치과대학병원 치과보존과로 전달된 진료 의뢰서의 충실도 분석 (Fidelity analysis of Patient's Referral Letters delivered to the Department of Conservative Dentistry in a Dental University Hospital)

  • In cases of a patient referral, the general dentist should communicate the patient's dental information as clearly as possible to the specialist to whom he/she is referring his/her patient. For this reason, it is necessary to write a referral letter in an accurate and complete manner. The purpose of this study is to analyze the referral letters of patients who are referred to the department of conservative dentistry in a dental university hospital. From January, 2017 to December, 2018, we gathered the information of the patients who were referred to the department of conservative dentistry in a dental university hospital with referral letter. We evaluated the fidelity of a total of 750 referral letters and recorded the information of the referral dentist and the dental clinic. The relationship between each item and fidelity was analyzed using SPSS 18.0 program. The analysis of the contents of the referral letters yielded the following results: The fidelity of the referral letter was higher when the patient was referred during the root canal treatment, or when there was a root canal filling or when there was pain during chewing. The fidelity of referral letter was low in cases where the referring clinic is close to and has cooperative relationship with dental university hospital. Among the factors affecting the fidelity, the year of graduation of the referral dentist was the most influential. This study confirmed the fact that the clinical practice of writing patient referral letters still leaves much to be desired. In order to solve this problem, it is necessary to create more clear and standardized guidelines for writing referral letters.

Determining the reliability of diagnosis and treatment using artificial intelligence software with panoramic radiographs

  • Purpose: The objective of this study was to evaluate the accuracy and effectiveness of an artificial intelligence (AI) program in identifying dental conditions using panoramic radiographs(PRs), as well as to assess the appropriateness of its treatment recommendations. Materials and Methods: PRs from 100 patients(representing 4497 teeth) with known clinical examination findings were randomly selected from a university database. Three dentomaxillofacial radiologists and the Diagnocat AI software evaluated these PRs. The evaluations were focused on various dental conditions and treatments, including canal filling, caries, cast post and core, dental calculus, fillings, furcation lesions, implants, lack of interproximal tooth contact, open margins, overhangs, periapical lesions, periodontal bone loss, short fillings, voids in root fillings, overfillings, pontics, root fragments, impacted teeth, artificial crowns, missing teeth, and healthy teeth. Results: The AI demonstrated almost perfect agreement (exceeding 0.81) in most of the assessments when compared to the ground truth. The sensitivity was very high (above 0.8) for the evaluation of healthy teeth, artificial crowns, dental calculus, missing teeth, fillings, lack of interproximal contact, periodontal bone loss, and implants. However, the sensitivity was low for the assessment of caries, periapical lesions, pontic voids in the root canal, and overhangs. Conclusion: Despite the limitations of this study, the synthesized data suggest that AI-based decision support systems can serve as a valuable tool in detecting dental conditions, when used with PR for clinical dental applications.

Comparison of the sealing ability of various bioceramic materials for endodontic surgery

  • Objectives: Endosequence Bioceramic Root Repair Material (BC-RRM) is used in endodontic microsurgery. It is available as a paste and a putty. However, no studies to date have examined the sealing ability of these forms alone or in combination as root-end filling materials. Hence, this study aimed to compare the sealing properties of these 2 forms of BC-RRM. Materials and Methods: Forty-two extracted upper anterior teeth were divided into 3 experimental groups, a positive and negative control. After the root canal treatment, the root ends were resected, retroprepared and retrofilled with either putty, paste + putty or mineral trioxide aggregate (MTA). The teeth were mounted in tubes so the apical 3 mm was submerged in Brain Heart Infusion (BHI) broth. The coronal portions of the canals were inoculated with Enterococcus faecalis and BHI broth and incubated for 30 days. The broth in the tubes was analyzed for colony forming units to check for leakage of bacteria from the canal. The teeth from the groups were sectioned and analyzed using scanning electron microscopy (SEM). The Kruskal-Wallis test and analysis of variance were used to analyze the data with a significance level p < 0.05. Results: The BC-RRM and MTA groups showed similar sealing ability. The positive control showed leakage in all samples. The SEM imaging showed the presence of bacteria in all experimental groups at the material-tooth interface. Conclusions: No significant differences were noted in the experimental groups, providing sufficient evidence that any combination could be effectively used during endodontic microsurgery.

Effect of post space preparation drills on the incidence of root dentin defects

  • Objectives: This study investigated the incidence of root dentin defects after the use of different post space preparation (PSP) drills. Materials and Methods: Seventy-two bovine incisors were selected and obtained 14-mm-long root sections. Twelve roots served as controls with no intervention (G1). The 60 root canals remaining were instrumented using the crown-down technique with the ProTaper Next system and obturated using the lateral condensation technique. Specimens were randomly distributed into 5 groups (n = 12) according to the operative steps performed: G2, root canal instrumentation and filling (I+F); G3, I+F and PSP with Gates-Glidden drills; G4, I+FI+F and PSP with Largo-Peeso reamers; G5, I+F and PSP with Exacto drill; and G6, I+F and PSP with WhitePost drill. Roots were sectioned at 3, 6, 9, and 12 mm from the apex, and digital images were captured. The presence of root dentin defects was recorded. Data were analyzed by the χ2 test, with p < 0.05 considered to indicate statistical significance. Results: Root dentin defects were observed in 39.6% of the root sections. No defects were observed in G1. G5 had significantly more cracks and craze lines than G1, G2, and G3 (p < 0.05), and more fractures than G1, G2, G3, and G4 (p < 0.05). When all root sections were analyzed together, significantly more defects were observed at the 12-mm level than at the 3-mm level (p < 0.05). Conclusions: PSP drills caused defects in the root dentin. Gates-Glidden drills caused fewer root defects than Largo-Peeso reamers and Exacto drills.

Modified Continuous Wave Technique을 이용한 근관충전시 형성된 치근단 크기가 치근단 폐쇄에 미치는 영향 (EVALUATION OF THE INFLUENCE OF APICAL SIZES ON THE APICAL SEALING ABILITY OF THE MODIFIED CONTINUOUS WAVE TECHNIQUE)

  • 본 연구에서는 근관형성의 치근단 크기가 근관 충전의 치근단 폐쇄에 미치는 영향에 대해 알아보고자 하였다. 36개의 단근관, 단근치를 대상으로 3가지 치근단 크기 (Master apical file : #25, #40, #60)에 따라 3가지 실험군으로 나누어 $Profile^{(R)}$$LightSpeed^{(R)}$를 이용하여 근관을 확대하고, Modified continuous wave technique으로 근관충전을 시행하였다. 미세누출의 측정을 위해 실험군당 10개의 치아를 1주일간 India ink에 넣어 색소 침투를 일으키고 치아를 투명화시켜 미세누출의 정도를 측정하였다. 미세누출에 치근단 크기가 미치는 영향을 분석하기 위하여 Kruskal-Wallis one-way ANOVA를 시행하였고, 사후 검정을 위하여 Student-Newman-Keuls method를 시행하였다. 또한, 치아 단면의 형태를 관찰하기 위하여 색소 침투까지 실행된 각 실험군당 2개의 치아를 근첨 1 mm에서 4 mm까지 1 mm간격으로 수평 절단하고 사진 촬영하였다. 각 실험군에서 측정된 미세누출양을 Krustal-Wallis one-way ANOVA에서 분석한 결과 치근단 크기는 유의성 있게 (P < 0.05) 미세누출에 영향을 미쳤으며 치근단 크기가 클수록 미세누출의 평균간과 표준편차 값은 감소하는 경향을 보였다. Student-Newman-Keuls method를 통한 사후 검정에서 치근단 크기가 #25 - #40, #25 - #60에서 유의 한 차이를 보였으며 #40 - #60간에는 통계적인 유의차는 없었다. 단면형태에서는 대부분의 치아에서 난원형의 근관 형태를 관찰 할 수 있었고, 치근단의 크기가 작을수록 성형된 근관에 불규칙한 내면이 존재하였고 gutta-percha나 sealer로 충전되지 않는 recess의 면적이 증가함을 알 수 있었으며 이러한 recess는 미세누출의 통로가 됨을 확인 할 수 있었다. 본 연구 결과, Modified continuous wave technique으로 근관충전시 근관 형성된 치근단 크기가 커질수록 보다 일관되고 성공적 인 결과를 예상할 수 있는 근관충전을 시행할 수 있음을 알 수 있었다.

Type III 치내치의 보존적 근관치료 : 증례보고 (Conservative Endodontic Treatment of Type III Dens Invaginatus : Case Report)

  • 치내치란 치아 조직의 석회화가 일어나기 이전에 법랑기가 치유두 내로 함입되어 나타나는 발육성 이상을 말한다. 치내치는 임상적, 방사선학적으로 다양한 변이를 나타내며, 치내치의 여러 분류법 중 Oehlers(1957)가 제안한 분류법이 가장 널리 이용되고 있다. 치수는 건강한 채로 남아 있으나 함입부가 치주염과 연관된 Type III 병소를 치료하는데 다양한 술식들이 설명되어져왔다. 가장 우선시 되어야 하는 목표는 가능하다면 치수를 건전하게 보존하는 것이며, 치수질환의 명백한 증거가 없다면, 함입부를 치근과 별개로 치료하는 보존적인 접근법이 필요하다. 그러나, Type III 치내치의 근관치료는 근관과 함입의 충분한 잔사제거, 재현가능한 근관장의 조절, 일관된 충전등을 충분히 달성하기 어렵다는 문제점이 있다. 본 증례는 치근단 병소를 보이는 Type III 치내치의 치료에 함입부에만 제한된 보존적 근관치료 및 Mineral Trioxide Aggregate(MTA)를 이용한 폐쇄를 통해 양호한 결과를 얻었기에 보고하는 바이다.

Retreatability of two endodontic sealers, EndoSequence BC Sealer and AH Plus: a micro-computed tomographic comparison

  • Objectives: Recently, bioceramic sealers like EndoSequence BC Sealer (BC Sealer) have been introduced and are being used in endodontic practice. However, this sealer has limited research related to its retreatability. Hence, the aim of this study was to evaluate the retreatability of two sealers, BC Sealer as compared with AH Plus using micro-computed tomographic (micro-CT) analysis. Materials and Methods: Fifty-six extracted human maxillary incisors were instrumented and randomly divided into 4 groups of 14 teeth: 1A, gutta-percha, AH Plus retreated with chloroform; 1B, gutta-percha, AH Plus retreated without chloroform; 2A, gutta-percha, EndoSequence BC Sealer retreated with chloroform; 2B, gutta-percha, EndoSequence BC Sealer retreated without chloroform. Micro-CT scans were taken before and after obturation and retreatment and analyzed for the volume of residual material. The specimens were longitudinally sectioned and digitized images were taken with the dental operating microscope. Data was analyzed using an ANOVA and a post-hoc Tukey test. Fisher exact tests were performed to analyze the ability to regain patency. Results: There was significantly less residual root canal filling material in the AH Plus groups retreated with chloroform as compared to the others. The BC Sealer samples retreated with chloroform had better results than those retreated without chloroform. Furthermore, patency could be re-established in only 14% of teeth in the BC Sealer without chloroform group. Conclusion: The results of this study demonstrate that the BC Sealer group had significantly more residual filling material than the AH Plus group regardless of whether or not both sealers were retreated with chloroform.

AH-26 및 산화아연유지놀 근관실러가 상아질에 대한 복합레진의 전단결합강도에 미치는 영향 (The Influence of AH-26 and Zinc Oxide-Eugenol Root Canal Sealer on the Shear Bond Strength of Composite Resin to Dentin)

  • AH-26 근관실러가 상아질에 대한 복합레진의 전단결합강도에 미치는 영향을 평가하기 위하여 144개의 발거된 대구치를 실린더형의 몰드에 매식한 다음 치관부의 상아질 표면을 노출시킨 후 편평하게 연마하고 AH-26을 도포한 군, ZOE paste를 도포한 군 및 실러를 도포하지 않은 대조군으로 나누어 One-step 상아질 접착제를 처리한 후에 Charisma (Heraeus Kulzer Germany) 복합레진을 적용하고 광중합시켰다. 시편을 분리하여 $37^{\circ}C$ 항온조에 24시간 보관후 Instron test machine (Model 4202, Instron Corp. USA)을 이용하여 시편의 전단 결합 강도를 측정하고 one-way ANOVA 및 Tukey's studentized rank test로 통계 분석 하였다. AH-26 근관실러로 처리한 군과 대조군은 ZOE 근관실러 처리군에 비해 유의하게 놓은 접착강도를 나타내었다(p<0.05). AH-26 근관실러 처리군과 아무 처리하지 않은 대조군 사이에는 결합강도에 있어 유의한 차이를 나타내지 않았다 (p>0.05). 본 연구의 조건에서는 ZOE 근관실러는 상아질에 대한 복합레진의 전단결합강도를 저하시키는 반면 AH-26 근관실러는 유의한 영향을 미치지는 않는 것으로 보인다. 따라서 임상에서 근관실러로 AH-26을 사용한 근관충전 후에는 즉시 복합레진 코어를 해 주어도 결합력에 저해가 없을 것으로 여겨진다.

치근단절제시 노출된 상아세관의 밀폐효과에 대한 Nd : YAG 레이저의 효과 (THE EFFECT OF ND : YAG LASER ON DENTINAL TUBULE SEAL AFTER ROOT END RESECTION)

  • If root and resection is done during surgical endodontic treatment, newly exposed dentinal tubules form pathways between the canal and the peripheral tissue. Nd : YAG laser was used to block this phenomenon, and its effect was studied with dye penetration and SEM techniques. 40 intact single rooted teeth were divided into 4 groups(10 each) : control group and test groups, in which retrograde cavity surface, cutting surface, retrograde cavity surface & cutting surface were treated with laser(1 watt 15pps) and finally retrograde filling with IRM was conducted. After that, they were stained with 2 % methylene blue, sectioned and evaluated by the maximum infiltration depth. And to observe surface change, they were prepared for SEM. The results were as follows ; 1. All experimental groups showed microleakage with variation in amount. 2. The 2nd group which treated both the retrograde cavity and cutting surface showed significantly less microleakage than the other groups(p<0.05). There was no significant difference between groups treated on one side only. 3. As a result of SEM observation of dentin surface, obstruction of dentinal tubules with marble shaped granules, which were different from normal dentin could be seen. Cracks could be seen also. 4. In summary of this experiment, it is thought that effort to obstruct the exposed dentinal tubules as well as retrograde cavity after root end resection is needed.

Post-Space형성시기가 치근단폐쇄에 미치는 영향에 관한 실험적 연구 (THE EFFECT OF POST-SPACE PREPARATION TIME ON THE APICAL SEAL OF ENDODONTICALLY TREATED TEETH)

  • The purpose of this study was to evaluate the effect of canal filling methods and post-space preparation time on the apical seal of endodontically treated teeth. Seventy-six single-rooted human teeth were divided into 6 groups and each tooth was obturated and prepared post-space according to the purpose of this study. After all specimens were immersed in Indian ink, decalcified and cleared, the degree of dye penetration into the root canals observed by magnifying glass (X20) and measured by caliper. The results were as follows: 1. In the thermatic condensation, no significant difference in ink penetration occured when the post-spaces were created immediately after obturation or when they were made a week later. 2. In the lateral condensation, no significant difference in ink penetration existed between teeth whose post-space were prepared immediately or delayed. 3. In comparison of experimental groups and control groups, no significant difference showed when the groups were obturated with McSpadden compactor but when the teeth were laterally condensed, the experimental groups showed less ink penetration than controls. 4. Thermatic condensed teeth with McSpadden compactor were less ink penetration thn laterally condensed teeth.
