Policy Tasks for bringing the Marine Healing Industry to Waterfront Spaces (해양친수공간에서 해양치유산업의 정책 과제)

  • The ocean is a unique space for exchange with the city, forming an organic relationship between people and waterfront areas. The marine healing industry improves quality of life through medical care and well-being, and creates added value by combining with the medical industry. The policy analysis results are as follows. First, the marine healing industry is the newest high-growth area. Furthermore, a professional medical service hospital is the first priority. This will include the development of marine healing facilities and strengthening forest healing connectivity, emphasizing the development of highly specialized medical technologies. The second priority is value creation in marine healing specialization, accompanied by an integrated and systematic policy for research and development of new medicines in marine healing effects. It is also important to apply new market trends and develop applied marine healing service programs. Thirdly, it is necessary to explore the possibility of a fragmented market for policy development.

Design of a Hospice Referral System for Terminally Ill Cancer Patients Using a Standards-Based Health Information Exchange System

  • Objectives: The demand for hospice has been increasing among patients with cancer. This study examined the current hospice referral scenario for terminally ill cancer patients and created a data form to collect hospice information and a modified health information exchange (HIE) form for a more efficient referral system for terminally ill cancer patients. Methods: Surveys were conducted asking detailed information such as medical instruments and patient admission policies of hospices, and interviews were held to examine the current referral flow and any additional requirements. A task force team was organized to analyze the results of the interviews and surveys. Results: Six hospices completed the survey, and 3 physicians, 2 nurses, and 2 hospital staff from a tertiary hospital were interviewed. Seven categories were defined as essential for establishing hospice data. Ten categories and 40 data items were newly suggested for the existing HIE document form. An implementation guide for the Consolidated Clinical Document Architecture developed by Health Level 7 (HL7 CCDA) was also proposed. It is an international standard for interoperability that provides a framework for the exchange, integration, sharing, and retrieval of electronic health information. Based on these changes, a hospice referral scenario for terminally ill cancer patients was designed. Conclusions: Our findings show potential improvements that can be made to the current hospice referral system for terminally ill cancer patients. To make the referral system useful in practice, governmental efforts and investments are needed.

The Effects of Exercise Program on Cognitive Function, Depression, and Life Satisfaction in Elderly (12주간 운동 프로그램이 노인의 인지기능, 불안 및 생활 만족도에 미치는 영향)

  • The purpose of this research was to examine conduction of 12 week exercise program for the aged of sanatorium on their cognitive function, depression, uneasiness, and living satisfaction. The subject of this research was 60 aged persons of H sanatorium in H-si and mental status examination and survey on depression criterion, uneasiness measurement, and living satisfaction of the aged were conducted. To examine the effects of the program, paired t-test was conducted. As a result, there were significant differences after 12-week exercise program in all factors such as cognitive function, depression, uneasiness and living satisfaction of the subjects. In conclusion, regular exercise program may positive effects on mental status cognitive status, depression, uneasiness, and living satisfaction of the aged. Therefore proper exercise form and program are necessary to the aged in long term care facilities.

A report on the regional oral health centers for the disabled in Korea (우리나라 권역장애인구강진료센터 현황과 전망)

  • Purpose : The purpose of this study was to survey the present status of the regional oral health centers for the disabled. Material and methods : Face to face interviews with oral healthcare professionals in eight regional oral health centers and literature reviews were conducted among eight regional oral health centers for the disabled during 2, July to 30, August 2017. Results : It is necessary to establish more regional oral health centers for the disabled in area where the number of disabled person as percentage of whole region population is high. Also, all the centers appeared a lack of professional manpower including dentist, dental hygienist and anesthesiologist, equipment and facilities for the demands of patients and their guardians. It is essential for the government to improve the support system to regional oral health centers for the disabled. Conclusions : This study can be used as a reference for the countermeasure to make a new policy and activate the accessibility of dental care services in the public for the disabled.

Airborne infection risk of respiratory infectious diseases and effectiveness of using filter-embeded mechanical ventilator and infectious source reduction device such as air cleaner (실내 공간에서의 호흡기 감염병 공기전파감염 위험도와 공기정화장치(필터 임배디드 기계식 환기설비 및 공기청정기 등 실내 감염원 저감 장치) 사용에 따른 효율)

  • Particulate infectious sources, including infectious viruses, can float in the air, causing airborne infections. To prevent indoor airborne infection, dilution control by ventilation and indoor air cleaners are frequently used. In this study, the risk of airborne infection by the operation of these two techniques was evaluated. In case of dilution control by ventilation, a high efficiency air filter was embedded at the inlet of supply air. In this study, infectious source reduction devices such as indoor air cleaner include all kinds of mechanical-filters, UV-photo catalysts and air ionizers through which air flow is forced by fans. Two mathematical models for influenza virus were applied in an infant care room where infants and young children are active, and the risk reduction efficiency was compared. As a result, in the case of individually operating the ventilator or the infectious source reduction device, the airborne infection risk reduction efficiencies were 55.2~61.2% and 53.8~59.9%, respectively. When both facilities were operated, it was found that the risk of airborne infection was reduced about 72.2~76.8%. Therefore, simultaneous operation of ventilation equipment and infectious source reduction device is the most effective method for safe environment that minimizes the risk of airborne infection of respiratory infectious diseases. In the case of a space where sufficient ventilation operation is difficult, it was found that the operation of an infectious source reduction device is important to prevent the spread of infectious diseases. This study is meaningful in that it provides an academic basis for strategies for preventing airborne infection of respiratory infectious diseases.

Development and Evaluation of Booklets and Video Clips to Prevent Children from Developing Picky Eating (유아 대상 활동북과 동영상 중심의 채소 편식 개선 교육자료 개발과 평가)

  • Objectives: The purpose of this study was to develop and evaluate booklets and video clips to prevent children from picky eating. Methods: Based on a survey conducted on food preferences of preschool children aged 2 to 5 years, 14 kinds of less preferred vegetables were selected. Accordingly, educational videos, activity books, and teaching-learning guides were produced for preschool children using the 'food bridge' theory, and the educational materials were named "Friendly vegetables". Educational materials were distributed to childcare institutions, and their effectiveness was investigated for preschool children who were instructed on these materials once every 30 days from March to November, 2019. The children were examined for changes in their knowledge of names, colors, taste/texture, methods of cultivation, and preferences for vegetables before and after the instructional course. Results: The awareness of vegetables increased significantly in younger children and the picky eating group. When the assessment was carried out in terms of vegetable knowledge, it was observed that the younger the age or the pickier the in eating food, the more effective the education is compared to the counter part. The preference for vegetables also increased after the instruction compared to the pre-instruction period, but significant changes were seen only in the 2~3 year age group for boys and girls. Also, only the picky group of girls showed changes in preference. The children's average interest in the education materials was 3.85 points out of 5 points. Conclusions: Through this study, we have developed educational materials for standalone use in childcare facilities and confirmed that they have a significant effect on improving awareness and preferences related to vegetables. In summary, the younger the age or the pickier the child in eating food, the more effective the education. It is believed that additional education on mealtime guidance is needed which can alter the eating behavior of preschool children and improve their diet. It is proposed to widen the scope of use of the materials by collecting diverse opinions from child care teachers.

Effectiveness of virtual reality immersion on procedure-related pain and anxiety in outpatient pain clinic: an exploratory randomized controlled trial

  • Background: The study investigated virtual reality (VR) immersion in alleviating procedure-related pain in patients with chronic pain undergoing fluoroscopy-guided minimally-invasive intervention in a prone position at an outpatient clinic. Methods: In this prospective randomized controlled study, 38 patients undergoing lumbar sympathetic ganglion block were randomized into either the VR or the control group. In the VR group, procedure-related pain was controlled via infiltration of local anesthetics while watching a 30-minute VR hypnotic program. In the control group, the skin infiltration alone was used, with the VR device switched off. The primary endpoint was an 11-point score on the numerical rating scale, indicating procedure-related pain. Patients' satisfaction with pain control, anxiety levels, the need for additional local anesthetics during the procedure, hemodynamic stability, and any adverse events were assessed. Results: Procedure-related pain was significantly lower in the VR group (3.7 ± 1.4) than in the control group (5.5 ± 1.7; P = 0.002). Post-procedural anxiety was lower in the VR group than in the control group (P = 0.025), with a significant reduction from pre-procedural anxiety (P < 0.001). Although patients' satisfaction did not differ significantly (P = 0.158) between the groups, a higher number of patients required additional local anesthetics in the control group (n = 13) than in the VR group (n = 4; P = 0.001). No severe adverse events occurred in either group during the study. Conclusions: VR immersion can be safely used as a novel adjunct to reduce procedural pain and anxiety during fluoroscopic pain intervention.

A Literature Review of Issues and Tasks by Period of Revision of Regulations Related to Convalescent Rehabilitation Wards in Japan: Focusing on Quality Evaluation

  • Purpose: Japan established the convalescent rehabilitation wards, corresponding to Korea's rehabilitation medical institutions, in 2000 and developed it into the present system through continuous revisions. This study sought to analyze the issues and tasks faced by Japan segregated by the period of revision of convalescent rehabilitation ward-related medical fee regulations, through a literature review and further aimed to explore the direction of development of domestic rehabilitation medical institutions. Methods: Ten revisions of the medical fee regulations were classified into three stages based on quality evaluation: (1) the quantitative expansion stage (2000-2006); (2) quality evaluation introduction stage (2008-2014); and (3) quality evaluation maturity stage (2016-2020). Results: The following issues and tasks emerged: (1) For the quantitative expansion stage; insufficient rehabilitation within the ward, insufficient after-hour rehabilitation, insufficient connection with acute-stage hospitals and maintenance facilities, and the low ratio of specialists. (2) For the quality evaluation introduction stage; disparity in the manpower between institutions, the necessity of a 365-day rehabilitation system, avoidance of critical patients, and the problem that an increase in the amount of rehabilitation did not lead to a qualitative improvement. 3) For the quality evaluation maturity stage; cream-skimming issues in selecting patients, inappropriate evaluation of rehabilitation effects, and the necessity of follow-up measures after discharge. Conclusion: It is worth referring to the established regulations in Japan, and concurrently it is necessary to strengthen the evaluation of the structures, processes, and results when operating and evaluating rehabilitation medical institutions in Korea taking into account the side effects that could be identified in Japan.

Qualitative Study for Barriers for Medication and Health Care Service Use among the Visually Impaired and Hearing Impaired in Korea (시각장애인 또는 청각장애인의 의료기관 이용 및 의약품 안전사용 저해요인 관련 심층면접조사)

  • Objective: The disabled are in a blind spot for obtaining information on drugs, and the pharmacies' counseling on drug use is centered on non-disabled people. Few studies have investigated the current statuses of drug use by type of disability. The purpose of this study is to understand the drug use by type of disability and by life cycle of visually impaired and hearing impaired in Korea. Methods: The study participants consisted of 16 people with visually impairments, 12 people with hearing impairments. One in-depth interview was conducted per participant, and each interview was recorded and documented. Results: Common barriers against safe medication and medical service uses across disability types are 'lack of consideration and service for the disabled, limited access to medical facilities due to disability, limited access to information regarding medication use, psychological anxiety about drug use and side effects, and inconvenience regarding COVID-19 epidemic. The specific factors were 'difficulties in identifying proper medicines and following prescribed dosages' in the case of visually impaired, and 'problems with sign language interpretation system' for the hearing impaired. Conclusion: Disabled people are hindered from using medicines properly due to various factors. Based on the content derived from this study, it is necessary to eliminate the inhibition factors and devise specific measures for the safety of each type of disorder such as developing a method for medication counseling considering disabilities and establishing communication support systems.

Gardening Program As Cognitive Rehabilitation Program For Mild Cognitive Impairment (경도인지장애 재활프로그램으로의 가드닝 프로그램)

  • The main purpose of this study is to quantitatively grasp the effectiveness of the gardening program by conducting a gardening program to induce active activities of the elderly and to induce healing functions and social activities, and then observing the results before and after implementation. In order to achieve the purpose of the study, as a result of conducting 30 gardening programs for 20 elderly people at two elderly care facilities, the measure of depression was found to have an improvement effect as a practice-oriented program, and it was found to revitalize the elderly. In addition, it was found that the quality of life was improved by the gardening program, and it was found to be helpful in activating the brain.
