A Study on the Result of Test Site on BMD (골다공증 검사 시 검사부위에 따른 결과에 대한 고찰)

  • The measurement of Korea people's Radial-terminal region were frequently measure of the left hand of Radial-terminal region due to the most Korean's are right-handed and it occasionally showed incorrect results. Therefore, in this study, we accessed a correlation with error of measurement and reduced the measurement error invalid. We reviewed 50 adults patients, from March 2012 for a certain period of time, visited the orthopedic center for the neutral position of forearm of plain radiography and measured the left side of the distal radial-terminal region containing the terminal region of the right distal radius. Then we have compared and analysed both T-score. As a result, the lower value of left wrist were 45 out of 75 which is approximately 60% of left wrist group while the lower value of right wrist were 30 out of 75 which is approximately 40% of right wrist group.

Quantitative Ultrasound for Osteoporosis Screening in Postmenopausal Women (폐경 후 여성에서 골다공증의 조기검진도구로서 골초음파의 유용성)

  • Objectives : To evaluate the diagnostic value of quantitative ultrasound (QUS) in the prediction of osteoporosis as defined by dual energy x-ray absorptiometry (DEXA) in postmenopausal women. Methods : Questionnaires and height and weight measurements were used in the investigation of 176 postmenopausal women. QUS measurements were taken on the right calcaneus while bone mineral density (BMD) measurements of the lumbar spine and femoral neck were made with DEXA. The areas under the curves (AUC) of the speed of sound (SOS) for osteoporosis in the lumbar spine and femoral neck were obtained through receiver operating characteristic (ROC) analysis and evaluated. A comparison was made, for osteoporosis in the lumbar spine and femoral neck, between the AUCs of the logistic model with clinical risk factors and SOS. Results : Pearson's correlation coefficients of SOS and lumbar spine BMD, and of SOS and femoral neck BMD were 0.26 and 0.37. The AUC for the logistic model in its discrimination for lumbar spine osteoporosis was 0.764, and for SOS 0.605. The AUCs for the logistic model in its discrimination for femoral neck osteoporosis and for SOS were 0.890 and 0.892, respectively. Conclusions : These results suggest that the diagnostic value of QUS as a screening tool for osteoporosis is moderate for the femoral neck, but merely low for the lumbar spine and that the predictability provided by SOS is no better than that by the sole use of clinical risk factors in postmenopausal women.

Effect of the Tai Chi Fall Prevention Program for Elderly Women Living in the Community (지역사회거주 여성노인에게 적용한 타이치 낙상예방 프로그램의 효과)

  • Purpose: The study aimed to examine the effects of Tai Chi fall prevention program on risk factors for fall, fear of falling, and quality of life among elderly women living in the community. Methods: A quasi-experimental pretest-posttest design with non-equivalent control group was used. Sixty participants were recruited from 4 senior citizen centers in a city. The 1-hour Tai Chi fall prevention program was provided three times a week for 6 months to the experimental group. Risk factors for fall including fracture risk and bone mineral density, fear of falling, and quality of life were measured at the baseline and at 6 months. Results: Participants were 75 years old in average. At the completion of 6 month program, the experimental group showed lower fracture risks, less fear of falling, and higher scores in several domains of quality of life than the control group, after controlling for the pretest scores, fall experience, and regular exercise habit. Conclusion: The Tai Chi fall prevention program was safely applied to elderly women with improvement in fracture risk, fear of falling, and several domains of quality of life. Future study is necessary to confirm the longer effect of the Tai Chi program for the prevention of fall episodes.

Analysis of Biomechanical Responses for the Anterior Cervical Plate Fixation in relation to Bone Mineral Density (골밀도에 따른 전방 내고정 장치 시술 후 경추부의 생체역학적 거동에 대한 분석)

  • 본 연구에서는 환자의 골다공증 유무에 따른 내고정 장치 시술 직후 및 융합 후의 안정성을 평가하기 위해 다양한 하중 모드에서 C5-C6 운동분절의 생체역학적 거동을 분석하였다. 이러한 목적으로 먼저, C5-C6 경추부의 유한요소 모델을 구현하여 검증하였다. 모델의 결과는 기존 실험치와 유사하여 신뢰성이 부여되었다. 검증된 모델은 Smith-Robinson 방식으로 골이식물을 삽입한 후 전방 내고정 장치를 적용한 시술 상황을 재현하기 위해 수정되었다. 수정된 모델은 두 종류로 구현되었다. (1) 첫 번째 모델에서는, 시술 직후의 상황을 재현하기 위해 골이식물과 종판의 경계면에 접촉요소를 사용하였다. (2)두 번째 모델에서는 완전히 융합된 상황을 나타내기 위해 골이식물을 종판에 고정하였다. 골다공증의 효과를 예측하기 위하여 두 모델의 해면골에 대한 탄성계수를 변화시켰다(정상: 100MPa, 골다공증: 40MPa). 각 모델의 C5 주체의 상위면에 73.6N의 압축 하중을 가한 후에 108Nm의 굴곡/신전, 굽힘, 비틀림 하중을 가하였으며, C6 추체의 하단면은 모든 방향에 대하여 구속하였다. 전체적인 결과에 있어서 상대적 회전운동, 미끄럼운동, 골이식물 내에서의 von Mises 응력의 경우 정상 모델에 비해 골다공증 모델에서 증가함을 보였으며, 특히 시술 직후의 모델에서 비틀림 하중이 가해진 경우, 상대적 회전운동 및 미끄럼 운동이 가장 높게 예측되었다. 이는 골다공증환자에게 전방 내고정 장치를 시술한 경우 골이식물의 파단 및 유합의 실패가 비틀림 하중에서 발생할 수 있음을 나타낸다. 해면골의 von Mises 응력은 시술 직후에 골다공증 모델의 모든 하중 모드에서, 유합 후에는 굽힘 하중 외의 모든 하중에서 ultimate strength를 초과하는 것으로 나타나 골다공증 환자에게 screw의 해리가 발생할 가능성이 높은 것으로 예측되었다. 따라서 골다공증 환자에게 과도한 운동이 발생하지 않도록 하기 위해서 시술 후 세심한 주의와 halo 같은 견고한 정형술이 필요할 것으로 사료된다.

A Study on the Bone Mineral Density and Related Factors in Korean Postmenopausal Women (폐경 여성의 골밀도 상태와 이에 영향을 미치는 요인에 관한 연구)

  • A total of 216 women of natural menopause over 40 years were interviewed to find the relationships among general environmental fctors, anthropometric measurements, daily nutrient intake, and BMD. The average age of the subjects was 54.0 years. Average ages of menarche and menopause were 16.7 years and 47.7 years, respectively. In general, energy and nutrients intake of the subjects were relatively low compared to the Korean recommended dietary allowances(RDA). Specifically, intakes of energy, protein, clcium, iron, and vitamin A were lower than Korean RDA, BMD levels of lumbar spin (LS), femoral neck(FN), ward's triangle(WT), and trochanter(TC) were significantly decreased wit age in descending order of LS, FN, TC, and WT BMD, accordingly. BMD levels of FN and TC of the subjects were higher compared to those of the same age but BMD levels of LS and WT were lower than those of the same age from the general population. For factors related to BMD levels, hight, weight, BMI, and hip circumference had positive correlations with BMD. On the other hand, menarche, and WHR had negative correlations with BMD. Protein, lipid, iron, vitamin B1, vitamin B2, and niacin intake among factors related nutrient intakes were positively correlated with BMD levels. Based on the stepwise regression analysis, the factors and vitamin B1 intakes. Overall, BMD was decreased with age as can be expected. For those being tall, moderately weighed, and smaller waist circumference, BMD levels were high when the relationships of body characteristics and BMD levels were considered. The higher BMD levels were found for those with high intake of protein and vitamin B1. From the findings, it is suggested that the women after menopause keep the optimum body weight and good eating habits. Specially intakes of good quality protein, vitamin B1 seemed to be important.

Effects of Herbal Medicines on Bone Mineral Density Score in Osteoporosis or Osteopenia: Study Protocol for a Systematic Review and Meta-Analysis (한약이 골다공증 혹은 골감소증 환자의 골밀도에 미치는 영향: 체계적 문헌고찰 및 메타분석을 위한 프로토콜)

  • Objectives The impact of social and economic losses on society due to the occurrence of osteoporosis patients is serious. The purpose of this study is to identify the types and effects of herbal medicine for osteoporosis and osteopenia. Methods We will investigate 8 online databases (MEDLINE/PubMed, Cochrane library, EMbase, China National Knowledge Infrastructure [CNKI], J-stage, National Digital Science Library [NDSL], Research Information Sharing Service [RISS], Koreanstudies Information Service System [KISS]) without language, publication date limitation for the clinical study of herbal medicine for osteoporosis and osteopenia published between the start of the database and 2021. The frequency, duration and pattern of the any type of herbal medicine utilization will be analyzed. Results Randomized controlled trials about herbal medicine or herbal medicine extracts for osteoporosis and osteopenia should be included in the study. Cochrane risk in bias tools will be used to evaluate the methodological quality of the study. A risk ratio or mean difference with a 95% confidence interval will show the effects of herbal medicine or herbal medicine extracts for osteoporosis and osteopenia. Conclusions The results of the systematic review will reflect the current status of herbal medicine treatment for osteoporosis and osteopenia, and it can be expected to verify the validity of the effectiveness, and provide it as a basic data for the use of herbal medicine in clinical utilization.

Studies on phosphorus deficiency in the Qianbei-Pockmarked goat

  • Objective: Qianbei-Pockmarked goats are affected by a disorder locally referred to as 'Ruanguzheng Disorder', which is characterized by emaciation, lameness, muscular relaxation, stiffness of the extremities, and abnormal curvatures of the long bones. Our objective was to determine the relationship between the disorder and phosphorus deficiency. Methods: Tissue samples were collected from affected and healthy animals, while soil and herbage samples were collected from affected and healthy pastures. Biochemical parameters were determined using an automatic biochemical analyzer (OLYMPUS AU 640, Olympus Optical Co., Tokyo, Japan). Mineral contents in soil, forage, and tissue were determined using a Perkin-Elmer AAS5000 atomic absorption spectrophotometer (Perkin-Elmer, Norwalk, CT, USA). Results: The results showed that phosphorus contents in herbages from affected pastures were markedly lower than those from healthy areas (p<0.01), and the ratio of calcium to phosphorus in the affected herbages was 12.93:1. The phosphorus contents of wool, blood, tooth, and bone from affected animals were also markedly lower than those from healthy animals (p<0.01). Serum phosphorus values in affected animals were much lower than those in healthy animals, while serum alkaline phosphatase values from affected animals were markedly higher than those from healthy animals (p<0.01). Inorganic phosphorus values from affected animals were approximately half of that in the control group. Supplementation of disodium hydrogen phosphate prevented and cured the disorder. Conclusion: This study demonstrates that Ruanguzheng disorder in Qianbei-Pockmarked goats is primarily caused by phosphorus deficiencies in herbage due to fenced pastures and natural habitat fragmentation.

Anti-osteoarthritic effects of a combination of pomegranate concentrate powder, Eucommiae cortex and Achyranthis radix in rats

  • Objectives: We examined the effects of a mixed formula consisting of dried pomegranate concentrate powder (PCP) and the aqueous extracts of Eucommiae cortex (EC) and Achyranthis radix (AR) in rats with surgically induced osteoarthritis (OA). Methods: Two weeks after OA-inducing surgery, a PCP:EC:AR 5:4:1 (g/g) combination or single formula was orally administered. Changes in body weight, knee thickness, maximum knee extension angle, bone mineral density of the knee joints, femoral and tibial articular surfaces, and compressive strength of the femoral and tibial articular cartilage (AC) were assessed, along with the prostaglandin E2 level, 5-lipoxygenase, matrix metalloproteinase (MMP)-2 and MMP-9 activity, and chondrogenic gene mRNA expression in the femoral and tibial AC with the synovial membrane (SM). In addition, the number of cleaved poly(ADP-ribose) polymerase, cyclooxygenase and tumor necrosis factor-${\alpha}$-immunoreactive cells in the femoral and tibial AC with SM were monitored, and the rate of cell proliferation was determined with a 5-bromo-2'-deoxyuridine uptake assay. Results : The signs of surgically induced OA in rats were significantly inhibited by both PCP, EC and AR combined and single formulas. In particular, the combination formula-treated OA model rats showed dose-dependent, significantly increased inhibitory activity against all tested criteria compared with single formula-treated rats. Conclusions: Taken together, our results suggest that the combination formula synergistically increased the anti-OA effects of its components through anti-inflammatory and chondrogenic activity in rats with surgically induced OA. In addition, 200, 100 and 50 mg/kg combination formula treatments showed dose-dependent inhibitory activity against all of the tested criteria.

A Study on the Dietary and Lifestyle Changes of Middle-Aged Women in the Gwangju Area in the COVID-19 Era (광주지역 중년여성의 코로나바이러스 감염증 19 시대 식생활 및 라이프 스타일 변화 연구)

  • Objectives: This study aimed to investigate the changes in the eating habits and lifestyle of middle-aged women in Gwangju during the COVID-19 pandemic. Methods: A total of 428 middle-aged women aged between 40 and 60 participated in a survey relating to general information, food and lifestyle, health functional food, and menopausal symptoms. The correlation between the variables was analyzed. Results: In the positive habits, the intake of nutritional supplements for immunity enhancement increased the most, followed by the use of media to learn healthy eating tips, and diets including healthy food. Negative habits increased in the order of frequency of taking delivery orders, levels of stress or anxiety, and time spent sitting or watching movies. In the case of recommended foods, the intake increased the most in the order of eggs, fruits, vegetables, milk/dairy products, and seaweed. Non-recommended foods increased in the order of meat, bread, rice, and noodles. The awareness of health functional foods was in the increasing order of interest, knowledge, consumption experience, and purchase amount. The type of health functional food intake was in the increasing order of probiotics, multivitamin and mineral supplements, vitamin C, collagen, and omega-3. Menopausal symptoms were in the increasing order of bone and joint pain, poor sleep quality, emotional ups and downs, loneliness, and feeling of emptiness. In the correlation of major variables, positive habits showed a significant positive correlation with recommended food intake and the recognition of health functional foods. Negative habits showed a significant positive correlation with non-recommended food intake and a significant positive correlation with menopausal symptoms. Recommended food intake showed a significant positive correlation with health functional food recognition and intake and menopausal symptoms. Conclusions: This study suggests that it is necessary to establish social measures to reduce the negative effects of the COVID-19 pandemic on middle-aged women and to ensure effective self-management through a healthy lifestyle since the pandemic has a long-term impact.

One-year experience of oral substrate reduction therapy in three patients with Gaucher disease type I

  • Purpose: Eliglustat is an oral substrate reduction therapy (SRT) approved for adults with Gaucher disease type I (GD1) who are extensive, intermediate, or poor CYP2D6 metabolizers. Here we report one-year experience of eliglustat switch therapy from long-term enzyme replacement therapy (ERT) in three adult patients with GD1. Materials and Methods: Medical history, clinical (hemoglobin concentration, platelet count, and bone mineral density) and biochemical parameters (angiotensin converting enzyme, total acid phosphatase, and lyso-gb1) of the patients were collected and evaluated by retrospective review of medical records at every 3, 6, or 12 month after switch to SRT. Results: Patient 1 was a 43-year old female diagnosed GD1 and her clinical and biochemical parameters were stabilized for more than 20 years by ERT. Due to the burden of regular hospital visit, she switched to SRT. During one-year of SRT, clinical parameters and biomarkers were maintained stable. However, after suffering acute febrile illness during SRT, she decided to re-switch to ERT due to concerns about drug interaction. Patient 2 was 41-year old male, younger brother of patient 1 and Patient 3 was 31-year old male. They switched to SRT in clinically stable condition with long-term ERT. The one-year SRT was tolerable without specific safety issue and the clinical parameters were maintained stable. Conclusion: One-year eliglustat therapy in three adult patients with GDI was generally tolerable and effective for maintaining the clinical parameters and biomarkers. However, the drug compliance, concurrent drug interactions, and long-term safety of eliglustat should be carefully monitored.
