개심술에 있어서 GIK 의 심근 보호효과에 대하여 (Effect of GIK Solution for Myocardial Protection)

  • Although anoxic cardiac arrest produces a dry, quiet field, the ability of the myocardium to withstand the anoxic insult is uncertain. The current growth of interest in the use of various cardioplegic solutions ` has resulted in the development of a number of different solutions. In this study, 51 consecutive cases of elective open heart surgery with the aid of extracorporeal circulation were reviewed retrospectively to compare two methods of myocardial preservation. All of these open heart operations had been performed, using hemodilution principle under the moderate hypothermia at the Department of Thoracic and Cardiovascular Surgery, College of Medicine, Kyungpook National University from December, 1975 to July, 1979. In the 31 consecutive cases that form the anoxic arrest group, the operations were done with intermittent aortic cross-clamping and topical cardiac hypothermia. The heart was cooled topically by cold normal saline, which was converted to ice slush before application to the pericardial sac. Twenty of 51 consecutive cases were assigned to the cold cardioplegic method [the cardioplegic group], in which two kinds of cold cardioplegic solutions [Young solution and G IK solution] were infused into the aortic root proximal to the aortic cross clamp for myocardial preservation. Mean total aortic cross clamp times were 43 minutes in the anoxic arrest group and 67 minutes in the cardioplegic group. In the post-operative period, spontaneous regular heart beatings were recovered in 80 percent of the cardioplegic group as opposed to 25.7 percent of the anoxic arrest group. Ventricular fibrillation requiring DC shock was seen in 32.3 percent of the anoxic arrest group and 10 percent of the cardioplegic group. In the cardioplegic group, mean CPK-MB was one positive value on the first post-operative day, and mean LDH 1 was elevated to 51 0 units/ml on the 2nd post-operative day. These results indicate that protecting the myocardium with cold cardioplegia is superior to use of the anoxic cardiac arrest.

심근정지후 심근세포의 회복과정과 심근정지용액에 관한 연구 (A Study on the Cardioplegic Solutions and the Cardioplegia of Guinea Pig Heart)

  • The changes in membrane potential and intracellular $Ca^{2+}$ activities along with adenosine triphosphate (ATP) and creatine phosphate (CP) contents of guinea pig hearts were studied during the cardioplegia and recovery phase. The results obtained were summarized as follows: 1) A low concentration of $Ca^{2+}$ in the cardioplegic solution caused significant changes in membrane potential during the cardioplegia and action potential configurations of the first beat after cardioplegia. 2) Intracellular $Ca^{2+}$ activities were decreased significantly during cardioplegia and increased when the cardiac tissue was reperfused with the normal Tyrode solution. The magnitude of increase in intracellular $Ca^{2+}$ activities was larger in the 0.1 mM $Ca^{2+}-cardioplegic$ solution than the 1.8 mM $Ca^{2+}-solution$. 3) ATP and CP contents of cardiac ventricular tissue were decreased during the cardioplegia, regardless of $Ca^{2+}$ concentration in the cardioplegic solutions, and CP contents were recovered with the reperfusion of normal Tyrode solution faster than those of ATP. And, there were no significant differences in the recovery of CP contents with different concentrations of $Ca^{2+}$ in the cardioplegic solutions tested, while the recovery of ATP contents was faster with $15mM\;K^{+},\;0.1mM\;Ca^{2+}$ cardioplegic solution. These results suggest that the $Ca^{2+}$ concentration in the cardioplegic solution is one of major factors influencing the recovery of cardiac tissue from the cardioplegia.

Crystalloid Cardioplegic Solution과 Blood Cardioplegic Solution을 사용한 선천성 심기형 환자에서의 술 후 심기능 평가에 대한 비교 연구 (A Comparative Study of the Postoperative Cardiac Performance after Repair of Congenital Heart Defects with Crystalloid and Blood Cardioplegic Solution)

  • This study was undertaken to hemodynamically determine the differences of myocardial protective effect between crystalloid and blood cardioplegic solution. Twenty nine children undergoing cardiac operations due to cyanotic congenital heart diseases were randomized into two groups receiving crystalloid or blood cardioplegia. Cardiac indices and other hemodynamic datum were examined postoperatively. Although there was no statistical differences between groups, postoperative stroke volume indices and left ventricular stroke work indices were slightly better with blood cardioplegia. We also found that postoperative left atrial pressures[p=0.0003], central venous pressures[p=0.004], and heart rates[p=0.014] were significantly lower with blood cardioplegia. The fact that relatively lower ventricular preloads [left atrial pressure and central venous pressure] were required to provide adequate cardiac output in blood cardioplegia group suggested superior myocardial protective effect of blood cardioplegic solution.

소아 연령군에서의 냉각-산소화-희석-혈심정지액을 이용한 심근 보호에 대한 임상적 고찰 (Clinical Trial of Myocardial Protection using Cold Oxygenated Diluted Blood Cardioplegia in Child Age)

  • Hypothermic cardioplegia is a well established method to optimize myocardial preservation during ischemic arrest, and it has been demonstrated that oxygenation of crystalloid cardioplegic solutions markedly enhances myocardial protection, The addition of a small amount of red blood cells to a crystalloid cardioplegic solutions improves capillary perfusion. Considering these results, we changed our cardioplegic solution from cold oxygenated crystalloid[Group 2] to cold oxygenated diluted blood[Group 1]. In this investigation, we examined the effects of two hypothermic potassium cardioplegic solutions on myocardial preservation in 50 patients[30 of Group 1 and 20 of Group 2] of child age group. Factors considered preoperatively included age, sex, body weight, preoperative diagnosis, and they showed no statistical differences, Intraoperative factors considered included duration of cardiopulmonary bypass, duration of aortic occlusion, operative mortality, which also revealed no statistically significant differences, We measured the serum levels of GOT[glutamate oxaloacetate transaminase] and CPK [creatine phosphokinase] during the first two days postoperatively, which, in both groups, showed significantly higher values until postoperative 1 day, and decreasing tendancy thereafter, however we failed to find any significant difference between two groups regarding the serum levels of those enzymes each day. Time for extubation and use of inotropics also revealed no significant differences. Defibrillation was needed less in Group 1 than in Group 2[p<0.05], and one case of supraventricular tachyarrhythmia occured in Group l. We conclude that cold oxygenated diluted blood cardioplegia provides no less preservation than does an oxygenated crystalloid cardioplegic solution in child age group.

St. Thomas Hospital 심정지액에 Creatine Phosphate 를 첨가한 후 심근 보호 효과 (Enhanced Myocardial Protection by Addition of Creatine Phosphate to the St. Thomas Hospital Cardioplegic Solution -Studies in the rat -)

  • The potential for enhancing myocardial protection by adding high-energy phosphate to cardioplegic solutions [St. Thomas Hospital solution] was investigated in a rat heart model of cardiopulmonary bypass and ischemic arrest. Creatine phosphate was evaluated as an additive to the St. Thomas Hospital cardioplegic solution. Creatine phosphate 10.0 mmol/L as the optimal concentration which improved recovery of aortic flow and cardiac output after a 30 minute period of normothermic [37oC] ischemic arrest. In comparing mechanical function in both groups the mean postischemic recoveries of aortic flow, cardiac output, stroke volume and stroke work [expressed as a percentage of its preischemic control] were significantly greater in STH-CP group than in CP- free control group. In addition to improving function and decreasing CK release, CP reduced reperfusion arrhythmias significantly decreasing the time between cross-clamp removal and return to regular rhythm from 81.8 * 13.9 [sec] in CP-free group to 35.9 * 6.8 [sec] in CP group [P< 0.05] so, exogenous CP exerts potent protective and antiarrhythmic effects when added to the St. Thomas Hospital cardioplegic solution. However, the mechanism of action remains to be elucidated.

흰쥐의 적출된 심장에서 심정지액의 온도가 심근보호에 미치는 영향 (The Effect of Temperature of Cardioplegic Soultion on Myocardial Protection from Ischemia - Experimental Study using Isolated Rat Heart Perfusion Technique -)

  • The effect of temperature of cardioplegic solution on myocardial preservation was studied using isolated rat heart perfusion technique. Twenty Sprague-Dawley rats, weighing 120~140gm, were pretreated with intraperitoneal injection of heparin sodium[300u/kg] and then the hearts were excised after cervical herniation 30 minutes later. The hearts were perfused in isolated working heart apparatus with oxygenated modified Tyrode solution at 37oC. After 10 minutes of non working heart perfusion, the hearts were subjected to arrest for 30 minutes by administration of 5cc cardioplegic solution at the temperature of 4oC [Group I ], 15oC [Group II], 25oC [Group III], 37oC[Group IV]. At the same time, the topical cooling of heart was performed using ice saline. After arrest, the hearts were reperfused by non working heart perfusion for 1 hour with modified Tyrode solution at 37oC. The CPK, GOT and LDH in reperfusate were measured at 5,20,40,60 minutes after start of reperfusion. With the values of those, we compared the effect of temperature of cardioplegic solution on myocardial preservation. The results were as follows; 1. The enzyme values in reperfusate were highest at 5 minute and after then declined. 2. At 5 minutes after reperfusion, the enzyme values in Group I were lower than those in other Groups. These results suggest that the cardioplegic solutions using for cardiac arrest and myocardial protection can be working better at 4oC than at any other temperature.

심혈관 확장제를 첨가한 심정지액의 심근보호에 미치는 효과 (An Experimental Study on the Effect of the Coronary Vasodilator-mixed Cardioplegic Solution on Myocardial Protection during Prolonged Aortic Cross-Clamping)

  • This study was experimentally undertaken to evaluate the effect of the coronary vasodilator-mixed cardioplegic solution on myocardial protection during prolonged aortic cross-clamping. The dogs were divided into two groups: control group A[received hypothermic cardioplegic solution without any additive coronary vasodilator], and comparing group 8[received hypothermic cardioplegic solution, mixed with various coronary vasodilators and Inderal]. Group A further was divided into two subgroups: subgroup A-1[ischemic time, 90 minutes], and subgroup A-2 [ischemic time, 240 minutes]. Group B further was divided into five subgroups: subgroup B-1 [received papaverine mixed hypothermic cardioplegic solution], subgroup B-2[received nitroglycerin mixed hypothermic cardioplegic solution], subgroup B-3 [received nitroprusside mixed hypothermic cardioplegic solution, subgroup B-4[received hydralazine mixed hypothermic cardioplegic solution], and subgroup B-5 [received inderal mixed hypothermic cardioplegic solution]. The specimens from all of the subgroups were studied by electron microscopic examination. The specimens of subgroups [B-l, B-2 8-3, and B-4], received coronary vasodilators mixed hypothermic cardioplegic solutions, were also compared by methylene blue induced staining of the myocardium and coronary vessels. The results obtained were as followings: l. On electron microscopic examination, all of the specimens, including subgroup A-2, showed no irreversible change of the myocardium. But the best result was obtained from the subgroup B-l, treated by papaverine mixed hypothermic cardioplegic solution. The subgroup B-2, treated by nitroglycerin, was next. And the subgroup B-5, treated by Inderal, was agreeable, comparing the electron microscopic finding with control group in the effect of myocardial protection. 2. The distribution in the myocardium of cardioplegic solution was demonstrated with the aid of methylene blue staining in the subgroups of B-l, B-2, B-3, and B-4, and they were the groups treated by papaverine, nitroglycerin, nitroprusside, and hydralazine in their grouping order. The best result was obtained from the subgroup B-1 [papaverine]. The subgroup B-2 [nitroglycerin] was next. The subgroup B-3 [nitroprusside] was moderate in finding of the colorization. The subgroup B-4 [hydralazine] was the poorest in the distribution of the cardioplegic solution in the myocardium. From these results, it appeared that myocardial protection during ischemic arrest for open heart surgery could be enhanced considerably when coronary dilatation was assured.

심근 정지 용액이 심근의 에너지원에 미치는 영향 (The Effects of Cardioplegic Solutions on the Energy Source of the Guinea Pig Heart)

  • 심근정지용액의 조성이 심근정지 및 심근정지 후 회복기의 에너지원에 미치는 영향을 조사하기 위해 guinea pig 심실 균질액에서 adenosine triphosphate (ATP), creatine phosphate (CP) 및 lactic acid (LA)의 변화를 관찰하여 다음과 같은 결과를 얻었다. 1) ATP 및 CP 양은 심근정지동안은 감소하고 회복기에는 다시 증가하였으며, 심근정지 후 정상용액으로 30분간 회복시켰을 때에는 ATP 양의 회복이 CP에 비해 느린것을 알 수 있었다. 2) LA 양은 심근정지동안은 증가하고 회복기에는 다시 감소하였다. 3) 심근정지 후 회복시간을 길게할수록 ATP 및 CP 양의 회복은 증가하였으며 CP 양이 더 많이 회복되었다. 4) LA 양도 회복시간을 길게할수록 감소하였다. 이상의 결과를 종합하여 볼 때, 심근정지용액의 $Ca^{2+}$$K^{+}$ 농도는 심근정지후 회복시 심근세포의 에너지원인 ATP, CP 및 lactic acid 양에 영향을 미친다고 생각된다. 이 연구 결과를 토대로 심근세포내 $Na^{+}$$H^{+}$ 활성도 변화 및 심근세포의 산증(acidosis)에 관한 연구를 첨가하면 심근정지시의 심근보호기전을 규명하는데 도움이 될 것으로 생각된다.

백서의 적출된 심장에서 심정지액의 산소화가 허혈성 심정지후 심기능 회복에 미치는 영향[I] (Effect of Oxygenation of Cardioplegic Solution on Electrical Stability and Postischemic Recovery of Cardiac Function after Ischemic Arrest in Isolated Rat Heart[ I ])

  • To evaluate the effect of oxygenation[95% O2+5% CO2] of St. Thomas Hospital No.2 cardioplegic solution[Plegisol], 20 isolated perfused rat hearts were studied under hyp-othermic[20oC] ischemic arrest for 2 hours with infusion of cardioplegic solution every 30 minutes throughout the ischemic period. Ten isolated hearts were studied with the oxygenated cardioplegic solution and 10 another isolated hearts with the nonoxygenated one. Mean oxygen tensions of the nonoxygenated and oxygenated cardioplegic solutions were 150mmHg and 470mmHg, respectively. Two in 10 hearts infused with the nonaxygenated cardioplegic solution were not recovered from nonworking heart due to persistent ventricular fibrillation. In comparing hem-odynamic parameters between both groups, the mean postischemic recovery[expressed as a percentage of its preischemic control value] was significantly greater with the oxygenated solution[in 10 recovered hearts] than the nonoxygenated solution[in 8 recovered hearts] [95.9$\pm$1.8% compared with 88.5$\pm$2.9% in peak aortic pressure, p<0.05, 75.7$\pm$5.2% compared with 43.5$\pm$6.5% in aortic flow, p<0.01, 75.5$\pm$4.6% compared with 54.1$\pm$5.6% in cardiac output, p<0.01, 78.3$\pm$4.6% compared with 60.3$\pm$4.6% compared with 60.3$\pm$6.2% in stroke volume, p<0.05, and 80.4$\pm$5.3% compared with 58.6$\pm$7.0% in dP/dT, p<0.05]. It is concluded that oxygenation of St. Thomas Hospital No.2 cardioplegic solution improves cardiac electrical stability and postischemic hemodynamic recovery after ischemic arrest in the isolated perfused rat heart.

한국산 잡견에서 허혈시 장시간심근보존을 위한 각 심정지액간 효과의 차이 (Comparison of Three Cardioplegic Solutions for Prolonged Cardiac Preservation During Ischemia in Korean Mongrel Dogs)

  • 한국산 잡견을 대상으로 세 종류의 심정지액으로 심장을 정지 보존 후,심장을 적출하여 $0^{\circ}C$에서 보존하면서 일정한 시간별로 6회에 걸쳐 좌심실 근육조직을 채취하여 -75$^{\circ}C$ 냉동고에 저장한 후,초고속 액체 크로마토그라피법으로 purine metabolites를 측정하였다. UW 용액(UWS)군의 ATP 농도는 St. Thomas 용액 (5751군과 혈성 심정 지액 (BCPS)군에 비하여 높으나, 575 군과 BCPS 군 간에는 특이한 차이가 없었다. UWS 군과 BCPS 군의 ADP 농도가 4,8, 12, 24 시간대의 575 군보다 높지만, UWS 군과 BCPS 군 사이에는 특이한 차이가 없었다. AMP 농도는 세군 모두에서 변화가 많지 않았고, adenosine, inoslne, hypoxanthlnc 농도는 시간 경과 에 따라 점차 증가하였고, xanthine의 농도는 매우 소량이어서 비교가 불가능하였다. 본 연구 결과, UW용액이 장시간의 심근보존능력에는 아직 문제점이 있으나, 전신 저체온법을 동시에 시행한 St. Thomas 용액이나 혈성 용액 보다는 보다 우수한 보존용액으로 사료된다.
