  • 데이터 마이닝은 특정분야에서만 관심을 갖는 분야가 아니라 현재 우리주변 여러 분야에서 많이 사용되고 응용되고 있다. 수많은 데이터 가운데 숨겨져 있는 유용한 상관관계를 발견하여, 미래에 실행 가능한 정보를 예측하여 추출해 내고 추후에 의사 결정에 이용하는 과정을 말한다. 따라서 데이터를 다양한 관점으로 해석하기 위해 데이터를 변환할 수 있다. 의료분야에서의 예를 들면 간단한 질환도 분석의 결과에서 큰 차이를 발견할 수 있다. 유방암에 관련된 속성들에 대해 베이즈 이론을 적용하여 유방암 발병 확률을 예측한다. 이를 통하여 과거 환자진찰 데이터로 얻은 자료를 적용하여 증거기반의 의료서비스를 제공하며, 또한 진찰결과에 대한 신빙성을 증가시킬 수 있다.

The Preventive Effects of Colon Cancer and Imflammatory Bowel Disease of Supercritical Heat-Treated Radish Extracts

  • There is a strong connection between the diet rich in antioxidants and the decreased incidence of inflammatory bowel disease and cancerous diseases. Diets that are rich in anti-oxidants particularly include fruits and vegetables containing the high amounts of vitamin A-E, carotenoids, and minerals. The supercritical heat-treated radish extracts of the research result had an inhibitory effect on the development of aberrant crypt foci (ACF), namely, preneoplastic lesions having a potential to become cancer cells and reduced the number of the aberrant crypt foci (ACF) consisting of four or more aberrant crypts (AC) having high risk to become tumors by about half. The supercritical heat-treated radish extracts can reduce the incidence of preneoplastic lesions having a high risk of developing cancer by about 28 %. DSS-treated mice developed symptoms similar to those of human UC, such as severe bloody diarrhea and weight loss. Supercritical heat-treated radish extracts, as well as sulfasalazine, suppressed colonic length and mucosal inflammatory infiltration. In addition, supercritical heat-treated radish extracts treatment significantly reduced the expression of pro-inflammatory signaling molecules through suppression both mitogen-activated protein kinases (MAPK) and nuclear factor-kappa B (NF-kB) signaling pathways, and prevented the apoptosis of colon. Moreover, supercritical heat-treated radish extracts administration significantly led to the up-regulation of anti-oxidant enzyme including SOD and Catalase.

언론 건강 위험 보도의 환경 감시 기능과 게이트키핑 (News Media's Surveillance and Gatekeeping in Representing Health Risk)

  • Objectives: This study investigates whether Korean news media pay more attention to emerging diseases than chronic ones, and whether they closely follow the changes in the magnitude of health risks of chronic or well-known diseases. These two features are expected to appear as the result of surveillance function served by health journalism that should be the main source of the public's risk perception. Methods: The number of stories published in 10 newspapers containing the words, 'SARS,' 'Bovine Spongiform Encephalopathy,' 'Avian Influenza,' and 'Influenza A virus' was compared with the number of stories on chronic or wellknown diseases. We also counted the annual number of stories, published in a 12-year period, containing following terms: 'cancer,' 'diabetes,' 'hypertension,' 'pneumonia,' and 'tuberculosis.' The number was compared with the actual mortality of each disease. Results: Although cancer represented the primary cause of mortality, the newspapers covered key emerging diseases more than cancer or other well-known diseases. Also, media coverage of 'pneumonia' and 'tuberculosis' did not vary in accordance with changes in the mortality of each disease. However, the news media coverage did vary in accordance with the mortality of 'cancer,' 'diabetes,' and 'hypertension.' Conclusions: Korean health journalism was found to have both strong and weak points. The news media reduced the relative level of attention given to pneumonia and tuberculosis. Bearing in mind the major influence of news coverage on risk perception, health professionals need to be more proactive about helping to improve Korean health journalism.

Art Therapy in Patients with Terminal Cancer and Their Families: A Multiple Case Study

  • Purpose: The study explored the meaning of experiences within a family art therapy process among terminal cancer patients and their families. Methods: Ten participants, including four terminal cancer patients currently admitted to the hospice ward at an inpatient hospice facility in S City and four caregiving family members, engaged in four cycles of family art therapy sessions. The sessions were conducted weekly or bi-weekly, and each lasted approximately 50 minutes. Results: Nine cross-case themes emerged: "feeling unfamiliar and intimidated by the idea of expressing my thoughts through art," "trying to accept the present and positively overcome sadness," "expressing hope through emotional bonds during the process of parting," "conveying and preserving personal and family beliefs," "feeling upset about family imbalances caused by deteriorating health," "valuing togetherness and striving for stability amidst the current challenges," "art as a medium of empowerment for patients and facilitator of family conversations, even amidst difficulties," "sharing a range of emotions-not just joy, but concerns and sorrow-through art," and "gratitude for art' s role in improving family communication and connection through artwork. Conclusion: The findings of this study lead to several conclusions. First, patients and their families faced psychological challenges when confronted with impending death, yet they strove to remain optimistic by seeking meaning in their struggles. Second, families practiced open and expressive communication, sharing a spectrum of complex emotions with one another. Third, even as the patient's condition worsened, resulting in family fatigue, their support and cohesion strengthened.

Practical Use of Cancer Control Promoters in Municipalities in Japan

  • The Cancer Control Act in Japan became effective in 2006. In Ibaraki, Toyama, and Hyogo prefectures, the Cancer Control Promoter (CCP) plan was created to strengthen partnerships for cancer prevention. This study aimed to examine the curre nt status of CCP utilization and analyze relationships with intersectoral collaboration, both within the government and with outside partners. In 2008, we mailed questionnaires to 100 administrators responsible for disease prevention and health promotion in municipal governments of the three prefectures. Ninety-one administrators responded (response rate, 91.0%). We analyzed responses to questions regarding whether or not the municipalities had used CCPs. Items assessing intersectoral collaboration examined municipality characteristics and relationships with outside partners and sectors specializing in areas other than community health. Among 90 administrators with valid data, 33 municipalities (36.7%) used CCPs while 57 (63.3%) did not. The Fisher's exact test revealed that intersectoral collaboration for using CCPs was associated with communication with all of the municipal government sectors not related to health. The present study indicated that CCPs were not consistently used in municipalities. However, we found that intersectoral collaborations, especially within the local government, may be related to the practical use of CCPs. This, in turn, may result in effective cancer control and prevention, as well as improvement in community health.

  • For the purpose of designing more successful cancer research, the strategy for cancer study in the field of Traditional Korean Medicine(TKM) during the 21th century was examined from the analysis of trends on cancer study in traditional korean medicine. The results were summarized as follows: 1. So far cancer research in TKM was chiefly done on cytotoxicity, side-effects by chemotherapy, tumor immunology, apoptosis, survival time with S-180 and pulmonary colonization assay and also clonogenic assay, cell adhesion assay, angiogenesis, cell-differentiation and side-effect by radiotherapy were partly performed. 2. It may be ideal that we should study synergistic effect between constituent drugs of prescriptions, tumor immunology, combined therapy between western and oriental medicines by reducing side-effect by radiotherapy and chemotherapy and antimetastasis according to the characteristics of oriental medicine chiefly and also supplement the studies on molecular biology, gene therapy, angiogensis and signal transduction. 3. We had better do specific-field research in cooperation between oriental medical colleges and Korea Institute of Oriental Medicine(KIOM) as well as study a target cancers such as hepatic cancer, pulmonary cancer and gastric cancer more intensively than all cancers domestically. 4. Our country must keep communication with China having many clinical data, Taiwan chiefly doing the combined tharapy between oriental and western medicines, Japan having done basic study actively on cancer.

Cancer Registration in India - Current Scenario and Future Perspectives

  • Cancer registration, an important component of cancer surveillance, is essential to a unified, scientific and public health approach to cancer prevention and control. India has one of the highest cancer incidence and mortality rates in the world. A good surveillance system in the form of cancer registries is important for planning and evaluating cancer-control activities. Cancer registration in India was initiated in 1964 and expanded since 1982, through initiation of the National Cancer Registry Program (NCRP) by the Indian Council of Medical Research. NCRP currently has twenty-six population based registries and seven hospital based registries. Yet, Indian cancer registries, mostly in urban areas, cover less than 15% of the population. Other potential concerns about some Indian registries include accuracy and detail of information on cancer diagnosis, and timeliness in updating the registry databases. It is also important that necessary data collection related quality assurance measures be undertaken rigorously by the registries to ensure reliable and valid information availability. This paper reviews the current status of cancer registration in India and discusses some of the important pitfalls and issues related to cancer registration. Cancer registration in India should be complemented with a nationwide effort to foster systematic investigations of cancer patterns and trends by states, regions and sub populations and allow a continuous cycle of measurement, communication and action.

Knowledge, Attitudes and Practices Regarding Cervical Cancer Screening Among Village Health Volunteers

  • Background: In the years 2014, coverage rates of cervical cancer screening in Nakornnayok province accounted to 76.5%. This was lower than the government's specified goal of 80%. Community health volunteers are members of a Thai healthcare alliance established to help promoting healthcare service communication and collaboration at the primary level. Such village health volunteers (VHVs) are established in most villages. Objective: To assess the knowledge and attitudes of cervical cancer screening among VHVs. Materials and Methods: The subjects were 128 VHVs from four Nakornnayok sub-districts; namely KlongYai, Chomphol, Buangsan and Suksara, Thailand. The study was conducted from December 2014 to January 2015. The questionnaire was designed to assess the knowledge and attitude of cervical cancer screening provided by the VHVs. In addition, cervical cancer screening coverage rates of each area were collected. The demographic data, scores of knowledge, attitudes, practices and the cervical cancer screening coverage rates were analyzed by one-way ANOVA. Results: The questionnaire reliability was assessed as 0.81. The total knowledge and attitude scores were 10 and 15 points. The mean knowledge scores of KlongYai, Chomphol, Buangsan and Suksara were 6.8, 7.0, 6.5 and 9.0 points, respectively. The VHVs had a high level of overall knowledge about cervical cancer screening. The mean attitude scores were 12.4, 13.2, 13.4 and 13.1 points. VHVs had a positive attitude to the promotion of cervical cancer screening at the overall level. The percentages of VHVs promoting cervical cancer information in respective districts were 72.2, 94.3, 94.9 and 50.0. However, the cervical cancer screening coverage rates were 62.4%, 34.7%, 80.3% and 47.3% respectively. Conclusions: The knowledge, attitudes and percentages of promoting information of cervical cancer screening among VHVs in the four sub-districts were high but did not correlate with the cervical screening coverage rates for each area. VHVs needed to understand socio-cultural beliefs of the women in the target population and design suitable strategies to encourage higher cervical screening coverage.

컴퓨터 텍스트 분석프로그램을 적용한 암환자의 투병수기 분석 (An Analysis of Cancer Survival Narratives Using Computerized Text Analysis Program)

  • Purpose: This study was done to explore experiences of persons living through the periods of cancer diagnosis, treatment, and self-care. Methods: With permission, texts of 29 cancer survival narratives (8 men and 21 women, winners in contests sponsored by two institutes), were analyzed using Kang's Korean-Computerized-Text-Analysis-Program where the commonly used Korean-Morphological-Analyzer and the 21st-century-Sejong-Modern-Korean-Corpora representing laymen's Korean-language-use are connected. Experiences were explored based on words included in 100 highly-used-morphemes. For interpretation, we used 'categorizing words by meaning', 'comparing use-rate by periods and to the 21st-century-Sejong-Modern-Korean-Corpora', and highly-used-morphemes that appeared only in a specific period. Results: The most highly-used-word-morpheme was first-person-pronouns followed by, diagnosis treatment-related- words, mind-expression-words, cancer, persons-in-meaningful-interaction, living and eating, information-related-verbs, emotion-expression- words, with 240 to 0.8 times for layman use-rate. 'Diagnosis-process', 'cancer-thought', 'things-to-come-after-diagnosis', 'physician husband', 'result-related-information', 'meaningful-things before diagnosis-period', and 'locus-of-cause' dominated the life of the diagnosis-period. 'Treatment', 'unreliable-body', 'husband people mother physician', 'treatment-related-uncertainty', 'hard-time', and 'waiting-time represented experiences in the treatment-period. Themes of living in the self-care-period were complex and included 'living-as-a-human', 'self-managing-of-diseased-body', 'positive-emotion', and 'connecting past present future'. Conclusion: The results show that the experience of living for persons with cancer is influenced by each period's own situational-characteristics. Experiences of the diagnosis and treatment-period are negative disease-oriented while that of the self-care period is positive present-oriented.

민간의료보험이 암 환자의 의료이용과 의료비에 미치는 영향: 일개 암전문의료기관의 우리나라 주요 암종을 중심으로 (Effects of Private Health Insurance on Health Care Utilization and Expenditures in Korean Cancer Patients: Focused on 5 Major Cancers in One Cancer Center)

  • Objectives : To identify the effects of supplemental private health insurance on health care utilization and expenditure under the mandatory National Health Insurance(NHI) system in Korea. Methods : The data were collected by the National Cancer Center in Korea. Cancer patients who were newly diagnosed with stomach (ICD code, C16), lung(C33-C34), liver (C22), colorectal cancer(C18-C20) or breast(C50) cancer were included as study subjects. Data were gathered using a structured questionnaire from face-to-face interviews, the hospital Order Communication System (OCS) and medical records. Clinical, socio-demographic and private health insurance related factors were also gathered. The differences of health care utilization and expenditure were compared between those who have private health insurance and those who do not using t-test and multivariable regression analysis. Results : Individuals with private health insurance spent larger inpatient costs than those without, but no differences were found in utilization in other service such as hospital admissions, hospital days and physician visits. Conclusions : We found that private health insurance exerts a significant effect on the health care expenditure in inpatient service. These study results can provide a rational basis to plan a national health policy regarding private health insurance. Further studies are needed to investigate the impacts of private health insurance on cancer patients' outcomes and survival rates.
