CT Fractional Flow Reserve for the Diagnosis of Myocardial Bridging-Related Ischemia: A Study Using Dynamic CT Myocardial Perfusion Imaging as a Reference Standard

  • Objective: To investigate the diagnostic performance of CT fractional flow reserve (CT-FFR) for myocardial bridging-related ischemia using dynamic CT myocardial perfusion imaging (CT-MPI) as a reference standard. Materials and Methods: Dynamic CT-MPI and coronary CT angiography (CCTA) data obtained from 498 symptomatic patients were retrospectively reviewed. Seventy-five patients (mean age ± standard deviation, 62.7 ± 13.2 years; 48 males) who showed myocardial bridging in the left anterior descending artery without concomitant obstructive stenosis on the imaging were included. The change in CT-FFR across myocardial bridging (ΔCT-FFR, defined as the difference in CT-FFR values between the proximal and distal ends of the myocardial bridging) in different cardiac phases, as well as other anatomical parameters, were measured to evaluate their performance for diagnosing myocardial bridging-related myocardial ischemia using dynamic CT-MPI as the reference standard (myocardial blood flow < 100 mL/100 mL/min or myocardial blood flow ratio ≤ 0.8). Results: ΔCT-FFRsystolic (ΔCT-FFR calculated in the best systolic phase) was higher in patients with vs. without myocardial bridging-related myocardial ischemia (median [interquartile range], 0.12 [0.08-0.17] vs. 0.04 [0.01-0.07], p < 0.001), while CT-FFRsystolic (CT-FFR distal to the myocardial bridging calculated in the best systolic phase) was lower (0.85 [0.81-0.89] vs. 0.91 [0.88-0.96], p = 0.043). In contrast, ΔCT-FFRdiastolic (ΔCT-FFR calculated in the best diastolic phase) and CT-FFRdiastolic (CT-FFR distal to the myocardial bridging calculated in the best diastolic phase) did not differ significantly. Receiver operating characteristic curve analysis showed that ΔCT-FFRsystolic had largest area under the curve (0.822; 95% confidence interval, 0.717-0.901) for identifying myocardial bridging-related ischemia. ΔCT-FFRsystolic had the highest sensitivity (91.7%) and negative predictive value (NPV) (97.8%). ΔCT-FFRdiastolic had the highest specificity (85.7%) for diagnosing myocardial bridging-related ischemia. The positive predictive values of all CT-related parameters were low. Conclusion: ΔCT-FFRsystolic reliably excluded myocardial bridging-related ischemia with high sensitivity and NPV. Myocardial bridging showing positive CT-FFR results requires further evaluation.

Eletromyographic Activities of Trunk and Lower Extremity Muscles During Bridging Exercise in Whole Body Vibration and Swiss Ball Condition in Elderly Women

  • The purpose of this study was to compare the trunk and lower extremity muscle activity induced by six different conditions floor, intensity 0, 1, 3, 5 of whole body vibration (WBV), and Swiss ball during bridging exercise. Surface electromyography (EMG) was used to measure trunk and lower extremity muscles activity. Ten elderly women were recruited from Hong-sung Senior Citizen Welfare Center. The collected EMG data were normalized using reference contraction (during floor bridging) and expressed as a percentage of reference voluntary contraction (%RVC). To analyze the differences in EMG data, the repeated one-way analysis of variance was used. A Bonferroni's correction was used for multiple comparisons. The study showed that EMG activity of the rectus abdominis, external oblique, internal oblique, erector spinae and rectus abdominis muscles were not significantly different between six different conditions of during bridging exercise (p>.05). However, there were significantly increased EMG activity of the rectus femoris (p=.034) in the WBV intensity 0, 1, 3, and 5 conditions compared with the floor bridging condition. EMG activity of the medial gastrocnemius were significantly increased in the WBV intensity 0, 1, 3, 5 and Swiss ball conditions compared with the floor bridging condition. Future studies are required the dynamic instability condition such as one leg lifting in bridging.

무게부하를 이용한 교각운동이 건강한 성인 남성의 몸통 및 다리 근육의 활성화에 미치는 영향 (Effects of Bridging Exercise Using Weight Loads on Trunk and Lower Limb Muscles Activity in Healthy Adult Males)

  • Purpose: This study observed the activity of trunk and lower limb muscles during a modified bridging exercise with various weight loads. Methods: The participants in this study consisted of 15 male adults. The muscle activity of the elector spinae, rectus abdominis, gluteus maximus, gluteus medius, vastus medialis, vastus lateralis, tibialis anterior, and soleus muscles was measured with an EMG-8 system during a modified bridging exercise performed with various weight loads (indicated as percentage of body weight). Differences in muscle activity during the bridging exercise according to the weight load applied were analyzed using a one-way ANOVA, and post hoc analysis was performed using LSD. Statistical significance was accepted at a p-level of 0.05. Results: When the subjects performed the modified bridging exercise with various weight loads, the muscle activity of the gluteus maximus and vastus medialis peaked at a load of 0.5%. The activity of the gluteus medius showed a remarkable difference when the modified bridging exercise was performed at loads of 0% and 0.5%, 0% and 1%. In addition, the activity of the vastus medialis showed a remarkable difference between modified bridging exercises performed at a load of 0% and that performed at a load of 0.5%. Conclusion: The results suggest that performing modified bridging exercises with a load of 0.5% of body weight results in significant differences in the activity of the gluteus medius and vastus medialis muscles. Thus, it is suggested that performing the modified bridging exercise at 0.5% of body weight may selectively strengthen the gluteus medius and vastus medialis muscles.

Effect of surfactant adsorption on the rheology of suspensions flocculated by associating polymers

  • Associating polymers act as flocculants in colloidal suspensions, because the hydrophobic groups (hydrophobes) can adsorb onto particle surfaces and create intermolecular cross-linking. The steady-shear viscosity and dynamic viscoelasticity were measured for suspensions flocculated by multichain bridging of associating polymers. The effects of surfactant on the suspension rheology are studied in relation to the bridging conformation. The surfactant molecule behaves as a displacer and the polymer chains are forced to desorb from the particle surfaces. The overall effect of surfactant is the reduction of suspension viscosity. However, the additions of a small amount of surfactant to suspensions, in which the degree of bridging is low, cause a viscosity increase, although the number of chains forming one bridge is decreased by the forced desorption of associating polymer. Since the polymer chains desorbed from one bridge can form another bridge between bare particles, the bridging density over the system is increased. Therefore, the surfactant adsorption leads to a viscosity increase. The surfactant influences the viscosity in two opposing ways depending on the degree of bridging.

  • Numerical experiments on biological interfacial layer, DEJ by finite element software ABAQUS have been conducted to study its fracture behavior including crack bridging / arresting characteristics in the model. Crack growth simulation has been carried out by numerical tool, XFEM, devoted to study cracks and discontinuities. The fracture toughness of DEJ has been estimated before and after crack bridging. The implications of bridging in numerical study of fracture behavior of DEJ-like biological interface have been discussed. It has been observed that the results provided by the numerical studies without proper accommodation of bridging phenomenon can mislead. This study can be helpful for understanding the DEJ-like biological interface in terms of its fracture toughness, an important material characteristics. This property of the material is an important measure that has to be taken care during design and manufacturing processes.

교각운동 시 복부 드로잉-인 방법이 요부 전만과 체간 및 하지의 근 활성도에 미치는 영향 (The Influence of Abdominal Drawing-In Maneuver on Lumbar Lordosis and Trunk and Lower Extremity Muscle Activity During Bridging Exercise)

  • An abdominal drawing-in maneuver (ADIM) with a pressure biofeedback unit can be used to prevent excessive lumbar lordosis during bridging exercise. Therefore, in this research, the effects of an ADIM on lumbar lordosis and lower extremity muscle activity during bridging exercise were investigated in thirty healthy adults. Surface electromyography (EMG) and VICON system were used to collect kinematic data and muscle activity, respectively. A paired t-test was used to determine a statistical significance. The results showed as follows: (1) When performing bridging exercise with an ADIM, the height of the anterior superior iliac spine and greater trochanter decreased significantly (p<.05). (2) When performing bridging exercise with an ADIM, the trunk extension angle and pelvic angle increased significantly (p<.05). (3) When performing bridging exercise with an ADIM, the EMG signal amplitude increased significantly in the rectus abdominis, internal oblique abdominis, external oblique abdominis, medial hamstring, and lateral hamstring (p<.05). (4) When performing bridging exercise with an ADIM, the EMG signal amplitude decreased significantly in the erector spinae (p<.05). From the result of this research, an ADIM trained with pressure biofeedback unit during bridging exercise is effective to prevent excessive contraction of erector spinae, to limit excessive motion of pelvis from sagittal plane and to increase muscle activity of abdominal muscles and hamstring muscle.

Effect of Sprinter Pattern Bridging Exercise using Theraband on Activation of Lower Extremity and Abdominal Muscle

  • Objective: The purpose of this study was to investigate the effect of sprinter pattern bridging exercise using theraband on activation of lower extremity and abdominal muscle and to find out postures that can effectively improve abdominal and lower extremity muscle strength and increase abdominal stability. Methods: This study was designed as a cross-sectional study. The following research was done with applicants attending S university in Seoul to compare the difference in muscle activity between one-leg-Support bridging exercise and sprinter-pattern bridging exercise using theraband. For 48 study participants, we first measured their MVC. Then, we applied one-leg-support bridging exercise and sprinter-pattern bridging exercise at random order. These data were expressed as the percentage of maximal voluntary contraction (%MVC).Electromyography analysis was performed by measuring the external obliques, internal obliques, biceps femoris, and gluteus maximus. Results: There was a statistically significant increment of muscle activity in external and internal oblique muscle(p<0.001)by sprinter-pattern bridging exercise using theraband. On the lower body, statistically significant increment of muscle activity in biceps femoris and gluteus maximus was found(p<0.05). On the other hand, on erector spinae, there was statistically significant decrease in muscle activity(p<0.05). Conclusions: Efficient treatment is expected when sprinter-pattern bridging exercise using theraband is applied clinically.For patients with chronic knee and ankle pain who have difficulty bearing weight, including low back pain and internal rotation of the femur, starting with a low weight bearing, we think it will be helpful in planning systematic training aimed at progressively strengthening the lower extremities.

슬관절 각도에 따른 교각운동이 하지근 활성도에 미치는 영향 (The Effect of Lower Extremity Muscle Activity on Bridging Exercise According to the Knee Joint Angle)

  • Purpose : The purpose of this study was to assess the effects of the lower extremity muscle activity on bridging exercise according to the knee joint angle. Methods : Twenty-five healthy adults volunteered to participate in this study. Subjects were required to complete following four bridging exercises; knee joint flexion $120^{\circ}$, $90^{\circ}$, $60^{\circ}$, $45^{\circ}$. Surface electromyography from selected lower extremity muscles was normalized to maximum voluntary isometric contraction. Muscle activity was measured by QEMG-4 system (LXM 3204, Laxtha Korea). A repeated measures of one-way ANOVA was used to determine the influence of bridging exercise on muscle activity for each muscle and descriptive statistics was used to determine muscle ratio. Results : The biceps femoris of all bridging exercises showed significantly(p<.05). The vastus medialis and lateralis of all bridging exercises showed significant excepted $120^{\circ}$(p<.05). The rectus femoris of all bridging exercises showed no significant. Median of vastus medialis/rectus femoris ratio of $120^{\circ}$ was 2.03, $90^{\circ}$ was 2.16, $60^{\circ}$ was 2.67, $45^{\circ}$ was 4.10. Median of vastus lateralis/rectus femoris ratio of $120^{\circ}$ was 1.70, $90^{\circ}$ was 1.70, $60^{\circ}$ was 2.08, $45^{\circ}$ was 2.58. Median of vastus medialis/vastus lateralis ratio of $120^{\circ}$ was 1.26, $90^{\circ}$ was 1.50, $60^{\circ}$ was 1.52, $45^{\circ}$ was 1.47. Conclusion : Angular motion decreasing with knee joint flexion made increase biceps femoris and vastus medialis activation. This result will be use knee joint stabilizing exercises during bridging or unstable surface training and biceps femoris strength training.

  • 에너지 저장 방식은 양질의 전원이 요구되는 중요 부하를 보호하기 위해 꾸준히 개발되어 왔다. 그 중 현재 가장 관심 있는 분야 중 하나인 것은 bridging power 시스템이다. 이 bridging power 시스템은 25kW에서부터 2,500k범위까지 다양한 제품이 용도에 맞게 출시되고 있다. Bridging Power 방식들은 4가지로 크게 요약될 수 있다. 즉, 저/고속의 플라이휠, 배터리, ultra capacitors 및 magnetic storage 기술 등이다. 이 논문은 bridging power 시스템의 개요에 대해 설명하고, 주요 기능 감시 시스템의 특징 및 장/단점에 대해 기술했으며, 현재 EPRI와 EPRI PEAC과 함께 TVA Public Power Institute가 제공하는 R&D 활동에 대해 요약하였다.

교각운동 시 공 적용이 체간근 활동에 미치는 영향 (Effect of Bridging Stabilization Exercises on Trunk Muscles Activity On and Off a Swiss Ball)

  • The purpose of this study was to assess the effect of bridging stabilization exercises on trunk muscles activity on and off a Swiss ball. 20 healthy university students volunteered to participate in this study. Subjects were required to complete following four exercises: exercise 1, single bridging exercise; exercise 2, feet on ball bridging exercise; exercise 3, calf on ball bridging exercise; exercise 4, back on ball bridging exercise. Surface electromyography from selected trunk muscles was normalized to maximum voluntary isometric contraction. A repeated measures of ANOVA with post-hoc Bonferroni's correction was used to determine the influence of exercise type on muscle activity for each muscle and descriptive statistics was used to determine local/global muscle ratios. The rectus abdominis of exercise 4 showed significantly higher muscle activity than rectus abdominis of exercise 1, 2, 3 (p<.05). The external oblique of exercise 2, 4 showed significantly higher muscle activity than external oblique of exercise 1 (p<.05). The internal oblique of exercise 2, 4 showed significantly higher muscle activity than internal oblique of exercise 1 (p<.05). The erector spinae of exercise 2, 3, 4 showed significantly higher muscle activity than erector spinae of exercise 1 (p<.05). Median of internal oblique/rectus abdominis ratio of exercise 1 was 1.16, exercise 2 was 2.43, exercise 3 was 2.45, exercise and 4 was 1.27. Median of internal oblique/external oblique ratio of exercise 1 was 1.01, exercise 2 was .91, exercise 3 was .99, and exercise 4 was .93. Muscle activity can be influenced by addition of a Swiss ball in bridging exercises. It is recommend to use a Swiss ball for trunk stabilization exercise.
