Community Care for Cancer Patients in Rural Areas: An Integrated Regional Cancer Center and Public Health Center Partnership Model

  • Purpose: The accessibility of medical facilities for cancer patients affects both their comfort and survival. Patients in rural areas have a higher socioeconomic burden and are more vulnerable to emergency situations than urban dwellers. This study examined the feasibility and effectiveness of a cancer care model integrating a regional cancer center (RCC) and public health center (PHC). Methods: This study analyzed the construction of a safety care network for cancer patients that integrated an RCC and PHC. Two public health institutions (an RCC in Gyeongnam and a PHC in Geochang County) collaborated on the development of the community care model. The study lasted 13 months beginning in February 2019 to February 2020. Results: The RCC developed the protocol for evaluating and measuring 27 cancer-related symptoms, conducted education for PHC nurses, and administered case counseling. The staff at the PHC registered, evaluated, and routinely monitored patients through home visits. A smartphone application and regular video conferences were incorporated to facilitate mutual communication. In total, 177 patients (mean age: 70.9 years; men: 59%) were enrolled from February 2019 to February 2020. Patients' greatest unmet need was the presence of a nearby cancer treatment hospital (83%). In total, 28 (33%) and 44 (52%) participants answered that the care model was very helpful or helpful, respectively. Conclusion: We confirmed that a combined RCC-PHC program for cancer patients in rural areas is feasible and can bring satisfaction to patients as a safety care network. This program could mitigate health inequalities caused by accessibility issues.

  • 최근 의료분야에서는 대규모의 데이터를 빠르게 검색 및 추출이 가능하게 의사결정트리 기법에 대한 연구들이 진행되고 있다. 현재 CART, C4.5, CHAID 등 여러 기법이 개발되었는데, 이러한 클레시파이 기법들은 몇몇 의사결정 나무 알고리즘이 이진분리로 분류를 하는데, 나머지 데이터의 결과가 손실될 우려가 있다. 그중 C4.5는 엔트로피의 측정값에 높고 낮음으로 트리 모양을 구성해 가는 방식이고, CART 알고리즘은 엔트로피 매트릭스를 사용하여 범주형 자료나 연속형 자료에 적용할수가 있다. 이에 본 논문에서는 클래시파이 기법 중 C4.5와 CART를 유방암 환자 데이터에 대해 적용하여 실험하여, 그 결과 분석을 통한 성능 평가를 수행하였다. 실험에서는 교차검증을 통해 그 결과에 대한 정확성을 측정하였다.

Growth-Suppressing Activity of the Transfected Cx26 on BICR-M1Rk Breast Cancer Cell Line

  • There are accumulating evidences suggesting that connexin (Cx), a gap junction channel-forming protein, acts as a growth suppressor in various cancer cells, and this effect is attributeed to the gap junction-mediated intercellular communication (GJIC). In order to characterize the relationship between the growth-arresting activity of Cx26 and its cytoplasmic localizations after expression, we linked a nuclear export signal (NES) sequence to Cx26 cDNA before transfecting into a rat breast cancer cell line. A confocal fluorescent microscopic observation revealed that the insertion of NES minimized the nuclear expression of Cx26, and increased its cytoplasmic expression, including plasma membrane junctions. Total cell counting and BrdUrd-labeling experiments showed that the growth of the breast cancer cells was inhibited by 74% upon transfection of Cx26-NES, whereas only 9% inhibition was observed with only Cx26 cDNA.

Discrimination of Cancer Cell by Fuzzy Logic in Medical Images

  • A new method of digital image analysis technique for medical images of cancer cell is presented. This paper deals with the cancer cell discrimination. The object images were the Thyroid Gland cell images that were diagnosed as normal and abnormal. This paper proposes a new discrimination method based on fuzzy logic algorithm. The focus of this paper is an automatic discrimination of cells into normal and abnormal of medical images by dominant feature parameters method with fuzzy algorithm. As a consequence of using fuzzy logic algorithm, the nucleus were successfully diagnosed as normal and abnormal. As for the experimental result, average recognition rate of 64.66% was obtained by applying single parameter of 16 feature parameters at a time. The discrimination rate of 93.08% was obtained by proposed method.

  • Artificial intelligence application in digital health care has been increasing with its development of artificial intelligence. The convergence of the healthcare industry and information and communication technology makes the diagnosis of diseases more simple and comprehensible. From the perspective of medical services, its practice as an initial test and a reference indicator may become widely applicable. Therefore, analyzing the factors that are the basis for existing diagnosis protocols also helps suggest directions using artificial intelligence beyond previous regression and statistical analyses. This paper conducts essential diagnostic prediction learning based on the analysis of blood cancer factors reported previously. Blood cancer diagnosis predictions based on artificial intelligence contribute to successfully achieve more than 90% accuracy and validation of blood cancer factors as an alternative auxiliary approach.

  • Endoscopic submucosal dissection (ESD) is a minimally invasive and effective treatment for early gastric cancer. The criteria for determining the suitability and success of ESD have expanded, with changes in the histopathological classification of gastric carcinoma, including results from molecular research. Treatment methods have also diversified to include targeted therapies and immuno-oncology agents. To improve communication between clinicians and pathologists, it is crucial to understand the standardized diagnostic forms of gastric cancer. This study aims to examine the handling method of ESD specimens and describe the pathological findings of gastric tumors.

  • In the new millennium people are facing serious challenges in health care, especially with increasing non-communicable diseases (NCD). One of the most common NCDs is cancer which is the leading cause of death in developed countries and in developing countries is the second cause of death after heart diseases. Cancer registry can make possible the analysis, comparison and development of national and international cancer strategies and planning. Information technology has a vital role in quality improvement and facility of cancer registries. With the use of IT, in addition to gaining general benefits such as monitoring rates of cancer incidence and identifying planning priorities we can also gain specific advantages such as collecting information for a lifetime, creating tele medical records, possibility of access to information by patient, patient empowerment, and decreasing medical errors. In spite of the powerful role of IT, we confront various challenges such as general problems, like privacy of the patient, and specific problems, including possibility of violating patients rights through misrepresentation, omission of human relationships, and decrease in face to face communication between doctors and patients. By implementing appropriate strategies, such as identifying authentication levels, controlling approaches, coding data, and considering technical and content standards, we can optimize the use of IT. The aim of this paper is to emphasize the need for identifying positive and negative effects of modern IT on cancer registry in general and specific aspects as an approach to cancer care management.

Comparative Analysis of the Perception of Family Functioning by Heads of Families with and without Cancer Members During Illness

  • Background: Cancer is a major health problem due to the aging population with increasing deaths. Family functioning is affected by cancer diagnosis and treatment. The aim of this study was to comparative analysis of the perception of family functioning by heads of families with and without cancer members during illness, focusing on changes or probable changes. Materials and Methods: This comparative study was conducted on two groups (families with a member of the cancer and controls without a family member with cancer). The families were of patients referred to the clinics and hospitals of Imam Khomeini, Taleghani and Omid of Urmia city, the number of samples being 148 for cases and 176for the control group. To collect the data, valid and reliable family functioning (FAD) was applied, a 60-item questionnaire with seven dimensions, with heads of families. To analyze the data SPSS- 23 Software was used for descriptive and analytical statistics. Significance level was defined p <0.05. Results: Among the seven items : problem solving, communication, roles, emotional response, emotional involvement, behavior control and overall functioning, only differences for average scores of problem-solving were statistically significant. Discussion: Contrary to common perception of severe damage for family functioning in families with cancer members, results of this study indicate that functioning in terms of family caregivers is more or less similar to that of the families with other diseases. Only in problem-solving item do these families experience more difficulty. Conclusion: According to the research findings, in nursing from families with cancer patient, it is recommended to focus more on the problem-solving item of the families.

Additive Properties of Crude, Age Specific and Age Adjusted Rates for Cancer Incidence and Mortality

  • Background: In National Cancer Registry Programme (NCRP) reports, various rates are routinely provided for 50 cancer sites of males and 54 cancer sites of females. Very often, depending on our interest, we wish to see these rates for group of cancers like head and neck cancers, oral cancers, and reproductive cancers. In such a situation, the desired rates are calculated independently from the actual data and reported. The question is can we derive the rates for groups of cancers from the published reports when the data is provided only for the individual sites? Objective: In the present paper, an attempt is made to explore the mathematical properties of various rates to derive them directly for the group of cancer sites from the published data when the rates are provided only for the individual sites. Source of data: The cancer incidence data collected by two urban Population Based Cancer Registries (PBCRs), under the network of NCRP for the period of 2006-08 was considered for the study purposes. The Registries included were: Bangalore and Bhopal. Results: In the present communication, we have shown that the crude rate (CR), age specific rates and age-adjuste rates (AAR) all possess additive properties. This means, given the above rates for individual sites, the above rates can be calculated for groups of sites by simply adding them. In terms of formula it can be stated that CR(Site1+Site2+++ SiteN) = CR(Site1)+CR(Site2) +++ CR(SiteN). This formula holds good for age specific rates as well as for AAR. This property facilitates the calculation of various rates for defined groups of cancers by simply adding the above rates for individual sites from which they are made up.

Event-Based Middleware for Healthcare Applications

  • In existing middleware for body sensor networks, energy limitations, hardware heterogeneity, increases in node temperature, and the absence of software reusability are major problems. In this paper, we propose an event-based grid middleware component that solves these problems using distributed resources in in vivo sensor nodes. In our multi-hop communication, we use a lightweight rendezvous routing algorithm in a publish/subscribe system of event-based communication. To facilitate software reuse and application development, a modified open services gateway initiative has been implemented in our middleware architecture. We evaluated our grid middleware in a cancer treatment scenario with combined hyperthermia, chemotherapy, and radiotherapy procedures, using in vivo sensors.
