Study on the fundamentals of the Five Viscera (오장(五臟)의 본(本)에 대한 연구)

  • In oriental medicine, there is the fundamentals to lead a stirring life, and it is an important matter that make a study of fundamentals for treatment. The su-wen(素問) mentions the fundamentals of sheng(生之本), the fundamentals of qi(氣之本), the fundamentals of feng-cang(封藏之本), the fundamentals of ba-ji(罷極之本) and the fundamentals of cang-lin(倉廩之本). They are terminologies expressing physiological functions of the five viscera - heart, lungs, kidneys, liver and spleen. The five viscera are expressed through face, fur, hair, nail and lips. And the five viscera are closely connected with blood, skin, bone, muscle and flesh. Through the changes of personal appearance, we can recognize those of their internal organs. In oriental medicine, they are called as the cang-xiang(藏象). Heart is the fundamentals of sheng(生), and it is related to spirit, blood and blood vessel. Lungs are the fundamentals of qi(氣), because they master the extensive meaning of qi(氣). Kidneys are the fundamentals of feng-cang(封藏) which means seclusion. Liver is the fundamentals of ba-ji(罷極) that have several view by concept. Spleen, stomach, large and small intestines, paunch and bladder are the fundamentals of cang-lin(倉廩), that is to say, gastrointestinal tract that ingest diet and digest it.

The Versatility of Cervical Vertebral Segmentation in Detection of Positional Changes in Patient with Long Standing Congenital Torticollis

  • Background Congenital muscular torticollis (CMT) is a benign condition. With early diagnosis and appropriate management, it can be cured completely, leaving no residual deformity. However, long-standing, untreated CMT can lead to permanent craniofacial deformities and asymmetry.Methods Nineteen patients presented to the author with congenital muscular torticollis. Three dimensional computed tomography (3-D CT) scans was obtained upon patient’s admission. Adjustment of skull’s position to Frankfort horizontal plan was done. Cervical vertebral segmentation was done which allowed a 3D module to be separately created for each vertebra to detect any anatomical or positional changes.Results The segmented vertebrae showed an apparent anatomical changes, which were most noticeable at the level of the atlas and axis vertebrae. These changes decreased gradually till reaching the seventh cervical vertebra, which appeared to be normal in all patients. The changes in the atlas vertebra were mostly due to its intimate relation with the skull base, while the changes of the axis were the most significantConclusion Cervical vertebral segmentation is a reliable tool for isolation and studying cervical vertebral pathological changes of each vertebra separately. The accuracy of the procedures in addition to the availability of many software that can be used for segmentation will allow many surgeons to use segmentation of the vertebrae for diagnosis and even for preoperative simulation planning.

  • Typhoid fever is an acute systemic febrile disease caused by Salmonella typhi. The classic picture of the disease consists of prolonged continuous or remitting fever, abdominal pain, diarrhea, rose spots and delirium. Salmonella infection can lead to diffuse organ involvement, including bone, lung, thyroid, kidney, liver, spleen, heart, pericardium, intestine and skin and cause a variety of complications. Pulmonary manifestations occur in only 1 percent of the patients. Mild cough with sticky sputum is the earliest symptom and bronchitis, pneumonia and lung abscess were presented. Recently we experienced a case of typhoid fever complicated by bronchitis, dysplasia in a 37-year-old male physician who was improved with ceftriaxone and ciprofloxacin We report this case with a review of the literature.

  • Purpose: Cemental tear is an uncommon form of root fracture that can lead to rapid localized attachment loss. When it is exposed in the periodontal pocket, it should be removed to prevent accumulation of dental plaque and calculus. Material and Methods: 2 patients were diagnosed as a cemental tear and they were treated with conventional flap operation and subgingival curettage. Additional treatments such as bone graft or guided tissue regeneration were not performed. Result: Symptoms subsided after the treatment. Periodontal pocket has been reduced but no gain of clinical attachment was observed. Remnant of cemental fragment remained after curettage. However, periodontal pocket was stably maintained and there was no recurrence. Conclusion: Periodontal attachment loss associated with cemental tear can be successfully treated with conventional periodontal surgical and nonsurgical procedures.

  • Dating and dosimetry using electron spin resonance (ESR) in 20th Century developed at both Yamaguchi University and Osaka University have been reviewed with emphasis on new prospects and strategies in 21th century. Natural radiation have been generating radicals that accumulated in archaeological and geological materials. ESR detects these radicals and the ESR signal intensity is proportional to the radiation dose and therefore the age. The assessment of the total dose of natural radiation and the annual dose rate give their ESR ages. The ESR dating of stalactites and stalagmites ant Akiyoshi cave in Yamaguchi prefecture in 1975 was extended to anthropological dating using bones and tooth enamel excavated in Greek Petralona cave. Fossils of shells and corals gave the ages of marine terraces and sea-level changes. Quartz grains gave the ages of geothermal alteration and fault movements. Future ESR dating of ices at outer planets anf their satellite are also investigated as basic studies for ices od $H_2O,\;CO_2,\;SO_2$ as well as terrestrial hydrates in laboratory. Atomic bomb radiation dosimetry at Hiroshima and Nagasaki using ESR lead to the dosimetry of personnel, Chemobyl and JCO criticality accidents. Monitoring of radiation dose with sensitive materials with tissue equivalence are being developed. finally a new scanning ESR imaging apparatus (a near field microwave microscope) developed in our laboratory gave ESR images of Radicals from fossils to Si-CVD and diamond films as summarized in my book in 2002.

Mandibular reconstruction using customized three-dimensional titanium implant

  • Mandibular defects lead to severe deformation and functional deficiency. Vascularized osteocutaneous tissue has been widely used to reconstruct the mandible. However, it is technically challenging to shape this type of grafts in such a manner that they resemble the configuration of the mandible. A 48-year-old female patient who underwent anterolateral thigh (ALT) flap coverage after a tongue cancer excision was diagnosed with a tumor recurrence during the follow-up. A wide excision mandibulectomy and mandibular reconstruction with an ALT flap and a titanium implant were performed. The prefabricated titanium implant was fixed to the condyle. Then, an ALT flap was harvested from the ipsilateral thigh and anastomosed. After confirming that the circulation of the flap was intact, the implant was fixed to the parasymphysis. On the radiograph taken after the surgery, the prosthesis was well positioned and overall facial shape was acceptable. There was no postoperative complication during the follow-up period, 1 year and 2 months. The prefabricated implant allows the restoration of facial symmetry without harvesting autologous bone and it is a safe and effective surgical option for mandibular reconstruction.

  • In patients with severely resorbed alveolar bone, it is difficult to gain retention in denture. Lack of retention makes denture unstable and lead to trouble in using denture. Suction denture seals the entire denture border with movable mucosa and this sealing mechanism forms negative pressure beneath the denture and produce higher retention and stability to denture. In this case, 4 edentulous patients visited for lack of retention with dentures. Considering their high expectation with retention, suction denture concept was used to fabricate retentive and stable denture. The purpose of this case report is to compare and analyze the considerations of suction denture restorations in edentulous patients.

  • Objectives: Automated audiometry provides an opportunity to do audiometry when there is no direct access to a clinical audiologist. This approach will help to use hearing services and resources efficiently. The purpose of this study was to review studies related to automated audiometry by focusing on the implementation of an audiometer, the use of transducers and evaluation methods. Methods: This review study was conducted in 2017. The papers related to the design and implementation of automated audiometry were searched in the following databases: Science Direct, Web of Science, PubMed, and Scopus. The time frame for the papers was between January 1, 2010 and August 31, 2017. Initially, 143 papers were found, and after screening, the number of papers was reduced to 16. Results: The findings showed that the implementation methods were categorized into the use of software (7 papers), hardware (3 papers) and smartphones/tablets (6 papers). The used transducers were a variety of earphones and bone vibrators. Different evaluation methods were used to evaluate the accuracy and the reliability of the diagnoses. However, in most studies, no significant difference was found between automated and traditional audiometry. Conclusions: It seems that automated audiometry produces the same results compared with traditional audiometry. However, the main advantages of this method; namely, saving costs and increased accessibility to hearing services, can lead to a faster diagnosis of hearing impairment, especially in poor areas.

  • Postoperative maxillary cyst (POMC) is relatively common complication among patients who underwent Caldwell-Luc surgery. Patients with POMC usually have no symptoms, although cyst extension can result in bone destruction or cystic infection with pain. The trigeminal nerve consists of the ophthalmic nerve, maxillary nerve, and mandibular nerve. Among these branches, the maxillary nerve runs to the lateral and frontal sides of the maxillary sinus wall. POMC can rarely lead to trigeminal neuropathy caused by cyst enlargement that compresses some branches of the trigeminal nerve. Recently, we experienced a case with trigeminal neuralgia due to POMC. The patient was successfully treated with inferior meatal antrostomy. We report this rare case with a literature review.

Overexpression of indoleamine 2,3-dioxygenase correlates with regulatory T cell phenotype in acute myeloid leukemia patients with normal karyotype

  • Background Production of immunosuppressive enzymes such as indoleamine 2,3-dioxygenase (IDO) is one of the strategies employed by hematologic malignancies, including acute myeloid leukemia (AML), to circumvent immune surveillance. Moreover, IDO has the ability to convert $CD4^+CD25^-$ conventional T cells into regulatory T cells (Tregs). In this study, we evaluated the expression of IDO in cytogenetically normal acute myeloid leukemia (CN-AML) patients and its correlation with the Treg marker, FOXP3, as well as clinical and laboratory parameters. Methods Thirty-seven newly diagnosed CN-AML patients were enrolled in our study along with 22 healthy individuals. The expression of the IDO and FOXP3 genes was analyzed by SYBR Green real-time PCR. Results Both IDO and FOXP3 were highly upregulated in CN-AML patients compared to control groups (P=0.004 and P=0.031, respectively). A positive correlation was observed between IDO and FOXP3 expression among AML patients (r=0.512, P=0.001). Expression of IDO and FOXP3 showed no significant correlation with laboratory parameters such as white blood cell and platelet counts, hemoglobin levels, bone marrow blast percentage, gender, and FLT3 mutation status (P>0.05). Conclusion Higher IDO expression in CN-AML patients may be associated with an increased Treg phenotype which may promote disease progression and lead to poor prognosis of CN-AML patients.
