Complete Transsection of Right Main Bronchus by Blunt Trauma -A Case Report- (둔상에 의한 우측 주기관지 완전 절단 치험 1례)

  • Main bronchial injury after blunt trauma is very rare in all bronchial injuries and the pathogenesis is variable and not well known in everycases. We report a case of complete transsection of right main bronchus by blunt trauma. This 24-year old patient was transferred from a local hospital with a chest tube. Because of the severe subcutaneous emphysema and tension pneumothorax, we inserted one more chest tube resulting no obvious interval change. With the impression of bronchial injury, we performed an exploratory thoracotomy. We couldn't proceed bronchoscopy in the operation room because of his unstable vital sign. After opening of the chest wall, we could identify completely transsected right main bronchus. We anastomosed the bronchus with 4~0 Vicryl interruptedly. After operation, the patient was recovered without any complication.

  • An accessory cardiac bronchus (ACB) is a rare congenital anomaly of the trachobronchial tree. It rarely presents with symptoms, and is usually diagnosed incidentally by chest CT, bronchoscopy, or during surgery. We experienced a case of accessory cardiac bronchus found incidentally in the bronchus intermedius with lung cancer in the right lower lobe, and surgically removed.

  • Prolonged air leakage is a major cause of morbidity in pneumothorax. When conservative management is not effective, surgery should be performed. However, surgery is not appropriate in patients with low pulmonary function. In these patients, occlusion of the airway with endobronchial blockers may be attempted under bronchoscopy. We treated two patients with prolonged air leakage using endobronchial Watanabe spigots under fibrobronchoscopy.

  • As a safety measure, dentures are routinely removed before surgery. Aspiration of a denture could be catastrophic, with medicolegal implications. Foreign body aspiration is uncommon in adults; however, aspirations may remain asymptomatic and undiagnosed for long periods of time. We report an adult male who presented with a cough for more than 6 months. On radiography, a foreign body was found migrating within the tracheobronchial tree from one mainstem bronchus to the other, at different time points. The foreign body was later found to be a portion of his denture. The aspiration may have occurred at the time of a surgical procedure.

  • From May 1986 to May 1992, 72 patients were diagnosed and operated for primary lung cancer, among them 65 patients were clinically evaluated at the department of Thoracic & Cardiovascular Surgery, Masan Koryo General Hospital. 1. There were 52 males 13 females[M:F=4:1], and 5th, 6th decade of life[72%] was peak incidence. 2. The preoperative diagnosis and its positive rate were sputum cytology 35%, bronchoscopy 47%, pleural effusion cytology 80%, and pleural biopsy 50%. 3. The classification histologic types were squamous cell cancer 71%, adenocarcinoma 17%, undifferentiated cell carcinoma 4.6%, and staging classification were Stage I 31%, Stage II 22%, Stage IIIa 26%, and Stage IIIb 20%. 4. The operative methods were lobectomy 52%, pneumonectomy 36%, and open biopsy 12%, and operability was 89%, resectability was 88%. 5. The postoperative complications developed 13 patients[22%], and operative mortality was 5%. 6. The overall actuarial survival rate was 1year 70%, 2year 42%, 3year 32%, 4year 26%, and 5year 22%, according to Stage 5year survival rate was Stage I 37%, Stage II 22%, Stage IIIa 3year 12%, Stage IIIb 2year 23%. And according to operative method lobectomy 23%, pneumonectomy 19%.

Bronchial obstruction following rupture by blunt trauma (외상성 기관지 단절과 폐쇄의 수술 치험례)

  • Rupture of the main bronchus following closed injury to the chest is a comparatively rare accident. The late recognition of this injury is attended by difficulties in management which may endanger life or expose the patient to distressing months and years of arduous therapy. This case was a 17 year old female who was a high school student. The patient had sustained a crushing injury to her right hemithorax and had been taken to an emergency hospital where right closed thoracostomy had been performed for a tension pneumothorax. She improved following this procedure but massive atelectasis of the right lung developed on the 13th day after trauma and transferred to our hospital. Bronchoscopy disclosed granulation tissue in the right main stem bronchus and end to end anastomosis of the bronchus was performed. Postoperative course was uneventful.

  • A 31-year-old female visited O.P.D. of internal medicine, Kyung Hee Medical center, with the chief complaint of generalized weakness and headache. She took a routine chest P-A and there was a 6.5 x 7.5 cm sized round mass in the right middle lung field. She admitted to the Dept. of Thoracic and Cardiovascular Surgery under the impression of malignant bronchogenic carcinoma with the remit of bronchoscopy and sputum cytology and tomogram. Middle and lower lobectomy of right lung was performed and postoperative surgical biopsy revealed out the plasma cell granuloma. The plasma cell granuloma may occur as a solitary nodule in the lung or be associated with systemic disease, plasma protein imbalance, or nonspecific local inflammatory reaction . More than two third of the reported patients were less than 30 year of age. Grossly the lesion appears reddish-brown and microscopic features include pallisade or a cartwheel distribution of plasma cells with Russel bodies and amyloid. Local excision or lobectomy has been curative in most cases.

  • 의료 영상에서 관심 있는 부위를 3차원으로 재구성하여 보는 것은, 정확한 진단을 위해서 매우 중요하다. 이러한 3차원 재구성을 위해서는 관심 있는 영역의 분할이 필수적인 선행작업이다. 본 논문에서는 관도계 기관의 분할을 위해서 슬라이스 영상의 정보를 이용한 3차원 영역 성장법을 제안한다. 제안된 방법은 2차원 슬라이스 영상에서 영역 성장법에 의해 영역을 확장시키고, 그 이웃한 슬라이스들에 씨앗점을 전달하여 재귀적으로 3차원 체적을 확장하여 영상을 분할한다. 이때, 이웃한 슬라이스간의 영역의 크기의 제약을 이용하여 새나감을 방지한다. 제안된 방법을 기관지의 분할에 적용한 결과, 새나감 없이 뾰족한 가지들까지도 성공적으로 분할했으며, 튜브의 중심 축이 고차원 곡선인 경우에도 성공적으로 분할했다.

  • Unilateral hypoplasia of the pulmonary artery is an uncommon anomaly, which commonly develops in combination with congenital cardiovascular defects such as tetralogy of Fallot, patent ductus arteriosus and septal defect of atrium or ventricle, but may also present as an isolated lesion. We have recently experienced a case of the left pulmonary artery hypoplasia in adult by chance of during the general health screen, which diagnosed by chest X-ray, chest spiral CT, lung perfusion and ventilation scan, digital substraction angiogram and bronchoscopy, then presented hereby with the review of relevant literature.

A Case of Leiomyoma of the Trachea (기관에 발생한 평활근종 1예)

  • Leiomyoma of the trachea is extremely rare but important to recognize early because they are curable. A case of leiomyoma of the trachea is described in 340-year-Old woman. She was admitted for dyspnea, coughing and sputum for 3 years. Under the impression of asthma she was treated but not improved. Chest CT showed an intraluminal tracheal mass just above the carina. Fiberoptic bronchoscopy revealed a round intraluminal mass on the membranous portion of trachea 4cm above the carina. The tumor was excised by wedge resection and end-to-end anastomosis of the trachea was performed. The pathologic examination revealed leiomyoma of the trachea. We report a case of leiomyoma of the trachea which was successfully resected.
