Purpose: Patients with serious burns are prone to chondritis due to lack of soft tissue in the auricle, which can cause severe defects in the auricular morphology. In addition, skin damage occurs frequently in the vicinity of post-burn wounds, presenting difficulties in reconstruction surgery. An auricular reconstruction has functional and cosmetic significance. The aim of this study is to develop appropriate reconstruction methods for auricular defects. Methods: Thirty seven patients, who were treated for auricular defects from 2005 to 2009, were enrolled in this study. A local flap, multiple regional flaps and cartilage framework with or without a temporal fascial flap were applied in reconstruction surgery according to the location of the auricular defect. Results: The age of the subjects ranged from 11 to 56. Some subjects had defects that cover more than half of the helical rim with most exhibiting post-burn scars in the vicinity, for whom a multiple regional flap was used. A single use of a tubed flap was sufficient for subjects with defects that covered less than half of the helical rim. A regional flap was also used for reconstruction in subjects with defects covering both the helical rim and antehelix. Conclusion: Achieving satisfactory results from the skin flaps and skin grafts for post-burn auricular defects in both functional and cosmetic aspects is a difficult task. Therefore, selecting an appropriate surgical method through proper diagnosis of the auricular defect and the state of the available skin in the vicinity is essential.

With the advent of microvascular free-tissue transfer, this single stage resurfacing method for large scar and soft tissue defects around the wrist in the patients of electrical burn has distinctive advantage over the conventional multistage pedicle-flap transfer. Between 1992 and 1996, we treated 9 cases of 8 patients who had large scar around the wrist due to old electrical burn with free flaps as a preparation of staged tendon graft. Mean age was 30.3 years and average scar area was $6{\times}11cm$. The length of time the injury and free flaps was 9 months on an average. Prior to the free flap, we performed the angiography to all patients in order to evaluate the circulation of the forearm and hand and to choose the recipient vessel. In all cases, proximal ulnar arteries in the forearm remained intact and all radial arteries remained intact in 8 of 9 cases on angiogram. The interosseous arteries were well visualized in all cases. We used the ulnar arteries as a recipient artery. The types of flaps used were f scapular cutaneous flaps, 2 dorsalis pedis flaps and a radial forearm flap. Flap survial was 100 percents with satisfactory functional and cosmetic results. Free flaps using ulnar artery as a recipient artery is one of the useful reconstruction methods for the resurfacing of large scar around the wrist in the patients of old electrical burn.

Configurations of the flame and combustion-field were caught by both high-speed camera and thermo-graphic camera, and those images were analyzed in detail through a post-processing. As a result, the combustion-field grew and reaction-temperature rose due to the strengthening of combustion reaction with the increasing flow-rate of carrier-gas. In addition, a phenomenon of flame flickering was discussed through the comparative analysis of the variational behaviors between the visible flame and IR (Infrared) flame-field.

In addition, the fuel consumption was measured using the balance during the combustion reaction. As a result, the consumption of atomized fuel increased with the increasing flow-rate of carrier-gas, but any correlation between the air/fuel ratio and carrier-gas flow-rate was not found at the low flow-rate condition. Also, the combustion-field grew and reaction-temperature rose due to the strengthening of combustion reaction with the increasing flow-rate of carrier-gas and power consumption of ultrasonic oscillator.
Burns are one of the most devastating forms of trauma and wound healing is a complex and multicellular process, which is executed and regulated by signaling networks involving numerous growth factors, cytokines, and chemokines. Recombinant human granulocyte macrophage colony-stimulating factor (rhGM-CSF) was specifically produced from rice cell culture through use of a recombinant technique in our laboratory. The effect of rhGM-CSF on promotion of deep second-degree burn wound healing on the back skin of a hamster model was evaluated through a randomized and double-blind trial. As macroscopic results, hamster skins of the experimental groups showed earlier recovery by new epidermis than the control groups. Immunohistochemical reactions of proliferating cell nuclear antigen and transforming growth factor-b1, which are indicators of cell proliferation, were more active in the experimental group, compared with the control group. On electron microscopy, basal cells in the epidermis of the experimental group showed oval nuclei, prominent nucleoli, numerous mitochondria and abundant free ribosomes. In addition, fibroblasts contained well-developed rough endoplasmic reticulum with dilated cisternae. Bundles of collagen fibrils filled the extracellular spaces. Particularly, ultrastructural features indicating active metabolism for regeneration of injured skin at 15 days after burn injury, including abundant euchromatin, plentiful free ribosomes, and numerous mitochondria, were observed. These findings suggest that use of rhGM-CSF could result in accelerated deep second-degree burn wound healing in animal models.
Objective The purpose of this review was to investigate exercise therapy by sasang constitution and to offer information for further research. Method The literature was investigated from KMBASE, KISS, ScienceON, JSCM, DBpia, Pubmed. The searching term were sasang, sasang constituion, sasang type, taeyangin, taeeumin, soyangin, soeumin, exercise, physical therapy, physical activity, and so on. Studies were extracted and analyzed exercise by sasang constitution, in addition, were also collected physiological differences related to sasang constitution, exercise ability, exercise response and effects, and exercise contraindications. Result & Conclusion It is helpful for Taeyangin to lower the energy through exercises that train the lower body and deep breathing, and there is no need to exercise excessively. Taeeumin tend to be obese compared to other constitutions. In order to be physically healthy, it is good to sweat a lot through regular exercise. It is good to do aerobic exercise that burns a lot of calories every day. Soyangin has a weak lower body, so it is good to exercise to reinforce the lower body. Soyangin has weak stamina, and since static exercise easily loses interest, it is helpful to do exciting exercise, and it can be performed at normal intensity. Soeumin are statistically less physically active. So soeumin need to improve exercise habits, but because physical strength is on the weak side, it is more appropriate to do an exercise that activates the body parts evenly rather than a strong one.

Background: Scalding burn injuries can occur in everyday life but occur more frequently in young children. Therefore, it is important to develop more effective burn treatments. Objectives: This study examined the effects of bee venom (BV) stimulation on scalding burn injury-induced nociception in mice as a new treatment for burn pain. Methods: To develop a burn injury model, the right hind paw was immersed temporarily in hot water (65℃, 3 seconds). Immediately after the burn, BV (0.01, 0.02, or 0.1 mg/kg) was injected subcutaneously into the ipsilateral knee area once daily for 14 days. A von Frey test was performed to assess the nociceptive response, and the altered walking parameters were evaluated using an automated gait analysis system. In addition, the peripheral and central expression changes in substance P (Sub P) were measured in the dorsal root ganglion and spinal cord by immunofluorescence. Results: Repeated BV treatment at the 2 higher doses used in this study (0.02 and 0.1 mg/kg) alleviated the pain responses remarkably and recovered the gait performances to the level of acetaminophen (200 mg/kg, intraperitoneal, once daily), which used as the positive control group. Moreover, BV stimulation had an inhibitory effect on the increased expression of Sub P in the peripheral and central nervous systems by a burn injury. Conclusions: These results suggest that a peripheral BV treatment may have positive potency in treating burn-induced pain.
Background: This paper was to investigate Korean medical disputes through the cases of asking Korean Acupuncture and Moxibustion Medicine Society(KAMMS) for medical consultation Methods: In this study, it was investigated 66 medical disputes requested to KAMMS for medical consultation from April, 2013 to December, 2017. The cases of disputes were classified according to the year, month, sex, age, area, original disease, treatment method and type of occurrence. Results : There were 66 cases from April, 2013 to December, 2017 that able to investigate. There were no annual increases and decreases or monthly trends in medical disputes. In characteristics of patients, female (53.03%) were more likely than male, and the age distribution was in in 50s (24.24%). It occurred in area, followed by Gyeongsang (33.33%), Gyeonggi-Incheon (30.30%), and Seoul (13.64%). The majority of original disease was musculoskeletal disease (81.82%), and treatment methods that have been assumed to cause medical disputes were 38 cases (57.58%) of acupuncture, followed by 12 cases (18.18%) of combined treatment. Analysis of occurred disease showed that 23 cases (34.85%) of musculoskeletal diseases were the most common, followed by 17 cases (25.76%) of infection. When original disease was musculoskeletal disease, the greatest type of occurrence was musculoskeletal disease (30.30%), but there was no statistical significance. Musculoskeletal disease was common after acupuncture (28.80%), and infection was common after combined treatment (12.10%). It was statistically significant. Although no statistically significant, pharmacopuncture tended to cause the immune response, while moxibustion tended to cause burns.

Purpose: This study conducts a comparative evaluation of the skin dose in CyberKnife (CK) and Helical Tomotherapy (HT) to predict the accurate dose of radiation and minimize skin burns in head-and-neck stereotactic body radiotherapy. Materials and Methods: Arbitrarily-defined planning target volume (PTV) close to the skin was drawn on the planning computed tomography acquired from a head-and-neck phantom with 19 optically stimulated luminescent dosimeters (OSLDs) attached to the surface (3 OSLDs were positioned at the skin close to PTV and 16 OSLDs were near sideburns and forehead, away from PTV). The calculation doses were obtained from the MultiPlan 5.1.2 treatment planning system using raytracing (RT), finite size pencil beam (FSPB), and Monte Carlo (MC) algorithms for CK. For HT, the skin dose was estimated via convolution superposition (CS) algorithm from the Tomotherapy planning station 5.0.2.5. The prescribed dose was 8 Gy for 95% coverage of the PTV. Results and Conclusions: The mean differences between calculation and measurement values were $-1.2{\pm}3.1%$, $2.5{\pm}7.9%$, $-2.8{\pm}3.8%$, $-6.6{\pm}8.8%$, and $-1.4{\pm}1.8%$ in CS, RT, RT with contour correction (CC), FSPB, and MC, respectively. FSPB showed a dose error comparable to RT. CS and RT with CC led to a small error as compared to FSPB and RT. Considering OSLDs close to PTV, MC minimized the uncertainty of skin dose as compared to other algorithms.
We conducted a nationalwide questionnaire to investigate accidental experiences and injuries at flashover or flame-fire for active firefighters in the line of duty. A total of 794 firefighters participated in this survey (764 males, 27 females, and 3 respondents; $39.2{\pm}8.4yr$ in age, $173.7{\pm}5.1cm$ in height, $73.4{\pm}8.6kg$ in body mass). The results showed that high risk circumstances the most frequently experienced while firefighting in Korea was flashover followed by backdraft, rollover and flameover. At the high risk circumstances the most frequently-experienced injuries were bruise, stabs and burns. Firefighters hoped to reduce the total mass of personal protective equipment (PPE), improve the mobility of the PPE and dexterity of protective gloves, so that they could escape from the high risk circumstances as fast as possible. In particular, requirements for improvement on protective gloves were greater than those on other PPE. The present study suggested that the need for improvements on the current firefighters' PPE to cope with emergency high risk situations in terms of PPE mass reduction and mobility.
