FINITE ELEMENT ANALYSIS ON MAXILLARY MOLAR IMPLANT UNDER DIFFERENT C/R RATIO (상악 구치부 임플랜트 보철수복시 치관/치근비에 따른 응력 분포에 대한 유한 요소 분석)

  • Statement of the problem: In cases of low bone level in maxilla followed by extraction due to severe periodontitis or enlarged maxillary sinus, crown-root ratio of implant prosthesis will increase. The prognosis of these cases is not good as expected. Purpose : The purpose is to compare stress distribution due to crown-root ratio and effect of splinting between two implants in maxillary molar area under different loads Material and methods: Using ITI($4.1{\times}10$ mm) implant. two finite element models were made(model S: two parallel implants, model A: one of two is 20 degree inclined). Each model was designed in different crown-root ratio(0.7:1, 1:1, 1.25:1) and set cement type gold crown to make it splinted or non-splinted clinical situations. After that, 300 N force was loaded to each model in four ways.(load 1 : middle of occlusal table, load 2 : middle of buccal cusp, load 3 : middle of lingual cusp, load 4 : horizontal load to middle of buccal cusp), and stress distribution was analyzed. Results: On all occasions, stress was concentrated on neck of implant near cortical bone. In the case of inclined implant, stress was increased compared with parallel implants. Under load 1, 2, 3, stress was not increased even when crown-root ratio increases, but under load 4, when crown-root ratio increases, stress also increased. And more stress was concentrated under load 1 than load 2, 3. When crown-root ratio was same, stress under load 1, 2, 3 decreased when splinting, but under load 4, stress did not really decrease. Conclusion: Under vertical load, stress distribution related to crown-root ratio did not change. But under horizontal load, stress increased as crown-root ratio increases. Under vertical load, splinting decreased stress but under horizontal load, effect of splinting was decreased as condition of implant changes for the worse such as increase of crown-root ratio, inclined implant.

Electroacupuncture and "Duhuojisheng-tang" Administration for Postoperative Treatment in 3 dogs with Traumatic Spinal Cord Injury (외상성 척수 손상에 대한 척추 수술을 한 3마리의 개에서, 술 후 처치로써 전침과 독활기생탕을 적용한 증례)

  • Three dogs with severe traumatic spinal cord injury (TSCI) due to falling wound were admitted to the Veterinary Medical Center, Chonbuk National University for evaluation of severe pelvic limbs paralysis without deep pain, normal defecation and urination. Based on physical examination, neurological assessment and computed tomogram (CT), the diagnosis was made as subluxation and compressed fracture. All the cases were surgically treated with dorsal laminectomy and a spondylosyndesis using pin and bone cements. For 2 weeks, the dogs didn't show any improvement. Consequently, the dogs were treated with electroacupuncture (EA) and Duhuojisheng-tang (DHJST). All the dogs got back the deep pain and presented wagged tail on 14-35 days after starting EA with DHJST. Especially, two of 3 dogs recovered almost normal ambulation and capacities of urination and defecation. But, one dog failed to regain normal ambulation due to inflammation of operative site which is thought to be caused by the bone cement. From these cases, it was thought that the combination of EA and DHJST mightbe one of the suitable therapies in dogs with no neurological improvement.

Surgical Treatment of Squamous cell Carcinomas Arising in Scalp Burn Wounds - Two Case Reports - (화상 후 두피에 생긴 편평 상피세포 종양에 대한 증례 보고 - 증례보고 -)

  • Marjolin's ulcer is a rare and often-aggressive cutaneous malignancy that arises in previously traumatized or chronically inflamed skin, particularly after burns. We experienced two cases after burns. Case I involved a forty eight year-old man who had suffered from a flame burn at the parietal scalp area, where had been initially described three years earlier as a full-thickness wound including the pericranium. The man consulted us for a persistent ulcerative and infected wound on the burned lesion during the last 24 months, which turned out on the contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) to be the squamous cell carcinoma with involving the skull and the dura mater. Although the posterior auricular lymph node was enlarged on the ipsilateral side, recent positron emission tomography (PET) CT did not show any metastatic lesion. It was impossible for us to resect the intracranial involvement of the tumor radically, and the postoperative PET CT still showed a focal fluorodeoxyglucose (FDG) uptake around the wall of the superior sagittal sinus. We think that an aggressive combined approach is essential for treatment in early stages for a high success rate, before the intracranial structures are involved because there is no consensus on the treatment for advanced disease, and the results are generally poor. Case 1 also did not involve a radical resection because of the intracranial invasion to the wall of superior sagittal sinus and the possibility of damage to the major cortical veins. He received adjuvant radiotherapy and must be followed periodically. Case 2 involved an eighty six year-old women who suffered from a painful scalp ulcer lesion after flame burns three years earlier. Unlike case 1, neither tumor infiltration into the dura nor lymph node enlargement was observed on the contrast-enhanced computed tomography (CT), magnetic resonance imaging (MRI), or positron emission tomography (PET) CT. We did a radical resection of the tumor, including the involved bone, and a cranioplasty with bone cement.

Surgical Treatment of Metastatic Tumor in Pelvis (골반부 전이성 종양의 수술적 치료)

  • Purpose: We studied to decide the operative indication of the metastatic tumor in pelvis according to the oncologic results, the Eastern Cooperative Oncologic Group (ECOG) performance status and complication. Materials and methods: From May 1994 to May 2003, 9 patients who were performed on palliative treatment and 10 paitents on operative treatment due to metastatic tumor of pelvic bone were investigated. On palliative/operative group, the mean age of patients was 57.6/48.0 years old and the ratio of male to female was 5:4/7:3. Primary origins were 3 cases from kidney, 3 from cervix and 2 of lung, 2 of myeloma, 2 of Non-Hodgkin's Lymphoma, and 1 from breast, bladder, testis, prostate, stomach, liver and retroperitoneal leimyosarcoma respectively. The palliative treatment was performed in 5 cases with radiotherapy, 1 with chemotherapy, 2 with combined chemo-radiotherapy and 1 with percutaneous cementation. The operative methods were 1 case of bone cement insertion after curettage, 2 of Girdlestone with internal hemipelvectomy and 7 of reconstruction after wide excision. Reconstructions were done.: 1 case of bone cementation, 5 of autograft prosthesis composite with irradiation or pastuerization and 1 of saddle prosthesis. We have observed the oncologic results, the ECOG performance status and complication. Results: The oncologic results of palliative/operative groups are NED 0/1, AWD 2/6, DOC 1/2 and DOD 6/1. The ECOG performance status was changed from 1.5 into 4.3 in palliative group and from 2.6 into 2.2 in operative group. The complications were 3 cases of the prosthesis failure and 2 of infection. Conclusion: The indication of operation of metastatic pelvic tumor is decided in consideration of the patient's condition, the grade of malignancy in primary tumor and the life expectancy.

Cemento-Ossifying Fibroma in the Maxilla: A Case Report (상악에 발생한 백악질골화성섬유종에 대한 증례보고)

  • Cemento-ossifying fibroma (COF) is a benign fibro-osseous tumor with fibrous tissue, abnormal cement and bone, or a combination of such elements. These are slow-growing lesions and are more frequent in women. Here, we report the case of a 28-year-old Korean woman. The patient having no underlying disease complained about facial swelling and asymmetry. A firm mass with impacted molars and teeth deviation on the right maxilla was observed. A computed tomography scan was taken and an incisional biopsy was performed. Following this, COF was diagnosed. Complete surgical removal of the lesion was carried out. A post-operative follow-up was conducted and 3 months later the patient reported no discomfort or any sign of recurrence in regards to the lesion. Differential diagnosis with fibrous dysplasia and the COF is important because of the treatment choice. We report a case of COF and offer a review of the literature on this article.

Efficacy and Safety of Balloon Kyphoplasty in the Treatment of Osteoporotic Vertebral Body Compression Fractures : Compared with Vertebroplasty

  • Objective : Kyphoplasty and vertebroplasty are two minimally invasive procedures for osteoporotic vertebral compression fractures. The purpose of this retrospective study was to compare the radiological findings and clinical outcomes between two procedures. Methods : Osteoporotic vertebral fractures were treated in 76 vertebrae, using kyphoplasty (n=35 vertebrae) and using vertebroplasty (n=41 vertebrae). Fractured vertebral bodies were diagnosed by correlating the clinical symptoms with radiologic study. The responses of pain symptoms were measured by a self-reported Visual Analog Scale (VAS) score. Plain X-rays were checked preoperatively and postoperatively at admission and 6 months. The vertebral body height and kyphotic angle were measured to assess the reduction of the sagittal alignment. Results : The mean pain scores were decreased significantly for both procedures postoperatively, but there were no significant differences between two groups. Kyphoplasty led to a significant reduction of the vertebral body height and improvement of kyphotic angle. There were no neurological deficits after kyphoplasty, but one patient experienced paraparesis after vertebroplasty. During the 6 months follow-up both procedures provided stabilization of the sagittal alignment. Conclusion : Kyphoplasty and vertebroplasty are considered effective minimally invasive techniques for the stabilization of osteoporotic vertebral body fractures, leading to a statistically significant reduction in pain. Kyphoplasty significantly restore sagittal alignment. Also, complications and the incidence of bone cement leakage are significantly lesser than vertebroplasty. Therefore, kyphoplasty seems to be reasonable procedure for osteoporotic vertebral body compression fractures when medical treatment fail.

A Case of Metastatic breast Cancer and Reconstruction of Superior Vena Cava by Woven Dacron Y Graft (전이성 유암에서 Woven Dacrorl Y graft를 이용한 상대공정맥 재건술 -치험 III-)

  • This 32 year old female patient underwent left radical mastectomy due to ductal carcinoma on May 1990, and treated with FAM (5-fluorouracil, Adriamycin and Mitomycin C) regimen postoperatively. However, right cervical Iymph node enlargement and facial edema progressively developed since December 199). On April 1994, operation was performed, and findings were as followes; x4$\times$5$\times$7 to 1 : 1 $\times$ 1 cm sized multiple enlarged and hyperemic Iymph nodes were scatterred throughout submandibular area to the junction of superior vents cave and pericardium, and partially invaded both anterior segmental lobe, sternum and both distal tip of clavicles. After radical dissection of the nodes of neck and mediastinal nodes, and wedge resection of both anterior segments of lung, and partial resection of both clavicle tips and total sternum. The both innominate veins and superior vena cava were partially obstructed by invaded cancer SVC reconstruction was done with preclotted 10$\times$ 10$\times$ 18mm Y shap d woven Dacron graft, which was anastomosed to the point of the junction of subclavian vein and jugular vein after cross clamping both veins and 2cm above the pericardial junction with one arm clamp. After maintaining blood drainage to the SVC from the right side, left innominate vein was anastomosed with 4-0 Prolene continuous running suture. Bone cement was used for resected sternal portion and clavicular ends were fixed to postal portion with 18 Gauge wires. The patient was treated with radiation and chemotherapy after discharge, and there were no evidence of regrowing of the mass nor obstruction of the graft inspite of no antithrombotic therapy.

Reconstruction of Soft Tissue Defects in the Finger using Arterialized Venous Free Flaps (유리 동맥화 정맥 피판을 이용한 수지 연부조직 결손의 재건)

  • Purpose: To report the clinical results of the use of arterialized venous free flaps in reconstruction in soft tissue defects of the finger and to extend indications for the use of such flaps based on the clinical experiences of the authors. Materials and Methods: Eighteen patients who underwent arterialized venous free flaps for finger reconstruction, between May 2007 and July 2009 were reviewed retrospectively. The mean flap size was 4.7${\times}3.2$ cm. The donor site was the ipsilateral volar aspect of the distal forearm in all cases. There were 8 cases of venous skin flaps, 5 cases of neurocutaneous flaps, 4 cases of tendocutaneous flaps, 1 case of innervated tendocutaneous flap. The vascuality of recipient beds was good except in 4 cases (partial devascuality in 2, more than 50% avascuality (bone cement) in 2). Results: All flaps were survived. The mean number of included veins was 2.27 per flap. Mean static two-point discrimination was 10.5 mm in neurocutaneous flaps. In 3 of 5 cases where tendocutaneous flaps were used, active ROM at the PIP joint was 60 degrees, 30 degrees at the DIP joint and 40 degrees at the IP joint of thumb. There were no specific complications except partial necrosis in 3 cases. Conclusions: An arterialized venous free flap is a useful procedure for single-stage reconstruction in soft tissue or combined defect of the finger; we consider that this technique could be applied to fingers despite avascular recipient beds if the periphery of recipient bed vascularity is good.

A PHOTOELASTIC STUDY ON THE STRESS ANALYSIS UNDER MADIBULAR DISTAL-EXTENSION REMOVABLE PARTIAL DENTURE WITH DIFFERENT DESIGN OF THE MAJOR CONNECTOR (주 연결장치의 설계변화에 따른 하악 유리단 국소의치의 광탄성 응력 분석에 관한 연구)

  • The purpose of this study was to analyze the magnitude and distribution of stress using a photoelastic model from a distal extension removable partial dentures with three kinds of mandibular major connectors, that is, lingual bar, linguoplate, and swing-lock attachment. A photoelastic model was made of the epoxy resin(PC-1) and hardener(PCH-1) and coated with plastic cement-1 (PC-1) at the lingual surface of the epoxy model and set with three kinds of chrome-cobalt removable partial dentures. A bilateral vertical load of 15kg to the middle portion of the metal bar crossing both the first molars of the right and the left, and a unilateral vertical load of 12.5kg to the right first molar were applied with the use of specially designed loading device and the reflective circular polariscope was used to analyze the photoelastic model under each condition. The following results were obtained : 1. When the bilateral vertical load was applied, the magnitude and distribution of the stress concentration of the edentulous area and the terminal abutment or adjacent teeth was in the order of lingual bar, linguoplate, swing-lock attachment. 2. When the unilateral vertical load was applied, the magnitude and distribution of the stress concentration of the edentulous area and the terminal abutment or adjacent teeth was in the order of lingual bar, linguoplate, swing-lock attachment. 3. When the unilateral vertical load was applied, the magnitude and distribution of the stress concentration of the termial abutment or adjacent teeth on the non-loaded side showed the least stress distribution in case of swing-lock attachment. 4. When the bilateral vertical load and the unilateral vertical load were applied the swing-lock attachment showed the mildest uniform stress distribution on the edentulous area and the alveolar bone around the abutment teeth.

Polymethylmethacrylate-Augmented Screw Fixation for Stabilization of the Osteoporotic Spine : A Three-Year Follow-Up of 37 Patients

  • Objective : The purpose of this study was to determine the efficacy, radiological findings, clinical outcomes and complications in patients with lumbar stenosis and osteoporosis after the use of polymethylmethacrylate (PMMA) augmentation of a cannulated pedicle screw. Methods : Thirty-seven patients with degenerative spinal stenosis and osteoporosis (T-score < -2.5) underwent lumbar fusion using the Dream Technology Pedicle Screw ($DTPS^{TM}$, Dream Spine Total Solutions, Dream STS, Seoul. Korea) between 2005 and 2007. The clinical outcomes were evaluated by using the visual analog scale (VAS) and the Prolo scale. Radiologic findings were documented through computed tomography (CT) and plain films. Results : Thirty-seven patients were evaluated and included, 2 males and 35 females with an average bone mineral density (BMD) of $0.47g/cm^2$. The average age of the patients was 68.7 (range, 57-88). The preoperative VAS for low back and leg pain ($7.87{\pm}0.95$ and $8.82{\pm}0.83$) were higher as compared with postoperative VAS ($2.30{\pm}1.61$ and $1.42{\pm}0.73$) with statistical significance (p = 0.006, p = 0.003). According to the Prolo scale, 11, 22, one and three patients were in excellent, good, fair and poor conditions, respectively. The average amount of the injected cement per one cannulated screw was $1.83{\pm}0.11\;mL$. Conclusion : The results show favorable outcome both clinically and radiographically for 37 patients who underwent lumbar fusion using $DTPS^{TM}$ and PMMA. Based on the results, the use of this surgical method can be a safe and effective option for the operation on the osteoporotic spine.
