치매 환자의 격앙행동과 주간호자의 대처행동 (Agitation in Home-dwelling Persons with Dementia and Coping Behaviors in Primary Care-givers to the Agitation)

  • Purpose: The purpose of this study was to investigate agitation in home-dwelling persons with dementia and coping behaviors of primary family care-givers to agitation. Methods: The research was designed as a descriptive study. A total of two hundred and five subjects had participated in this study. To measure agitation in persons with dementia, Cohen-Mansfield Agitation Inventory was used. To measure coping behaviors of primary family care-givers to the agitation, a questionnaire was used. Descriptive statistics, t-test, ANOVA, Scheff$\acute{e}$ test were used to answer the research objectives. Results: 'Repetitive mannerisms' and 'repeating sentences' were the most commonly occurred agitation in home-dwelling persons with dementia. 'Leaving the situation', 'allowing certain time', and 'soothing by talking' were the most common coping behaviors occurred in primary care-givers. Conclusion: The findings of the current study would provide meaningful data to develop nursing programs to control agitation for home-dwelling persons with dementia and education programs for primary care-givers to cope with persons' agitation.

중환자 가족의 신체억제대 사용에 대한 인식: Q방법론적 접근 (Perceptions of Family Care-givers toward Use of Physical Restraints: An Application of Q-methodology)

  • Purpose: The purpose of this study was to identify on the perceptions of family care-givers toward use of physical restraints according to their values, beliefs, and perceptions using Q methodology. Methods: Thirty-three family care-givers classified 41 selected Q-statements into 9 points standard. The obtained data were analyzed by using a pc QUANL program. Results: Principal component analysis identified 4types of the perceptions of family care-givers toward the use of physical restraints. Type I is 'Rational accepted', which means that they perceived the restraints are essential therapeutic devices and had cooperative attitude to use of medical staffs' restraints. Type II is 'Sardonic sensibility', which means that they have a negative and a cynical attitude to use of physical restraints. Type III is 'Ambivalent', which means that they have conflicts between rationality and emotion, and type IV is 'Practical claim of a right', which means they insist that patients and their family members must be provided with a detailed explanation regarding the application of physical restraints. Conclusion: The findings of this study suggest that perceptions toward the use of physical restraints among family care-givers should be understood for patients' safety and dignity in medical circumstance. Based on the results, this study will be useful in developing the customized nursing intervention for supporting family care-givers' subjectivity considering the Korean context.

요양보호사의 직무만족결정요인 연구: 충남지역 입소시설요양보호사를 중심으로 (An Empirical Study on the Determinants of Job Satisfaction among Care Givers: Focus on Care Givers Working in Care Facilities of Chung Nam Province)

  • 본 연구는 요양보호사의 직무만족 결정요인을 탐구하는데 있다. 연구를 위하여 직무만족결정요인을 개인 조직 직무적요인으로 구분하였다. 개인적요인으로 성장욕구와 자아효능감을, 직무적요인은 직무다양성, 직무중요성, 직무자율성, 역할명료성, 업무량을, 조직적요인으로는 동료관계와 교육훈련을 사용하여 직무만족에 미치는 영향관계를 분석하였다. 연구를 위한 자료는 충청남도 소재 입소요양시설 52개소를 대상으로 479명의 요양보호사들로부터 구조화된 설문지로써 수집하였으며, 수집된 자료 분석은 SPSS 20.0 통계패키지를 사용하여 회귀분석을 실시하였다. 연구결과 직무다양성, 직무중요성, 대인관계, 교육훈련은 직무만족에 긍정적 영향을, 업무량은 부정적 영향을 미치는 것으로 나타났으며, 성장욕구, 자아효능감, 직무자율성, 역할명료성은 유의미한 영향관계를 보이지 않았다. 연구 결과에 근거하여 요양보호사의 직무만족 결정요인에 관한 이론적 함의를 논의하고 향후 연구방향을 제시하였다.

노인요양병원의 공간구성에 대한 간호사 요구 (Nurses' Needs for the Spatial Composition of Geriatric Hospital)

  • Purpose : The purpose of this study was to examine the nurses' needs for the spatial composition of geriatric hospital. Methods : The survey method was used for collecting the data. The subjects were 110 nurses who worked at 6 geriatric hospitals in Busan. They answered the questionnaire by the self-administered method. The questionnaire consisted of items which asked the spatial composition of patient room, bathroom, dining space, resting space, care-givers' room, and garden. Results : 1) The patient room using a bed on ondol floor would be useful. Also, it would be desirable that most patient rooms consisted of 4 or 6 persons per patient room and the others were 1 or 2 persons per room. 2) The bathroom for only patients and the shower room for their families or care-givers are needed in the geriatric hospital. 3) The dining room for patients in each floor would be useful to old patients. In addition to that, the cafeteria for their families or care-givers are needed. 4) The resting space for patients and their families, for the staff, and for care-givers are needed. 5) The personal locker and refrigerator are necessary fixtures in care-givers' room. Also, table, sofa, sink, and shower booth are also needed. 6) On the base of activities of care-givers, the space for preserving, washing, and heating foods are needed. Also, the storage space for personal items and the fitting space are needed. Implications : The results of this study would be the fundamental data for space planning of the geriatric hospital.

요양보호사의 직무환경에 따른 직무스트레스와 소진이 직무만족에 미치는 영향 (The Effect of Job Stress and Burnout by Job Environment of Care-givers on Job Satisfaction)

  • 본 연구는 노인장기요양기관 종사자인 요양보호사들의 직무환경과 관련된 속성 및 스트레스와 소진을 파악하고 이들 요인이 직무만족에 어떠한 영향을 미치는지 효과를 분석하여 요양보호사의 조직업무를 향상시켜 노인의 삶의 질과 노인복지 발전에 도움이 되는데 목적을 갖고 연구하였다. 연구대상은 전북 군산시에 소재한 노인장기요양기관(요양시설, 재가노인복지시설)에 근무하는 요양보호사 129명을 대상으로 하였다. 분석결과 요양보호사의 직무환경의 하위요인으로 인적환경과 보상체계, 업무전문성 등이 직무소진, 직무스트레스, 직무만족에 유의적인 영향을 미치는 요인임을 실증시켜 주었다. 이는 그만큼 인적환경을 위한 업무전문성 및 능력과 혁신적 사고를 가진 업적에 비례한 보상체계가 잘 되어 있을수록 긍정적인 관계 형성과 동시 직무소진과 스트레스의 감소 및 직무만족이 향상됨을 의미한다. 이는 최근 장기요양을 위한 노인성 환자의 급증과 장기요양기관에서의 요양보호사의 역할과 직무가 중요시되는 현실에 비추어 볼 때 전문직으로서의 다양한 복지 수혜적 봉사자로서의 요양보호사들의 직무환경 개선을 위한 직무전문화 보장과 이들 직무능력에 따른 적절한 보상시스템의 체계화가 필요시 됨을 시사한 것으로 볼 수 있다. 따라서 직업에 대한 자긍심이 높은 전문 인력의 배출과 참여로 요양보호사의 잦은 직무소진과 스트레스를 낮출 수 있는 경감요인을 찾아 제거함으로써 보다 많은 인력의 참여와 조직헌신을 통한 직무만족 극대화를 통해 조직몰입을 높일 수 있을 것이다.

요양보호사의 노인에 대한 지식 및 태도가 직무스트레스에 미치는 영향 (The Effect of Care Giver's Knowledge and Attitude toward the Elderly on Job Stress)

  • The purpose of this study was to identify the level of care-givers' knowledge, their attitude towards the elderly and their stress levels within the job, and to apprehend the cause which has had an influence on job stress. The research subjects were 230 care-givers who work in a sanitarium for the elderly in the East of Jeollanamdo. The tool used to measure their knowledge of the elderly was FAQ1 and the semantic differential scale was used to determine the attitude. Job stress was measured by the tool which was used by sun-ju Park. The data analysis was carried out by SPSS Win 13.0. The results of this study are as follows; The care-givers' knowledge level point towards the elderly was relatively low at 44.6%. The average point of attitude toward the elderly was 3.91, which is relatively moderate. The percentages of job stress was generally low. The results showed us that there was no correlation with the knowledge level of the elderly, the attitude towards the elderly and job stress. Since the results indicated that care-givers' knowledge level for the elderly was low, we reached the conclusion that further education where care-givers can get general knowledge for the elderly and aging is necessary. Moreover, research for strengthening positive attitudes towards the elderly is also necessary.

만성신부전환자의 주보호자로 살기 (The Lived Experience of Caregivers of Chronic Renal Failure Patient)

  • Purpose: The research aims to understand the lived experience of the caregivers of chronic renal failure(CRF) patients and its essential meaning. The results of the study can be used as basic materials for developing comprehensive intervention methods of care givers of CRF patients. Method: The research used van Manen's hermeneutic and phenomenological research methods in order to describe the lived experience and to understand its meaning. It concentrates on the understanding the essence of experience and consists of existential survey, hermeneutic and phenomenological reflection and hermeneutic writings. Participants in this research were five women care givers of CRF patients who had hemolysis at C university hospital in a metropolitan city, the period of data collection was from July 27 to Sep. 4, 2004 and major data of results in the following 5 essential themes. "sole responsibility for the patient enduring everything", "creating their own field", "heavy and painful life without hope of their private life", "wishing not to be inherited and consoling each other". Conclusion: The above findings point out that the experience of care givers of CRF patient affected and changed all parts of an individual life and his or her family life. Therefore, it suggests that total family nursing care must be considered in order to provide the holistic caring for CRF patients and their care givers.

노인장기요양기관의 변혁적 리더십이 조직효과성에 미치는 영향 : 요양보호사의 임파워먼트 매개효과를 중심으로 (Effects of Transformational Leadership on the Organizational Effectiveness of Long-Term Care Facilities : Focusing on the Care Giver's Mediating effect of Empowerment)

  • 본 연구는 요양보호사가 인식한 상사의 변혁적 리더십과 직무만족 및 조직몰입으로 대표되는 조직효과성과의 관계에서 임파워먼트가 미치는 영향을 살펴보기 위하여 실시되었다. 이를 위해 서울, 경인지역 장기요양기관에서 근무하는 요양보호사를 대상으로 서베이를 실시하여 283명의 응답을 최종 분석에 활용하였다. 분석결과 첫째, 장기요양기관의 변혁적 리더십은 직무만족과 조직몰입에 유의미한 영향을 미치고 있었다. 둘째, 요양보호사의 임파워먼트는 직무만족과 조직몰입에 유의미한 영향을 주는것으로 나타났다. 셋째, 장기요양기관의 변혁적 리더십과 요양보호사의 직무만족 및 조직몰입간의 관계에서 임파워먼트가 매개역할을 하는 것으로 나타났다. 이러한 결과를 토대로 장기요양기관 관리자의 변혁적 리더십 향상을 위한 실천방안과 요양보호사들의 임파워먼트 향상을 위한 프로그램 개발 및 제도적 장치 마련에 대한 시사점을 제시하였다.

두경부암 환자 가족의 돌봄 경험에 관한 현상학적 연구 (A Phenomenological Study of Experiences of Family Care Givers of Patient with Head and Neck Cancer)

  • 목적 : 본 연구는 두경부암 환자를 돌보는 가족들의 돌봄 경험의 의미와 본질을 탐색하는 것을 목적으로 하고 있다. 방법 : 개인들의 공통된 체험의 본질과 의미를 기술하는데 적합한 현상학적 연구 방법을 이용하여 5명의 두경부암 환자 가족을 대상으로 심층 면담을 실시하였다. 결과 : 자료 분석 결과 24개의 주제, 9개의 주제 모음이 도출되었고, '충격적이고 막막한 미로 같은 현실', '가족구성원의 신체상의 변화로 세상과 단절됨', '가족 탈진', '성숙한 돌봄을 위한 가족의 도약'의 4개의 범주가 드러났다. 결론 : 본 연구를 통해 두경부암 환자 가족의 돌봄 경험을 더 깊이 있게 살펴보고 이해할 수 있었다. 본 연구 결과를 기반으로 두경부암 환자 가족의 어려움을 지지해 줄 수 있는 전문적인 지지 프로그램의 개발이 마련되었으면 한다.

보육시설의 실외놀이 환경실태 및 환경특성에 대한 교사의 평가와 요구 (Care-giver's Needs and Evaluation on the Actual Condition of the Playgrounds in Child Care Facilities)

  • The purpose of this study is to classify playground types according to the physical characteristics of playgrounds in child care facilities, to analyze the needs of care-givers and to evaluate the adequacy of playgrounds according to playground types. The specific areas discussed in this study were the evaluation of the adequacy in the physical characteristics of playgrounds for children's outdoor play activities by the care-givers, who assist in the outdoor play activities of children and manage safety in the playground, and a report on playground requirements of them. Data was collected from field survey carried out to investigate the physical characteristics of playgrounds of 21 child care facilities. This survey incorporated a structured-questionnaire for the purpose of evaluating the adequacy of the physical characteristics of playgrounds (location and size of the playground, play equipments, the composition of play areas, ground covers, and location of sidewalk in the playground) by the 181 care-givers from the facilities and investigating the needs of them. The major results showed the following. 1) In field survey, it was observed that while the location and the size of the playgrounds varied widely, ground cover, play equipments, and the composition of play area turned out to be identical, regardless of the location and the size of the playgrounds. 2) The playgrounds were classified into five types according to the number of children and the size of the playground. The five types include A-type as a large-scale facility/smallsize playground, B-type as a small-scale facility/large-size playground, C-type as a small-scale facility/small-size playground, D-type as a middle-scale facility/large-size playground, and E-type as a large-scale facility/large-size playground. 3) The adequacy of the physical characteristics of playground in the D-type was evaluated to be higher than that of the other types. The adequacy of the C-type playground was evaluated to be lower than that of the other types in terms of size and the composition of play areas within the playground. 4) The care-givers of the D-type and the E-type playground expressed a desire to install various play equipments, while the care-givers of the C-type playground did not wish to install play equipment. 5) Various outdoor play areas were needed in the D-type playground.
