Pulmonary Mucormycosis with an Appearance of Consolidation (경화 양상으로 나타난 폐의 Mucormycosis)

  • The mucormycosis is a group of serious opportunistic infections caused by fungi of the class Zygomycetes and order Mucorales. Pulmonary mucormycosis is a relatively rare disease but typically manifested by a rapidly progressive, often fatal pneumonia in patients with diabetes mellitus, hematologic malignant neoplasms, or organ transplants. The radiologic manifestations of pulmonary mucormycosis are nonspecific and include progressive lobar or multilobar consolidations, pulmonary masses and pulmonary nodules. Recently, we experienced a pulmonary mucormycosis in 32-year-old man with uncontrolled diabetes. He complained of cough, left pleuritic chest pain and generalized weakness. Initial chest X-ray finding was the consolidation on the lower lobe of the left lung. On the sixth hospital day, bronchoscopic examination with lung biopsy revealed broad, non-septate hyphae with right-angle branching, diagnostic of mucormycosis, and consequently amphotericin B therapy was started. We performed a lobectomy of the left lower lobe of the lung on 29th hospital day.

Differentiation between Morgagni Hernia and Pleuropericardial Fat with Using CT Findings (CT 소견을 이용한 Morgagni 탈장과 심막주위지방의 감별)

  • Background: Generally hernia is diagnosed with simple chest or gastrointestinal x-ray. Sometimes CT or MRI can give lots of information for the diagnosis. However, there was no study for the differentiation with using CT findings between Morgagni hernia and pleuropericardial fat. The aim of this study was to evaluate the useful CT findings for differentiating Morgagni hernia from pleuropericardial fat. Material and Method: We retrospectively analyzed CT scans of eight patients with Morgagni hernia and 20 patients with abundant pleuropericardial fat without peridiaphragmatic lesions. All CT scans were performed with coverage of the whole diaphragm in the inspiration state. We evaluated 1) the presence of the defect of the anterior diaphragm, 2) the interface between the lung and fat, 3) the angle between the chest wall and fat, 4) the continuity between the extrapleural fat and fat, 5) the presence of the vessels within fat, and 6) the presence of a thin line surrounding fat. Result: In all cases with Morgagni hernia, the defect of the anterior diaphragm was seen. The interface was well-defined, smooth, and convex to the lung. The angle with the chest wall was acute. The continuity with the extrapleural fat was not seen. In the cases with abundant pleuropericardial fat, the defect of the anterior diaphragm was seen in three (15%). The interface was usually irregular (n=10) and flat (n=17). The angle with the chest wall was variable. The continuity with the extrapleural fat, that was markedly increased in amount, was usually seen (n=16). The thin line surrounding fat was seen in four cases with Morgagni hernia, however, not seen in all cases with pleuropericardial fat. All of the above findings were statistically significant, however, vessels within fat was not significant to differentiate Morgagni hernia (n=8/8) from pleuropericardial fat (n=14/20). Conclusion: The useful CT findings of Morgagni hernia were fatty mass with sharp margin, convexity toward lung, acute angle with chest wall, and thin line surrounding hernia. Branching structure within fatty mass representing omental vessels that has been known as a characteristic finding of Morgagni hernia was not useful for differentiating Morgagni hernia from pleuropericardial fat.

Taxonomic Characteristics of Korean-native Anacardiaceae (한국산(韓國産) 옻나무과(科)의 분류학적(分類學的) 연구(硏究))

  • This study was conducted to establish a systematized taxonomic problems of through the leaf morphological characters and leaf venation patterns, and stomatal cell patterns and cell characteristics of abaxial and adaxial surface of the leaflets by SEM, of 6 native species in Korea and 2 foreign species of the Genus Rhus in the Family Anacardiaceae. The results obtained from this study are summarized as followings: 1. Morphological study measured 32 characters of leaves from herbarium specimen and field-collected samples for each species. The results of cluster analysis based on the Euclidean distance showed that the species could be classified into 3 groups: R. sylvestris. R. typhina, R. succedanea: R. trichocarpa. R. chinensis. R. verniciflua: and R. ambigua. R. radicans subsp. orientale, Analysis of principal components showed 5 groups: The major factors in the first principal component group was length of petiole of the terminal leaflets, that in the second group angle of left side in the terminal leaflet bash, that in the third group area ratio between first and terminal leaflets, that in the forth group angle ratio between right and left side in the terminal leaflet base, and that in the fifth group was angle of main and secondary vein at midrib of terminal leaflet. Cumulative contribution by the first, second and third principal component group was explained with 82.6%, a large percent of all information. 2. The leaf venation pattern investigated using soft X-ray photography revealed clado-and reticulo-camptodromous types according to branching angle of the secondary vein. And three groups by the developing degree of secondary vein were R. trichocarpa, R. ambigua. R. chinensis, R. typhina; R. radicans subsp. onentale, R. succedanea, R. verniciflua: and R. sylvestris. Classification key for the Rhus of Korean-native Anacardiaceae was made by the venation pattern and devevoping degree of the secondary vein. 3. The stomatal cell patterns were greatly classified into paracytic and anomocytic types, specific among species according to stomatal and subsidiary cell patterns, and various differences among the species was determined. Microstructure of the adaxial and abaxial surfaces could be divided into synclinal and anticlinal cell wall patterns, and were specific-species. Stomatal cells of R. chinensis were surrounded with characterized villus-like cells.

  • In the fall of 1992, growth characteristics and cone-bearing behaviour of crown top shoots were investigated on Pin us koraiensis trees, belonging to age class VII and IV, in the School Forests, College of Forestry, Kangwon National University to obtain basic information for the design of Korean pine cone harvesting machine. The results were summarized as follows. 1. The length of terminal main shoots and lateral shoots of the trees in age class IV was 27~56cm and 18~47cm, respectively, both of which were significantly higher than those of the trees in age class VII. 2. There was no significant difference between two age classes in the branching angle ($24{\sim}89^{\circ}$) of lateral shoots and in the diameter (10~30mm) of two-year-old main shoots. However, one-year-old terminal main shoots of age class IV showed the higher diameter (9~18mm) than those of age class VII. 3. Mean cone number per tree was 5, and mean fresh weight of cone was 272g. No significant difference was found in these traits between both age classes. 4. Cones, on an average, hung on the two-year-old main shoots 15cm above the basal ends in age class VII and 33cm above in age class IV. Both lengths are equivalent to one half and four fifths of the shoots, respectively. 5. The fresh weight range of crown-top shoots containing cones was 1.0~7.4kg.

High Voltage Electron Microscopy of Structural Patterns of Plastid Crystalline Bodies in Sedum rotundifolium (HVEM에 의한 둥근잎꿩의 비름 (Sedum rotundifolium L.) 색소체의 결정체 구조)

  • Major contributions has been made in cellular ultrastructure studies with the use of high voltage electron microscopy (HVEM) and tomography. Applications of HVEM, accompanied by appropriate image processing, have provided great improvements in the analysis of three-dimensional cellular structures. In the present study, structural patterns of the crystalline bodies that are distinguished in mesophyll plastids of CAM-performing Sedum rotundifolium L., have been investigated using HVEM and tomography. Tilting, and diffraction pattern analysis were performed during the investigation. The titlting was performed at ${\pm}60^{\circ}\;with\;2^{\circ}$ increments while examining serial sections ranging from 0.125 to $1{\mu}m$ in thickness. The young plastids exhibited crystalline inclusion bodies that revealed a peculiar structural pattern. They were irregular in shape and also variable in size. Their structural attributes affected the plastid morphology. The body consisted of a large number of tubular elements, often reaching up to several thousand in number. The tubular elements typically aggregated to form a fluster The elements demonstrated either a parallel or lattice arrangement depending on the sectioning angle. The distance between the elements was approximately 20nm as demonstrated by the diffraction analysis. HVEM examination of the serial sections revealed an occasional fusion or branching of elements within the inclusion bodies. Finally, a three-dimensional reconstruction of the plastid crystalline bodies has been attempted using two different image processing methods.

  • This study was performed to compare the characteristics of hydraulic conductivity such as relative conductivity(RC), leaf specific conductivity(LSC), Huber value(HV), specific conductivity(SC), and diameter of vessels(${\mu}m$) and number of vessels($No./mm^2$) in branch junctions of the twenty-one deciduous broad-leaved species. The hydraulic conductivities of branch junctions decreased with increasing junction angle between stem and branch, and with decreasing diameter of branch. The RC and LSC of branch junctions related to branching types(ㅏ, Y, ${\Psi}$ type) were much lower in ㅏ and ${\Psi}$ types than in Y type. The diameter and number of vessels remarkably reduced in branch junctions as compared with the stem and branch.

Chromosome Redundancy and Tree Phenotype Variation in Autotetraploid Trifoliate Orange (동질 사배체 탱자에서 염색체 배가와 수체 표현형의 변이)

  • The study was conducted to investigate the possibility that epigenetic DNA methylation causes tree phenotypic variation in autotetraploids through evaluating the phenotypic variation and DNA methylation in autotetraploids occurred spontaneously from diploid trifoliate orange. Chromosome analysis confirmed that fourteen trifoliate orange trees of selected by flow cytometry were tetraploids (2n = 4X = 36) without any aneuploids. Chromomycin A3 staining determined that these trees were all autotetraploid with doubled chromosome set. Tree phenotypes, such as tree height and width, branching number, length, and angle, internode length, and leaf characteristics, varied in the autotetraploids. Chlorophyll indices were diverse in the autotetraploids, but photosynthetic rates were not significantly different. In addition, a wide range of variation was observed in stomatal density and guard cell length. Analysis of global cytosine DNA methylation showed that there was a variation of the methylation level in autotetraploids. More than half of 14 autotetraploids had at least 2 times higher methylation level than diploid trifoliate orange. The results indicate that tree phenotypic variation in autotetraploids might be related to global DNA methylation for reducing gene redundancy.

Assessment of Carotid Geometry by Using the Contrast-enhanced MR Angiography (조영증강 MR 혈관 조영술을 이용한 경동맥 기하학의 평가)

  • Purpose : To evaluate the geometry of carotid artery by assessing the images of contrast-enhanced MR angiography (CE-MRA) and interrelationships between the geometry of carotid artery and clinical factors. Materials and Methods : 216 consecutive patients who performed supraaortic CE-MRA with fast spoiled gradient-echo imaging were included. Their medical records were reviewed for variable information including risk factors predictive of generalized atherosclerotic disease (age, hypertension (HTN), diabetes mellitus, hyperlipidema, and smoking), sex, body weight, height, and body mass index (BMI). We reviewed the CE-MRA with carotid origin (3 types), carotid artery tortuosity, angle of internal carotid artery bifurcation, the type of aortic arch branching, and the presence of the coiling of carotid artery. Results : Multinomial logistic regression analysis showed that significantly contributed clinical backgrounds for carotid origin were the age and the BMI. With an increase of age at 1, the probability that the type of carotid origin become from type 1 to type 2 was 0.9 times (p=0.004) in right carotid artery (RCA), 0.9 times (p = 0.031) in left carotid artery (LCA), 0.9 times that are likely to be type3 from type 2 (p<0.001) in RCA and 0.9 times in LCA (p=0.009). Increase in BMI at 1 increased odds of becoming type 2 as 1.1 times (p = 0.067) in RCA, 1.1 times (p=0.009) in LCA and increased chance of becoming type 3 as 1.2 times (p = 0.001) in RCA, 1.2 times (p=0.003) in LCA. Mean value of right and left carotid tortuosity were $240.9{\pm}69.0^{\circ}$and $154.4{\pm}55.0^{\circ}$, respectively. Conclusion : The BMI, age, sex and presence of HTN affects the geometry of carotid arteries, the site of origin and tortuosity of carotid artery specifically.

The Variation of Natural Population of Pinus densiflora S. et Z. in Korea (V) -Characteristics of Needle and Wood of Injye, Jeongsun, Samchuk Populations- (소나무 천연집단(天然集團)의 변이(變異)에 관(關)한 연구(硏究)(V) -인제(麟蹄), 정선(旌善), 삼척집단(三陟集團)의 침엽(針葉) 및 재질형질(材質形質)-)

  • As a successive work of the variation studies of natural Pinus densiflora stands, some characteristics of individual trees of the three natural populations selected from the Kwang-won Province, the middle-east part of Korean peninsula, as shown in the location map, were investigated. And the statiscal differences between individuals within population, and between populations were analysed. Twenty trees from each population were selected for this study purpose. Doing this, those trees lagged in growth, usually showing poorer form, were eliminated. The results obtained are summarized as follows: 1. Though the average population ages had the ranage between 50 and 63, the growth of height or diameter was similar. Population No.9 is, however, considered to have better tree forms at glance. Population No.8 showed the heighest value not only in the clear-stem-length ratio. 0.53 but also in the crown-index 0.91. The higher value can be result from those trees having long lateral branches and relatively short crown height, meaning undesirable crown shape. In regard to the fine branchedness and the acuteness of branching angle, the population No.9. is considered to be a better one, whereas there was almost no difference in crown height among populations. 2. Checking the frequency distributions of the ratio of the clear-stem-height to the total height and the crown-indices, some difference between populations are considered. These might be attributed to the previous way of stand mangement which alters the density. 3. In the serration density, the average number of 54 per 1cm needle length, the significant differences exist between individual trees within population but not between populations. A few trees which extremly high serration density were observed. As in serration, so tendencies were in the number of stomata row and resin duct. 4. The population 8 had the resin duct index value of 0.074 as the highest which was twice or triple of the other ones. 5. The patterns of increasing process of the average 10-year-ring-segment were not similar till the 30 years of age, but beyond this, the tendency lines were aggregated. 6. Regading the average summer wood ratio, no diffrence between populations, but in the ranges, i.e. 23 to 30 in population No.8. and 16 to 36 in population No.9., with regad to the specific gravity of wood, there were hardly observed any difference between populations even in the ranges values. As the increase of tree ages, the increase of specific gravity was followed but the increasing patterns were not similar between populations. 7. No significant differences between populations in the average tracheid length and the range were detected. However, the length was increased according to the age increase. The increasing pattern was same between populations.
