관자뼈 HRCT 스캔 시 선량감소 방법에 관한 연구 (A Study on the Dose Reduction Method for Temporal Bone HRCT Scan)

  • 고 해상력 CT에 해당하는 관자뼈 CT는 높은 관전압과 얇은 단면두께 등의 적용으로 스캔 선량이 인접 부위 검사보다 높다. 이에 검사조건 중 재구성 알고리즘을 변화 적용하여 검사 선량을 줄이면서 병변에 대한 민감도가 우수한 알고리즘을 찾아 유의성과 임상 기초자료 제공 가능성을 알아보았다. 그 결과 100 kVp로 관전압을 낮추어 적용 시 선량이 약 35.6% 감소하였고, 100 kVp로 획득한 Raw data에 Definition 알고리즘 적용 시 SNR, CNR이 우수하였으며 다른 알고리즘과 비교 시 통계적으로 유의한 차이를 보였다(p<0.05). 그리고 구조적 유사도를 비교한 결과 SSIM index가 ROI 별 0.776, 0.813, 0.741로 분석되었다. 따라서 관자뼈 CT 스캔에서 알고리즘 변경적용은 CT 검사로 발생하는 선량을 일부 감소시킬 수 있고 임상 기초자료 측면에서 매우 의미가 있다고 생각한다.

유아성 흑백 신경외배엽성 종양 (Melanotic neuroectodermal tumor of infancy)

  • The melanotic neuroectodermal tumor of infancy is a rare neoplasm arising in the first year of life. A 33-day-old female infant had an enlarged alveolar ridge on the right anterior maxilla. Intraoral examination revealed a nonulcerative swelling at the site. An intraoral radiograph showed an ill-defined radiolucency on the right anterior maxilla and displacement of primary incisors from their alveolar sockets. CT scans revealed an expansion of the surrounding bone and partial destruction of the anterior wall of the premaxilla. Histopathologic examination showed the cytoplasm of neuroblastic cells and eosinophilic, epithelioid cells frequently contained a dark brown granular pigment that stained positively to vimentin and HMB45, focally positive to NSE and cytokeratin. Four weeks after the operation, CT scans showed a rapidly growing soft tissue mass occupying right maxillary sinus encroaching to the orbit and nasal cavity. The final diagnosis was made as a malignant melanotic neuroectodermal tumor of infancy.

자가 머리뼈 이식 후 뼈결손부의 면적 변화 (A Size Change of Bone Defect Area after Autogenous Calvarial Bone Graft)

  • Calvarial bone grafting in craniomaxillofacial trauma and facial reconstructive surgery is now widely recognized and accepted as a standard procedure. One of the commonly reported problems of calvarial bone graft is the contour defect caused by partial resorption of the graft. But, there are few reports that discuss the fate of the calvarial bone graft based on the quantitative data. In this article, the changes of grafted calvarial bone were evaluated using 3-dimensional computed tomography(CT). 9 patients were observed with the CT scans at 2mm thickness immediately after operation and at the time of last follow-up. The area of the bone defect was segmented on the 3-dimensional CT image and calculated by AnalyzeDirect 5.0 software. The immediate postoperative bone defect area of the recipient site and the donor site were $612.9mm^2$ and $441.5mm^2$, respectively, which became $1028.1mm^2$ and $268.8mm^2$, respectively at the last follow-up. In conclusion, the bone defect area was less increased on the donor site of calvarial bone graft than on the recipient site. And the CT scan is a valuable imaging method to assess and follow-up the clinical outcome of calvarial bone grafting.

피부근염-다발성근염(皮膚筋炎-多發性筋炎)에서 골주사(骨走査)를 이용(利用)한 근염활동성(筋炎活動性)에 관(關)한 연구(硏究) (A Study on Clinical Activity of Myositis by the Use of Radioisotope Bone Scan in the Patients with Dermatomyositis-polymyositis)

  • To evaluate the diagnostic usefulness of radioisotope bone scan and clinical activity of myositis, bone scan using $^{99m}Tc-Methylene$ diphosphonate was serially done before and after treatment with prednisolone in 10 patients with well-documented dermatomyositis-polymyositis. The observed results were as follows. 1. In all 10 patients before treatment with prednisolone, the bone scans showed evidence of increased muscle uptake. Muscle uptake was markedly increased in 4 patients, moderately increased in 3 patients and minimally increased in 3 patients. 2. The site of increased muscle uptake was consistent with the site of clinically involved muscle which was weak and tender. 3. The degree of muscle uptake correlated with the severity of the muscle weakness and tenderness at the scan was done. In all 10 patients treated with high dose prednisolone, muscle uptake was decreased following therapy. Above results suggest the radioisotope bone scanning may be useful in the diagnosis and treatment of patient with dermatomyositis-polymyositis.

Complex open elbow fracture-dislocation with severe proximal ulna bone loss: a case report of massive osteochondral allograft surgical treatment

  • We report a case of a 69-year-old right-dominant man who had an open Monteggia-like lesion of the right elbow (Gustilo-Andersen IIIA) with severe proximal ulna bone loss associated with an ipsilateral ulnar shaft fracture due to a motorcycle accident. The patient underwent two-stage surgery. Wound debridement and bridging external fixation were performed at first. Three months later, a frozen massive osteochondral ulnar allograft was implanted and fixed with a locking compression plate. A superficial wound infection appeared 5 weeks after the second surgery. Superficial wound debridement, negative pressure therapy, and antibiotics were administered for 3 months, achieving infection healing. At 3 years post-surgery, the elbow range of motion was satisfactory with a Disabilities of the Arm, Shoulder and Hand (DASH) score of 16.7. Radiographs and computed tomography scans showed good allograft-bone integration without allograft reabsorption or hardware loosening. Although not complication-free, massive ulna osteochondral allograft implantation can be considered a valid option in cases of open Monteggia-like lesions associated with ulnar shaft fracture and severe bone loss in active patients, whenever osteosynthesis or joint replacement is not a proper solution. This type of bone stock restoration allows for future surgery, if needed.

Effectiveness of Temporomandibular Joint Disorder Follow-Up Using Bone Scans

  • Purpose: To evaluate the effectiveness of temporomandibular joint (TMJ) disorder follow-up and determine the factors that affect the TMJ bone scan hot spot numerical value (bone scan value), and to compare this value to the diagnosis of patients with temporomandibular joint disorders (TMD), their treatment options, and the resolution of their symptoms. Materials and Methods: A retrospective cohort study was performed on 24 patients (four males, 20 females) who received TMD treatment in the Section of Dentistry, Seoul National University Bundang Hospital (Seongnam, Korea) from 2007 to 2014. An analysis of the significance test and correlation between TMD diagnosis, treatment options, a baseline the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) questionnaire, treatment before and after the clinical examination and subjective progress, and TMJ bone scan value change were completed by using SPSS version 12.0. Result: Although only 14 patients had bony factors that caused TMD, the average pre-treatment bone scan value of the all patients was $4.29{\pm}0.31$, which is higher than the finding for osteoarthritis (3.88), and reduced post-treatment bone scan value was found to be without a statistically significant difference (P=0.056). After the treatments, clinical symptoms in 18 patients disappeared, and six patients did not require additional treatment, although they still displayed subjective symptoms. It was observed that the higher the pre-treatment bone scan value, nonspecific physical symptoms, chronic pain index, characteristic pain intensity, disability score, were, the lower the post-treatment bone scan value was. And this reduced post-treatment bone scan value tendency was not shown with the pre-treatment depression index, but there was not a statistical difference. Conclusion: The post-treatment TMJ bone scan value tended to be insignificantly reduced in the 24 patients whose clinical symptoms were improved (P=0.056). Moreover, the TMJ bone scan value showed no relation to the TMD type or its related symptoms.

Bone regenerative effect of recombinant human bone morphogenetic protein-2 after cyst enucleation

  • Background: The aim of this study is to quantitatively evaluate the effect of rhBMP-2 for repair of bone defects after cyst enucleation using the osteogenesis index (OI). Methods: Under general anesthesia, 10 patients (12 lesions) underwent oral or maxillofacial surgery for cyst enucleation. Postoperatively, 12 lesions were divided into two groups: group A (six lesions) was treated with absorbable collagen sponge (ACS) in combination with rhBMP-2, and group B (six lesions) was treated with ACS alone. After 3 months, cone-beam computed tomographic scans were obtained to measure changes in the volume of the lesions. We then calculated the OI of each group at two different Hounsfield units to determine any statistically significant difference between these two groups (Mann-Whitney U test). Results: As tested at the level of new bone, the mean OI was 72.37 % in group A and 55.08 % in group B -a statistically significant difference (p = 0.041). As tested at the level of mature bone, the mean OI was 27.47 % in group A and 18.88 % in group B, but the difference was not statistically significant (p = 0.394). Conclusions: The application of rhBMP-2 after maxillofacial cyst enucleation accelerated new bone formation in the bone defects. Thus, the use of rhBMP-2 in combination with ACS may be considered an alternative to conventional bone grafting in some patients with postoperative bone defects.

Cone Beam Computed Tomography를 이용한 상악 임플란트 식립 전후의 골밀도 변화에 관한 연구 (ASSESSMENT OF BONE DENSITY ON MAXILLA AFTER IMPLANTATION WITH CONE BEAM COMPUTED TOMOGRAPHY)

  • Purpose: This study examined the significance of increased bone density according to time after implantation on maxilla using demographic data with CBCT and compared the bone density between before vs. after implantation using the Hounsfield index. Materials and Methods: Twenty-five implant site on maxilla were selected. Cone-beam computerized tomography (CBCT) scans were used for the analysis. The implant sites were evaluated digitally using the Hounsfield scale with EzImplant TM and the results were compared over time. Statistical data over time was carried out to determine the correlation between the recorded Hounsfield unit (HU) over time and gender difference using repeated ANOVA. Results: The bone density of implantation site over time showed an increase in the HU mean values. Immediately after implantation, bone density was significantly increased than bone density before implantation. Until 6 month follow-up, bone density showed stable increasement. There is no significant difference on gender. Conclusions: Using CBCT, bone density increased over time after implantation on maxilla. Bone density measurements using CBCT might provide an objective assessment of the bone quality as well as the correlation between bone density and stability of implant.

조영증강 $^{18}F$-FDG PET/CT가 이중에너지 X-선 흡수 계측법을 이용한 골밀도 및 체성분 측정에 미치는 영향 (Effect of Contrast-Enhanced $^{18}F$-FDG PET/CT on Measurements of Whole Body Bone Mineral Density and Body Composition by Dual-Energy X-Ray Absorptiometry)

  • PET/CT에 사용되는 방사성의약품 FDG와 CT 조영제가 DXA를 이용하여 측정하는 BMC, BMD, TFM, LBM 등의 결과에 미치는 영향을 알아보고자 하였다. 2011년 12월부터 2012년 3월까지 본원 PET/CT를 시행한 환자 중 본 연구에 참여를 동의한 환자 총 22명을 대상으로 하였다. 실험은 두 집단으로 나누어 실험을 진행하였는데 첫 번째 집단은 17명으로 PET/CT 용 방사성의약품인 FDG 정맥 주사 전에 WB DXA를 시행한 다음 FDG 정맥주사 1시간 후 CT 조영제 사용 없이 PET/CT (Non-Contrast Enhancement $^{18}F$-FDG PET/CT)시행 후 즉시 다시 WB DXA를 진행하였다. 두 번째 집단은 총 5명으로 첫 번째 집단과 마찬가지로 FDG 정맥 주사 전 WB DXA를 시행한 다음 FDG 정맥 주사 1시간 후 CT 조영제 사용하여 PET/CT (Contrast Enhancement $^{18}F$-FDG PET/CT)시행 후 즉시 다시 WB DXA 진행하였다. 결과는 PET/CT 전 후 DXA에서 WB-BMC, WB-BMD, TFM 그리고 LBM 등을 측정하였고 각 항목의 변화율을 산출하여 비교하였다. Non-Contrast Enhancement $^{18}F$-FDG PET/CT 전 후의 골밀도 및 체성분의 변화율은 WB-BMC, +2.4%; WB-BMD, +1.8%; TFM, -6.8%; LBM, +2.2%였고 각 항목의 변화는 통계적으로 유의한 차이가 없었다. Contrast Enhancement $^{18}F$-FDG PET/CT 전 후의 변화율은 WB-BMC, +13.7%; WB-BMD, +9.3%; TFM, -34.1%; LBM, +13.6%였고 Contrast Enhancement $^{18}F$-FDG PET/CT 전과 후의 골밀도 및 체성분 측정치의 변화는 통계적으로 유의하였다. 본 연구에서 Non-Contrast Enhancement $^{18}F$ PET/CT 후에는 DXA 시행은 결과에 영향이 없어 가능할 것이라 생각되어지나 Contrast Enhancement $^{18}F$-FDG PET/CT검사 후에 시행한 DXA 검사의 결과는 신뢰도가 감소될 것으로 사료된다.

Evaluation of alveolar bone loss following rapid maxillary expansion using cone-beam computed tomography

  • Objective: To evaluate the changes in cortical bone thickness, alveolar bone height, and the incidence of dehiscence and fenestration in the surrounding alveolar bone of posterior teeth after rapid maxillary expansion (RME) treatment using cone-beam computed tomography (CBCT). Methods: The CBCT records of 20 subjects (9 boys, mean age: $13.97{\pm}1.17$ years; 11 girls, mean age: $13.53{\pm}2.12$ year) that underwent RME were selected from the archives. CBCT scans had been taken before (T1) and after (T2) the RME. Moreover, 10 of the subjects had 6-month retention (T3) records. We used the CBCT data to evaluate the buccal and palatal aspects of the canines, first and second premolars, and the first molars at 3 vertical levels. The cortical bone thickness and alveolar bone height at T1 and T2 were evaluated with the paired-samples t-test or the Wilcoxon signed-rank test. Repeated measure ANOVA or the Friedman test was used to evaluate the statistical significance at T1, T2, and T3. Statistical significance was set at p < 0.05. Results: The buccal cortical bone thickness decreased gradually from baseline to the end of the retention period. After expansion, the buccal alveolar bone height was reduced significantly; however, this change was not statistically significant after the 6-month retention period. During the course of the treatment, the incidence of dehiscence and fenestration increased and decreased, respectively. Conclusions: RME may have detrimental effects on the supporting alveolar bone, since the thickness and height of the buccal alveolar bone decreased during the retention period.
