Development of a Home-Based Multimedia Tutoring System for Postpartum Depression Management (산모의 산후우울 간호관리를 위한 멀티미디어 재택관리 시스템 개발)

  • Postpartum depression is one of the most serious problems in maternal health because it affects not only the mother but also her family. Postpartum depression disturbs maternal -infant interaction and attachment. However, most postpartum depression patients ignore this problem and do not seek treatment. Thus this study in conducted to development of a Home-Based Multimedia Tutoring System for postpartum depression management. With this computerized system, mothers in the postpartum periods can check the level of postpartum depression using a personal computer. This system will go through each mother's data and screen those who have abnormal values. In addition this system includes intervention programs -education for nutrition, hygiene care, sleep, postpartum exercise, methods of relaxation, deep breathing, visualization, music therapy and family therapy-to relieve postpartum depression. Using this system, a mother who has a minor level of depression can manage it by herself. Computer language used in this study were html 3.2, OS used was Microsoftware NT Server 4.0, the graphic tool was Adobe Photoshop 4.0, and the Webpage tool was Notepade. The results of this study are show at internet “URL : http : //203.241.225.42/”. Finally, the author suggests that this system could be adequately applied to assessing postpartum depression and as a intervention strategy for mothers during the postpartum period. Further this study contributes to designing an appropriate postpartum depression prevention strategy.

  • Evaluating the patient's voice before thyroidectomy is useful for the purpose of identifying patients with vocal cord paralysis without symptoms, identifying other patient's voice abnormalities, and whether it is related to voice disorders that may occur after surgery. Also voice evaluation after thyroid surgery is helpful in diagnosis, treatment, and rehabilitation and follow-up of voice disorders that occur without clear nerve damage after thyroidectomy. And it is helpful for rapid recovery through active early rehabilitation treatment for patients who complain of speech impairment without paralysis. In particular, neck exercise can improve the adhesion of the surgical site and increase the range of motion of the neck as well as improve subjective neck discomfort. In addition, hearing, voice and breathing functions should be improved, and voice hygiene education and counseling should be provided. Vocal cord injection is the first treatment option for unilateral vocal cord palsy. By establishing a protocol for voice disorders before and after thyroid surgery and providing appropriate treatment, the quality of life of patients can be improved.

A Study on the Pulmonary Functional Characteristics of Amateur Wind Instrument Players and Choir Members

  • Purpose: This study was undertaken to identify the effect of amateur wind musical performance and choir activity on pulmonary function, and to determine the usefulness as a respiration training program by measuring the pulmonary functions of subjects. Methods: A total of 90 subjects (wind instrument players group=30, choir members group=30, control group=30) participated in the experiment. Pulmonary function test (FVC, FEV1, FEV1/FVC ratio, MVV, SVC, PEF, FEF 25-75%, IRV, ERV) was conducted using a spirometer (CardioTouch 3000S, Bionet, Seoul, Republic of Korea). Each factor was measured 3 times to meet the American Thoracic Society criteria, and the highest value was used in the analysis. Results: Comparing pulmonary function between the amateur wind instrument players (WP), amateur choir members (CH), and control (CG) groups revealed significant differences in FEV1, FVC, FEV1/FVC, and ERV (p<0.05). Highest values were obtained in the WP group. Significant differences were obtained for various factors in the multiple regression analysis of practice year (PY), practice time per week (PTPW), and exercise time per week (ETPW): FEV1 and FVC in PY, FEV1/FVC in PTPW, and FEV1/FVC, MVV, PEF, and FEF (25-75%) in ETPW. Conclusion: Amateur wind instrument performance effectively improves lung function and is useful as a breathing training program for preventing debilitation and improving respiratory function.

  • Background : Generally $VO_2$ max is higher in treadmill exercise than cycle ergometer exercise. According to Hassen and Wasserman, $VO_2$ max with treadmill exercise is higher at ratio of 1.11 than that with cycle ergometer. $VO_2$ max also is influenced by race, sociocultural background, exercise habit In this study, $VO_2$ max and AT were evaluated between Treadmill and cycle exercise in male Korean college students. Method: Study subjects were 44 male college students. We randomized them into 2 groups; 24 students did treadmill exercise at first and 1 week later did cycle ergometer. Another 20 students did in opposite method. They made symptom limited maximal exercise. Author defined maximal exercise as followings: 1) respiratory exchange ratio(RER)> 1.1, 2) plateau>30 sec, 3) heart rate reserve(HRR) <15%, or 4) breathing reserve (BR)<30%. Otherwise their results are excluded as submaximal exercise. Anaerobic threshold(AT) was estimated by V-slope method. Results: $VO_2$ max and AT was $45.1{\pm}6.66m\ell$/kg/min and $26.0{\pm}6.78m\ell$/kg/min in treadmill and $34.9{\pm}5.89m\ell$/kg/min, $19.5{\pm}4.77m\ell$/kg/min in Cycle Ergometer. The measured-$VO_2max$/pred-$VO_2max$ was $98.8{\pm}13.24%$ in treadmill; $84.4{\pm}13.42%$ in cycle ergometer. Comparing $VO_2$ max in treadmill with that obtained by Hassen's method, there were significant differences.(p<0.01). At maximal exercise there were differences in HRR, $O_2$/pulse, BR, $V_E$/MVV, $V_E/VCO_2$ between treadmill and cycle but not in $V_E/VO_2$, Vd/Vt, Ti/Ttot. At AT there were differences in $O_2$/pulse, BR, $V_E$/MVV, Ti/Ttot between treadmill and cycle, otherwise not. Conclusion: According to the result of this study, there are larger gap between treadmill and cycle ergometer in normal Korean adults than foreign data, and it needs further study to obtain reference value of Korea.

Studies of the Effect of Vinegar Ingestion after the Strenuous Wingate Test on Energy Substrates during Recovery Periods (고강도의 윙게이트 테스트 후 식초섭취에 의한 회복기의 에너지기질 변화 연구)

  • We investigated the effect of brown-rice vinegar (BRV) ingestion after strenuous Wingate tests on energy substrates during a 2 hr recovery period. For this, seven healthy male adolescents were chosen as subjects. They performed 3 Wingate tests to induce fatigue, after which they ingested brown-rice vinegar (BRV) drink and/or water as a control (CON) after 15 min of the test. Blood was obtained pre-exercise and 30 min, 60 min, and 120 min post-exercise, and blood glucose, lactate, free fatty acids, ammonia, and cortisol were analyzed. After 120 min of recovery period, the 4th Wingate test was conducted to calculate the power recovery ratio of the 3rd and 4th trials. Breathing frequency, oxygen saturation, and heart rate did not show significant changes. Blood glucose level was lower in CON than BRV after 120 min of recovery, and blood lactate and ammonia levels were lower in BRV at 60 and 120 min. The higher free fatty acids were found at 60 and 120 min in BRV. In addition, Wingate power recovery ratio of peak power and peak power/body weight was significantly higher in BRV compared to CON. These results suggested that BRV ingestion after strenuous exercise facilitates fatigue recovery. Therefore, BRV might be effectively used as an ergogenic aid for events in which competitors compete two or more times a day.

The Effects of Acute Respiratory Training Feedback upon a Change on HRV-Autonomic Nervous System in Middle-aged Women (일회성 호흡훈련 피드백이 중년여성의 HRV-자율신경시스템 변화에 미치는 영향)

  • The purpose of this study was to analyze the effect of acute respiratory training feedback upon a change on HRV-Autonomic nervous system in middle-aged women. The research subjects were totally 24 middle-aged women(40-60 years old), were randomly allocated 12 people to the respiratory training group and 12 people to the control group, and then were carried out the acute respiratory training. The feedback exercise in the respiratory training group was conducted for totally 15 minutes. Following the 10-minute breath awareness training according to the expert's guidance, the 5-minute autonomous breathing exercise was implemented. The data analysis was carried out Repeated Measures ANOVA with SPSS WIN 20.0. The conclusions that were obtained through this are as follows. The middle-aged women got significantly higher in SDNN, RMSSD, LF, HF after the acute respiratory training. Compared to the control group. the respiratory training group was indicated to have gotten higher significantly in SDNN, RMSSD, LF, HF. Mean HR and LF/HF were not shown a significant difference in both the main effect of group & period and the interaction effect of group & period. Above of a result the acute respiratory training feedback is effective for SDNN, RMSSD, sympathetic activity, parasympathetic activity in the middle-aged women. Thereby, the respiratory training program improves autonomic nervous system, being considered to be possibly expected the effective value of exercise intervention available for relieving stress and recovering autonomic dysfunction in the middle-aged women.

An Analysis of Research Trend about Oriental Nursing Published in Korea (한방간호연구 논문분석)

  • The study attempts to analyze the trends of study related with oriental nursing. This article reviewed 81 researches on oriental nursing from 1985 to July 2000, by examining them according to the articles' characteristics, sources by publication period, research design, nursing intervention effect of experimental study and concept, and content of literature review. The findings of the analysis can be summarized as follows; 1. The number of studies related with oriental nursing has progressively increased by year from 1985, and that of articles was the highest during the period between 1995 to 1999. Since the 1990s, the number of articles including thesis and dissertation increased, and a third of subject articles were thesis among them. 2. The majority of research design were survey and correlational studies, and most of them were published in the 1990s. Literature review articles also occupied a certain portion, and most of them were published after 1995. In addition, the research design of dissertation revealed in various types. 3. Nursing intervention used in the experimental study was Dan Jeon Breathing, San-Yin-Jiao (SP-6) Pressure, Percutanous Electrical Nerve Stimulation, Guided Imagery, Diet for Sasang Constitution, Yoga Exercise, Qigong Exercise, Meridian Massage, Bed Exercise Program, Health Contract and Structured Rehabilitation Education. Among the study subjects, patients with chronic illness were at the highest rate, and the most nursing intervention effects presented physiological effect, activities of daily living and relieving pain. In most studies, the affirmative effect proved. 4. The key concept of literature review articles is energy, oriental nursing, health promotion, oriental medicine management of illness and etc. Being important concepts to understand oriental nursing, they confirm similar perspective of oriental medicine and nursing and show simultaneously the importance of nursing considered into the cultural perspective. The summary of the above results shows a tendency that the thesis and dissertation have been relatively considered with various research designs, while the others have focused on literature review. Especially, experimental study proved affirmative effect through various experimental treatment of nursing intervention related with oriental medicine. Besides in the literature review, inquiry of theories and practical perspective, which is necessary for the establishment of oriental nursing, was introduced. However, for the establishment of oriental nursing, more studies are to be required along with development of nursing intervention related with health promotion.

Experience of Self-management and Coping with Acute Exacerbation of Chronic Obstructive Pulmonary Disease (만성폐쇄성폐질환자 자가관리와 증상악화에 대한 대처 경험)

  • This study aimed to identify the experience of daily self-management (SM) and coping with acute exacerbation (AE) in COPD patients. A interview was conducted with 32 COPD patients, and data were analyzed according to the content analysis method. As a result of this study, the experience of daily SM was 'Implementing activities preventing from disease', 'Performing endurable exercise', 'Compliance with the medication regimens', 'Enforcing the dietary management', and 'Taking a rest.' Five major themes were drawn. And, for the experience of coping with AE, four major themes were derived: 'Using medical services', 'Getting enough rest', 'Taking prescription drugs', and 'Taking breathing technique.' COPD patients need continous education to detect earily specific symptoms and to cope with worsening symptoms.

  • Objects : The purpose of this study was to investigate Urinary Incontinence improvement effect of Moosim Gi-Gong Riding stance and to propose urinary incontinence treatment Program. Methods : We analyzed the effect of Moosim Gi-Gong Riding stance, and compared to Behavior theraphy which includes Kegel Exercise, Riding Stance of Ki-chum, Zhan-Zhuang-Gong. Results : 1. Moosim Gi-Gong Riding stance can correct the pelvic strain with principles such as horseback riding and help restore organs in the abdominal cavity. 2. Moosim Gi-Gong Riding stance can restore the ability to recover bladder and proximal urethra in right place through changes in the abdominal pressure by breathing and posture 3. Moosim Gi-Gong Riding stance can help restoring the ability to control the urination by increasing the intensity of the abdominal pressure and reinforcing Kidney, Liver, Spleen Meridian muscles. 4. Reinforcing Kidney, Liver, Spleen Meridian muscles can help to treat urinary incontinence through strengthening the tension between organs and activating the intestinal tract. Conclusions : This study shows that treatment program for Urinary Incontinence using Moosim Gi-Gong Riding stance can be useful to patient.

Diaphragmatic hernia in a Jeju horse (crossbred) broodmare

  • Diaphragmatic hernias, whether congenital or acquired (traumatic), are rarely observed in the horse. Acquired diaphragmatic hernias typically occur secondary to trauma or an increase in intraabdominal pressure due to falling, heavy exercise, or parturition. Diaphragmatic herniorrhaphy is difficult to perform in adult horses and the horses with symptomatic diaphragmatic hernias usually die. A 10- year old, 340 kg, Jeju horse (crossbred) broodmare with sudden onset of gait disorder and a moderate emaciation was examined. Findings on physical examination included conjunctivitis, dehydration, shallow breathing, dyspnea, weaken heart beat, lack of auscultatable sounds from the gastrointestinal tract, and anorexia. Rectal temperature was $38.4^{\circ}C$ and respiratory rates were moderately increased. There were slight signs of acute colic. The broodmare died one day after non-specific treatment of fluids, nutriment, antibiotics and non-steroidal anti-inflammatory drug. The cause of death was strangulation of the small intestine through a diaphragmatic hernia. The rent was about 2 cm in diameter and located in the central right part of diaphragm. Around 60 cm of small intestine was protruded into thoracic cavity through the rent. The cause of the hernia could not be ascertained. The broodmare had been pastured with many other horses, and the groom had not noticed any aggressive behavior among them. It was, however, speculated that trauma by stallion's attack may have been the cause of the diaphragmatic hernia, because the new horse may be the object of behaviors ranging from mild threats to seriously aggressive kicking, squealing, rearing, and biting.
