Cement Augmentation for Lateral Row Fixation in Rotator Cuff Repair: A Case Report

  • One of the most important factors leading to a successful healing of rotator cuff tear is good bone quality to secure the suture anchor in the bone for a stable fixation. However, rotator cuff tear are commonly found in elderly patients, and their proximal humerus often shows osteoporosis or cystic lesions. Especially when the transosseous repair prevails for a torn rotator cuff, a weak metaphyseal cancellous bone is often the case, which associated with difficulty in stable fixation of the lateral row suture anchor. In this situation, we were able to augment the lateral row fixation with polymethylmethacrylate bone cement. Although there is a concern of disturbance in the blood flow and healing potential, our case showed good clinical results with respect to healing. If we suspect a weak fixation of the lateral row suture anchor, bone cement seems to be a good option for augmentation.

Clinical Experience of the Brushite Calcium Phosphate Cement for the Repair and Augmentation of Surgically Induced Cranial Defects Following the Pterional Craniotomy

  • Objective : To prevent temporal depression after the pterional craniotomy, this study was designed to examine the safety and aesthetic efficacy of the brushite calcium phosphate cement (CPC) in the repair and augmentation of bone defects following the pterional craniotomy. Methods : The brushite CPC was used for the repair of surgically induced cranial defects, with or without augmentation, in 17 cases of pterional approach between March, 2005 and December, 2006. The average follow-up month was 20 with range of 12-36 months. In the first 5 cases, bone defects were repaired with only brushite CPC following the contour of the original bone. In the next 12 cases, bone defects were augmented with the brushite CPC rather than original bone contour. For a stability monitoring of the implanted brushite CPC, post-implantation evaluations including serial X-ray, repeated physical examination for aesthetic efficacy, and three-dimensional computed tomography (3D-CT) were taken 1 year after the implantation. Results : The brushite CPC paste provided precise and easy contouring in restoration of the bony defect site. No adverse effects such as infection or inflammation were noticed during the follow-up periods from all patients. 3D-CT was taken 1 year subsequent to implantation showed good preservation of the brushite CPC restoration material. In the cases of the augmentation group, aesthetic outcomes were superior compared to the simple repair group. Conclusion : The results of this clinical study indicate that the brushite CPC is a biocompatible alloplastic material, which is useful for prevention of temporal depression after pterional craniotomy. Additional study is required to determine the long-term stability and effectiveness of the brushite calcium phosphate cement for the replacement of bone.

Anti-Ionizing Radiation Effect of Selenium on Osteoporosis Model during Bone Repair Process (골다공증 모델의 뼈 재생기에 있어 셀레늄(Selenium)의 방사선 보호작용)

  • Selenium (Se) is an essential trace element for humans and animals, and several findings suggest that dietary Se intake may be necessary for bone health. Accumulating evidence indicates that Se compounds possess anticancer properties. Se is specifically incorporated into proteins in the form of selenocysteine and non-specifically incorporated as selenomethionine in place of methionine. This study evaluated protection by Se in the bone repair process in ovariectomized rats after irradiation. For such purpose, 80 ovariectomized female Sprague-Dawley rats were randomly divided into 4 experimental groups: ovariectomized (Ov), Ov/Se, Ov/irradiated (Irr) and Ov/ Se/Irr. A bone defect was created on the tibia of all animals 40 days after ovariectomy. Two days after surgery, only the Ov/Se and Ov/Se/Irr rats received 0.8 mg Se/kg. Three days after surgery, only the Ov/Irr and Ov/Se/Irr rats received 10 Gy of X-rays on the lower limb region. The animals were euthanized at 7, 15, 22 and 29 days after surgery to assess the repair process, which was evaluated by analysis of trabecular bone number (Masson Trichrome) and birefringence analysis (Picrosirius). It was possible to observe a delay in the bone repair process in the ovariectomized/irradiated group and similarity between the ovariectomized, Ov/ Se and Ov/Se/Irr groups. Our findings suggest that sodium selenite may influence a radioprotective effect in the bone repair of tibia of ovariectomized rats without toxicity.

  • The cleft alveolus occurs about 75% of cleft lip and palate patients. The purpose of bone grafting is improve the maxillary growth, rehabilitation of continuty of maxillary arch and providing bone for periodontal support for unerupted teeth. The bone grafting for alveolar cleft defect repair are classsified; primary bone grafting, early secondary bone grafting secondary bone grafting and late secondary bone grafting. In this article, we reported the cases of PMCB grafts for repair of the alveolar clefts showed potential benifit to the patient to induce a normal maxillary growth and providing bone foor periodontal support of unerupted teeth.

Repair of sports bone injury based on multifunctional nanomaterial particles

  • Nanoparticles have lower size and larger specific surface area, good stability and less toxic and side effects. In recent years, with the development of nanotechnology, its application range has become wider and wider, especially in the field of biomedicine, which has received more and more attention. Bone defect repair materials with high strength, high elasticity and high tissue affinity can be prepared by nanotechnology. The purpose of this paper was to study how to analyze and study the composite materials for sports bone injury based on multifunctional nanomaterials, and described the electrospinning method. In this paper, nano-sized zirconia (ZrO2) filled micro-sized hydroxyapatite (HAP) composites were prepared according to the mechanical properties of bone substitute materials in the process of human rehabilitation. Through material tensile and compression experiments, the performance parameters of ZrO2/HAP composites with different mass fraction ratios were analyzed, the influence of filling ZrO2 particles on the mechanical properties of HAP matrix materials was clarified, and the effect of ZrO2 mass fraction on the mechanical properties of matrix materials was analyzed. From the analysis of the compressive elastic modulus, when the mass fraction of ZrO2 was 15%, the compressive elastic modulus of the material was 1222 MPa, and when 45% was 1672 MPa. From the analysis of compression ratio stiffness, when the mass fraction of ZrO2 was 15%, the compression ratio stiffness was 658.07 MPa·cm3/g, and when it was 45%, the compression ratio stiffness is 943.51MPa·cm3/g. It can be seen that by increasing the mass fraction of ZrO2, the stiffness of the composite material can be effectively increased, and the ability of the material to resist deformation would be increased. Typically, the more stressed the bone substitute material, the greater the stiffness of the compression ratio. Different mass fractions of ZrO2/HAP filling materials can be selected to meet the mechanical performance requirements of sports bone injury, and it can also provide a reference for the selection of bone substitute materials for different patients.

Bone repair in defects filled with AH Plus sealer and different concentrations of MTA: a study in rat tibiae

  • Objectives: This study aimed to evaluate the effects on bone repair of different concentrations of mineral trioxide aggregate (MTA) added to AH Plus. Materials and Methods: Bone tissue reactions were evaluated in 30 rats (Rattus norvegicus) after 7 and 30 days. In the AH + MTA10, AH + MTA20, and AH + MTA30 groups, defects in the tibiae were filled with AH Plus with MTA in proportions of 10%, 20% and 30%, respectively; in the MTA-FILL group, MTA Fillapex was used; and in the control group, no sealer was used. The samples were histologically analyzed to assess bone union and maturation. The Kruskal-Wallis and Mann-Whitney tests were performed for multiple pairwise comparisons (p ≤ 0.05). Results: At the 7-day time point, AH + MTA10 was superior to MTA-FILL with respect to bone union, and AH + MTA20 was superior to MTA-FILL with respect to bone maturity (p < 0.05). At the 30-day time point, both the AH + MTA10 and AH + MTA20 experimental sealers were superior not only to MTA-FILL, but also to AH + MTA30 with respect to both parameters (p < 0.05). The results of the AH + MTA10 and AH + MTA20 groups were superior to those of the control group for both parameters and experimental time points (p < 0.05). Conclusions: The results suggest the potential benefit of using a combination of these materials in situations requiring bone repair.

Effects of Diet of Korean Safflower(Carthamus tinctorious L.) Seed Powder on Bone Tissue in Rats during the Recovery of Rib Fracture (토종홍화씨의 급여가 실험동물의 늑골골절 회복중 골조직에 미치는 영향)

  • This study was carried out to investigate the effects of diet of Korean safflower(Carthamus tinctorious L.) seed powder on bone tissue during the recovery of rib-fracture in rats. Male Sprague-Dawley rats of 10 weeks old, weighing 370$\pm$5g, were divided into two groups including the control group(C group, AIN-76 semipurified diet) and safflower seed group(S group, AIN-76 semipurified diet+10% safflower seed powder) and were fed experimental diets for 12 days after adaptation period. After this period, the 9th right rib was fractured surgically and sham-operation was also performed. Rats were fed with experimental diets for up to 30 more days after rib-fracture. The degree of bone repair was evaluated during the recovery period at the 8th, 11th, 16th, 21st, 30th days after the surgical operation by microscopic observation of the fractured rib tissue. In callus formation, the portion of hyaline cartilage was noticably higher in S group than C group. The intracatilagenous ossification was observed at the 8th day in S group, but at 11th day in C group. The intramembranous ossification in callus was widely found over the 8th day to the 11th day in S group, but it was shown over the 11th day to the 16th day in C group. Bone resorption was also occured more rapidly in S group as indicated by large numbers of osteoclasts observed. At the 30th day, most of trabecular bones were disappeared in S group, whereas still shwon in C group over wide ranges of fractured ribs. These results imply that the supplementation of Korean safflower seed powder influences in the recovery of bone fracture by accelerating the process of bone repair.

Open Repair of Massive Rotator Cuff Tears (광범위 회전근 개 파열의 개방적 봉합술)

  • Purpose: To evaluate the usefulness and functional results of open repair of massive rotator cuff tears combined with or without the tenoplasty of biceps long head. Materials and Methods: From March 2003 to August 2004, we evaluated 18 cases of the patient treated with open repair of massive rotator cuff tears. The mean age was 56 years and mean follow-up period was 15 months. We performed open repair of massive rotator cuff tears by tendon to bone repair, but in irrepairable 4 cases for tendon to bone repair performed open repair combined with tenoplasty of biceps long head. The functional results were assessed using the Constant score, the parameters of which were pain, dialy activity, mobility, strength and satisfaction. Results: The functional results were excellent in 4 cases, good in 8 cases, fair in 3 cases and poor in 3 cases. And the results of open repair combined with tenoplasty of biceps long head were good in 2 cases and fair in 2 cases. Re-repair was done in 1 case for re-rupture. And the functional result of this case was fair. The 3 poor cases were suspected re-rupture, the factor of which old age and weakened rotator cuff. It was impossible to do re-repair. Conclusion: The open repair combined with or without tenoplasty of biceps long head is a useful and effective method for massive rotator cuff tear.

  • The purpose of this retrospective study was to evaluate the treatment outcome of proximal tibial graft. Thirty-nine patients were grafted with proximal tibia for the repair of maxillofacial defect. Complications can be considered incidental and neglectable clinical significance. It is concluded that the proximal tibial metaphysis offers a promising alternative sources of cancellous bone for the repair of maxillofacial defect.

  • This study was performed to evaluate the histopathologic changes of the rabbit tibial compact bone using internal irrigation with both cold and room - temperature saline on drilling. The medial surface of the rabbit tibia was drilled with specially designed pilot drill (2.0mm in diameter) at 300 rpm. When drilling, two different temperature salines were injected (experimental group I : $4^{\circ}C$ saline, experimental goup Ⅱ : room - temperature saline). And the control group was drilled without cooling agent. The three rabbits in each two experimental and control groups were sacrificed 1, 2, 4, and 8 weeks after. The bone tissues including the bony defects were fixed with 10% neurtal buffered formalin, decalcified with formic acid, embedded in paraplast, and sterile sectioned at 5-6${\mu}m$. And then tissue specimens were stained with H - E and observed under light microscope. The results were as follow : The experimental groups showed early bone repair than the control group at all intervals. They underwent the same course of bone repair until 4 weeks. But the experimental group I showed slightly better bone maturation than the experimental group Ⅱ at 8 weeks.
