The Relationship between Exercise, Bone Mineral Density and Antioxidant Enzyme Activity of Postmenopausal Women

  • This study was carried out to elucidate the relationship among exercise, bone mineral density and antioxidant enzyme activity of postmenopausal women. 60 women residing in the Iksan, Korea area were recruited. The questionnaires were designed to find out exercise habits. Bone mineral density (BMD) was measured by dual energy X-ray absorptiometry. Parameters of antioxidative capacity, including the activities of superoxide dismutase (SOD), glutathione peroxidase (GPx), catalase (CAT) and total antioxidant capacity (TA) were analyzed in fasting blood. The mean age, height, weight, and BMI of subjects were 65.0 years, 151.1 cm, 59.5 kg $26.0\;m/kg^2$, respectively. The mean BMDs of subjects were $0.85\;g/cm^2$ (lumbar spine), $0.6\;g/cm^2$ (Femoral neck), $0.49\;g/cm^2$ (trochanter), and $0.40\;g/cm^2$ (Ward's triangle). There was a significant difference in BMD among different age groups (50's, 60's and 70's) showing lower value with increasing age (p<0.05). The mean SOD, GPx, and CAT activities were 138.5 U/mL, 1,273.8 U/mL and 314.3 kU/L respectively, and TA was 1.16 mmol/L. TA of the group which exercised 3$\sim$4 times a week was significantly higher than those of the other exercise groups (p<0.05). The subjects with higher SOD activity also have a higher the T values in the lumbar spine, femoral neck, trochanter, and Ward's triangle. In conclusion, this study revealed that the levels of antioxidant enzyme activity were closely associated with the exercise status and bone mineral density in postmenopausal women.

이중에너지 방사선흡수 골밀도 장치의 품질관리 현황 (A Status Report on Dual Energy X-ray Absorptiometry Quality Control in Korea)

  • DEXA는 골다공증 진단에 가장 많이 사용되는 표준 측정법이다. 현재 국내에서 운영되고 있는 DEXA 장치는 5,671대이다. 본 연구에서는 국내에서 설치 운영되고 있는 DEXA장비의 품질관리 실태를 조사하였다. 3,000개 기관을 대상으로 한 설문 조사에서 12.6%의 응답을 받아 그 결과를 분석하였다. 결과에서 77.5%가 일일 품질관리를 시행하고 있으나 Shewhart chart는 68.5%, CUSUM chart는 86.0%가 모르거나 사용하지 않는다고 답하여 정확한 품질관리가 되지 않는 것으로 조사되었다. 정확한 골밀도 검사는 환자의 골절의 위험도를 산정하기 위한 자료로 활용된다. 하지만 부적절한 검사는 오진을 유발할 가능성을 높여준다. 따라서 정확한 골밀도 검사를 위해서는 적절한 보조기구를 사용하고 장치의 품질관리를 반드시 시행하여 장치와 자세에 대한 오차를 줄이고 검사 의료진의 교육을 통해 오류 요인을 줄이는 것이 필수적이다.

Investigation of the Factors Affecting Bone Mineral Density in Children with Celiac Disease

  • Purpose: Children with celiac disease (CD) are at an increased risk of low bone mineral density (BMD) owing to malabsorption of fat-soluble vitamins, inflammation, and malnutrition. This study aimed to determine the prevalence and risk factors for low BMD in Iranian children with CD. Methods: This prospective cohort study examined 149 Iranian children with CD between 2011 and 2018 at Zabol University of Medical Sciences. BMD was measured using dual-energy X-ray absorptiometry. Demographic, clinical, and laboratory data were collected from patients' medical records. Logistic regression analysis was performed to identify the factors associated with low areal BMD (BMD-Z <-2) in the lumbar spine and femoral neck. Descriptive data were analyzed using the mean, standard deviation, and relative frequency. Data were analyzed using the chi-square test, t-test, and analysis of variance. Results: Of the 149 children with CD, 27.5% had osteoporosis. The mean body mass index (BMI) Z score was -1.28±1.2. Lower BMI was associated with a higher likelihood of BMD-Z (odds ratio 2.17; p≤0.05). Conclusion: Overall, the findings of this study showed that there was no correlation among Marsh classification, presence of specific human leukocyte antigens, and low BMD in Iranian children with CD. BMI can be a predictor of bone density in children with CD and may be applied clinically in early screenings to evaluate the bone health status in these children.

의치장착자의 의치사용만족 및 행태가 행복감에 미치는 영향 (Effect of Dentures Wearers' Denture Satisfaction and Behavioral Characteristics on their Happiness)

  • Objectives : The purpose of the study was to provide basic data for the improvement of their happiness through the use of effective denture-related services and restoration of healthy oral function. Methods : The subjects of the study were 452 denture wearers using the elderly welfare facilities in Seoul. Results : The main variables influencing the happiness of the denture wearers were living with the family, subjective health, oral health status, duration of denture use, and regular oral examinations. Living with one's family had the greatest influence on the happiness of the denture wearers. Conclusions : This study discusses the effect of satisfaction and behavior factors on effective denture use with reference to the happiness of denture wearers. This information will help improve denture management programs.

Association between vitamin D intake and bone mineral density in Koreans aged ≥ 50 years: analysis of the 2009 Korea National Health and Nutrition Examination Survey using a newly established vitamin D database

  • BACKGROUND/OBJECTIVES: Vitamin D plays an important role in skeletal growth and maintenance and in the prevention of various diseases. We investigated the relationship between vitamin D intake and bone mineral density (BMD) in Korean adults aged ${\geq}50$ years using the 2009 Korea National Health and Nutrition Examination Survey data. SUBJECTS/METHODS: This study was conducted in 1,808 subjects aged ${\geq}50$ years with BMD data in Korea. Dietary vitamin D levels were assessed by the 24-hour recall method. BMD was measured using dual-energy X-ray absorptiometry. We investigated general characteristics and the association between these characteristics, vitamin D status, and BMD. RESULTS: Vitamin D intake was significantly lower in the osteoporosis group among women (P < 0.05). Among all subjects, the higher the serum 25(OH)D concentration, the higher the whole-body total BMD (WBT-BMD), femoral total hip BMD, and femoral neck BMD (P < 0.01). In the serum vitamin D-deficient group of both the total population and women, serum 25(OH)D concentration was associated with WBT-BMD (P < 0.05). Among women with a calcium intake < 537.74 mg/day, BMD of those with a vitamin D intake > $2.51{\mu}g/day$ (average intake of women) was higher than that of women with a vitamin D intake ${\leq}2.51{\mu}g/day$ (P < 0.001). CONCLUSIONS: Korean adults should increase their BMD by increasing serum 25(OH)D concentration. Furthermore, increasing vitamin D intake could improve BMD, especially in Korean women who consume less calcium than the estimated average requirement.

생애전환기 건강진단 노인수검자의 성별에 따른 관련 요인분석 (The Analysis on Related Factors of the Aged Examines Who Get Diagnosed in Their Climacteric Period According to Sex)

  • Objectives & Method:Based on the statistic from January 2008 to December 2008 include 66-year-old 375 people (159 men, 216 women) who got examined in National Health Insurance Corporation, this study set a goal to establish a new health index by analyzing the relation between the cardio-cerebra vascular disease and risk factor such as obesity, smoking, exercise, alcohol consumption, blood pressure, blood sugar and cholesterol. Results:1) Examine results, health status according to gender;People who are diagnosed as abnormal health status have higher risk for both men and women. As for the detailed diseases, high blood pressure was highest followed by hyperlipidemia and obesity. In case of the women, 96.3% in bone mineral density was highest. 2) Correlation of disease and health risk factors based on gender;Correlation of disease according to gender in the obesity(p<0.001), lipid abnormalities(p<0.001), kidney diseases(p<0.001), chest diseases(p<0.05), osteoporosis(p<0.001) showed a statistically significant difference. The analysis showed that the men have more obesity and lipid abnormalities as well as thoracic disease than the women, but the women have more kidney disease than the men. As for the osteoporosis examine which is conducted only for the women, most of the women were abnormal even though it cannot compare the result according to gender. Obesity(p<0.001), alcohol(p<0.001), smoking(p<0.001) and LDLcholesterol(p<0.001) showed statistical correlation between gender and health risk factor. And only high blood pressure(p<0.001) showed a statistical correlation between gender and risk of cardio-cerebra vascular disease. 3) Risk of cardio-cerebra vascular disease related to health risk factor characteristic;Obesity assessment(p<0.001), alcohol(p<0.05), smoking(p<0.05), blood pressure(p<0.001), glucose(p<0.001) showed correlation between health risk factor and stroke. Drinking(p<0.001), smoking(p<0.05), exercise(p<0.001), blood pressure(p<0.001), LDL cholesterol(p<0.001) showed health risk factor and myocardial infarction. Obesity assessment(p<0.001), smoking(p<0.05), blood pressure(p<0.001), glucose(p<0.001) and showed correlation between health risk factor and diabetes. Obesity assessment(p<0.001), blood pressure(p<0.001) showed correlation between health risk factor and high blood pressure. Conclusion:Therefore, we have to take one step farther after the health examination. In order to prevent and reduce the risk of disease, people have to understand their health status exactly first, and correct their habit with recognizing the amount of risk related to cardio-cerebra vascular disease.

부산지역 치과환자의 임플란트 유형 및 분포 경향 (Types and Distribution of Implant Dental Clinic Patients in Busan Area)

  • 본 연구는 부산지역 S치과에 내원한 임플란트 시술을 받은 299명이 식립한 871개의 임플라트를 대상으로 임플란트의 종류, 골 이식 종류, 치주상태, 임플란트 분포를 조사하여 임플란트 구강위생관리 및 환자관리에 기초 자료를 마련하고자 하였다. 임플란트의 종류는 국산 임플란트 83.6%, 수입 임플란트 16.4%로 나타났다. 골 이식에서는 혼합 뼈로 골 이식을 한 환자가 17.1%로 가장 많았으며, 질병이 없는 환자는 있는 환자보다 자가 뼈로 골 이식을 많이 하였다($X^2$=14.66, p<.01). 치주상태는 남자보다는 여자가 좋았으며(t=-2.73, p<.01), 연령별에서는 연령이 적을수록(F=14.20, p<.001), 질병 유 무에서는 질병이 없는 환자(t=-4.67, p<.001)에서 치주상태가 좋았다. 치아별 임플란트 분포에서 하악에 많았고, 하악 좌측 임플란트가 2개 있는 환자가 16.1%로 가장 많았다($X^2$=33.14, p<.01). 총 치아의 임플란트 유 무에서 연령별로는 20대의 경우 1개, 60대 이상은 2개 로 나타나 연령이 증가할수록 개수가 많아짐을 보였다($X^2$=74.09, p<.001). 또한 치주상태에 따라서도 양호한 경우 1개를 식립한 경우가 많았고 불량할수록 2개 가 많았다($X^2$=38.28, p<.01). 이상의 결과에서 임플란트의 식립 상태 및 치주 상태를 정확히 파악하여 환자 상태에 맞는 1:1 맞춤 구강위생관리 프로그램 개발이 조속히 마련되어야 할 것이다.

경남 일부지역 폐경 전·후 여성들의 골밀도와 생화학지수들과의 관련성에 관한 비교연구 (Comparison of Relationship between Biochemical Indices and Bone Mineral Density of Pre- and Post- Menopausal Women in Gyeongnam Area)

  • This study was carried out to compare the relationship between biochemical indices and bone mineral density (BMD) in 50 pre-menopausal and 50 post-menopausal women. The subjects were divided into normal and risk groups according to their bone status, as determined by T-scores of the lumbar spine and femur. The average T-score of the lumbar spine was higher (p<0.05) in pre-menopausal women ($0.42{\pm}0.18$) than post-menopausal women ($-0.08{\pm}0.21$). Serum levels of HDL-cholesterol, P, and Fe were significantly higher in the risk group than the normal group in pre-menopausal women (p<0.05). Serum levels of total protein, globulin, alkaline phosphatase (ALP), and osteocalcin were lower in the risk group than the normal group, whereas the level of estrogen was higher in the normal group than the risk group in post-menopausal women (p<0.05). In pre-menopausal women, P was positively correlated with Ca (p<0.01), and ALP was positively correlated with osteocalcin (p<0.01) and parathyroid hormone (PTH) (p<0.05). Further, insulin-like growth factor-I (IGF-I) was negatively correlated with the vitamin $25(OH)D_3$ and vitamin K (p<0.05). In post- menopausal women, the Ca was positively correlated with vitamin $25(OH)D_3$ (p<0.05) and vitamin K (p<0.01), and P was positively correlated with vitamin K (p<0.01), Ca (p<0.01), and IGF-I (p<0.05) and negatively correlated with PTH (p<0.05). IGF-I was negatively correlated with PTH (p<0.01) and estrogen (p<0.05), and ALP was positively correlated with osteocalcin (p<0.01) and negatively correlated with vitamin K and estrogen (p<0.05). In pre-menopausal women, the lumbar spine BMD was positively correlated with vitamin K level (p<0.01) and negatively correlated with P level (p<0.05). In post-menopausal women, the femur BMD was positively correlated with estrogen level and negatively correlated with PTH leves (p<0.05). These results suggest that vitamin K and P levels are associated with bone health in pre-menopausal women, and estrogen and PTH levels are associated with bone health in post-menopausal women.

농촌지역 폐경후 여성의 골밀도 변화에 영향을 미치는 식이 요인 연구 (Dietary factors affecting bone mineral density in Korean rural postmenopausal women)

  • 본 연구에서는 골다공증의 취약집단인 농촌지역 폐경 후 여성의 골밀도 변화에 영향을 미치는 식이요인을 추적관찰 하였다. 연구시작년도에 골밀도를 측정하고 계절별 식이섭취 조사를 실시하였으며 2년 후에 골밀도를 다시 측정하였다. 1) 대상자를 골밀도 감소율에 따라 세 집단으로 분류하여 집단 간 영양소 섭취량 차이를 분석한 결과, 2년간 골밀도 변화율과 칼륨, 비타민C, 비타민A, 엽산 섭취량이 상관관계가 있었으며 식품군은 채소류, 난류의 섭취와 유의적인 상관성이 있었다. 2) 상관분석을 통하여 얻어진 변수들과 골밀도의 관계를 고려해 각 부위의 골밀도 변화율에 영향을 미치는 변수들로 모델을 검증한 결과에서는, 2년간의 골밀도 변화율은 연구시작시 골밀도, 비타민A, 채소류, 난류섭취량이 유의한 변수로 나타났다. 종합하면, 2년간 추적조사결과 골밀도 감소율이 큰 집단에 비해 감소율이 작은 집단에서 칼륨 섭취량이 많았으며, 연구 시작 시 골밀도, 비타민A, 채소류, 난류 섭취량이 요골의 골밀도 감소에 영향을 미치는 것으로 나타났다. 본 연구를 통하여 50세 이상 연령층일지라도 체중 (제지방량)유지, 비타민C 및 비타민A, 무기질 (칼슘, 인, 칼륨)이 풍부한 식품 섭취 등 일상생활에서 조절 가능한 식이요인이 골 건강유지의 중요한 구성요소임을 확인하였다. 이러한 결과는 폐경이후 농촌여성 골밀도 유지를 위한 영양교육 자료로 활용될 수 있으며, 앞으로 폐경이후 농촌여성의 골밀도 변화에 영향을 미치는 식생활 요인에 대한 장기간에 걸친 추적연구가 필요한 것으로 생각된다.
