Endoscopic ultrasound-guided gastrojejunostomy with a direct technique without previous intestinal filling using a tubular fully covered self-expandable metallic stent

  • Background/Aims: Endoscopic ultrasonography-guided gastrojejunostomy is a minimally invasive method for the management of gastric outlet obstruction. Conventionally, a lumen-apposing metal stent (LAMS) is used to create an anastomosis. However, LAMS is expensive and not widely available. In this report, we described a tubular fully covered self-expandable metallic stent (T-FCSEMS) for this purpose. Methods: Twenty-one patients (15 men [71.4%]; median age, 66 years; range, 40-87 years) were included in this study. A total of 19 malignant (12 pancreatic, 6 gastric, and 1 metastatic rectal cancer) and 2 benign cases were observed. The proximal jejunum was punctured with a 19 G needle. The stomach and jejunum walls were dilated with a 6 F cystotome, and a 20×80 mm polytetrafluoroethylene T-FCSEMS (Hilzo) was deployed. Oral feeding was initiated after 12 to 18 hours and solid foods after 48 hours. Results: The median procedure time was 33 minutes (range, 23-55 minutes). After two weeks, 19 patients tolerated oral feeding. In patients with malignancy, the median survival time was 118 days (range, 41-194 days). No serious complications or deaths occurred. All patients with malignancy tolerated oral food intake until they expired. Conclusions: T-FCSEMS is safe and effective. This stent should be considered as an alternative to LAMS for gastric outlet obstruction.

Timing and predictors of death during treatment in patients with multidrug/rifampin-resistant tuberculosis in South Korea

  • Background/Aims: This study aimed to investigate the timing and predictors of death during treatment among patients with multidrug/rifampin-resistant tuberculosis (MDR/RR-TB) in South Korea. Methods: This was a retrospective cohort study that included MDR/RR-TB cases notified between 2011 and 2017 in South Korea. Results: Among 7,226 MDR/RR-TB cases, 699 (9.7%) died at a median of 167 days (IQR 51-358 d) from the initiation of MDR-TB treatment. The cumulative proportion of all-cause death was 35.5% at 90 days and 52.8% at 180 days from treatment initiation. TB-related deaths occurred at a median of 133 days (IQR 32-366 d), which was significantly earlier than the median of 184 days (IQR 68-356 d) for non-TB-related deaths (p = 0.002). In a multivariate analysis, older age was the factor most strongly associated with death, with those aged ≥ 75 years being 68 times more likely to die (aHR 68.11, 95% CI 21.75-213.26), compared those aged ≤ 24 years. In addition, male sex, comorbidities (cancer, human immunodeficiency virus, and end stage renal disease), the lowest household income class, and TB-specific factors (previous history of TB treatment, smear positivity, and fluoroquinolone resistance) were identified as independent predictors of all-cause death. Conclusions: This nationwide study highlights increased deaths during the intensive phase and identifies high-risk groups including older people and those with comorbidities or socioeconomic vulnerabilities. An integrated and comprehensive strategy is required to reduce mortality in patients with MDR/RR-TB, particularly focusing on the early stages of treatment and target populations.

Association between Elevated Plasma Homocysteine and Low Skeletal Muscle Mass in Asymptomatic Adults

  • Background Homocysteine has been drawing attention with a closed linkage with skeletal muscle. However, the association of hyperhomocysteinemia with decreased skeletal muscle mass remains unclear. We aimed to investigate the association of hyperhomocysteinemia with low skeletal muscle mass (LMM) in asymptomatic adults. Methods This was a cross-sectional study of 114,583 community-dwelling adults without cancer, stroke, or cardiovascular diseases who underwent measurements of plasma homocysteine and body composition analysis from 2012 to 2018. Hyperhomocysteinemia was defined as >15 µmol/L. Skeletal muscle mass index (SMI) was calculated based on appendicular muscle mass (kg)/height (m)2. Participants were classified into three groups based on SMI: "normal," "mildly low," and "severely low." Results The prevalence of hyperhomocysteinemia was the highest in subjects with severely LMM (12.9%), followed by those with mildly LMM (9.8%), and those with normal muscle mass (8.5%) (P for trend <0.001). In a multivariable logistic regression model, hyperhomocysteinemia was significantly associated with having a mildly LMM (odds ratio [OR], 1.305; 95% confidence interval [CI], 1.224 to 1.392) and severely LMM (OR, 1.958; 95% CI, 1.667 to 2.286), respectively. One unit increment of log-transformed homocysteine was associated with 1.360 and 2.169 times higher risk of having mildly LMM and severely LMM, respectively. Conclusion We demonstrated that elevated homocysteine has an independent association with LMM in asymptomatic adults, supporting that hyperhomocysteinemia itself can be a risk for decline in skeletal musculature.

사차원전산화단층촬영과 호흡연동 직각 Kilovolt 준비 영상을 이용한 간 종양의 움직임 분석 (Evaluation of the Positional Uncertainty of a Liver Tumor using 4-Dimensional Computed Tomography and Gated Orthogonal Kilovolt Setup Images)

  • 목 적: 4-dimensional computed tomography (4DCT) 영상과 on board imaging (OBI) 및 real time position management (RPM) 장치로 매 회 치료 시마다 얻은 호흡연동 직각 kilovolt (KV) 준비 영상(gated orthogonal kilovolt setup image)을 이용해 간암 환자를 치료하는 동안 발생하는 종양 위치의 불확실성을 평가하고자 했다. 대상 및 방법: 3차원입체조형치료가 예정된 20명의 간암 환자를 대상으로 RPM과 전산화단층촬영모의치료기를 이용해 치료계획용 4DCT를 시행했다. 표적 근처에 위치한 간동맥화학색전술 후 집적된 리피오돌(lipiodol) 혹은 횡격막을 종양의 위치 변이를 측정하는 표지자로 선택했다. 표지자의 위치 차이를 이용해 온라인 분할간 및 분할중 내부 장기 변이와 움직임 진폭을 측정했다. 측정된 자료의 정량적 평가를 위해 통계 분석을 실시했다. 결 과: 20명 환자로부터 측정된 표지자의 분할간변이의 중앙값은 X (transaxial), Y (superior-inferior), Z (anterior-posterior) 축에서 각각 0.00 cm (범위, -0.50~0.90 cm), 0.00 cm (범위, -2.4~1.60 cm), 0.00 cm (범위, -1.10~0.50 cm) 였다. 4명의 환자에서 X, Y, Z축 중 하나 이상에서 0.5 cm를 초과하는 변이가 관찰되었다. 4DCT와 호흡연동 직각 준비 영상으로부터 얻은 표적의 움직임 진폭의 차이는 X, Y, Z 축에서 각각 중앙값이 -0.05 cm (범위, -0.83~0.60 cm), -0.15 cm (범위, -2.58~1.18 cm), -0.02 cm (범위, -1.37~0.59 cm) 였다. 두 영상간 표적의 움직임 진폭 차이가 1 cm를 초과하는 환자가 Y축 방향으로 3명 관찰되었으며, 0.5 cm 초과 1 cm 미만의 차이를 보이는 환자도 Y축과 Z축 방향을 합쳐 5명 관찰되었다. 분할중 표지자 위치 변이의 중앙값은 X, Y, Z축에서 각각 0.00 cm (범위, -0.30~0.40 cm), -0.03 cm (범위, -1.14~0.50 cm), 0.05 cm (범위, -0.30~0.50 cm)였으며 2명의 환자에서 1 cm를 초과하는 변이가 Y축 방향으로 관찰되었다. 결 론: 4DCT와 호흡연동 직각 KV 준비 영상으로 얻은 표지자의 분할간, 분할중 및 움직임 진폭에서 큰 변이가 관찰되었다.

빅데이터환경에서 텍스트마이닝 기법을 활용한 한국의 석면 트렌드 (1918년~2027년) (Asbestos Trend in Korea from 1918 to 2027 Using Text Mining Techniques in a Big Data Environment)

  • 석면은 악성중피종과 폐암 등 치명적인 질병을 유발하기 때문에 국내에서 2009년부터 그 사용이 전반적으로 금지되었다. 그러나 국내에서 지난 수십년 간 석면이 생산 및 수입되어 다양한 산업에서 사용되어 왔기 때문에 우리 주변에는 여전히 석면함유물질이 많이 존재하고 있어 안전한 관리가 절실하다. 이 연구는 지난 32년(1991년 ~ 2022년)동안의 빅데이터를 바탕으로 석면관련 주요키워드를 이용하여 석면관련 트렌드 변화를 살펴보고자 한다. 또한 국내 과거(1990년 이전)의 석면의 생산, 수입, 사용 실태와 더불어 현재(2023년~2027년)의 석면관련 정책 동향을 살펴보고자 하였다. 1991~2000년에는 국내 석면의 생산과 수입 그리고 이용으로 인하여 석면에 대한 발암성이 부각되는 시기로 연구, 근로자, 발암물질, 환경등과 관련된 키워드가 주를 이루었다. 2001~2010년에는 석면의 발암성과 관련하여 미국, 일본 등에서 소송이 시작되었던 시기로 폐암, 소송, 발암물질, 노출, 기업 등의 키워드가 주를 이루었다. 2011~2020년에는 국내에서도 석면문제의 심각성을 인지하는 키워드인 발암물질, 야구장, 학교, 슬레이트, 건축물, 폐석면광산 등이 지속적으로 상위권 키워드로 검색되었다. 2021년부터는 주로 검색되는 키워드는 학교, 슬레이트, 건축물 등과 조경석, 환경영향평가, 아파트, 시멘트 등의 키워드가 나타났다.

호흡동조 방사선치료에 사용되고 있는 RPM (Real-time Position Management) Respiratory Gating System의 호흡변화에 따른 정확성에 대한 고찰 (Consideration of the Accuracy by Variation of Respiration in Real-time Position Management Respiratory Gating System)

  • 목 적: 본원에서 시행하고 있는 호흡동조 방사선치료(Respiratory Gated Radiation Therapy, RGRT)는 RPM (Real-time Position Management) Respiratory Gating System (version 1.7.5, Varian, USA)을 이용하여 실시하고 있다. 본 연구는 호흡변화에 따른 방사선치료의 정확성을 분석하고 평가하고자 한다. 대상 및 방법: 움직임을 임의로 조정할 수 있는 구동 팬텀(Motion Phantom)인 QUASAR Programmable Respiratory Motion Phantom (Moudus Medical Device Inc. CANADA)을 이용하였다. 폐암과 간암 환자 50명의 호흡을 분석하여 호흡의 변화를 관찰한 결과를 기준으로 주기 3초, 진폭 1.5 cm인 총 60초간의 기준호흡에서 매번 기저호흡(Baseline)을 기울기를 갖고 점진적으로 내려가는 경우와, 간헐적으로 기준 기저호흡 보다 더 내려가는 경우의 호흡 모양을 만들어 본원에서 사용되고 있는 Phase gating, 보통 30~70% gating 치료 방법과 동일하게 방사선을 조사하였다. 결 과: 점진적으로 매번 기저호흡이 0.01 cm, 0.03 cm, 0.05 cm씩 내려가는 호흡의 경우 모든 조건에서 방사선이 조사되었다. 간헐적으로 기저호흡이 0.2 cm, 0.4 cm, 0.6 cm, 0.8 cm 내려가는 경우도 모든 조건에서 방사선이 조사되어 RPM Respiratory Gating System에서 Phase gating 방식은 기저호흡 변화에 대하여 방사선이 모두 조사되었다. 결 론: 최적의 방사선치료를 위해 종양의 움직임을 고려하여 시행하는 호흡동조 방사선치료에서 RPM Respiratory Gating System의 Phase gating 방식은 본 연구에서와 같이 기저호흡의 변화가 호흡동조에 정확히 반영되지 않음을 알 수 있었다. 이에 호흡동조 방사선치료에는 무엇보다 환자의 호흡 관찰이 중요하다고 사료되며 호흡의 변화가 관찰되었다면 즉시 방사선치료를 멈추고 호흡을 관찰한 후 변화된 호흡상태에서 투시를 시행하여 치료부위를 다시 확인한 후에 치료를 재개하여야 한다고 사료된다.

  • Incidental thyroid nodules are commonly detected on ultrasonography (US). This has contributed to the rapidly rising incidence of low-risk papillary thyroid carcinoma over the last 20 years. The appropriate diagnosis and management of these patients is based on the risk factors related to the patients as well as the thyroid nodules. The Korean Society of Thyroid Radiology (KSThR) published consensus recommendations for US-based management of thyroid nodules in 2011 and revised them in 2016. These guidelines have been used as the standard guidelines in Korea. However, recent advances in the diagnosis and management of thyroid nodules have necessitated the revision of the original recommendations. The task force of the KSThR has revised the Korean Thyroid Imaging Reporting and Data System and recommendations for US lexicon, biopsy criteria, US criteria of extrathyroidal extension, optimal thyroid computed tomography protocol, and US follow-up of thyroid nodules before and after biopsy. The biopsy criteria were revised to reduce unnecessary biopsies for benign nodules while maintaining an appropriate sensitivity for the detection of malignant tumors in small (1-2 cm) thyroid nodules. The goal of these recommendations is to provide the optimal scientific evidence and expert opinion consensus regarding US-based diagnosis and management of thyroid nodules.

대학병원 내 외래간호사의 포름알데히드 노출 평가 (Exposure to Formaldehyde of Ambulatory Care Nurses in University Hospital)

  • Objective: This study aimed to evaluate the exposure status of formaldehyde(FA) among the nurses in ambulatory care departments of university hospital. Methods: Two university hospitals were surveyed. The FA concentration in air were measured to target 62 nurses in 8 ambulatory care departments(89 samples). Air sampling and analysis of FA were carried out according to the OSHA Method ID-205. The survey was conducted with questionnaire asking about how to control FA and whether the nurses use the formaldehyde protectors or not. Results: FA was detected in all samples. The maximum concentration of FA was 0.258 ppm and the geometric mean was 0.023 ppm. There was no sample that exceeded any of exposure standards by OSHA-TWA whereas there were 54 samples(60.7%) that exceeded the standards by NOISH-TWA. Among 62 nurses handling FA, 13 nurses(21.0%) used the protective gloves while nobody used a gas mask. It was assessed that any of 8 common ambulatory care departments did not use a safety cabinet for FA in which local exhaust ventilation was fixed. Conclusions: Nurses in ambulatory care departments were exposed to FA. Therefore, the environment management of a workplace, the health management of a nurses, FA handling education and installing a FA cabinet with a local exhaust ventilation were needed because FA as a carcinogen was able to cause any cancer to a human body if it was emitted in air for long time.

  • 전통적으로 인류를 위협해 오던 감염성 질환이 줄어드는 반면, 심뇌혈관 질환을 포함하여 암, 당뇨와 같은 만성질환이 빠른 속도로 퍼져나가고 있다. 오랜 시간에 걸쳐 발병하며 증세가 장기간 지속되지만 치료가 어려운 만성질환의 특성을 반영, 그 위험요인의 관리와 이에 대응하는 건강증진정책의 마련은 세계 각국의 중요 관심사로 부상하게 되었다. 일차 의료 시스템이 점점 더 복잡한 형태로 변화하는 현대사회의 건강요구에 부합, 건강증진 및 질병 예방 정책, 지역사회에 대응 솔루션 등 이른바 환자 중심의 다 부문 접근방식을 제공하면서 변화된 의료 환경에서 일차 의료의 가치는 다시 한 번 부각 되고 있다. 기존의 의료전달체계로는 급속한 고령화 및 질병 패러다임의 변화, 삶의 질 향상을 향한 민중의 욕구를 감당하기 어려워지면서 유럽연합국가들은 지속적이며 포괄적이고 조정된 관리를 제공하는 일차 의료 시스템을 구축, 그 품질 향상을 위하여 다각적인 노력을 기울이고 있다. 본 논문은 우리나라 보건의료 전달체계의 현황 및 문제점 등을 검토하고, 일차 의료 강화와 관련된 유럽연합의 예를 중심으로 그 시사점을 도출, 환자 중심의 미래형 의료전달체계 구축 방안에 관하여 논의하고자 한다.

  • Toxicogenomics is now emerging as one of the most important genomics application because the toxicity test based on gene expression profiles is expected more precise and efficient than current histopathological approach in pre-clinical phase. One of the challenging points in Toxicogenomics is the construction of intelligent database management system which can deal with very heterogeneous and complex data from many different experimental and information sources. Here we present a new Toxicogenomics database developed as a part of 'Toxicogenomics for Efficient Safety Test (TEST) project'. The TEST database is especially focused on the connectivity of heterogeneous data and intelligent query system which enables users to get inspiration from the complex data sets. The database deals with four kinds of information; compound information, histopathological information, gene expression information, and annotation information. Currently, TEST database has Toxicogenomics information fer 12 molecules with 4 efficacy classes; anti cancer, antibiotic, hypotension, and gastric ulcer. Users can easily access all kinds of detailed information about there compounds and simultaneously, users can also check the confidence of retrieved information by browsing the quality of experimental data and toxicity grade of gene generated from our toxicology annotation system. Intelligent query system is designed for multiple comparisons of experimental data because the comparison of experimental data according to histopathological toxicity, compounds, efficacy, and individual variation is crucial to find common genetic characteristics .Our presented system can be a good information source for the study of toxicology mechanism in the genome-wide level and also can be utilized fur the design of toxicity test chip.
