (N = 8) were used. All canal samples were prepared by reaching an apical canal size of #30. Pre- and post-instrumentation digital images were recorded and an assessment of canal shape was determined using a computer image analysis program SigmaScan Pro (Systat Software Inc., San Jose, CA, USA). The changes of the dimension of inner walls of canals, (2) the changes of the dimension of outer walls of canals, and (3) the centering ratio were measured at 7 measuring points, and then data were statistically analyzed using one-way ANOVA and Duncan's test. The results were as below; 1. The root canal shaping ability of Profile was significantly faster than that of other rotary NiTi instruments (p < 0.05). 2. The deformation and fracture of all instruments used for this study were not experienced. 3. In the degree of changes of the dimension of inner walls of canals, Profile demonstrated the lowest changes of the dimension of inner walls of canals except at the measuring points of the 1 and 2 mm (p < 0.05). However, the ProTaper Universal showed the highest changes of the dimension of inner walls of canals at all measuring points (p < 0.05). 4. In the degree of changes of the dimension of outer walls of canals, Mtwo demonstrated the lowest changse of the dimension of outer walls of canals except at the measuring point of the 1 mm (p < 0.05). However, Profile exhibited the highest changes of the dimension of outer walls of canals at the measuring points of 3 and 4 mm and ProTaper Universal and K3 showed the largest changes of the dimension of outer walls of canals at the measuring points of 1, 2, 6, and 7 mm (p < 0.05). 5. In degree of centering ratio, Profile demonstrated the least centering ratio comparing with the centering ratio shown by other NiTi instruments at the measuring points of 1, 4, 5, and 6 mm. Results suggest that in the coronal part of canal preparation, active cutting files such as ProTaper Universal may efficiently flare the canal orifice and form a better taper, and in the apical part of the canal, files which have a better centering ability such as Profile may maintain the original canal curvature and reduce the shaping time.

Reduction of Radiation Dose to Eye Lens in Cerebral 3D Rotational Angiography Using Head Off-Centering by Table Height Adjustment: A Prospective Study

  • Objective: Three-dimensional rotational angiography (3D-RA) is increasingly used for the evaluation of intracranial aneurysms (IAs); however, radiation exposure to the lens is a concern. We investigated the effect of head off-centering by adjusting table height on the lens dose during 3D-RA and its feasibility in patient examination. Materials and Methods: The effect of head off-centering during 3D-RA on the lens radiation dose at various table heights was investigated using a RANDO head phantom (Alderson Research Labs). We prospectively enrolled 20 patients (58.0 ± 9.4 years) with IAs who were scheduled to undergo bilateral 3D-RA. In all patients' 3D-RA, the lens dose-reduction protocol involving elevation of the examination table was applied to one internal carotid artery, and the conventional protocol was applied to the other. The lens dose was measured using photoluminescent glass dosimeters (GD-352M, AGC Techno Glass Co., LTD), and radiation dose metrics were compared between the two protocols. Image quality was quantitatively analyzed using source images for image noise, signal-to-noise ratio, and contrast-to-noise ratio. Additionally, three reviewers qualitatively assessed the image quality using a five-point Likert scale. Results: The phantom study showed that the lens dose was reduced by an average of 38% per 1 cm increase in table height. In the patient study, the dose-reduction protocol (elevating the table height by an average of 2.3 cm) led to an 83% reduction in the median dose from 4.65 mGy to 0.79 mGy (P < 0.001). There were no significant differences between dose-reduction and conventional protocols in the kerma area product (7.34 vs. 7.40 Gy·cm2, P = 0.892), air kerma (75.7 vs. 75.1 mGy, P = 0.872), and image quality. Conclusion: The lens radiation dose was significantly affected by table height adjustment during 3D-RA. Intentional head off-centering by elevation of the table is a simple and effective way to reduce the lens dose in clinical practice.

Performance-based seismic design of a spring-friction damper retrofit system installed in a steel frame

  • This study investigates a new seismic retrofit system that utilizes rotational friction dampers and axial springs. The retrofit system involves a steel frame with rotational friction dampers (RFD) at beam-column joints and linear springs at the corners, providing energy dissipation and self-centering capabilities to existing structures. The axial spring acts as a self-centering mechanism that eliminates residual deformations, while the friction damper mitigates seismic damage. To evaluate the seismic performance of the proposed retrofit system, a series of cyclic loading tests were carried out on a steel beam-column subassembly equipped with the proposed devices. An analytical model was then developed to validate the experimental results. A performance point ratio (PPR) was presented to optimize the design parameters of the retrofit system, and a performance-based seismic design strategy was developed based on the PPR. The retrofit system's effectiveness and the presented performance-based design approach were evaluated through case study models, and the analysis results demonstrated that the developed retrofit system and the performance-based design procedure were effective in retrofitting structures for multi-level design objectives.

Comparison of the centering ability of Wave.One and Reciproc nickel-titanium instruments in simulated curved canals

  • Objectives: The aim of this study was to evaluate the shaping ability of newly marketed single-file instruments, Wave One (Dentsply-Maillefer) and Reciproc (VDW GmbH), in terms of maintaining the original root canal configuration and curvature, with or without a glide-path. Materials and Methods: According to the instruments used, the blocks were divided into 4 groups (n = 10): Group 1, no glide-path / Wave One; Group 2, no glide-path / Reciproc; Group 3, #15 K-file / Wave One; Group 4, #15 K-file / Reciproc. Pre- and post-instrumented images were scanned and the canal deviation was assessed. The cyclic fatigue stress was loaded to examine the cross-sectional shape of the fractured surface. The broken fragments were evaluated under the scanning electron microscope (SEM) for topographic features of the cross-section. Statistically analysis of the data was performed using one-way analysis of variance followed by Tukey's test (${\alpha}$ = 0.05). Results: The ability of instruments to remain centered in prepared canals at 1 and 2 mm levels was significantly lower in Group 1 (p < 0.05). The centering ratio at 3, 5, and 7 mm level were not significantly different. Conclusions: The Wave One file should be used following establishment of a glide-path larger than #15.

The Effect of Glide Path on Canal Centering Ability in Reciprocating File System (Reciprocating 파일 시스템에서 Glide Path가 근관만곡도 유지에 미치는 영향)

  • The purpose of this study was to evaluate the influence of glide path on canal centering ratio after instrumentation with different single file systems; WaveOne and Reciproc. Reciproc R25 (VDW), WaveOne Primary (Dentsply Maillefer) and PathFile #13, 16, 19 (Dentsply Maillefer) were used in this study. In no glide path groups, Reciproc files and WaveOne files used for canal preparation without glide path. In glide path groups, the PathFile were used before canal preparation. Methylene blue dye was introduced into the canal to obtain a clear pre-instrumentation image. Pre-instrumentation images and post-instrumentation images were scanned using Epson Perfection V700 Photo scanner (Epson, Nagano, Japan). Transparencies of post-instrumentation images were changed and superimposed on pre-instrumentation images using Adobe Photoshop CS 3 (Adobe Systems Incorporated, San Jose, CA, USA). The centering ratio was calculated for each instrumented canal using the following formula: CR=|X1-X2|/Y. It was statistically analyzed using two-way ANOVA at 95% confidential level. The centering ratio in glide path groups were significant less than it in no glide path groups at 3, 4, 5 and 6 mm level. Except 1 and 6 mm level, WaveOne groups had significant less centering ration than Reciproc groups. At 6 mm level, there was no significant difference between WaveOne and Reciproc. In the limitation of this study, creation of a previous glide path before reciprocating motion instrumentation in curved canal appears to be appropriate and WaveOne system can be used for preparation of curved canal without severe aberrations.

Seismic performance of a rocking bridge pier substructure with frictional hinge dampers

  • The rocking pier system (RPS) allows the columns to rock on beam or foundation surfaces during the attacks of a strong earthquake. Literatures have proved that seismic energy dissipated by the RPS through the column impact is limited. To enhance the energy dissipation capacity of a RPS bridge substructure, frictional hinge dampers (FHDs) were installed and evaluated by shaking table tests. The supplemental FHDs consist of two brass plates sandwiched by three steel plates. The strategy of self-centering design is to isolate the seismic energy by RPS at the columns and then dissipate the energy by FHDs at the bridge deck. Component tests of FHD were first conducted to verify the friction coefficient and dynamic characteristic of the FHDs. In total, 32 shaking table tests were conducted to investigate parameters such as wave forms of the earthquake (El Centro 1940 and Kobe 1995) and normal forces applied on the friction dampers. An analytical model was also proposed to compare with the tested damping of the bridge sub-structure with or without FHDs.

Aesthetics of Samjae and Inequilateral Triangle Found in Ancient Triad of Buddha Carved on Rock - Centering on Formative Characteristics of Triad of Buddha Carved on Rock in Seosan - (고대(古代) 마애삼존불(磨崖三尊佛)에서 찾는 삼재(三才)와 부등변삼각(不等邊三角)의 미학(美學) - 서산마애삼존불의 형식미를 중심으로 -)

  • This study was attempted in order to offer basic data for implementing and applying Samjonseokjo(三尊石造), which is one of traditional stone construction method, by confirming how the constructive principle is expressed such as proportional beauty, which is contained in the modeling of Triad of Buddha Carved on Rock that was formed in the period of the Three States, centering on Triad of Buddha Carved on Rock in Susan. The summarized findings are as follows. 1. As a result of analyzing size and proportion of totally 17 of Triad of Buddha Carved on Rock, the average total height in Bonjonbul(本尊佛) was 2.96m. Right Hyeopsi(右挾侍) was 2.19m. Left Hyeopsi(左挾侍) was 2.16m. The height ratio according to this was 100:75:75, thereby having shown the relationship of left-right symmetrical balance. The area ratio in left-right Hyeopsi was 13.4:13.7, thereby the two area having been evenly matched. 2. The Triad of Buddha Carved on Rock in Seosan is carved on Inam(印岩) rock after crossing over Sambulgyo bridge of the Yonghyeon valley. Left direction was measured with $S47^{\circ}E$ in an angle of direction. This is judged to target an image change and an aesthetic sense in a Buddhist statue according to direction of sunlight while blocking worshipers' dazzling. 3. As for iconic characteristics of Buddha Carved on Rock in Seosan, there is even Hyeopsi in Bangasang(半跏像) and Bongjiboju(捧持寶珠) type Bosangipsang. In the face of Samjon composition in left-right asymmetry, the unification is indicated while the same line and shape are repeated. Thus, the stably visual balance is being shown. 4. In case of Triad of Buddha Carved on Rock in Seosan, total height in Bonjonbul, left Hyeopsi, and right Hyeopsi was 2.80m, 1.66m, and 1.70m, respectively. Height ratio in left-right Hyeopsibul was 0.60:0.62, thereby having been almost equal. On the other hand, the area ratio was 28.8:25.2, thereby having shown bigger difference. The area ratio on a plane was grasped to come closer to Samjae aesthetic proportion. 5. The axial angle of centering on Gwangbae was 84:46:50, thereby having been close to right angle. On the other hand, the axial angle ratio of centering on Yeonhwajwa(蓮華坐: lotus position) was measured to be 135:25:20, thereby having shown the form of inequilateral triangle close to obtuse angle. Accordingly, the upper part and the lower part of Triad of Buddha Carved on Rock in Susan are taking the stably proportional sense in the middle of maintaining the corresponding relationship through angular proportion of inequilateral triangle in right angle and obtuse angle. 6. The distance ratio in the upper half was 0.51:0.36:0.38. On the other hand, the distance ratio in the lower half was 0.53 : 0.33 : 0.27. Thus, the up-down and left-right symmetrical balance is being formed while showing the image closer to inequilateral triangle. 7. As a result of examining relationship of Samjae-mi(三才美) targeting Triad of Buddha Carved on Rock in Susan, the angular ratio was shown to be more notable that forms the area ratio or triangular form rather than length ratio. The inequilateral triangle, which is formed centering on Gwangbae(光背) in the upper part and Yeonhwajwa(lotus position) in the lower part, is becoming very importantly internal motive of doubling the constructive beauty among Samjae, no less than the mutually height and area ratio in Samjonbul.

COMPARISON OF SHAPING ABILITY OF ROTARY Ni-Ti FILE SYSTEMS USED BY UNDERGRADUATES (학생들이 사용한 엔진 구동형 Ni-Ti file systems의 근관 성형 효율 비교)

  • The purpose of this study was to compare the shaping ability of three Ni-Ti file systems used by dental students or the experts and consequently to aid in choosing a proper systems for educational courses of dental students and beginners. Fifty students and ten dentists who have clinical experience over two years prepared 180 simulated root canals in resin blocks with three Ni-Ti systems; $ProFile^{(R)}\;(PF),\;HeroShaper^{(R)}\;(HS),\;K3^{TM}\;(K3)$. After preparation, the Ni-Ti files were evaluated for distortion and canal preparation time was recorded. The images of pre- and post-instrumented canals were scanned and superimposed. Amounts of increased canal widths, deviation, and centering ratio were calculated at apical 1, 3 and 5 mm levels and statistical analysis was performed The results were as follows : 1. HS showed the shortest preparation time and instrumented canal width in K3 was significantly larger than other groups (P<0.05). 2. At 1 and 3mm levels, all groups had outward deviation. In student group, at the 1mm level, PF had the least deviation (P<0.05). 3. In the centering ratio, the PF had the best centering ability compared to the others at 5mm level. At 1 and 3mm levels, HS and PF had better abilities than K3. Student group had better ratio than the expert at 3mm level with PF (P<0.05). Based on the results, it is surmised that the $ProFile^{(R)}$ is the safest and most ideal instrument for students and beginners.

THE EFFECT OF DIFFERENT FLUTE DESIGN AND TORQUE-CONTROLLED MOTOR ON THE SHAPING ABILITY OF SIMULATED RESIN ROOT CANALS (엔진의 토크 조절 능력과 니켈-티타늄 파일의 삭제 성향이 근관 성형에 미치는 영향)

  • The purpose of this study was to compare the shaping ability of the two different Ni-Ti file systems and the two different engine systems in simulated canals. A total of four groups of each 10 were tested. Each group was instrumented with HeroShaper and Endo-Mate2 (Croup HE), HeroShaper and Tecnika (Croup HT), ProFile and Endo-Mate2 (Group PE), and ProFile and Tecnika (Croup PT). Canal preparation time was recorded. The images of pre- and post- instrumented root canals were scanned and superimposed. The amounts of increased width and centering ratio were measured and calculated at apical 1, 3 and 5mm levels. These data were statistically analyzed with one-way ANOVA and Duncan's multiple range test The results of this study were as fellows ; 1. Canal preparation time of HT group was the shortest (p<0.05). 2. The amount of increased canal width in HE group was significantly larger than PT group at apical 1mm level (p<0.05) At apical 3mm level, PT group was significantly smaller than other groups (p<0.05). At apical 5mm level, PE group was significantly larger than PT group (p<0.05). 3. The amount of centering ratio in HE group was significantly larger than other groups (p<0.05). At apical 5mm level, HT group was significantly larger than PE group and PT group (p<0.05). Under the condition of this study, torque-controlled endodontic motor is safer than no torque controlled motor, especially when the active file is used.

A Study on the Projected Workforce of Advanced Oncology Certified Nurses (종양전문간호사인력 수요추계에 관한 연구)

  • Purpose: The purpose of this study was to suggest the projected manpower of oncology nurses & Advanced oncology certified nurses needed in the future. Method: Need models, ratio models and expert opinion were used for projecting the number of oncology nurses & Advanced oncology certified nurses. Resul: 1. The demand for the oncology nurses was estimated 6,043 in the year 2003, 6,548 in the year 2005, and 4,556 in the year 2010. 2. The demand range of the advanced oncology certified nurses centering on the number of hospitalized patients were estimated 358-538 in the year 2003, 388-583 in the year 2005, and 448-672 in the year 2010. 3. The demand range of the middle and long term advanced oncology certified nurses centering on the number of sickbed were estimated 507-787 in the year 2005, 523-810 in the year 2010, and 540-837 in the year 2020. 4. The demand range of the advanced oncology certified nurses centering on the number of organizations were estimated 374 in the year 2003, 399 in the year 2005, 410 in the year 2010. 5. The total demand range, concerning the numbers of hospitalized patients, hospital bed, and organizations are estimated 358-538 in the year 2003, 388-787 in the year 2005, 428-840 in the year 2010, and 540-837 in the year 2020. 6. In the demand of developed country's level oncology nurses are OCN's level 1,495, AOCN's level 102 in the year 2003, OCN's level 1,613, AOCN's level 111 in the year 2005, OCN's level 1,879, AOCN's level 128 in the year 2010 in case of the United States, and in case of Canada, 765 in the year 2003, 826 in the year 2005, and 956 in the year 2010.
