  • Purpose: One of the main reasons why mothers quit breast feeding is that the volume of breast milk is inadequate due to insufficiency in suckling. We believe suckling experience may be a factor affecting nipple confusion. So an alternative feeding method, namely cup, spoon, finger, or nasogastric tube feeding may be needed to prevent nipple confusion. The purpose of this study was to construct a predictive model for demand for alternative feeding education by nurses. Methods: A descriptive design with structured self-report questionnaires was used for this study. Data from 175 nurses working in hospitals in Busan were collected between April 1 and 15, 2009. Data were analyzed by decision tree method, one of the data mining techniques using SAS 9.1 and Enterprise Miner 4.3 program. Results: Of the nurses, 81.1% demanded alternative feeding education and 5 factors showed that most of them expressed intention to pay, desire to know about alternative feeding, age, and learning experience. From these results, the derived model is considered appropriative for explaining and predicting demand for alternative feeding education. Conclusion: This confirms that knowledge and compliance in alternative breast feeding for newborn babies should be correct and any inaccuracies or insufficient information should be supplemented.

초기 모유 수유와 인공 수유에 따른 초산모의 신생아에 대한 지각 비교 (Differences in Breast feeding and Bottle feeding Primiparas′ Perceptions of their Babies during the Early time of Post-partum Period)

  • This study was done to compare the breast-feeding primiparas' perceptions of their babies with bottle-feeding primiparas' at 2-3 days after delivery. The subjects of this study included 42 breast-feeding mothers and 56 bottle-feeding mothers. Data were collected by using the Neonatal Perception Inventory(NPI) devised by Broussard (1963) and added by Lee, Ja Hyung(1986) from December, 1996 to June, 1997. The results of this study are as follows, 1. Primiparas' perceptions of their babies according to their general characteristics. 1) There was no significant difference in mothers' perceptions according to mothers' age and whether they have a job or not. 2) There was a difference in mothers' perceptions according to their educational level (P<0.1). 2. The comparisons of breast-feeding mothers' perceptions of their babies with bottle-feeding mothers'. 1) There was no significant difference in the two groups' perceptions of other babies 2) There was no significant difference in the two groups' perceptions of their own babies. (1) Breast-feeding mothes' general perceptions of their own babies were better than bottle-feeding mothers', but there was no significant difference between the two groups. (2) Breast-feeding mothers' perceptions about the basic needs of their own babies were better than bottle-feeding mothers', and there was a significant difference between the two groups (P<0.05). (3) Breast-feeding mothers'perceptions about the sense and motor abilities of their own babies were better than bottle-feeding mothers', but there was no significant difference between the two groups. (4) Breast-feeding mothers' perceptions about the necessary care of their own babies were lower than bottle-feeding mothers', but there was no significant difference between the two groups. 3) There was no significant difference between the two groups' positive-negative perceptions of their babies (1) 69% of breast-feeding mothers and 69.9% of bottle-feeding mothers had positive perceptions about general conditions, but there was no significant difference between the two groups (2) 57.1% of breast-feeding mothers and 46.4% of bottle-feeding mothers had positive perceptions about the needs of babies, but there was no significant difference between the two groups. (3) 50% of breast -feeding mothers and 48.2% of bottle-feeding mothers had positive perceptions about the sense and motor abilities of babies, but there was no significant difference between the two groups. (4) 57.1% of breast-feeding mothers and 60.7% of bottle-feeding mothers had positive perceptions about the necessary care for babies, but there was no significant difference between the two groups. 3. 54.8% of breast-feeding mothers tried to feed their own babies on the 1st day after delivery. 83.3% of breast-feeding mothers chose the feeding method for themselves. Only 9.5% of breast-feeding mothers took recomendations for breast-feeding from nurses. 78.6% of breast-feeding mothers had no chance to learn about breast-feeding. 69% of breast-feeding mothers complained about the difficulties of breast-feeding. The most serious problems were : 'insufficient milk supply', 'lack of knowledge about feeding technique' and 'problems with nipples'.

  • Purpose : Mothers and primary encounter and Korean traditional Gynecology clinician's perspective on breastfeeding, the importance and general knowledge, familiar with the breastfeeding-related issues to help resolve the maternal education and treatment. treatment options for a specific note down the need for Maternity and think that the existing studies and articles about breast diseases with reference to the view of the contemplated. Method : Maternity and breast disease with reference to research and review papers. Results : The first, from the past to encourage nursing mothers to eat a lot of nursing was given recently is changing the perception of adequate calories. and second, the posture of breastfeeding in prevention of mother's fatigue and pain and the baby's milk intake should be considered effective. The third, breast feeding is heavier than normal because of the surrounding muscle tissue in the breast to support the training may be helpful to. At last is emotional stability, and adequate nutritional intake and their milk is not enough awareness and interest in and education about breast-feeding mothers to breastfeed will help you look. Conclusion : Prenatal care program in order to facilitate breastfeeding and breast-feeding from the breast managed Postpartum Care Program consists of a map for the breast-related discomfort in advance to lead to prevention and to increase women's satisfaction seems to be.

모유수유에 대한 산모의 의식과 실천 현황 (Current Understanding and Practices of Breast feeding by Mothers)

  • 목 적 : 실질적인 모유수유 교육 계획을 세우기 위하여 산모의 모유수유의 실태와 의식을 조사하고 6년 전 시행했던 유사 연구와의 비교를 통해서 우리나라 모유수유의 추세를 파악하고자 하였다. 방 법 : 이화여자대학교 의과대학 동대문병원에서 건강한 신생아를 출산한 127명의 산모를 대상으로 하였다. 산모는 출산 후 1일에 면담하였고 1, 2, 3개월에 다시 전화 추적 면담으로 모유수유 상태와 계획을 조사하였다. 결 과 : 분만 직후 96.1%의 산모들이 모유수유를 계획하고 있었다. 이중 73.2%는 완전 모유수유를, 22.8%는 혼합수유를 계획하고 있었다. 모유수유 기간은 대부분에서 7-12개월을 계획하고 있었다. 모유수유를 주위에서 권장한 사람은 가족이 가장 많았다. 3개월이 경과되면서 완전 모유수유를 하는 산모의 비율은 50%정도로 감소하였고, 혼합 수유하던 산모들은 대부분 인공수유로 이행되었다. 이러한 추세는 1999년에 Jung 등이 시행했던 연구에서와 비슷하나 3개월까지 완전모유수유를 유지하는 비율은 증가하는 경향이다. 모유수유 실패 요인은 젖 부족이(35.0%), 직장 복귀가(27.5%)로 의료인의 지지와 교육으로 도와줄 수 있는 원인들이었다. 결 론 : 1999년 Jung 등의 연구와 비교하여 볼 때 생후 3개월까지 완전 모유수유를 유지하는 비율은 27%에서 50%로 증가하였으나, 모유수유를 실패하는 대부분은 생후 1개월에 인공수유로 전환됨을 알 수 있었다. 혼합 수유를 시행하는 산모에게 실질적이고 지지적인 교육을 통해서 모유수유 성공률은 증가될 수 있다.

임신시 모유 수유 계획의 실천에 대한 추적 관찰 (A Follow-up Survey of Mothers' Antenatal Breast-feeding Plans)

  • 목 적 : 임신 시 모유 수유를 계획한 수유모의 모유 수유 실천과 이에 영향을 주는 요인을 알아보고자 본 조사를 시행하였다. 방 법 : 임신 시 모유 수유를 계획했던 1,128명 중에서 지역과 병원급을 층화하여 무작위 추출한 152명을 연구의 대상으로 하였다. 2001년 11월부터 2002년 3월까지 연구 대상자의 출산 후 4개월에 전화로 설문을 실시하여 결과를 분석하였다. 결 과 : 1) 임신 시 모유 수유를 계획한 수유모 중에서 출산 4개월 후의 모유 수유율은 37.5%였고, 실패 이유는 66.3%가 모유가 나오지 않거나 양이 적어서라고 대답하였고 그 외 수유모의 직장 또는 일 13.7%, 수유모의 질환 7.4%, 변이 묽어서 5.3%, 아기의 질병 4.2% 등이었다. 2) 수유모의 연령, 자녀의 유무, 모유 수유를 결정한 사람, 가족의 형태에 따른 차이는 없었으나, 임신 시 모유 수유를 4개월 이상으로 계획했던 경우 모유 수유율이 2.3(95% 신뢰구간 1.15-4.62)배로 유의하게 높았다(P<0.05). 3) 전문대졸 이상의 수유모의 모유 수유율이 고졸 수유모보다 0.43(95% 신뢰구간 0.21-0.86)배로 유의하게 낮았으며 취업 수유모의 모유 수유율이 0.37(95% 신뢰구간 0.17-0.83)배로 유의하게 낮았다(P<0.05). 4) 모유 수유 장점과 수유의 방법에 대한 인식항목 20개에 대한 점수의 중앙값 12점을 기준으로 할 때 12점 이상인 사람의 모유 수유율은 41.5%였던 반면, 12점 미만인 경우 34.5%의 모유 수유율을 보여 인식이 높은 군이 낮은 군에 비해 1.35(95% 신뢰구간 0.70-2.62)배의 모유 수유율을 보였다(P>0.05). 5) 출산 장소가 산부인과 의원인 경우 종합병원 보다 모유 수유율이 3.97(95% 신뢰구간 11-14.23)배로 유의하게 높았다(P>0.05). 출산아 성별, 병원내 모유 수유, 모자동실 및 분만 형태에 따른 모유 수유율의 유의한 차이는 없었다. 결 론 : 모유 수유율 높이기 위해서 적극적인 산전 교육과 출산 후 병의원에서 모유 수유를 할 수 있도록 모자동실, 모유 수유 상담 및 교육이 적절하게 제공되어야 할 것이다. 또한 취업 수유모의 모유 수유를 위한 공간과 시설 및 휴가 문제 등이 사회 정책이나 국가 정책으로 제도화 되어야 할 것이다.

모유수유 저해요인에 대한 산모와 신생아실 간호사간의 인식정도 비교 연구 (A Comparative Study on the Perception of the Impediment Factors between Mothers and Nurses in Nursery)

  • The condition of nutrition during infancy will greatly affect infants' physical, emotional growth, especially breast feeding is important in their growth and development, and emotional stability, too. Despite such advantages of breast-feeding, its rate has continued to fall year after year in Korea : the 95% rate in 1960's has fallen to 25.4% in 1990. It is known that such a downfall of breast-feeding rate is associated with various factors. The purpose of the study is to examine mother's and nurses' in nursery perception of the impediment factors of breast-feeding : to compare those between of breast-feeding : to compare those between two groups : to provide fundamental data for developing strategies for increasing breast-feeding. The subjects were 45 new mothers from one hospital and the same number of nurses sampled from 3 university hospitals, in Seoul. The data were collected for 11 days from April 12 to April 23, 1994 and a questionnaire was developed based on the interview with 14 nurses and 10 mothers and the literature reviews. Liker's Five-Point Scale was used as measurement. The Data were analyzed using SPSS / PC and descriptive statistics, t-test, ANOVA. The results of study are as follows : 1) Most new mothers get the information about breast-feeding from their mothers, books or newspaper, and relatives in rank than professionals(nurses or doctors). 2) The impediment factors the breast-feeding are categorized as mothers, hospital system, medical personnels and social factors. The most frequently mentioned impediment factors from mothers are 'Difficulty by operational wounds'(3.13), 'Lack of will for breast-feeding'(3.47), 'Insufficient rest and sleep'(3.52) and 'Opposition from husband or his family members'(4.77)in rank. On the other hand those factors from nurses are 'Inadequate nipple condition'(2.37), 'Decreasing milk secretion given medicine after operation'(2.63), 'Mothers knowledge deficit by poorly prepared education'(2.79) and 'Mothers abhorance of breast-feeing'(3.87) in rank. 3) As for the hospital system, the highly perceived impediment factors by mothers are 'Seperation of baby from mother after birth'(2.78), 'Lack of space for breast-feeding in the hospital'(2.93), 'Lack of facility for informing the time for breast-feeding'(3.18) and 'Because of babys' preference artificial nipple by being accustomed to it in hospital(3.97), in rank. Meanwhile, those from nurses are "Seperation of baby from mother after birth",(1.92), "Inconsistency between hospital's nursing time and mother's breast secretion time" scretion time(2.97), "Lack of space for breast-feeding in the hospital"(3.39), and "Lack of facility for informing the time for breast-feeding"(3.74) 4) As for the medical personnels, the highestly perceived Impediment factor from mothers in "Lack of professional nurses for breast-feeding"(2.96), and the lowestly perceived one is "Doctors' reluctance"(4.75). Nurses perceived same as mothers, too. 5) As for the social factors, the highestly perceived impediment factor by mothers and nurses is "Inconvience of social activities"(2.83) and the lowestly ones are "The sense of self-sacrifice"(4.22) by mothers, and "The sense of old fashioned"(4.13) by nurses. 6) The difference of the perception of impediment factors between mothers and nurses is statistically significant only in mother factor.

영유아의 이유실태(離乳實態) -대전지역(大田地域)- (Weaning Practice for Infants in Daejeon City)

  • For the purpose of investigating the weaning practice for infants, this study of 251 infants aged from 3 months to 2 years old was conducted in Daejeon City during the the period of August 9 through 23, 1978. The results obtained were as follows: About 60% of the infants were breast-fed, 27.6%, mixed-fed, and 12.0%, bottled-fed. The reason for bottle-feeding was either the lack of breast-milk secretion or ill-health of mothers. There was no significant difference in the feeding methods according to a level of mother's education. Sixty precent of the infants began to be weaned 6. months. The most of the infants were completely weaned within 18 months. The knowledge on weaning was mainly obtained from the publications, such as newspapers, journals for women, child care booklets, etc. The difficulties in the weaning practice were the shortage of knowledge on weaning and the unacceptability by the infants. Liquid or semi-liquid foods were firstly given to the more than 60% of the infants as introducing weaning foods, while 23.3% of the babies were given solid foods. Interestingly, the earlier the birth order was, the more babies were given liquid or semi-liquid foods, The motives for weaning was either the boby's demand for other foods than milk(41.3%) or the lack of breast-milk secretion (31.3%). In the selection of supplementary foods, the nutritional value was given the highest consideration) (54.9%). many mothers (64.2%) prepared supplementary foods for infants at home, while 23.1% used commercial body foods and 12.7% gave infants adult's meals as supplementary foods. The major materials for home-made supplementary foods were fruits and vegetables (41.5%) and cereals (37.5%). The commercial supplementary foods (mainly powdered from) were not used much (never used; 53.6%) and even in the babies, who had consumed them once, nearly half of the babies (42.9) did not eat well. Accordingly, the many mothers (68.8%) welcomed the development of the domestic supplementary foods in good quality and one-third (31.2%) responded that they would consume them regardless of the price.

효과적인 모유수유 개념 분석 (Concept Analysis of Effective Breastfeeding)

  • Purpose: This study was conducted to analyze and clarify the concept of the effective breastfeeding. Methods: This study used Walker and Avant's process of concept analysis. Results: Effective breastfeeding is how to give infant adequate milk, which fulfills needs of mother and infant, from her breast so that they can be satisfied with the process and results. It included the effective breastfeeding properties as follows: feeding behaviors (positioning, latch on, and suckling), transferring a breast milk to an infant, mother-infant interaction, and satisfying their desires (satisfaction, comfort, mother's self-confidence, infant's adequate weight gain and defecation, adequate breastfeeding interval). The antecedent of effective breastfeeding were anatomical and functional normal breast, breastfeeding knowledge and steady-state of mother, and feeding desire, rooting reflex and normal oral cavity of infant. The consequences of effective breastfeeding were exclusive breastfeeding, infant and maternal health and wellbeing, and achievement of mother and infant attachment. Conclusion: The meaning of effective breastfeeding defined in this study will contribute to develop the effectiveness breastfeeding assessment tool and the nursing intervention for ineffective breastfeeding.

초산부부의 모유수유에 대한 지식, 태도 및 실천의지 (Primiparous Couples' Knowledge, Attitude and Practice regarding Breastfeeding)

  • Breastfeeding is an excellent way of feeding infants and continues to be an important source of nutrition and antibodies for infants. Although breast-feeding is believed to be important, the rate of breastfeeding among Korean women is very low. One reason for the low breastfeeding rate is that the health professionals in the past have given little practical help couples to overcome difficulties encountered during breastfeeding. The promotion of breastfeeding has recently become a high priority among health professionals because of the undisputed physiological, psychological, social, economic, and nutritional benefits. Fathers have been found to influence the course of breastfeeding, but no one has conducted a systematic investigation into in and included fathers perspectives of breastfeeding in Korea. It is important to find strategies to solve the problems by assessing the knowledge, attitude and practice of breastfeeding with fathers as well as mothers. The purpose of this research is to find ways to promote primiparous(first child) couples's breastfeeding by assessing their general knowledge, attitude and practice of it. More specifically its purposes is to identify the relationship between knowledge, attitude, practice and the background factors of breastfeeding of primiparous couples in order to promote and support breastfeeding. The data will help health professionals to guide first-pregnancy couples to prepare and overcome any difficulties encountered during breastfeeding. Data were collected through a questionnaire which included both structured and open-ended questions. The questionnaire was composed referencing earlier literature, studies and surveys. This survey was made postpartum 2-3days after childbirth on 96 couples. The numbers of subjects for data analysis were selected 51 couples through pilot screening test. Data were coded and analyzed using the Statistical Package for Social Sciences : Cronbach's alpha coefficient, T-test, ANOVA, Stepwise pearson's correlation coefficient. The relations between the variables of breastfeeding were examined using a Pearson's correlation coefficient. This study showed that, the internal validity of the instrument was tested by Cronbach's alpha. The result was : mother knowledge 0.72, mother attitude 0.88, and father knowledge 0.70, father attitude 0.92, practice 0.76 and planning of breast feeding 0.95. The average age of mothers in the sample was 28.9 years old, and the average age of fathers in the sample was 31.3 years old. The other descriptive informations available included. the study also found that fathers were more important than doctors or nurses. However results of the study indicate that there were no differences between father and mother knowledge of breastfeeding(t=-0.39, p=0.698), and father attitude was statistically significant higher than mother attitude of breastfeeding (t=2.24, p=0.030). In analysis, the variable 'practice' with breastfeeding was the relationship with mother knowledge (p=0.031) and mother attitude(p=0.015). In this study, the correlation between one couple' knowledge and attitude was not significant. Primiparous couples surveyed in the postpartum period regarding knowledge, attitude and practice about breastfeeding might be affected by recall bias, the effects of the euphoria of the first 24-48 hours postpartum, but these variables were not examined. This study results add to the body of knowledge about nursing care for breastfeeding mothers and fathers. This indicates that a need for education about breastfeeding and, that fathers be included in it as well as mothers.

A Survey on the Awareness of Human Milk Bank in Korean Female Health Care Providers

  • Objective: Human milk bank is known as best alternative option for sick infants. However, only two human milk banks (HMBs) are working in Korea. In the present study, we evaluated the awareness on the HMB in female health care provider to find out the issues that would help facilitating HMB establishment in Korea. Methods: The survey questionnaire was developed by a team composed with neonatologists, obstetricians, nurses, nutritionists, and health care specialists. The survey was performed for female health care workers in Soon Chun Hyang Cheonan Hospital between March and June in 2016. Eighty-eight of 110 questionnaires were collected and the results were analyzed. Results: Most of subjects were found to have correct knowledge on the breast milk feeding. However, only 49% of subjects had awareness of the presence of HMB. Unfortunately, less than half of them could get information about HMB by official way including mass media. In addition, 76% of subjects were found not to want to use HMB. Most important reason was found as a concern on the safety of donor milk including life style of donors, infection controls, and possible nutritional loss of banked milk. Conclusion: The purpose and function of HMB was not widely educated and it seems to be one of most important reasons why women have negative concept about HMB in Korea. Thus, further study is warranted with more numbers of subjects after active public education about the HMB.
