  • A bronchus-associated lymphoid tissue(BALT) lymphoma of the lung is a rare disorder of patients with Sj$\ddot{o}$gren's syndrome. A 49-year-old woman was admitted for an evaluation of exertional dyspnea and general weakness which had persisted for two years. The patient had suffered from dry mouth and dry eyes for five years. The physical examinations showed a coarse breath sound with inspiratory crackles on the whole lung field, particularly on the both basal lungs. The laboratory data disclosed high titers of anti-nuclear antibodies, and anti-SSA (Ro), and anti-SSB (La) antibodies. Chest radiographs demonstrated the presence of bilateral, diffuse, reticulonodular densities in both lungs. Thin-section CT scans showed diffusely distributed mosaic pattern of an inhomogeneous attenuation extending over the entire lung zone. The histological findings from an open-lung biopsy specimen revealed an accumulation of lymphoid cells around the bronchioles and an extension of malignant lymphoma cells from the bronchiolar epithelium toward the alveolar space. Immunohistochemically, the neoplastic cells reacted positively to the CD 20 antigen and were focally positive for the UCHL 1 antigen. The histological diagnosis was consistent with a low grade marginal zone B-cell lymphoma originating in the BALT. Here, we present a case of a histologically proven BALT lymphoma of the lung in a patient with primary Sj$\ddot{o}$gren's Syndrome.

Complicated Anterior Mediastinal Teratoma -Report Of Two Cases- (합병증을 동반한 종격동 기형종 -2예 보고-)

  • We have experienced two cases of complicated teratoma in anterior mediastinum. One case, 1-year-old female, has a huge anterior mediastinal teratoma involving almost all of the left pleural cavity with atelectasis of the left lung. The other case, 12-year-old female, has a anterior mediastinal teratoma with fistula between the teratoma and the bronchus of anteromedial basal segment of left lung. The bronchus has a bronchiectatic change with cell infiltration. We performed exploratory thoracotomy with complete excision of tumor mass in both cases and with additional resection of anteromedial basal segment of the left lung due to inflammation and necrosis in latter case. The patients had uneventful postoperative courses and were discharged in good condition. Histopathologic diagnosis of these were benign teratoma consist of skin, skin appendages, cartilage, connective tissue, and gut.

A clinical pathogenetic study of broncholithiasis (기관지 결석증의 임상적 연구)

  • Broncholithiasis is defined as a cor9ition in which a concretion is present within a bronchus or a cavity in the lung communicating with a bronchus. The usual causes of broncholithiasis are known as tuberculosis, histoplasmosis, silicosis, aspirated calculi, and a few fungal infections. It is generally accepted that the constant motion created by respiration and beating of the heart may cause the peribronchial calcified lymph node to erode into the tracheobronchial tree and to form broncholith. After the analysis of our 6 cases of broncholithiasis which were treated surgically in the Department of Thoracic Surgery, Seoul National University Hospital from 1960 to December, 1985, we could suggest that intrinsic formation of calculi should be regarded as the pathogenesis of broncholithiasis in addition to the extrinsic formation of calculi.

  • The mucoepidermoid carcinoma of the bronchus is very rare neoplasm. The histological appearance is an intimate mixture of mucous-producing cell and epidermoid cell. The malignant potency of the tumor was determined by histologic pattern. Bronchoplasty techniques represent the ideal form of excisional therapy for benign endobronchial tumor as well as tumors of low-grade malignant potential, such as bronchial adenoma, and for repair of traumatic airway injuries and benign stricture and selected group of patients with carcinoma of the lung. We experienced a case of-low-grade mucoepidermoid carcinoma in a patient of 21-year old male who has been suffered from hemoptysis episodes for several years. The sleeve resection of left main bronchus and left lower lobectomy due to bronchiectatic change were carried out. The patient are being followed up without specific problem.

Bronchial Rupture by Blunt Chest Trauma -a case- (외상성 기관지 단절의 수술 치험 -1례-)

  • Rupture of the main bronchus followed by blunt chest trauma is comparatively very rare. Early recognition of bronchial rupture and emergency thoracostomy and management is essential for reducing of morbidity and mortality and late complications. This case was 11 years old female who was a primary school student. The patient was sustained a crushing injury to her right hemithorax by traffic accident and had been taken emergency closed thoracostomy at her second intercostal space, midclavicular line at emergency room. In the course of the next 2 hours, the girl`s condition remained critical with tension pneumothorax and abnormal arterial blood gas analysis. Induction of anesthesia started 3 hours after the accident. During the general anesthesia, cardiac arrest was occurred and cardiac resuscitation was performed. Right upper lobectomy and end-to-end anastomosis of ruptured right main bronchus was performed. Postoperative course was satisfactory.

  • The glomus tumor is a distinctive benign neoplasm with a small painful nodule, occurs most commonly in extremities but may be found elsewhere in the body. Its occurrence in the trachea or lung parenchyme has been recognized with extreme rarity and there has not been any report, to our knowledge, of its occurrence in the main bronchial glomus tumor. We report a case of a glomus tumor in the left main bronchus in a 67-year-old man who was presented with blood-tinged sputum and dyspnea, which was completely relieved by surgical resection.

  • Mucoepidermoid cancer of the bronchus in childhood is extremely rare. These tumors generally produce symptoms of airway obstruction, often misdiagnosed as unresolved tuberculosis. Distant metastasis is an uncommon finding of this malignancy, therefore complete surgical resection is treatment of choice. The prognosis of these tumors correlates with the histologic grade of the tumor. We report our clinical experience of mucoepideromoid cancer in a 10-year-old child who visited our hospital with symptoms of recurrent cough and fever. The patients underwent successful removal of tumor by bilobectomy via thoracotomy after bronchoscopic biopsy.

  • Pulmonary arteriovenous fistula can be either congenital or acquired. The vast majority are congenital, and about 60% have been associated with hereditary hemorrhagic telangiectasia [Rendu-Osler-Weber disease . Secondary or acquired pulmonary arteriovenous fistula occurs with trauma, schistosomiasis, long-standing hepatic cirrhosis, metastatic carcinoma, and actinomycosis. Pulmonary hemorrhage secondary to acquired pulmonary arteriovenous fistula is a rare event associated with mortality. We have experienced 64 year-old female patient with the hemoptysis secondary to acquired pulmonary arteriovenous fistula due to the infection of pulmonary parasite. The chest PA and CT scan was showed calcified nodule to the distal portion of lateral segmental bronchus of RML. The bronchial angiogram was demonstrated slightly hypertrophied bronchial artery supplying RML bronchus and the presence of hypervascularization around the calcified nodule, rapid A-V shunting is noted by fluoroscopy. The patient was successfully treated by the right middle lobectomy.

A Bronchial Lipoma: Occurrence at an Unusual Site and Its Successful Bronchoscopic Resection with an Electrosurgical Snare

  • Primary tracheobronchial lipoma is an extremely rare entity, the diagnosis of which is often missed initially. Cases are generally diagnosed late after initial treatment for asthma and bronchitis. We report a case of a 42-year-old man with a left main bronchus lipoma that caused near-total obstruction. The lipoma was treated by bronchoscopic resection with an electrosurgical snare and cryoablation.

  • An Athma is a hypersensitive reaction of bronchus on various stimulus. It has a cough and laboring breath caused by narrowing of inflammatory bronchus. In this study, the three asthma patients classified by Sasang constitutional medicine had a notable medical effects. So we report the healing process and results of these three patients in this study.
