조기 유방암에서 보존적 수술후 방사선치료성적 (The Results of Primary Radiotherapy following Breast-Conserving Surgery for Early Breast Cancer)

  • 목적: 조기 유방암에서 보존적 수술후 방사선치료는 외국에서 과거 20여년 동안 근치적 수술과 비슷한 생존율을 보였다. 그러나 아직 우리 나라에서는 보편화되지는 못한 실정이다. 이 논문은 원자력병원에서 시행한 조기 유방암환자에서 보존적 수술의 성적 및 부작용의 결과를 알기 위함이다. 방법: 1987년 1월부터 1989년 12월까지 원자력 병원 치료방사선과에서 보존적 수술후 방사선치료를 받은 조기 유방암 환자 45명에 대해 후향적 연구를 시행하였다. 추적 관찰기간은 4-82개월이었고 중앙값은 54개월이었다. 수술은 조직 생검만 시행한 경우 및 Tumorectomy 또는 Quadrantectomy를 시행한 경우가 있었다. 28예에서 액와림프절 박리를 시행하였다. 방사선 치료는 전유방에 Tangential field로 하루 1.8-2 Gy씩 총 50 또는 50.4 Gy를 조사하였다. 항암 요법은 30예의 환자에서 시행되었고 호르몬 치료는 11예에서 시행되었다. 결과: 5년 생존율, 5년 무병생존율 및 5년 국소제어율은 각각 87.2%, 86.5%, 및 97.6%였다. 전신 요법을 시행하는 군이 시행하지 않은 군에 비하여 5년 생존율에서 좋은 성적을 보였으나 통계학적인 의미는 없었다 (0.05

초기 유방암의 유방보존수술과 방사선치료의 장기추적결과 (Long-term Results of Breast-conserving Surgery and Radiation Therapy in Early Breast Cancer)

  • 목 적: 초기 유방암에서 유방 보존 수술과 방사선치료 후 실패양상과 생존율 및 미용효과를 알아보고자 하였다. 대상 및 방법: 1992년 1월부터 2002년 12월까지 계명대학교 동산의료원에서 유방보존수술과 방사선치료를 받은 초기 유방암환자 154명을 대상으로 하였다. TNM 병기는 1기 90명, IIa기 50명, IIb기 11명이었다. 모든 환자는 유방의 부분절제술과 동측 액와림프절 곽청술을 시행하고 방사선치료를 하였다. 유방의 방사선치료는 동측 전체 유방에 6 MV 광자선으로 50~54 Gy를 5주에서 6주간에 조사하였고 원발병소에 추가 방사선은 전자선으로 10~16 Gy를 조사하였다. 항암화학요법은 75명에서 방사선치료 전후에 시행하였고 호르몬치료는 92명에서 Tamoxifen을 사용하였다. 추적관찰기간은 13개월에서 179개월로 중앙값이 92.5개월이었다. 결 과: 전체환자의 5년, 10년 생존율은 97.3%, 94.5%이었다. 5년, 10년 무병생존율은 92.5%, 88.9%이었고 구제 치료후 5년, 10년 무병생존율은 93.9%, 90.2%이었다. 다변량 분석상 수술 후 방사선치료까지의 기간이($\leq$6 weeks vs. >6 weeks, p=0.017) 무병생존율에 있어서 통계적으로 유의한 예후 인자였다. 주된 실패양상은 원격전이였고 다발성으로 치료 후 중앙값 55.5개월에 전이를 하였다. 원격전이의 호발부위는 폐였다. 미용결과는 80.7%에서 좋음에서 매우 좋음으로 나타났다. 결 론: 초기 유방암에서 유방보존수술과 방사선치료는 우수한 생존율과 미용결과를 보인다고 생각된다. 또한 유방 보존수술 후 방사선치료는 가능한 한 빨리, 6주 이내에 시행되는 것이 좋을 것으로 생각된다.

HER2-enriched Tumors Have the Highest Risk of Local Recurrence in Chinese Patients Treated with Breast Conservation Therapy

  • Purpose: The purpose of this study was to investigate the recurrence pattern and characteristics of patients based on the 2013 St. Gallen surrogate molecular subtypes after breast-conserving surgery (BCS) in Chinese women. Methods: This retrospective analysis included 709 consecutive breast cancer patients undergoing BCS from 1999-2010 at our institution. Five different surrogate subtypes were created using combined expression of the estrogen receptor, progesterone receptor, and human epidermal growth factor receptor-2. Locoregional relapse-free survival (LRRFS), distant metastasis-free survival (DMFS), and disease-free survival (DFS) rates were calculated. Results: The 5-year LRRFS, DMFS, and DFS rates were 90.5%, 88.2%, and 81.5%, respectively. Multivariate analysis revealed that young age, node-positive disease, and HER2 enrichment were independent prognostic factors in LRRFS patients. There was also an independent prognostic role of lymph node-positive disease in DMFS and DFS patients. Patients with luminal A tumors had the most favorable prognosis, with LRRFS, DMFS, and DFS rates of 93.2%, 91.5%, and 87.4% at 5 years, respectively. Conversely, HER-2-enriched tumors exhibited the highest rate of locoregional recurrence (20.6%). Conclusion: Surrogate subtypes present with significant differences in RFS, DMFS, and LRRFS. Luminal A tumors have the best prognosis, whereas HER2-enriched tumors have the poorest.

Long-term results of forward intensity-modulated radiation therapy for patients with early-stage breast cancer

  • Purpose: To observe long-term clinical outcomes for patients with early-stage breast cancer treated with forward intensity-modulated radiation therapy (IMRT), including local control and clinical toxicities. Materials and Methods: We retrospectively analyzed a total of 214 patients with stage I-II breast cancer who were treated with breast conserving surgery followed by adjuvant breast radiation therapy between 2001 and 2008. All patients were treated using forward IMRT. The whole breast was irradiated to a dose of 50 to 50.4 Gy followed by an 8 to 12 Gy electron boost to the surgical bed. Results: The median age was 46 years (range, 21 to 82 years) and the medial follow-up time was 7.3 years (range, 2.4 to 11.7 years). Stage T1 was 139 (65%) and T2 was 75 (35%), respectively. Ipsilateral breast recurrence was observed in 3 patients. The 5- and 10-year local control rates were 99.1% and 97.8%, respectively. The cosmetic outcome was evaluated according to the Harvard scale and 89.4% of patients were scored as excellent or good. Conclusion: The whole breast radiation therapy as an adjuvant treatment using a forward IMRT technique showed excellent long-term local control as well as favorable outcomes of toxicity and cosmesis.

Comparison of breast volume change between oncoplastic breast-conserving surgery with radiation therapy and a simultaneous contralateral balancing procedure through the inverted-T scar technique

  • Background Reduction mammoplasty or mastopexy is performed as an additional balancing procedure in patients with large or ptotic breasts who undergo breast-conserving surgery (BCS). Radiation therapy on breasts that have undergone surgery may result in changes in the volume. This study presents a comparative analysis of patients who received post-BCS balancing procedures to determine whether volume changes were larger in breasts that received radiation therapy than on the contralateral side. Methods Thirty-six participants were selected among patients who received BCS using the inverted-T scar technique between September 2012 and July 2017, were followed up for 2 or more years, and had pre-radiation therapy computed tomography images and post-radiation therapy images taken between 12 and 18 months after completion. The average age of the participants was 53.5 years, their average body mass index was 26.62 kg/㎡. Results The pre- and post-radiation therapy volumes of the breasts receiving BCS were 666.08±147.48 mL and 649.33±130.35 mL, respectively. In the contralateral breasts, the volume before radiation therapy was 637.69±145.72 mL, which decreased to 628.14±166.41 mL after therapy. The volume ratio of the affected to the contralateral breasts was 1.05±0.10 before radiation therapy and 1.06±0.12 after radiation therapy. Conclusions The ratio of the volume between the two breasts immediately after surgery and at roughly 18 months postoperatively was not significantly different (P=0.98). For these reasons, we recommend a simultaneous single-stage balancing procedure as a reasonable option for patients who require radiation therapy after BCS without concerns regarding volume change.

  • 목 적: 유방 보존수술 후 방사선 치료를 받은 환자에서 치료의 성적과 무병 생존율 및 재발에 영향을 주는 위험인자에 대해 알아보고자 하였다. 대상 및 방법: 1997년 3월부터 2003년 12월 까지 유방 보존수술 시행 후 방사선 치료를 받은 환자 77명을 대상으로 후향적으로 분석하였다. 추적 관찰 기간의 중앙값은 58.4개월($43.8{\sim}129.4$개월)이었다. 전체 환자의 평균 연령은 41세, 중앙 연령은 40세이었다. 수술 후 조직학적 T 병기는 Tis가 7명, T1 38명(49.3%), T2 28명(36.3%), T3 3명, 미확인이 1명이었다. 액와 림프절 전이가 없는 경우가 52명(67.5%), $1{\sim}3$개 전이가 14명(18.1%), 4개 이상이 3명(0.03%)이었다. 절제연이 음성인 환자는 59명이었고, 2 mm 이내로 근접한 경우는 15명, 양성인 경우는 2명이었다. 방사선치료는 전체 유방에 접사면으로 조사한 후 원발 병소 부위에 전자선으로 추가조사 하여 총 59.4 Gy에서 66.4 Gy를 시행하였다. 액와 림프절의 개수가 4개 이상인 경우에는 액와 림프절과 쇄골 상 림프절을 포함하여 $41.4{\sim}60.4$ Gy를 조사하였다. 항암화학요법은 59명에서 시행되었고, 호르몬 치료로는 tamoxifen 또는 fareston을 사용하였으며 29명에서 시행하였다. 결 과: 5년 생존율은 98.1%이었으며, 5년 무병 생존율은 93.5%이었다. 총 77명의 환자 중 4명(5.2%)의 환자가 재발을 하였다. 1명은 쇄골 상 림프절 재발, 1명은 쇄골 상 림프절과 동시에 다발성 원격전이, 다른 2명은 원격전이가 발견되었다. 원발 병소 주위의 국소 재발은 추적 관찰 기간 중 발견되지 않았다. 림프절 전이 유무나 숫자는 재발이나(p=0.195)무병 생존율(p=0.30)에 영향을 미치지 않았다. 절제연이 양성인 2명 중 1명이 7개월 만에 재발을 하였고, 재발 기간까지 걸린 기간인 무병 생존기간이 통계적으로 의미 있게 짧은 것으로 나타났으며(p<0.0001), 재발 빈도도 절제연이 음성이거나 가까운 경우에 비해서 통계적으로 의미가 있는 것으로 나타났다(p=0.0507). 그러나 절제연이 근접한 경우에는 절제연 음성인 경우와 비교하여 통계적으로 재발 빈도에 차이가 없었다(p=1.000). 재발된 4명은 모두 40세 이하로 9.2%의 재발률을 보인 반면, 40세 이상에서는 재발이 없었으나 두 그룹 간에 통계적으로 유의한 차이는 없었다(p=0.1255). 수술 후 조직학적 T 병기는 T2의 경우 4명(14%)이 재발한 반면, T1에서는 재발이 발견되지 않았으며 통계적으로 유의한 차이를 보였다(p=0.0284). 무병 생존율도 T2 환자의 경우에 통계적으로 의미 있게 낮은 것으로 나타났다(p=0.0379). 무병 생존율에 영향을 미치는 단변량 분석에서 수술 후 조직학적 T 병기, 절제연의 상태, p53 수용체 변이가 통계적으로 의미 있게 나왔다. 결 론: 초기 유방암에서 유방 보존수술 후 방사선치료는 높은 국소제어율과 무병 생존율을 보이는 안전한 치료이다. 절제연 양성, 수술 후 조직학적 T병기, p53 수용체 변이 등은 통계적으로 유의하게 재발 위험이 높은 군에 속하였으며, 무병 생존율에 영향을 미치는 것으로 밝혀졌다.

Volumetric changes in the lumpectomy cavity during whole breast irradiation after breast conserving surgery

  • Purpose: This study was performed to evaluate the change in the lumpectomy cavity volumes before and after whole breast radiation therapy (WBRT) and to identify factors associated with the change of volume. Materials and Methods: From September 2009 to April 2010, the computed tomography (CT) simulation data from 70 patients obtained before and after WBRT was evaluated. The lumpectomy cavity volumes were contoured based on surgical clips, seroma, and postoperative changes. Significant differences in the data from pre-WBRT CT and post-WBRT CT were assessed. Multiple variables were examined for correlation with volume reduction in the lumpectomy cavity. Results: The mean and median volume reduction in the lumpectomy cavity after WBRT were 17.6 $cm^3$ and 16.1 $cm^3$, respectively with the statistical significance (p < 0.001). The volume reduction in the lumpectomy cavity was inversely correlated with time from surgery to radiation therapy (R = 0.390). The presence of seroma was significantly associated with a volumetric change in the lumpectomy cavity after WBRT (p = 0.011). Conclusion: The volume of lumpectomy cavity reduced significantly after WBRT. As the time from surgery to the start ot WBRT increased, the volume reduction in the lumpectomy cavity during WBRT decreased. A strong correlation was observed between the presence of seroma and the reduced volume. To ensure appropriate coverage and to limit normal tissue exposure during boost irradiation in patients who has seroma at the time of starting WBRT, repeating CT simulation at boost planning is suggested.

Consideration of fractionation or field size prior to radiation therapy in patients with breast cancer and psoriasis: a case report

  • We present three cases of patients with breast cancer and psoriasis who received radiotherapy following breast-conserving surgery. One patient developed an extensive flare-up of psoriasis during chemotherapy. After discontinuing chemotherapy, she received conventional radiotherapy to the ipsilateral whole breast, axillary, and supraclavicular lymph nodes with 50.4 Gy in 28 fractions and boosted with 10 Gy in five fractions to the tumor bed. Two patients received hypofractionated whole-breast radiotherapy at a total dose of 40.05-42.4 Gy in 15-16 fractions. In all three cases, there was no flare-up of psoriatic events at least 6 months after the completion of radiotherapy. We hypothesized that there is a close relationship between psoriatic events and the extent of trauma rather than the daily dose of the fraction. Therefore, we confirmed that the effect of radiotherapy on psoriatic events is minimal if the radiation field size does not exceed that of the whole breast.

A Case of Dural Arteriovenous Fistula of Superior Sagittal Sinus after Tamoxifen Treatment for Breast Cancer

  • We are reporting an unusual case of dural arteriovenous fistula (AVF) of the superior sagittal sinus (SSS) after tamoxifen treatment for breast cancer. A 30-year-old female arrived at the emergency room with a sudden headache and left sided weakness and sensory loss. In her past medical history, she was diagnosed with breast cancer 1 year prior, and subsequently underwent a breast conserving mastectomy with whole breast radiation and adjuvant chemotherapy with tamoxifen. At the time of admission, computed tomography showed a small acute intracerebral hemorrhage at the right parietal cortex, and magnetic resonance imaging showed that a dural AVF at the SSS with a prominent and tortuous venous enhancement along the centrum semiovale was present. Cerebral angiography showed that the dural AVF at the mid-portion of the SSS with meningeal arterial feeding vessels entering the wall of the SSS, then draining through the dilated cortical veins. Our patient had no signs of active malignancy or any abnormalities in her coagulation profile, so it can be concluded that the tamoxifen was the likely cause of the SSS thrombosis and dural AVF. The dural AVF was treated by an endovascular coil embolization for the arterialized segment of the SSS. The patient dramatically recovered favorably from left side motor and sensory deficit. The best clinical approach is to screen potential patients of tamoxifen hormonal therapy and educate them on the sign and symptoms of life threatening thromboembolic events while taking tamoxifen.

Thyroid Function after Postoperative Radiation Therapy in Patients with Breast Cancer

  • Objective: The aim of this study was to assess thyroid function in breast cancer patients exposed to therapeutic external beam radiation. The focus was on possible progressive changes and any relationships between the incidence of primary hypothyroidism, the time required to become hypothyroid, and factors such as chemotherapy, hormonotherapy and immunotherapy. Materials and Methods: Seventy females undergoing 3D conformal and IMRT radiation therapy for breast cancers were enrolled in a non-randomized prospective study. The patients was divided into two groups: those after mastectomy or breast conserving surgery (BCS) were irradiated to a scar of the chest wall/breast and the ipsilateral supraclavicular and the axillary areas (supraclavicular radiotherapy group - SC-RT group - 32 patients) and the control group receiving adjuvant chest wall/breast RT only (BCT group - 38 patients).The total doses were 50.0 to 70 Gy in 5 to 7 weeks. The median follow-up term was 24 months (range, 1-40 months). Thyroid function was evaluated by measuring thyroid stimulating hormone (TSH), free thyroxine (fT4), and free triiodothyronine (fT3) levels. The minimum, maximum and mean thyroid gland doses for 20 Gy (V20) were calculated for all patients. Results: Statistically significant results were obtained for the SC-RT group. Two yearsa fter the end of RT the chance of an event was increased in 6% of the population (p=0.009) in the SC-RT group. In the BCT group no significance was noted. No statistically significant differences were found for V20, chemio-, immunotherapy and hormonotherapy or Ki67 values (p=0.12). No significant results were obtained for development of hypothyroidism and clinical factors (age, thyroid volume, treatment modalities). Conclusion: Radiotherapy is associated with a higher incidence of thyroid toxicity in breast cancer patients. Routine thyroid function monitoring should be recommended in such cases.
