Treat-to-Target Strategy for Asian Patients with Early Rheumatoid Arthritis: Result of a Multicenter Trial in Korea

  • Background: To evaluate the therapeutic benefits of the treat-to-target (T2T) strategy for Asian patients with early rheumatoid arthritis (RA) in Korea. Methods: In a 1-year, multicenter, open-label strategy trial, 346 patients with early RA were recruited from 20 institutions across Korea and stratified into 2 groups, depending on whether they were recruited by rheumatologists who have adopted the T2T strategy (T2T group) or by rheumatologists who provided usual care (non-T2T group). Data regarding demographics, rheumatoid factor titer, anti-cyclic citrullinated peptide antibody titer, disease activity score of 28 joints (DAS28), and Korean Health Assessment Questionnaire (KHAQ) score were obtained at baseline and after 1 year of treatment. In the T2T group, the prescription for disease-modifying antirheumatic drugs was tailored to the predefined treatment target in each patient, namely remission (DAS28 < 2.6) or low disease activity (LDA) ($2.6{\leq}DAS28$ < 3.2). Results: Data were available for 163 T2T patients and 162 non-T2T patients. At the end of the study period, clinical outcomes were better in the T2T group than in the non-T2T group (LDA or remission, 59.5% vs. 35.8%; P < 0.001; remission, 43.6% vs. 19.8%; P < 0.001). Compared with non-T2T, T2T was also associated with higher rate of good European League Against Rheumatism response (63.0% vs. 39.8%; P < 0.001), improved KHAQ scores (-0.38 vs. -0.13; P = 0.008), and higher frequency of follow-up visits (5.0 vs. 2.0 visits/year; P < 0.001). Conclusion: In Asian patients with early RA, T2T improves disease activity and physical function. Setting a pre-defined treatment target in terms of DAS28 is recommended.

방임 및 보호 아동·청소년 원가정 회복지원 시범사업의 현황과 과제 (Recovery Support Service for Neglected Children and Their Families of Origin: Status and Suggestions)

  • 정부의 아동학대 대응체계가 전면 개편되고 있으나 최근에도 아동·청소년 대상의 학대와 방임은 계속 증가하고 있다. 학대 행위자의 75%가 부모라는 점은 재발 위험이 높을 가능성을 나타내며, 따라서 가해 부모에 대한 교육과 치료를 통한 가족 관계 및 양육환경의 개선 및 가족기능회복이 시급하다. 본 연구의 목적은 방임 및 보호 아동·청소년과 부모의 지속적인 관계 개선을 통해 원가정의 건강성 회복을 도모하는 시범사업의 현황 및 실태 분석을 통해 지역사회 기반의 원가정 기능회복지원 사업의 확대 및 개선 방안을 제시하고자 이루어졌다. 시범사업은 2020년 6월부터 11월까지 진행되었으며, 지역에 따라 코로나-19의 확산 및 사업운영 인력의 개인역량, 타 가족사업과의 협력, 지역 자원 활용 정도 등에 따라 서비스 제공 정도에 차이가 있었다. 그럼에도 원가정 회복지원 서비스 제공과정이 보다 체계화 되었고, 학대 재발 방지를 위한 지역사회 네트워크 기반이 형성되었다. 이에 원가정 회복지원 사업이 보다 확대되기 위해서 첫째, 기존 아동 학대 보호 기관과의 서비스 제공을 위한 범정부적 협업 시스템이 마련되어야 하며, 둘째, 원가정 대상자 발굴을 위한 지역별 사례 발굴 경로가 다양하게 구축되어야 한다. 셋째, 시범사업 운영기관의 노하우를 타 지역 센터와 공유함으로써 동반성장 체계를 마련하여 안정적인 사업운영 체제가 조성되어야 한다. 마지막으로 지역 내 기존 보호 아동 기관과의 서비스 중복을 피하기 위해 예방이나 재발 방지 차원에서 '원가정 보호' 및 모니터링에 집중할 필요가 있다. 이를 토대로 지역별 특색을 살리며 서비스 표준화를 모색할 수 있는 운영지침과 성과평가 방안이 마련되어야 할 것이다.

낙남정맥의 서식지 유형별 조류 군집 분석 (Analysis of Bird Community by Habitat Type in Nak-nam Jeongmaek)

  • 본 연구는 낙남정맥의 체계적인 관리 및 보전을 위한 기초자료를 제공하고자 수행하였다. 낙남정맥의 6개의 중점조사지역(길마재, 천왕산, 여항산, 무학산, 천주산, 신어산)에서 지형, 환경특성 등을 고려하여 각 지점별로 조류 서식지 유형별 2개씩의 고정조사구를 선정하여 2018년 5월부터 10월까지 수행하였다. 낙남정맥 6개의 중점조사지역을 조사한 결과 총 48종 1,181개체의 조류가 확인되었다. 서식지 유형별 조류 출현종은 개발지에서 35종 567개체로 가장 많았으며 종다양도는 임도에서 2.66로 가장 높았다. 낙남정맥의 서식지 유형별 영소길드 분석결과(Pearson Chi-square test, 𝛘2=16.681, p<0.05) 개발지, 계곡부, 임도 모두 수관층(Canopy) 영소길드에 속한 군집의 빈도가 높았다. 채이길드 분석결과(Pearson Chi-square test, 𝛘2=16.217, p<0.005) 개발지는 관목층(bush), 계곡부와 임도는 수관층 채이길드에 속한 군집의 빈도가 높았다. 길드 분석결과 낙남정맥의 임도구간은 대경목 임목과 천이된 초본류가 지빠귀류를 비롯하여 다양한 조류에게 충분한 영소 및 먹이자원을 제공하고 있었으며, 이러한 연구결과는 낙남정맥의 다양한 서식지가 조류서식에 다양한 자원을 제공하고 있는 것으로 판단된다.

2015~2020년 동안 인천 지역 단일기관에서의 비결핵항산균 분포 및 항균제 내성률 (Distribution and Antimicrobial Resistance of Non-Tuberculous Mycobacteria during 2015~2020: A Single-Center Study in Incheon, South Korea)

  • 본 연구는 인천지역에 위치한 단일기관의 비결핵항산균의 분포 및 항균제 내성률과 동반 세균 빈도를 조사한 것으로, 2015년부터 2020년까지 TB/NTM real-time PCR 검사가 의뢰된 8,258건의 환자 데이터를 후향적으로 검토하였다. 총 296건의 검체가 NTM 양성이었고, 발생률은 2015년 2.5% (30/1,209)에서 2020년 3.8% (66/1,740)로 증가하였다. NTM으로 확인된 296건 중, 54.7% (162/296)는 M. avium complex (MAC)에 속하는 균종으로 확인되었고, 그다음으로 M. abscessus complex (MABC) 20.9% (62/296), M. fortuitum 6.4%(19/296) 및 M. flavescens 3.4% (10/296) 순이었다. NTM 양성 검체 중, 약제내성검사가 의뢰된 검체는 76.7% (227/296)였다. 다제내성 NTM은 40.1% (91/227)였고, 광범위 내성 NTM은 59.9% (136/227)였다. NTM과 동시 감염은 43.4% (63/145)이었고 가장 흔한 균종은 Klebsiella pneumonia (23.8%, 15/63)였다. 본 연구는 인천지역의 NTM 분포 및 항균제 내성률과 동반 세균 빈도에 관한 최초 보고이다. NTM 감염의 비율은 점차적으로 증가하는 추세이며 효과적인 관리를 위해 적극적인 진료와 철저한 감염관리가 필요할 것이다.

서울시 세대별 창업 분포와 영향 요인에 대한 연구: 청년층과 중년층의 비교를 중심으로 (A Study on the Distribution of Startups and Influencing Factors by Generation in Seoul: Focusing on the Comparison of Young and Middle-aged)

  • 이 연구는 서울시 세대별(청년층, 중년층) 창업의 공간적 분포와 영향 요인을 분석하는데 목적이 있었다. 이를 위해 창업가의 연령대 자료를 포함하고 있는 서울시 사업체조사(2018) 데이터를 활용하여 서울시 424개 행정동의 세대별 창업 분포를 분석했으며, 산업, 인구구조 및 창업지원 기관 관련 변인이 포함된 연구모형을 설정하여 세대별 창업의 영향 요인을 분석하였다. 분석 방법은 서울시 행정동의 세대별 창업과 주요 변인들의 빈도, 평균 및 표준편차를 확인하기 위해 기술통계를 활용했으며, 전역적, 국지적 공간자기상관 분석을 통해 세대별 창업의 공간적 분포를 분석하였다. 특히 세대별 전체 창업과 주요 산업별 창업을 구분하여 분석함으로써 서울시 창업의 공간 분포를 심층적으로 확인하였다. 이후 라그랑주 검정을 통해 공간회귀분석 모형을 선택하였으며, 이를 바탕으로 세대별 창업에 미치는 지역적 영향 요인을 분석하였다. 연구결과를 통해 도출한 주요 결과는 다음과 같다. 첫째, 청년층과 중년층 창업의 공간적 분포에 유의미한 차이가 있었다. 청년층은 서초·강남-용산-마포-강서구로 이어지는 벨트 지역에서 창업이 활발하게 이루어졌지만, 중년층은 서초·강남·송파·강동으로 대표되는 동남권 지역의 창업이 활발한 편이었다. 둘째, 서울시 세대별 창업은 업종에 따라 다양한 공간적 분포를 보였다. 지식첨단산업(정보통신, 전문서비스)은 세대 공통적으로 서초, 강남, 마포, 구로, 금천구가 중심지역이었으며, 제조업은 기존 집적지를 중심으로 창업이 집중되어 있었다. 반면 생활서비스업의 경우 청년층은 용산, 마포, 관악 등 대학과 문화 중심지역에서 창업이 활발했으며, 중년층은 신시가지 중심으로 창업이 집중되었다. 셋째, 서울시의 세대별 창업 입지에 대한 영향 요인에 차이가 있었다. 청년층은 첨단산업, 대학, 문화자본 및 인구 밀집이 창업에 유의미한 영향 요인이었으며, 중년층은 전문서비스 특화도, 낮은 평균연령, 창업지원 기관 밀집 수준이 창업에 유의미한 영향을 미쳤다. 또한 이러한 입지요인은 산업별로 차별적인 영향이 있었다. 연구를 통해 제시한 제언은 다음과 같다. 첫째, 서울시 지역, 산업 및 세대별 특성을 고려한 체계적인 창업지원이 필요하다. 세대별 창업 지역과 산업에 상당한 차이가 있는 만큼 지역 및 산업적 특성을 고려한 맞춤형 창업지원 체계를 강화할 필요가 있다. 둘째, 연구 방법적 측면에서 데이터 축적을 통해 자치구 단위에서 문화, 재정 등을 종합적으로 고려한 후속 연구가 필요하다.

담낭 절제술 후 총담관 직경의 장기 변화에 대한 CT 평가 (CT Evaluation of Long-Term Changes in Common Bile Duct Diameter after Cholecystectomy)

  • 목적 본 연구는 담낭절제술 후 보상적 총담관 확장의 빈도와 정도를 조사하고, 담낭절제술과 common bile duct (이하 CBD) 확장 사이의 시간을 평가하고, 폐쇄성 CBD 확장을 암시하는 잠재적으로 유용한 CT 소견을 식별하는 것을 목표로 한다. 대상과 방법 2009년에서 2011년 사이에 단일 센터에서 담낭절제술 전후에 여러 차례 CT 스캔을 받은 담도 폐쇄가 없는 121명의 환자를 대상으로 한 후향적 연구를 진행하였다. 또한 담낭절제술 후 CBD 결석 또는 팽대부 종양으로 인해 초기에 연구에서 제외되었던 11명의 환자의 임상 및 CT 소견을 조사하여 폐쇄성 및 비폐쇄성 CBD 확장의 특징을 확인하였다. 결과 121명의 환자의 평균(표준편차) 단축 최대 CBD 직경은 담낭절제술 전 축면에서 5.6(± 1.9) mm였지만 담낭절제술 후 7.9 (± 2.6) mm로 증가했다(p < 0.001). 담낭절제술 전 축성 CBD 직경이 8 mm 미만인 106명의 환자 중 39명(36.8%)이 담낭절제술 후 ≥ 8 mm의 CBD 확장을 보였다. 장기(> 2년) 연속 추적 CT 스캔을 받은 17명의 환자 중 6명(35.3%)은 결국 모두 담낭 절제술 후 2년 이내에 축성 CBD 직경이 유의미하게(> 1.5배) 증가한 것으로 나타났다. 폐색 또는 관련 증상이 없는 121명의 환자 중 단 1명(0.1%)만이 담낭 절제술 후 intrahepatic duct (이하 IHD) 확장 > 3 mm를 보였던 반면, CBD 폐쇄가 있는 11명의 환자 모두 복통과 비정상 검사실 지수가 있었고 81.8% (9/11)가 IHD 및 CBD의 상당한 확장을 보였다. 결론 보상적 비폐쇄성 CBD 확장은 일반적으로 폐쇄성 확장과 비슷한 정도로 담낭절제술 후에 발생한다. 그러나 담낭절제술 후 담관 폐색과 관련 증상이 있거나, 의미 있는 IHD 확장 또는 2-3년 후 추가적인 CBD 확장이 발생하는 경우 CBD 폐쇄를 의심해야 한다.

Flash 3D 재구성을 적용한 뇌 혈류 부하 단층 촬영 시 LEAP 검출기의 적용에 관한 연구: One Day Subtraction Method (The Study about Application of LEAP Collimator at Brain Diamox Perfusion Tomography Applied Flash 3D Reconstruction: One Day Subtraction Method)

  • 목적 : Siemens사의 Flash 3D(Pixon(R) method, 3D OSEM)는 검사 시간을 단축하면서 재구성을 통해 영상의 질을 높일 수 있도록 개발된 소프트웨어 프로그램으로써 핵의학 단층 촬영 시 유용하게 적용되고 있는 영상처리기법이다. 그러나 감산된 영상을 Flash 3D로 재구성하여 시행하는 뇌 혈류 부하 검사 시에 영상 획득시간을 짧게 하여 검사를 시행하면 재구성된 감산 영상의 신호 대 잡음비가(SNR, signal to noise ratio) 기저 영상에 비해 낮아지는 문제점이 있었다. 감산 영상의 SNR을 높이기 위해 LEAP 검출기를 사용하였고, 뇌혈관의 해상력보다는 혈관 확장의 예민도에 더 중점을 두었다. 본 실험은 뇌혈관 부하 단층 촬영 시 LEAP 검출기의 적용 가능성을 확인하고, Flash 3D를 이용한 적정 수준의 재구성 매개 변수를 파악하는 데 목적이 있다. 실험재료 및 방법 : (1) 팬텀 평가: $^{99m}Tc$을 넣은 Hoffman 3D Brain $Phantom^{TM}$을 이용하였다. LEAP와 LEHR 검출기로 첫 번째 영상을(부하 영상에 해당) 획득하고 $^{99m}Tc$의 반감기인 6시간 후 동일한 방법으로 두 번째 영상을(기저 영상에 해당) 획득하였다. 또한, 각각의 기저 영상과 감산 영상의 SNR 및 백질과 회백질의 비를 측정하였다. (2) 환자 영상의 평가: 2008년 5월부터 2009년 1월까지 LEAP 검출기로 촬영하여 정상으로 판독된 15명과 LEHR 검출기로 촬영하여 정상으로 판독된 13명의 환자를 대상으로 영상을 정성분석 하였다. Phantom에서 얻은 재구성 매개 변수를 대입하여 평가하였다. 하루 검사 프로토콜로 시행하였으며 기저에서 925 MBq, 부하에서 925 MBq의 $^{99m}Tc$-ECD를 투여하였다. 결과 : (1) 팬텀 평가: 각 검출기에서 획득한 계수치를 측정한 결과 LEHR 기저에서는 41~46 kcount, 부하에서 79~90 kcount, 감산에서 40~47 kcount가 측정되었다. LEAP의 경우 기저에서 102~113 kcount, 부하에서 188~210 kcount, 감산에서 94~103 kcount가 측정되었다. LEHR 감산 영상의 SNR은 LEHR 기저 영상과 비교하면 37% 감소하여 나타났고, LEAP 감산 영상의 SNR은 LEAP 기저 영상과 비교하면 17% 감소하여 나타났다. 회백질과 백질의 비는 LEHR 기저에서 2.2:1 감산에서 1.9:1로 측정되었고, LEAP 기저에서는 2.4:1 감산에서 2:1로 측정되었다. (2) 환자 영상의 평가: LEHR 검출기로 획득한 계수는 기저에서 대략 40~60 kcount, 부하에서 80~100 kcount 사이였다. 기저 및 부하 영상은 FWHM을 7 mm로 (타 장비의 Cutoff에 해당), 감산 영상은 FWHM을 11 mm로 설정하는 것이 적절하였다. LEAP는 기저에서 대략 80~100 kcount, 부하에서 180~200 kcount로 측정되었다. LEAP 영상은 기저 및 부하에서 FWHM을 5 mm로, 감산에서 7 mm로 설정해야 영상의 흐림을 줄일 수 있었다. 기저 및 부하 영상은 LEHR 영상이 LEAP 영상보다 해상력이 우수했다. 그러나 감산 영상의 경우 팬텀 실험과 같이 LEHR 영상의 SNR이 떨어져 영상이 거칠게 보였다. 감산 영상은 LEAP 영상이 LEHR 영상에 비해 SNR 및 예민도가 높게 평가되었다. LEHR과 LEAP 검출기의 모든 영상에서 subset과 iteration은 8회가 적절하였다. 결론 : LEAP 검출기를 이용해 적정 수준의 필터를 사용함으로써 SNR을 높여 보다 선명한 감산 영상을 획득할 수 있게 되었다. 하루 검사 프로토콜을 적용하여 Flash 3D로 재구성하는 경우, 보다 나은 감산 영상을 얻기 위해 LEAP 검출기의 적용을 고려해 볼 수 있을 것으로 판단된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. 말기 암 환자의 마지막 24시간 동안 진통제 사용의 분석 : 내과의사와 외과의사의 비교 (The Use of Analgesics in the Last 24 hours of Life of Patients with Advanced Cancer : A Comparison of Medical Physicians and Surgeons)

  • 목적 : 암은 우리나라 사망원인의 첫째이며 말기암환자에서 보이는 신체증상 중 통증은 의료인이 반드시 조절을 해주어야 한다. 암환자의 90%이상에서 통증조절이 가능하며, 말기 암환자의 임종 24시간 전 $33{\sim}60%$에서는 의식이 명료하여 진통제의 경고, 투여가 가능하다. 마지막 24시간 동안의 진통제 사용실태와 내과계와 외과계 의사에따른 통증조절 방법의 차이를 조사함으로서 보다 계획적이고 적극적인 통증 조절 중요성의 중요성을 강조하고자 본 연구를 시행하였다. 방법 : 1994년 7월 1일부터 1995년 6월 30일까지 모대학병원에 입원하여 암으로 사망한 환자 160명을 대상으로 의무기록지, 간호기록지를 조사하는 후향적 코호트 방법을 시행하여 인구통계학적 자료와 사망 24시간전의 의식상태, 통증여부, 암의 종류 등을 조사하였다. 대상자를 내과의사와 외과의사에 의해 치료받은 군으로 나누어 임종전 24시간 동안 투여한 진통제의 용량, 투여방법, 투여종류, 필요시 마타 투여된 진통제의 평균 횟수를 비교 분석하였다. 진통제의 용량은 경구몰핀 등가량으로 환산해서 계산하였다. 결과 : 1) 전체 160명 중 남자가 102명(63.8%)명, 여자는 58명(36.2%) 이었고 평균연령은 56.4세였다. 2) 평균입원 27.8일이었으며 원발부위에 따른 암의 종류는 위암 42명(26.3%), 폐암 29명(18.1%), 간암 29명(18.1%), 췌장암 10명(6.2%), 대장암 9명(5.6%), 자궁경부암 6명(3.7%), 유방암 5명(3.1%)의 순이었다. 3) 160명 중 125명(78.13%)이 통증을 호소했고 진통제를 투여받은 환자는 내과계 103명중 66명(64.08%), 외과계 57명 중 31명(54.39%)이었다. 97명 중 50명(51.55%)에서 경고, 투여가 가능했다. 4) 임종 24시간 전에 86명(53.75%)에서 의식이 명료했다. 5) 규칙적으로 투여된 진통제의 종류는 경구용 모핀이 34명(내과계 24명, 외과계 10명)으로 제일 많았고, 주사용 모르핀이 26명(내과계 20명, 외과계 6명), 경구용 코데인이 8명 (내과 5명, 외과 3명)의 순이었다. 비정기적으로 통증조절을 위해 투여된 약물로는 주사용 모르핀이 16회, 데메롤 근주가 12회, nubain 근주가 9회의 순이었다. 6) 규칙적으로 투여된 진통제의 평균량은 내과계에서 115.41 OME에서 외과계 52.7 OME 보다 많았으나(P<0.05), 환자 요구시마다 투여된(p.r.n.) 진통제의 양은 외과계가 66.64 OME 로서 내과계 23.49 OME 보다 유의하게 많았다(P<0.01). 7) 통증호소시에 투여한 진통제의 횟수에 있어서도 내과계 평균은 0.62회, 외과계는 1.88로 유의한 차이가 있었다(P<0.001). 8) 보조진통제의 사용은 내과계에서 57명(55.34%), 외과계에서 25명(43.86%)이 사용했다. 결론 : 임종 24시간 전에 과반수 이상이 의식이 명료했으며 내과의사가 외과의사보다 적극적으로 통증조절을 하고 있으나 많은 의사들이 규칙적인 통증조절보다는 비정기적인 투약과 진통제의 비경구투여를 선호하였다.
