심혈관질환약물과 향정신성약물의 약물상호작용 (Drug Interactions between Cardiovascular Agents and Psychotropic Drugs)

  • 많은 심혈관질환약물과 향정신성약물 간에 다양한 약물상호작용이 존재하며 이러한 약물들의 대부분이 시트크롬(cytochrome, CYP)450 효소의 기질, 억제제, 유도제로 작용하면서 약물상호작용이 일어나게 된다. 주로 CYP2D6와 CYP3A4를 억제하는 향정신성약물로 인해 같이 투여되는 심혈관질환약물의 효과가 변할 수 있고 부작용까지 나타날 수 있다. 이런 상황을 고려하고 반대의 경우도 포함하여 흔히 처방되는 두 종류의 약물을 병용 투여하는 경우 고려해야 할 부분에 대해서 심혈관질환약물 분류에 따라 논하였다. 대부분의 베타차단제는 CYP2D6의 대사에 의존하므로 이 대사를 억제하는 bupropion, chlorpromazine, haloperidol, SSRIs, quinidine 등을 사용했을 때 베타차단제의 독성이 나타날 수 있다. 앤지오텐신 관련 약물과 이뇨제가 lithium의 농도를 변화시키는 점도 고려하여야 한다. 칼슘통로차단제 및 콜레스테롤강하제를 CYP3A4의 강력한 억제제인 amiodarone, diltiazem, fluvoxamine, nefazodone, verapamil 등과 함께 사용하였을 때 약물 상호작용에 따른 부작용에 유의하여야 한다. 항부정맥제를 복용하는 환자에서 QT 간격 증가를 야기하는 약물이나 관련 CYP450 효소를 억제하는 약물을 동시에 투여하는 것은 삼가거나 적극적인 관찰이 필요하다. Digoxin과 warfarin이 병용 투여되는 향정신성약물로 인해 혈중 농도가 변하는 것도 임상적으로 중요하다.

Effect of Dietary Protein Levels on the Performance, Nutrient Balances, Metabolic Profile and Thyroid Hormones of Crossbred Calves

  • An experiment was conducted to study the effect of different dietary protein levels on the performance, nutrient balances, blood biochemical parameters and thyroid hormones of crossbred calves. Thirty crossbred (Bos taurus${\times}$Bos indicus) calves aged 3-5 months were divided into 3 equal groups of 10 each and fed graded levels of crude protein, namely 100 (NP), 75 (LP) and 125 (HP) percent of the Kearl recommendations for 105 d. The calves had access to ad libitum oat hay as the basal roughage. A metabolism trial of 6 d duration was conducted at 90 d of the study. Blood collection and its analysis for various hematological and biochemical parameters as well as thyroid hormones was done both during the pre- and post-experimental periods. The fortnightly body weight changes and the net gain did not differ significantly due to dietary variation. The average daily gain was $367{\pm}21.6$, $347{\pm}22.9$ and $337{\pm}26.4g$ in calves fed NP, LP and HP diets, respectively. Averaged across the feeding trial, oat hay intake was higher (p<0.05) in NP animals than HP or LP fed groups. The dry matter (DM) intake showed no significant difference between the 3 groups but the DM digestibility was higher (p<0.05) in the HP fed animals. The digestibility of crude protein, organic matter, crude fiber and nitrogen-free extract was significantly higher (p<0.05) on HP diets compared to LP or NP diets. The calves on all 3 diets were in positive nitrogen (N) balance, however the N retention was higher (p<0.05) in HP than in LP fed calves. The intake and retention of calcium and phosphorus were similar between the treatments. The blood biochemical profile revealed no significant influence of the dietary treatments on hemoglobin, packed cell volume as well as serum levels of glucose, total protein, albumin, globulin, Ca, P, and alkaline phosphatase. Serum levels of the circulating thyroid hormones ($T_3$ and $T_4$) tended to be lower (p>0.05) on feeding of the LP diet besides showing an increasing trend with the advancement of age. Considering the similar performance and metabolic profile, it could be concluded that crossbred calves can be satisfactorily reared on 25% lower protein level as recommended by Kearl for developing countries, which would not only economize the cost of production but also help to reduce environmental pollution attributable to livestock production.

재발성 종양성 석회증 - 증례 보고 - (Recurrent Tumoral Calcinosis - A Case Report -)

  • 종양성 석회증은 칼슘과 인산의 대사장애 없이 관절주위의 연부조직에 석회성분의 침착을 일으키는 질환으로 그 원인은 정확히 알려져 있지 않으며, 발생빈도는 매우 드물며 주로 흑인에서 발생한 예들이 보고되어 있으나 황인종에서 수 차례 재발되어 보고된 예는 아직 없다. 이에 저자들은 우측 슬관절과 대퇴부에 발생하여 수 차례 재발된 종양성 석회증 1예를 경험하였기에 문헌고찰과 함께 보고하고자 한다. 21세 여자 환자로서 우측 슬관절과 대퇴부 후방의 종괴를 주소로 내원하였으며 과거력상 14년 전부터 타 병원에서 4차례에 걸쳐 생검 및 제거술을 실시하였으나 재발을 반복하였다. 치료로서 제거 가능한 종괴의 철저한 변연 절제술을 시행하였으며 조직학적 소견상 종양성 석회증으로 진단되었던 예이었다. 술 후 24개월 이상 추시 관찰하여 현재까지 재발의 소견이 없었다. 저자들은 황인종에서 발생빈도가 드문 재발된 종양성 석회증 1예를 보고하는 바이다.

귀신환이 난소적출 백서의 골다공증에 미치는 영향 (Effects of Guisinhwan on the Ovariectomized Rat Model of Postmenopausal Osteoporosis)

  • Aging is characterized by an age-dependent reduction in bone density. Pronounced bone loss by postmenopausal estrogen deficiency induces a bad effect on dynamic balance of bone metabolism. When the bone density is reduced to the level below the fracture threshold, the risk for fracture is greatly increased. The purpose of this study is to examine what are the effects of the Guisinhwan(GSH : 歸腎丸) on the ovariectomized rat model of postmenopausal osteoporosis. The results of the experiment are as follows: Body weight in control group showed significant increase in comparison with sham, but that in GSH-treated showed no change in comparison with control. The level of serum albumin in control group showed significant decrease in comparison with sham. That in GSH-treated was slightly decreased in comparison with control. The level of serum ALP activity in control group showed significant increase in comparison with sham, but that in GSH-treated was significantly decreased in comparison with control. The level of serum phosphorous, calcium, GOT, GPT, T3 did not significant change among the three groups. The level of serum estrogen in control group showed slightly decreased in comparison with sham, but that in GSH-treated showed no change in comparison with control. The level of serum T4 in control group showed significant increase in comparison with sham, but that in GSH-treated was significantly decreased in comparison with control. Trabecular bone area as well as trabecular thickness in control group showed significant decrease In comparison with sham. Those in GSH-treated showed significant increase in comparison with control. Trabecular number did not significant change among the three groups. Trabecular separation only in GSH-treated showed significant decrease in comparison with control. Osteoclast number in control group showed significant increase in comparison with sham. That in GSH-treated showed significant decrease in comparison with control. Oseoblast surface in control group showed significant increase in comparison with sham, but that in GSH-treated showed no change in comparison with control. According to the above these results, GSH has shown to be capable of preventing and curing osteoporosis caused by old-aged and postmenopause.

총순환정지후 혈중 크레아티닌 카이네이즈 BB의 변화에 관한 연구 (Changes of Plasma Creatinine Kinase-BB after Total Circulatory Arrest)

  • 개심술시 총순환정지는 매우 유용한 방법이나 뇌손상등 부작용으로 그 사용이 제한되고 있다. 이러한 뇌손상의 분석에 유용한 지표로서 뇌 허혈성 손상의 특이한 효소인 크레아티닌 카이네이즈 BB(CK-BB)의 유용성을 재고하고 뇌손상에 영향을 미치는 인자들의 분석을 시도하였다. 총순환정지를 이용하여 개심술을 시행한 18명의 환자를 대상으로 하였다. 이들은 다시 비청색증형 심기 형군과 청색증형 심기형군으로 나누어 각각 6명과 12명으로 하였다. 각 환자에서 총순환정지전 및 후 15, 30, 60, 120, 240, 480, 720분에 동맥혈을 채취하여 CK-BB분획을 측정하였고 이와 동시에 혈색소농도, 이 온화 칼슘농도, 혈당량 등을 측정하였다. 총순환정지시간과 CK-BB와 통계적으로 유의한 관계를 보이는 채혈시간은 찾을 수 없었다. 또 총순환 정지전의 문제로 인한 영향을 줄이기 위하여 각각의 측정치에서 총순환정지전의 측정치를 뺀 값을 구하 여 각기 CKBBD15, CKBBD30....등으로 표시하고 상관관계를 구하였으나 역시 유의한 상관관계를 보이 지 않았다. CKBB30과 CKBBD30값은 비청색증형 군과 청색증형 군 사이에 유의한 차이를 보이지 않았 다. CK-BB농도와 혈색소 농도는 유의한 상관관계를 보였다. 단일 채혈에 의한 CK-BB농도의 지표는 순환정지시간과 유의한 관계가 없음이 확인되었다. 또한 청색 증형 심질환군이 총순환정지에 의한 뇌손상에 비청색증형 심질환군에 비해 더 취약하지는 않은 것으로 생각된다.

정보 성분과 상대위험도를 이용한 clopidogrel의 약물상호작용 시그널 검색 : 건강보험데이터베이스를 대상으로 한 데이터마이닝 연구 (Use of Information Component (IC) and Relative Risk (RR) for Signal Detection of Drug Interactions of Clopidogrel : Data-mining Study Using Health Insurance Review & Assessment Service (HIRA) Claims Database)

  • Health Insurance Review & Assessment Service (HIRA) claims database has a high potential to detect signals of new drug interactions. The aim of this study was to evaluate the usefulness of information component (IC) and relative risk (RR) as a tool for signal detection, and to analyze the possible drug interactions caused by clopidogrel using HIRA claims database. This study was performed in elderly patients over 65 years of age who administered clopidogrel from January 2005 to June 2006 in South Korea. Serious Adverse Events (SAEs) as drug interactions of clopidogrel were defined as any ambulatory hospitalization for ischemic diseases within comcomitant medication period of clopidogrel. Information Component (IC) and Relative Risk (RR) were calculated to compare the proportion of drug-SAE pairs in order to select drug specific SAEs. IC and RR signals of clopidogrel drug interaction were screened when IC's 95% confidence interval was greater than 0 and RR's 95% confidence interval was greater than 1 respectively. All detected signals were compared to references such as $Micromedex^{(R)}$ and 2010 Drug Interaction $Facts^{TM}$. Sensitivity, specificity, positive predicted value and negative predicted value were used to evaluate usefulness of this method. Among 13,252,930 cases of elderly patients who co-administered clopidogrel and other drugs, 47,485 cases were detected as SAE. Of these, one-hundred nine cases were detected by the IC-based data-mining approach and ninety one cases were detected by the RR-based data-mining approach. Total One-hundred sixty three unrecognized signals were detected by IC or RR. Twelve signals from IC-based data-mining (57.1%) were corresponded with drug interactions from references and eight signals from RR-based data-mining (38.1%) were corresponded with drug interactions from references. These signals include proton pump inhibitors, calcium channel blockers and HMG CoA reductase Inhibitors, which were known to affect CYP450 metabolism. Further studies using HIRA claims database are necessary to develop appropriate data-mining measure.

Trabecular number and did not significant change among the three groups. Trabecular separation only in YLZ-treated showed significant decrease in comparison with control. Osteoclast number and oseoblast surface in control group showed significant increase in comparison with sham. Those in YLZ-treated showed slightly decreased in comparison with control. According to the above these results, YLZ has shown to be capable of preventing and curing osteoporosis caused by old-aged and postmenopause.

Randomized, Double-blind, Placebo-controlled Trial of the Effects of Polycan, β-glucan Originating from Aureobasidium Pullulans, on Bone Biomarkers in Healthy Women

  • Polycan originating from Aureobasidium pullulans is mostly composed of β-1, 3/1, 6 glucans and possesses an anti-osteoporotic effect. We conducted a randomized, double-blind, placebo-controlled trial to examine the efficacy and safety of the polycan on bone biochemical markers in healthy perimenopausal women. Sixty subjects were randomly allocated to 2 groups-group 1 received 400 mg of polycan and group 2 received placebo-these were administered once daily for 28 days. Fasting blood and urine samples were collected at baseline and 4 weeks after treatment. The primary outcome was change in osteocalcin (OSC) and bone-specific alkaline phosphatase (BALP). Changes in calcium (Ca), phosphorus (P), C-telopeptide of collagen cross-links (CTx), N-telopeptide of collagen cross-links (NTx), and deoxypyridinoline (DPYR) were the secondary outcomes. A safety assessment was performed using adverse event (AE) and laboratory data. After 4 weeks of polycan treatment, OSC, DPYR, and BALP levels changed (P < 0.05) significantly from baseline in both groups. However, no significant differences were observed in any markers between the 2 groups, except for P (P < 0.05). Interestingly, group 2 showed a significant increase in CTx (65.2%, P < 0.05), while CTx in group 1 slightly increased (17.2%). Both groups showed no significant differences in AE. Although 4 weeks of polycan treatment did not have a statistically significant effect on bone metabolism biomarkers, increases in CTx were modestly inhibited by polycan. Further studies in a large population and longer treatment periods are needed to confirm the effect of polycan on bone turnover.

Effect of Soymilk and Exercise on Bone Mineral Density and Bone Metabolism Related Markers in Underweight College Women with Low Bone Density

  • The objective of this study was to investigate the effect of soymilk and exercise on bone mineral density (BMD) in underweight college women of 19-22 years of age, who had lower bone mass. The BMD of the lumbar spine and femoral neck was measured for 52 underweight college women. Among them, 33 subjects, whose t-score value was below -1, were selected. Questionnaire survey, anthropometrical measurements, dietary recall, analysis of BMD, fasting serum osteocalcin and urinary deoxypyridinoline (DPD) were conducted before and after the 10 week study. The 33 subjects were divided into 2 groups: soymilk group (n=19), and soymilk + exercise group (n=14). The soymilk group was given 400$m\ell$ soymilk containing 60mg of isoflavones on a daily basis and the soymilk + exercise group exercised three times a week with a daily intake of 400$m\ell$ soymilk for 10 weeks. The average ages of the soymilk group and the soymilk + exercise group were 21.1 years and 20.4 years, respectively and, there were no significant differences between the soymilk group and the soymilk + exercise group in the areas of height, weight or Body Mass Index (BMI). At the baseline, the mean daily energy intake of the soymilk group and the soymilk + exercise group was 1,597.9kcal (79.43% of RDA) and 1,704.2kcal (85.2% of RDA), respectively. The mean calcium intake of the soymilk group (408.3mg) was not significantly different from that of the soymilk + exercise group (389.4mg). Despite the 400$m\ell$ soymilk supplementation, there were no significant changes of nutrient intake in either group after treatment. However, there were significant increases in BMD's of lumbar spine and femoral neck in both groups. There were some increases in the serum osteocalcin level and decreases in the urinary deoxypyridinoline level as well. BMD change of the soymilk group was not significantly different from that of the soymilk + exercise group. In conclusion, supplementary intake of soymilk (containing 60mg of isoflavones) resulted in a significant increase in the BMD's of the lumbar spine and femoral neck in underweight college women with low bone mass. However, exercise did not result in any significant changes in the BMD's, implying the necessity for more intensive and specific long-term physical training for any substantial changes. Further investigation is necessary to determine the exercise that most strongly affects BMD.

Production Performance and Carcass Characteristics of Malpura and Mutton Synthetic Lambs Fed Low and High Energy Rations in a Semiarid Region of India

  • Forty eight lambs (24 each of Malpura and Mutton synthetic breeds) were weaned at 90 days of age and divided into two groups. One group of 12 lambs from each of the two breeds was maintained on low energy (52% TDN) and the other group of 12 on high energy (58% TDN) feedlot ration until 6 of the lambs attained 20 kg and the other 6 attained 25 kg live weight in each of the two groups. Daily feed intake and weekly body weights were recorded. Conventional metabolism trials were conducted on the two breeds and the two rations. The growth rate was found to be higher for lambs on high energy ration (p<0.01). The growth rate was also higher for higher target weight groups (p<0.01). The breed did not significantly affect the live weight gains. Mutton synthetic lambs required less number of days to reach 20 kg live weights but took more time to reach 25 kg target weight as compared to Malpura lambs (p<0.01). The lambs of both the breeds reached 25 kg live weight earlier on high energy than those on low energy ration (p<0.01). The dry matter intake, irrespective of breed, was significantly higher (4.57% or 93.4 g per kgW$^{0.75}$) on low energy than that on high energy ration (4.20% or 87.2 g per kg W$^{0.75}$). The digestibility coefficients of all the nutrients excepting crude fibre were significantly higher on high energy diet irrespective of the breed. Lambs on low energy ration, however, digested the crude fibre more than those on the high energy ration (p<0.05). Although there were no significant differences in the intakes of nitrogen, calcium and phosphorus, the balance of nitrogen was higher on high energy ration. The DCP and TDN values were 9.70 and 52.76% for low energy and 9.89 and 57.68 % for high energy ration, respectively. The dressing percentages on live weight basis were 50.2 on low and 51.6 on high energy ration, 50.5 at 20kg and 51.3 at 25 kg slaughter weight and 51.0 in Malpura and 50.8 in Mutton synthetic lambs, respectively. The percent of bones in the carcass was higher on low energy ration in Mutton synthetic lambs at 20 kg slaughter weight than others. It was concluded that the performance of the lambs in respect of mutton production was significantly better on high energy ration fed upto 25 kg slaughter weight with no or little breed differences.
