Development of Enzymatic Recombinase Amplification Assays for the Rapid Visual Detection of HPV16/18

  • Human papillomavirus (HPV) types 16 and 18 are the major causes of cervical lesions and are associated with 71% of cervical cancer cases globally. However, public health infrastructures to support cervical cancer screening may be unavailable to women in low-resource areas. Therefore, sensitive, convenient, and cost-efficient diagnostic methods are required for the detection of HPV16/18. Here, we designed two novel methods, real-time ERA and ERA-LFD, based on enzymatic recombinase amplification (ERA) for quick point-of-care identification of the HPV E6/E7 genes. The entire detection process could be completed within 25 min at a constant low temperature (35-43℃), and the results of the combined methods could be present as the amplification curves or the bands presented on dipsticks and directly interpreted with the naked eye. The ERA assays evaluated using standard plasmids carrying the E6/E7 genes and clinical samples exhibited excellent specificity, as no cross-reaction with other common HPV types was observed. The detection limits of our ERA assays were 100 and 101 copies/µl for HPV16 and 18 respectively, which were comparable to those of the real-time PCR assay. Assessment of the clinical performance of the ERA assays using 114 cervical tissue samples demonstrated that they are highly consistent with real-time PCR, the gold standard for HPV detection. This study demonstrated that ERA-based assays possess excellent sensitivity, specificity, and repeatability for HPV16 and HPV18 detection with great potential to become robust diagnostic tools in local hospitals and field studies.

Combined Screening of Cervical Cancer, Breast Cancer and Reproductive Tract Infections in Rural China

  • Objectives: To investigate the current prevalence and knowledge of cervical cancer, breast cancer and reproductive tract infections (RTIs) in rural Chinese women, and to explore the acceptance and feasibility of implementing a combined screening program in rural China. Methods: A population-based, cross-sectional study was conducted among women aged 30 to 59 years old in Xiangyuan County, Shanxi Province from 2009 to 2010. Socio-demographic characteristics, knowledge of cervical cancer, breast cancer and RTIs, and the attitude toward single or combined screening were collected by an interview questionnaire. Each participant received a clinical examination of the cervix, breast and reproductive tract. Examinations included visual inspection, mammography, laboratory tests and pathological diagnosis. Results: A total of 1,530 women were enrolled in this study. The prevalence of cervical precancerous lesions, suspicious breast cancer, suspicious benign breast disease and RTIs was 1.4%, 0.2%, 14.0% and 54.3%, respectively. Cervicitis, trichomonas vaginitis, and bacterial vaginitis were the three most common RTIs among our participants. Television, radio broadcast, and public education during screening were the major source of healthcare knowledge in rural China. Moreover 99.7% of women expressed great interest in participating in a combined screening project. The affordable limit for combined screening project was only 50 RMB for more than half of the rural women. Conclusion: A combined screening program would be more effective and popular than single disease screening projects, while appropriate accompanied education and a co-pay model for its successful implementation need to be explored, especially in low-resource settings.

암으로 자녀를 잃은 가족의 경험에 대한 질적연구 (The Experience of the Family Whose Child Has Died of Cancer)

  • The purpose of this study was to build a substantive theory about the experience of the family whose child has died of cancer The qualitative re-search method used was grounded theory. The interviewees were 17 mothers who had cared for a child who had died of cancer Traditionally in Korea, mothers are the care givers in the family and are considered sensitive to the family's thoughts, feelings. The data were collected through in-depth interviews by the investigator over a period of nine months. The data were analyzed simultaniously by a constant comparative method in which new data are continuously coded into categories and properties according to Strauss and Corbin's methodology. The 16 concepts which were found as a result of analyzing the grounded data were, -left over time, the empty place, meaninglessness, inner sadness, situational sadness, heartache, physical pain, guilt, resentment, regret, support / stigmatization, finding meaning in the death, changing attitudes about life and living, changing attitudes about health, changing religious practice and changing family relations. Five categories emerged from the analysis. They were emptiness, consisting of left over time, the empty place and meaninglessness ; sadness, consisting of inner sadness and situational sadness ; pain, consisting of heartache and physical pain ; bitterness, consisting of guilt, resentment, regret, sup-port / stigmatization and finding meaning in the death : and transition, consisiting of changing attitudes about life and living, changing attitudes about health, changing religious practice and changing family relations. These categories were synthesized into the core concept, -the process of filling the empty space. The core phenomenon was emptiness. Emptiness varied with the passing of time, was perceived differently according to support / stigmatization and finding meaning in the death, was followed by sad-ness, pain, and bitterness, and finally resulted in changes in attitudes about life and living and about health, and in changes in religious practice and family relations. The process of filling the empty space proceeded by ① accepting realty, ② searching for the reason for the child's death, ③ controlling the bitter feelings, ④ reconstructing the relationships ameng death, illness and health and ⑤ filling the emptiness by resolving causes of child's death, adopting, having another child or with work. Six hypotheses were derived from the analysis. ① The longer the bereavement, the mere the empty space becomes filled. ② The longer the hospitalization, the more sup-port the family needs. ③ The more the sadness, pain and bitterness are expressed, the mere positive changes emerge. ④ Family support faciliates the process of filling the empty space. ⑤ Higher family cohesiveness faciliates the process of filling the empty space. ⑥ The greater the variety of reasons attributed to the child's death, the greater the variety of patterns of change. Four propositions related to emptiness and bitter-ness were developed. ① When the sense of emptiness is great and bitterness is manifested by severe feelings of guilt and resentment, the longer the process of fill-ing the empty space. ② When the sense of emptiness is great and the family is highly motivated to get rid of the bitterness, the shorter the process of filling the empty space. ③ When the sense of emptiness is less and bitter-ness is manifested by severe feelings of guilt and resentment, the process of filling the empty space is delayed. ④ When the sense of emptiness is less and the family is highly motivated to get rid of the bitterness, the process of filling the empty space goes on to completion. Through this substantive theory, nurses under-stand the importance of emptiness and bitterness in helping the family that has lost a child through cancer fill the empty space. Further research to build substantive theories to explain other losses may con-tribute to a formal theory of how family health is restored after human tragedies are experienced.

In Vivo Evaluation of Curcumin-loaded Nanoparticles in a A549 Xenograft Mice Model

  • Curcumin (Cum) has been reported to have potential chemo-preventive and chemotherapeutic activity through influencing various processes, inducing cell cycle arrest, differentiation and apoptosis in a series of cancers. However, the poor solubility of Cum limits its further applications in the treatment of cancer. We have previously reported Cum-loaded nanoparticles (Cum-NPs) prepared with amphilic methoxy poly(ethylene glycol)-polycaprolactone (mPEG-PCL) block copolymers. The current study demonstrated superior antitumor efficacy of Cum-NPs over free Cum in the treatment of lung cancer. In vivo evaluation further demonstrated superior anticancer effects of Cum-NPs by delaying tumor growth compared to free Cum in an established A549 transplanted mice model. Moreover, Cum-NPs showed little toxicity to normal tissues including bone marrow, liver and kidney at a therapeutic dose. These results suggest that Cum-NPs are effective to inhibit the growth of human lung cancer with little toxicity to normal tissues, and could provide a clinically useful therapeutic regimen. They thus merit more research to evaluate the feasibility of clinical application.

Evaluation of an Educational Program on Cervical Cancer for Rural Women in Mangalore, Southern India

  • Cervical cancer is one of the leading causes of cancer in women worldwide. One way by which the incidence of this malignant disease can be minimized is by imparting knowledge through health education. This study aimed at developing an educational package on cervical cancer (EPCC) and determining its effectiveness in terms of significant increase in knowledge of rural women regarding cervical cancer. A one group pre-test, post-test design was adopted. Thirty rural women were selected using a convenient sampling method. Data were collected using a demographic questionnaire and a structured knowledge questionnaire developed by the researchers. The EPCC was designed for a duration of one hour and 10 minutes. The structured knowledge questionnaire was first administered as the pre-test, following which knowledge on cervical cancer was imparted using the EPCC. On the 8th day, the post-test was administered. Data were analyzed using descriptive and inferential statistics. The mean post-test knowledge score of the women regarding cervical cancer was significantly higher than that of their mean pre-test score, indicating that the EPCC was effective in improving the knowledge of rural women on cervical cancer. The association between pre-test knowledge scores and selected demo-graphic variables were computed using chi-square test showed that pre-test knowledge score of the women regarding cervical cancer was independent of all the socio-demographic variables. It was concluded that the EPCC is effective in improving the knowledge of women, regarding cervical cancer. Since the prevalence of cervical cancer is high, there is an immediate need to educate women on prevention of cervical cancer.

소아암 청소년의 복원력 예측 요인 (Predictors of Resilience in Adolescents with Cancer)

  • 목적 본 연구는 소아암 청소년 복원력과 가족 복원력의 관계를 파악하고, 청소년 연령에 따른 복원력 영향 요인을 확인하여 이를 증진하기 위한 간호중재 개발의 기초자료를 제공하고자 수행된 서술적 조사 연구이다. 방법 상급종합병원 2곳에서 소아암으로 진단받은 지 최소 6개월 이상 경과된 외래 치료 중이거나 치료 종료 후 추후 관리 중인 만 10-18세 청소년 107명과 그 부모 107명을 대상으로 설문 조사를 하였다. 결과 가족 강인성, 사회적 지지는 가족의 월평균 수입에 따라 유의한 차이가 있었고, 종교 유무에 따라 소아암 청소년의 복원력은 유의한 차이가 있었다. 병행 치료 수가 3가지 이상인 경우 가족 문제해결 의사소통과 소아암 청소년의 복원력이 높았다. 사회적 지지가 높을수록 가족 강인성, 가족 문제해결 의사소통이 높게 나타났고, 가족 강인성은 가족 문제해결 의사소통, 소아암 청소년의 복원력과 양의 상관관계가 있으며, 가족 문제해결 의사소통은 소아암 청소년의 복원력과 양의 상관관계가 있었다. 소아암 청소년의 복원력에 영향을 미치는 요인은 만 10-12세 소아암 청소년의 경우 종교 유무, 가족 문제해결 의사소통이었고, 만 13-15세 소아암 청소년의 경우 가족 강인성, 가족 문제해결 의사소통으로 나타났다. 결론 본 연구의 결과는 소아암 청소년의 복원력과 가족 복원력을 증진하기 위하여 소아암 청소년의 연령에 따른 복원력 중심의 간호 중재 프로그램 개발의 필요성을 시사한다.

암생존자의 스트레스가 우울과 삶의 질에 미치는 영향: 외상 후 성장의 조절효과를 중심으로 (The Effects of Stress on Depression and Quality of Life in Cancer Survivors : Focusing on the Moderating Effects of Posttraumatic Growth)

  • 본 연구에서는 암생존자의 스트레스가 우울과 삶의 질에 영향을 미치는 과정에 외상 후 성장이 조절효과를 나타내는지 확인하고자 하였다. 이를 위해 서울시, 경기도, 충청도, 전라도에 거주하는 암생존자 173명을 대상으로 설문조사를 실시하였고 응답한 자료로 위계적 회귀분석을 실시하였다. 분석 결과 첫째, 외상 후 성장은 스트레스와 상호작용하여 암생존자의 우울 및 삶의 질에 영향을 미쳤다. 즉 스트레스 수준이 높더라도 외상 후 성장이 높으면 외상 후 성장이 낮을 때보다도 우울 수준이 더 낮고, 삶의 질 수준이 더 높은 것으로 나타났다. 둘째, 외상 후 성장은 삶의 질의 하위요인 중 정서적 및 기능적 삶의 질에서만 조절효과를 나타냈다. 본 연구결과를 바탕으로 암생존자의 우울 감소와 삶의 질 증진을 위해서는 외상 후 성장을 높이는 교육 및 상담적 개입이 필요함을 논의하였다.

젊은 유방암 환자의 자녀 특성에 따른 양육 스트레스, 양육 행동 및 부모교육 요구도 (Parenting Stress, Parental Behaviors and Need for Parental Education Relative to Children Characteristics of Young Women with Breast Cancer)

  • Purpose: This study was designed to explore the parental stress, behaviors and need for parental education of young women with breast cancer and how these variables are related to their children characteristics. Methods: The subjects were 110 young women with breast cancer who have a child. Data were collected using questionnaire of parenting stress, parental behaviors and need for parental education. Results: The subjects reported many unmet needs for parental education, and have the highest level of need for education related to breast cancer. Women with daughters have higher level of need for parental education compared to subjects with sons. There was a negative correlation between parenting stress and parental behaviors and between parenting stress and need of parental education. Conclusion: As these results, the development and application of parental education program for mothers with breast cancer in the future should be considered with the demographic characteristics, the social and cultural environment, parenting stress, and parental behaviors of parents and children.

소아·청소년 암환자의 신체상과 질병적응에 관한 연구 (Children with Cancer: Adjustment to Disease and Body Image)

  • 의학기술의 발전으로 소아암 환아의 생존율이 1960년대 30%미만에서 최근 75%이상으로 향상되었다. 이에 따라 소아암은 더 이상 불치병이 아닌 만성질환으로 분류되고 있으나, 장기간의 투병생활로 인해 소아암 환아들은 신체적, 심리사회적 문제를 겪고 있다. 특히 외모에 관심이 많고, 또래관계가 중요한 10대 소아암 환아들에게 있어 신체적, 심리사회적 변화는 이들의 신체상과 질병적응에 영향을 미칠 것으로 예상된다. 이에 본 연구에서는 소아암 환아의 신체상(신체왜곡도) 및 질병적응 수준을 알아보고, 인구사회학적 특성, 질병관련 특성 및 신체상이 소아암 환아의 질병적응에 미치는 영향에 대해 알아보았다. 본 연구는 2007년 10월 22일부터 11월 16일까지 자기기입식 설문조사로 이루어졌으며, 조사대상자는 암진단 후 3년 이내의 10세에서 18세 환아 82명이었다. 수집된 자료는 기술통계, t-검정, 상관관계, 위계적 다중회귀방법으로 분석하였다. 분석결과, 연령, 학교생활여부, 신체상이 소아암 환아의 질병적응에 영향을 미치는 요인으로 나타났다. 즉, 연령이 높을수록, 학교생활을 하고 있는 환아가 그렇지 않은 환아보다, 신체왜곡도가 낮을수록 질병적응에 긍정적 영향을 미치는 것으로 나타났다. 소아암 환아의 질병적응을 돕기 위해서는 소아암 환아가 학교생활을 지속할 수 있도록 돕는 정책적 제도 구축, 긍정적 신체상 형성을 위한 프로그램 개발, 소아암 환아의 연령을 고려한 심리사회적 접근 등이 필요하며, 이를 위해 사회복지사의 적극적인 개입이 이루어져야 할 것이다.

Epidemiological Characteristics and Prediction of Esophageal Cancer Mortality in China from 1991 to 2012

  • Background: To analyze the mortality distribution of esophageal cancer in China from 1991 to 2012, to forecast the mortality in the future five years, and to provide evidence for prevention and treatment of esophageal cancer. Materials and Methods: Mortality data for esophageal cancer in China from 1991 to 2012 were used to describe its epidemiological characteristics, such as the change of the standardized mortality rate, urban-rural differences, sex and age differences. Trend-surface analysis was used to study the geographical distribution of the mortality. Curve estimation, time series, gray modeling, and joinpoint regression were used to predict the mortality for the next five years in the future. Results: In China, the incidence rate of esophageal cancer from 2007 and the mortality rate of esophageal cancer from 2008 increased yearly, with males at $8.72/10^5$ being higher than females, and the countryside at $15.5/10^5$ being higher than in the city. The mortality rate increased from age 45. Geographical analysis showed the mortality rate increased from southern to eastern China, and from northeast to central China. Conclusions: The incidence rate and the standardized mortality rate of esophageal cancer are rising. The regional disease control for esophageal cancer should be focused on eastern, central and northern regions China, and the key targets for prevention and treatment are rural men more than 45 years old. The mortality of esophageal cancer will rise in the next five years.
