Effects of Calcaneus Fixation Taping on Quadriceps Angle and Lower Limb Muscles During Stairway Gait of a Patient with Patellofemoral Pain (무릎넙다리통증증후군 환자의 계단보행 시 발뒤꿈치뼈 교정 테이핑이 슬개대퇴각과 하지근육에 미치는 영향)

  • Purpose: This study was conducted to determine the structural change in knees caused by quadriceps angle and the muscle activity in lower limbs. Indirect intervention was provided by using taping for stability in the ankle joints, which affected patellofemoral pain. Methods: The subjects in this study were 20 patients with patellofemoral pain who visited ${\bigcirc}{\bigcirc}{\bigcirc} $ Hospital in Busan. The visual analogue scale measured the dynamic quadriceps angle and the degree of pain felt by the patients while walking down stairs, which was a known factor of patellofemoral pain. In addition, muscle activities in the rectus femoris, vastus lateralis, vastus medialis, tibialis anterior, peroneus longus, and biceps femoris, which affect the knees and ankles, were measured using surface electromyography. The muscle activities were converted into %RVC for this study. The data obtained in this study were analyzed with the Wilcoxon signed-rank test using the SPSS Ver. 25.0 statistical program. The significance level ${\alpha}$ was 0.05. Results: The study results showed that the pain and dynamic quadriceps angle were significantly reduced statistically when applying the calcaneus fixation taping (p<0.05). Muscle activity in the lower limbs was significantly decreased in the vastus medialis, vastus lateralis, and tibialis anterior (p<0.05). Conclusion: The summary of the study results verified that the calcaneus fixation taping reduced the pain and dynamic quadriceps angle by providing stability in the ankle joints. It also produced efficient movement due to the difference in lower-limb muscle activity.

  • Simple bone cysts are common, benign bone tumor and for the treatment, curettage with bone grafting, resection and intralesional steroid or autogenic bone marrow injections were usually performed. Simple bone cysts of the calcaneus are relatively uncommon and curettage with autologous bone grafting were proposed for management rather than intralesional steroid injections. We would like to report 9 year-old male with calcaneal simple bone cyst treated satisfactorily by minimally invasive endoscopic curettage and autologous bone grafting with review of the relevant literature.

  • Osteochodromas are the most common benign osseous neoplasm, covered by a distinct hyaline cartilage cap, originating from the physis. They involve typically metaphyseal or metadiaphyseal region of a long bone, while they involve the foot rarely. In the foot, the osteochondromas mostly occur in short tubular bones such as metatarsal bones and phalanges. They are seldom found in calcaneus. We report a rare case of osteochondroma involving calcaneus.

  • Ewing's sarcoma is rarely foundoccurs in the bones of the hands and feet. We report a case of Ewing's sarcoma of the left calcaneus in a 15-year-old girl who complained of left heel pain and swelling. An open biopsy was performed and histological examination showed the proliferation of uniform small round cells. Immunohistochemical staining for CD99 showed diffuse strong positivity in the cytoplasmic membrane of the tumor cells. After preoperative chemotherapy, a below knee amputation was performed.

Effects of Running Speed on the Foot Segments Motion at the Stance Phase (달리기 시 속도 증감에 따른 지지국면에서 발 분절 움직임 분석)

  • The aim of the present study was to investigate effect of running speed conditions on the kinematic pattern of the metatarsus, mid-foot, calcaneus. Twenty-two healthy young adults were made to run on treadmill at three different running speeds(normal speed, 9.2; slow speed, 7.4; fast speed, 11.1km/hr.) and the trajectories of the 10 reflective markers for each subject were recorded by an eight-camera motion capture system at 200 Hz. Three-dimensional angles for the foot segment in the support phase during running were calculated according to Euler's technique. Results showed that running speed did not affect the peak of the dorsi/plantar flexion, inversion/eversion, and adduction/abduction or their range of motion for each foot segment. However, when the running speed was fast, significant differences were found in the peak of the plantar flexion, eversion, and adduction and ROM(range of motion) of the dorsi/plantar flexion, inversion/eversion, and adduction/abduction between the foot segments, metatarsus, mid-foot, and calcaneus. It was proposed that the foot segment should be analyzed from a multi-segment system point of view on the basis of anatomical reference during locomotion.

  • Secondary soft tissue injuries can occur from the pressure of the displaced fragment of posterior calcaneal tuberosity in calcaneal tongue-type fractures and calcaneal tuberosity avulsion fractures. The soft tissue injury can be prevented by immediate reduction of the displaced fragments. Various techniques can be used to fix the fracture fragments, but the stability of fixation and minimal invasiveness to soft tissue should be considered. This paper reports the successful outcomes of patients with soft tissue compromises in calcaneal tongue-type fractures and calcaneal tuberosity avulsion fractures. The fixation technique of a large cannulated screw and simple cerclage wiring is believed to be a useful surgical option for the treatment of secondary soft tissue compromised calcaneal fractures.

Use of Subtalar Arthroscopy in Intra-Articular Calcaneus Fractures (관절 내 종골 골절에서 거골하 관절경술의 이용)

  • Displaced intra-articular calcaneal fractures are difficult to treat because of complex anatomy and high soft tissue complications. Various surgical approaches have been introduced to solve these problems, but the treatment remains complex. Recently, clinically and radiographically superior results were reported using a subtalar arthroscopy in reducing the posterior facet in both percutaneous and open approaches. In the percutaneous approach, the arthroscopically assistant percutaneous approach must be selected carefully for mild-to-moderately displaced fractures because of the limited view. In the open approach, there is little evidence of the utility of subtalar arthroscopy. Therefore, intraoperative arthroscopy should always be used in conjunction with fluoroscopy to achieve reduction and assess the internal fixation placement.

The histological studies on the development of cartilage canal in calcaneus of the Korean native cattle fetus (한우태자 뒷발꿈치골에서 연골관에 관한 조직학적 연구)

  • The development and change of the cartilage canals ralated to chondrocyte growth and the major histological pattern were investigated in the calcaneus of the Korean native cattle fetuses from CRL 28mm to 175mm. The results as follows; 1. The cartilage canal first appeared in the caudal subperichondral region of the sustentaculum tali of the CRL 28mm fetus, and well developed after the period. 2. The three major cartilage canals, one entered from the head of calcaneous into where achilles tendon insert and the others from the caudal region of sustentaculum tali and the calcaneal sulcus, growth distinctively. The latters participated in the ossification center prior to the former. 3. The number of cells in this canals and the size increased synchronously with chondrocytic hypertrophy. In the intermediate and deep canals, the chondrogenic change showed distinctinvely. Our results demonstrated that the cartilage canals from the lower part were important the formation of the ossification center, and the growth and development of canals associated with the hypertropic changes of chondrocytes in the calcaneus of the Korean native cattle fetus.

Limited Posterior Approach for the Surgical Treatment of Intraarticular Fracture of Calcaneus (제한적 후방 도달법을 이용한 관절내 종골 골절의 치료)

  • Purpose: To evaluate the clinical efficacy of the limited posterior approach for the surgical treatment of intraarticular fracture of calcaneus. Materials and Methods: From March 2000 to February 2006, we studied retrospectively 186 patients, 203 cases who were treated with open reduction and internal fixation through limited posterior approach and were followed up for more than 1 year. The clinical results were evaluated with Creighton-Nebraska score and circle draw test after 1 year. We checked simple AP, lateral, axial and Broden's view preoperatively and 1 year after surgery, and compared $B{\ddot{o}}hler$ angle and Gissane angle. Results: By Creighton-Nebraska score, Sanders type 2 was 86.4, type 3 was 74.3, type 4 was 62.4. And by circle draw test, type 2 was 8.9 cm, type 3 was 7.2 cm, type 4 was 5.9 cm. $B{\ddot{o}}hler$ angle and Gissane angle were $7.6^{\circ}$, $102.4^{\circ}$, and it increased to $23.5^{\circ}$, $128.6^{\circ}$ after postoperative 1 year. Conclusion: Limited posterior approach for the surgical treatment of intraarticular fracture of calcaneus was considered to an effective treatment modality.
