모유수유모의 모유수유 임파워먼트와 어머니 역할 수행 자신감과의 관계 (A Study on the Relationship between Breastfeeding Empowerment and Self-confidence in the Maternal Role of Breastfeeding Mothers)

  • Purpose: This study was conducted to evaluate the relationship between breastfeeding empowerment and self-confidence in the maternal role of breastfeeding mothers. Method: Breastfeeding empowerment was measured on a scale of 5 developed by Kim & Park(2004), while self-confidence was measured on a scale of 4 by Lederman et al.(1981). Data was collected during a Breastfeeding Infant Contest held in September 2006 and 2007 in Changwon-si, Gyeongsangnam-do. The subjects were 278 breastfeeding mothers. The collected data was processed with the SPSS win 12.0 version. Result: Breastfeeding empowerment scored 4.21 on average (5.0 full score). The score showed a statistical difference only in breast milk quantity and breastfeeding knowledge. On the otherhand self-confidence in the maternal role showed a statistical difference only in breastfeeding frequency and know ledge. Self-confidence in the maternal role showed a statistically significant relationship with breastfeeding skill(${\gamma}=0.50$), with breastfeeding will(${\gamma}=0.45$), with adherence to exclusive breastfeeding(${\gamma}=0.35$) and with group empowerment(${\gamma}=0.39$). Conclusion: The positive relationship between breastfeeding empowerment and self-confidence in the maternal role implies that breastfeeding empowerment may be utilized to promote self-confidence in the mother's role.

유방울혈이 있는 제왕절개분만 산모를 위한 모유수유 증진 프로그램의 개발 및 평가 (Development and Evaluation of Breastfeeding Promotion Program for Mothers with Breast Engorgement following Cesarean Birth)

  • Purpose: This study was to develop a breastfeeding promotion program and to test effects of the program on levels of breast discomfort, breast size, sodium in breast milk, and type of feeding in mothers with breast engorgement following cesarean birth. Methods: A non-synchronized non-equivalent control group pretest-posttest design was used in this study. The participants were 70 postpartum mothers who were admitted to a postpartum care center and experienced breast engorgement following cesarean birth. The planned nursing intervention was the breastfeeding promotion program consisting of breast massage and 1:1 breastfeeding education, counseling, and support focusing on individualized problem solving provided for 10 days. Fifty-three women completed the program (experimental group 26, control group, 27). Measurements were level of breast discomfort, breast size, sodium in breast milk and type of feeding at pre and posttest. Results: Women who participated in the program experienced lower scores for breast discomfort, greater decrease in breast size, lower levels of sodium in breast milk, and practiced breastfeeding more than those in the control group. Conclusion: Results indicate that this breastfeeding promotion program is effective in reducing breast engorgement and improving breastfeeding practices, and is therefore recommended to enhance breastfeeding promotion practice in postpartum care centers.

모유수유 임파워먼트 측정도구 개발 (A Study on the Development of the Korean Breastfeeding Empowerment Scale)

  • Purpose: The purpose of this study was to develop an instrument to measure breastfeeding empowerment for the Korean mother, and to test the validity and reliability of the instrument. Method: The items of instruments used was based on literature review. Nineteen item scales were developed with five domains. In order to test reliability and validity of the scale, data was collected from 99 mothers who were within 4weeks, postpatum and 88 mothers who had had children within 1 year. Data was collected between September 2nd and 24th by a self-reported questionnaire. Result: The results were as follows: As a result of factor analysis three times, 19 items were selected from the total of 24 items. Five factors evolved by factor analysis, which explained 65.7% of the total variance. The first factor 'breastfeeding skill' explained 34.1%, the 2nd factor 'group empowerment' 10.6%, the 3rd factor 'breastfeeding will' explained 7.8%, the 4th factor 'social policy empowerment' 7.0%, and the 5th factor 'adherence to exclusive breastfeeding' 6.2%. Cronbach's alpha coefficient of this scale was .8781. Conclusion: The study supports the reliability and validity of the scale. Because the main concept of breastfeeding empowerment was breastfeeding skill, there were distinct differences in the dimensions of breastfeeding empowerment scales.

모유수유관리 프로그램이 모유수유지속에 미치는 효과 (Effect of a Breastfeeding Management Program on Breastfeeding Duration)

  • Purpose: The purpose of this study was to measure the effects of a breastfeeding management program for promotion of breastfeeding. Method: The study design was a non-equivalent control group pre-post test. The study subjects were pregnant women receiving a breastfeeding education class in one hospital in Ilsan. The total number of study subjects was 153 pregnant women, namely 75 mothers in the experimental group and 78 in the control group. Data was collected between June and November, 2006. A breastfeeding management program was provided to the experimental group while admitted for delivery, and in weeks 2, 4, 8, 12, 16, 20, and 24 of the postpartum period. The rates of breastfeeding were measured in weeks 2, 4, 8, 12, 16, 20, and 24 of the postpartum period. Result: There were significant differences in the scores of rate of breastfeeding between the two groups(all cases), which were 93.1 in the experimental group and 85.8 in the control group at week 4; 88.1 vs 71.8 at week 8; 83.4 vs 56.7 at week 12; 71.4 vs 53.9 at week 16; and 65.7 vs 48.4 at week 20 respectively. However, there were not significant differences in the scores of rate of breast-feeding between the two groups, which were 97.7 in the experimental group and 96.0 in the control group in the early postpartum period at week 2, and 59.6 vs 45.3 at week 24. Add to this, the study analyzed only people who had jobs. As a result, the scores of rate of breastfeeding were 94.8 in the experimental group which were significantly higher than 88.5 in the control group at week 4; 93.2 vs 73.3 at week 8; 89.6 vs 57.2 at week 12; and 75.0 vs 46.0 at week 20 respectively. However, there were not significant differences at week 2, week 16 and week 24. Conclusion: The results of this study suggest that a breastfeeding management program during the last trimester of the perinatal period and postpartum period may increase the duration of breastfeeding in postpartum clients who intend to breastfeed. This study supports the literature which found that prenatal education and postpartum support are important to the outcome of breastfeeding.

여성들의 수유양상 및 수유방식의 선택에 영향 주는 요인들 (Infant Feeding Practices and the Factors that Influence feeding Practices among Women in Seoul and the Chungbuk Area)

  • In order to obtain baseline data for the development of an educational program on breastfeeding, a survey was carried out to investigate infant feeding practices, the characteristics the subjects have in common, and the factors that influence the feeding methods among women in Seoul and the Chungbuk area. Subjects included 671 lactating mothers who visited public health centers or pediatric clinics between December 1999 and February 2000, and were interviewed using a questionnaire. The results were as follows : With respect to feeding methods, the incidence of breastfeeding, formula feeding, mixed feeding and formula feeding switched from breastfeeding was 20.6%, 29.8%, 11.6%, and 38.0%, respectively. With respect to the characteristics the subjects had in common, the incidence of women who planned their infants' feeding methods before pregnancy, during pregancy, and after delivery was 48.7%, 31.0%, and 20.3%, respectively. The incidence of women who started breastfeeding in the hospital after delivery was 38.8%. About seventy seven percet of the subjects had not previously attended an educational program on breastfeeding, and most of these wanted to participate in the future in an educational program to learn about breastfeeding. Some of the topics they were interested in were“Nutritional Management for Sufficient Breast Milk”(60.3%),“Breast Care”(25.0%), and“Correct Nursing Positions”(9.8%). Most (88.2%) of the women who breastfeed suffered from physical discomforts including discomfort of the waist, and legs and discomfort due to cracked or sore nipples. “Insufficient breast milk”was the main reason for breastfeeding cessation or for switching to formula feeding. With respect to formula feeding practices, the main reasons for selecting a specific brand of formula were“the same brand the hospital used after delivery”(34.3%) and“an advertised brand”(23.3%). The strongest factor for promoting breastfeeding was“the support of husband or parents”, next were “breastfeeding in the hospital after delivery”and“planning to breastfeed before pregnancy”in that order. The characteristics the subjects had in common relating to formula feeding were“mother's job”,“high economic level”,“Caesarian section”and “planning to breastfeed after delivery”. In conclusion, it is recommended that breastfeeding be pro-moted, and educational programs be developed and offered as soon as possible to each group which had unfavorable attitudes toward breastfeeding. In addition, the monitoring and supervision of formula advertisements is required to protect consumers from the adverse effects of exaggerated advertising.

모유수유에 관한 의사, 간호사의 신념 및 태도조사 (Breastfeeding Belief and Attitudes of Physicians and Nurses)

  • The purpose of this study was to investigate belief and attitudes on breastfeeding of physicians and nurses. A questionnaire was mailed to obstetricians, pediatricians and nurses at the department of pediatrics or obstetrics of university hospital and private hospital in Seoul and Choong-Chung Province) in South Korea. Total numbers of study subjects were 346 (pediatrician 67, obstetrician 41, nurse 238). The results were as follows : 1. The main reasons not to educate breast feeding in the hospitals were a lack of interest of the health professionals, a lack of educators and education programs. 2. The respondents thought that the best ways for lactating mother to get advices about breastfeeding during the first month were to give a call to health professionals in the hospitals where she had been delivered, or call to relatives or friends. 3. Breastfeeding attitudes of physicians and nurses did not differ according to gender, job, or type of the hospitals they work. 4. Breastfeeding attitudes were related with personal breastfeeding experience, breastfeeding knowledge, extracurricular education experience, encouragement experience. Physicians and nurses should give appropriate advices and support to lactating mothers to increase breastfeeding rate. They were, however, ill-prepared to counsel breastfeeding mothers. Therefore, it is necessary to instruct breastfeeding in the curriculum of the medical and nursing schools, and incorporate clinically based breastfeeding training into continuing education workshops. Improved breastfeeding education is a critical step in ensuring that health professionals are adequately prepared for this important role.

모유수유지식에 따른 수유형태에 관한 연구 (A Study on the Relationship between Knowledge of Breast feeding and Practice of feeding pattern)

  • The purpose of this study was to investigate the relationship between knowledge of breast feeding and practice of feeding pattern. A sample of 117 mothers who have the babies between age from 1 month to 12 months were selected from one university hospital and one public heath care center in J city. The data were collected with the questionnaire during the period from the January. 5, 2002 through January. 30, 2002. Analysis was done by using frequency, percentage, t-test, analysis of variance, and scheffe test. The results of this study can be summarized as follows. Comparision of the method of feeding by general characters. Income state, rearing (p=.019), rearing supporters(p=.026), infant age(p=.03) have shown significant difference as the variables affecting Breast practice rate. Breastfeeding rate were 24.8%, mixedfeeding type, 14.5%, milkfeeding type, 60.7%. The mean of knowledge on breastfeeding by feeding patterns were the highest in the group of breastfeeding. There was statistical significance between the group of breast feeding and milkfeeding(p=.000). In the conclusion, this study revealed that knowledge of breastfeeding promoted practice of breastfeeding. Therefore, to promote practice of breastfeeding, it is necessary that nurses provide adequate information about knowledge of breastfeeding.

초산모의 조기 모유수유 상태에 영향을 미치는 요인 (Factors Related to Neonatal Suckling in the Initiati of Breastfeeding in Primiparous Mothers)

  • Purpose: This study was conducted to discover the related factors of neonatal suckling in the initiation of breastfeeding in primiparous mothers and to provide basic data for promoting nursing intervention strategies to improve the practice of breastfeeding. Method: The subjects of this study were 71 primiparous mothers who had normal vaginal deliveries at one obstetric hospital in P metropolitan city and one delivery center in J city. The collected data was analyzed using the SPSS program. Result: The average IBFAT(Infant Breastfeeding Assessment Tool) score was 9.6$\pm$2.3. The general characteristics studied that had a significant influence on neonatal suckling in the initiation of breastfeeding were the place of delivery (ex: Hospital, Postnatal Unit), whether oxytocin was used, no usage of analgesic medication, amount of satisfaction after the first breastfeeding weight of the newborns and the Apgar score at one minute. Conclusions: A higher IBFAT score was related to primiparous mothers who had a hospital delivery, received oxytocin, received maternal labour analgesics, neonatal weight, Apgar score at one minute, and satisfaction after the first breastfeeding.

임부 남편의 모유수유에 대한 태도 유형 분석 (Attitudes of Pregnant women s husbands to Breast Feeding)

  • By using Q-methodology, this study examines the attitudes of pregnant womens' husbands toward breastfeeding. Also, the research provides basic data necessary to develop a strategy for recommending breastfeeding. A total of 112 items for the Q-population were collected from related literature and interviews with the general public, specialists, pregnant women and their husbands. Finally, 38 statements were selected. Twenty one husbands of pregnant women classified these statements on each card on a 1 to 9 point scale(forced normal distribution) and wrote the reasons for both the most supported and the most resisted statements. The materials collected were analyzed by using pc QUANL program. The analysis drew down following fact that even though the attitudes of the husbands of pregnant were very similar, they could be classified to three types according to the motivation and recognition the degree of choosing breastfeeding. Type 1 is the mother's duty supporter, who insists that breastfeeding is completely natural and the proper duty of the mother. Type 2 is the emotional value supporter, who thinks that breastfeeding emotionally affects both the baby and the mother in a positive way. Type 3 is the conditional choice supporter, who chooses the most proper suckling way of feeding according to given conditions.

산전 자가유방 마사지 교육이 초산모의 모유수유 자기효능감, 모유수유 적응과 실천에 미치는 효과 (Effects of Prenatal Breast Self-massage Education on Breastfeeding Self-efficacy, Adaptation, and Practice in Primiparous Women)

  • Purpose: This study aimed to verify the effects of prenatal breast self-massage education on first-time mothers' breastfeeding self-efficacy, breastfeeding adaptation, and breastfeeding practice. Method: The experimental group (n=26) received prenatal breast self-massage education focused on self mamma care (SMC) at 34-36 weeks gestation and 2 weeks later face-to-face. The control group (n=25) was recruited immediately following birth and received usual care. Breastfeeding self-efficacy was measured 4 hours after childbirth and 3 days later, and breastfeeding practice, breastfeeding adaptation, and breastfeeding-related characteristics were measured 2 weeks and 4 weeks after childbirth through online survey. The data were analyzed by $x^2$ test and t-test using SPSS/WIN 24.0. Results: There was no significant difference in breastfeeding self-efficacy of the experimental group at 4 hours after childbirth (t=0.83, p=.410) whereas it was statistically significantly higher at 3 days postpartum (t=2.86, p=.006). There were no significant differences in breastfeeding adaptation between the two groups at both 2 weeks (t=1.76, p=.084) and 4 weeks postpartum (t=0.87, p=.388). For breastfeeding practice a statistically significant difference was found at 4 weeks for the experimental group ($x^2=4.77$, p=.036). Conclusion: This antenatal SMC intervention was found to be a feasible intervention for use in clinical practice.
