Relationship Between Mitochondrial DNA Copy Number, Metabolic Abnormalities and Hepatic Steatosis (지방간 및 대사 인자들과 말초혈액 백혈구의 사립체 DNA copy 수와의 연관성)

  • Insulin resistance plays a central role in fatty liver, a part of the metabolic syndrome. This study examined the relationship between fatty liver, metabolic abnormalities and mitochondrial DNA [mtDNA] copy number in peripheral blood that is correlated with diabetes or metabolic markers. Fatty liver was assessed by questionnaire on alcohol consumption and abdominal ultrasonography. MtDNA copy number in peripheral leukocytes was measured by a real-time quantitative polymerase chain reaction [PCR]. Among 445 subjects, 148 subjects had hepatic steatosis and 297 were controls. mtDNA copy number was significantly lower in fatty liver group in comparison with that of normal finding group. This result is similar in both groups, alcoholic or non-alcoholic fatty liver group. MtDNA copy number was inversely correlated with alanine aminotransferase [ALT], aspartate aminotransferase [AST], gamma-glutamyltransferase [$\gamma$-GTP], body mass index [BMI], waist circumference, diastolic blood pressure, and free fatty acid. MtDNA copy number in peripheral leukocytes was associated with fatty liver and insulin resistance related factors.

Vitamin D deficiency is an independent risk factor for cardiovascular disease in Koreans aged ${\geq}50$ years: results from the Korean National Health and Nutrition Examination Survey

  • Vitamin D deficiency is a risk factor for metabolic syndromes. We examined whether vitamin D deficiency altered the prevalence of cardiovascular disease (CVD) in older Koreans. Cross-sectional analysis of data from the Korean National Health and Nutrition Examination Survey IV 2008-2009 was used to examine the association between serum 25-hydroxyvitamin D (25(OH)D) levels and the prevalence of CVD in a representative population-based sample of 5,559 men and women aged ${\geq}50$ years. CVD was defined as angina pectoris, myocardial infarction, or stroke. The prevalence of CVD (7.0%) in the older Korean population was lower than that in the older US population, although average serum 25(OH)D levels were much lower in the Korean population. Additionally, serum 25(OH)D levels did not differ significantly between the CVD and non-CVD groups. However, subjects in the lowest category (< 25 nmol/l) of serum 25(OH)D level had the greatest prevalence of CVD, about two-fold higher than subjects in the highest category (> 75 nmol/l), after adjusting for age, gender, body mass index, education level, residence location, and region. The prevalence of other risk factors for CVD, including higher waist circumference, fasting glucose, low-density lipoprotein (LDL) cholesterol, and triglyceride levels and lower high-density lipoprotein (HDL) cholesterol levels, was also higher among subjects in the lowest category than among those in the highest category. In conclusion, low serum 25(OH)D may be an independent risk factor for CVD in older Koreans.

Adiponectin gene SNP 276G${\rightarrow}$T, nutrient intakes, and cardiovascular disease risk in Korean type 2 DM patients

  • Single nucleotide polymorphism (SNP) in adiponectin gene has been associated with insulin resistance, diabetes, and cardiovascular disease (CVD). This study was performed to investigate the association of SNP 276G${\rightarrow}$T at adiponectin gene with CVD risk factors in Korean type 2 diabetes mellitus (DM) patients. The subjects were 351 type 2 DM patients visited a DM clinic in Seoul, and the patients with known CVD were excluded. The adiponectin SNP 276G${\rightarrow}$T was analyzed and dietary intakes were assessed by a Food Frequency Questionnaire. The prevalence of G/G, G/T, and T/T genotype was 47.6%, 43.3%, and 9.1%, respectively. Male subjects with T/T genotype showed significantly lower level of adiponectin and HDL-cholesterol and significantly higher C-reactive protein (CRP) level compared to G/G and G/T genotypes. In G/G genotype, protein intake was negatively correlated to body weight, BMI, and waist circumference, and there were positive correlation between carbohydrate intake and BMI, waist-hip ratio, and ApoB/apoA-1 ratio in G/T genotype. However, in T/T genotype, there was no significant association between macronutrient intakes and anthropometric and hematological values. In conclusion, CVD risk would be high in type 2 DM patients with T/T genotype, and the association of macronutrient intakes with anthropometric and hematologic factors was different among the three adiponectin genotypes. These results may imply the need for different dietary management regime according to adiponectin genotype to lower CVD complications in Korean type 2 DM patients.

Relationship between dietary intakes and the double burden of malnutrition in adults of Malang, Indonesia: An exploratory study

  • BACKGROUND/OBJECTIVES: Urbanization and lifestyle changes have resulted in nutrition transition. Over-nutrition causes obesity increase, although malnutrition still exists. This phenomenon is called a double burden of malnutrition (DBM). This study was conducted to confirm the existence of DBM and to investigate the dietary factors related to DBM in Indonesian adults. SUBJECTS/METHODS: Data for the subjects (51 men and 89 women) who are the adults resided in Malang, Indonesia were collected between July 17 and August 14, 2017, by using questionnaire. Height, weight, waist and hip circumference, blood pressure, blood glucose, total cholesterol, and hemoglobin were also measured for the subjects. RESULTS: The average age of the subjects was 47.2 years. The average height, weight, and body mass index for men were 160.08 cm, 62.6 kg and $25.45kg/m^2$, respectively, and those for women were 148.74 cm, 58.09 kg, and $26.21kg/m^2$, respectively. Of the subjects, 3.6% were underweight, 24.3% were normal or healthy weight, while 72.2% were overweight and obese. Analysis of the dietary intakes revealed high for cereal (7.73 points), but very low for milk (0.25 points) and fruits (0.51 points). Dietary diversity was very few overall (< 5). The occurrence of both anemia (23.6%) and chronic diseases such as hypertension (57.1%), diabetes (12.1%), and hypercholesterolemia (3.6%) was considerably high. The existence of DBM was confirmed by 16.4% of the subjects. DBM was observed significantly higher in women than in men. Dietary diversity and DBM occurrence were inversely correlated. On average, the number of chronic diseases was 1.08 in men and 1.79 in women. Dietary diversity inversely affected systolic blood pressure. CONCLUSIONS: DBM existed in adults in Malang, Indonesia due to inadequate dietary intakes, and a high rate of chronic diseases.

Assessments of nutrient intake and metabolic profiles in Korean adolescents according to exercise regularity using data from the 2008 Korean National Health and Nutrition Examination Survey

  • The purpose of this study was to assess the metabolic profile and nutrient intake data in Korean adolescents according to exercise regularity using the forth Korean National Health and Nutrition Examination Survey from 2008 data. A total of 834 Korean adolescents (440 boys, 394 girls) aged 12 to 18 years were classified into exercising male (EM), non-exercising male (NM), exercising female (EF), and non-exercising female (NF) groups. EM exhibited significantly higher weight (P<0.001), waist circumference (WC) (P<0.01) and body mass index (EMI) (P<0.001) than NM. EF showed significantly greater height (P<0.01), weight (P<0.001), we (P<0.001), and BMI (P<0.001) than NF. Although ratios of macronutrient intake were within the appropriate range in all groups, energy, riboflavin, vitamin e and calcium were assessed as in adequate by the nutritional adequacy ratio (NAR) in all groups. Moreover, EF and NF had inadequate intake of vitamin A and iron according to NAR, respectively. There were significant correlations between height and NAR protein (r=0.249, P<0.001), and mean adequacy ratio (MAR) (r=0.177, P<0.01), and between weight and NAR protein (r=0.180, P<0.01), and MAR(r=0.136, P<0.05) in EM. On the other hand, there were significant correlations between weight and NAR protein (r=0.270, P<0.01), we and NAR protein (r=0.271, P<0.01), and BMI and NAR protein (r=0.326, P<0.01) in NM. There were significant correlations height and NAR Fe (r=0.153, P<0.05) in EF. However, there were no significant correlations between metabolic factors and nutritional adequacy in NF. Although we noted no significant differences in MAR between the groups, the exercising groups showed higher MAR values than the non-exercising groups. Therefore, practicing of ideal dietary behaviors appears to be induced through physical activity and regular exercise in Korean adolescents.

Dietary iodine intake and the association with subclinical thyroid dysfunction in male workers (직장인 남성에서 식이 요오드섭취량과 불현성 갑상선기능이상과의 연관성)

  • The prevalence rate of thyroid dysfunction (hypothyroidism and hyperthyroidism) has increased within the Korean population and seems to be affected by iodine dietary habits. Some studies reported that the prevalence of thyroid dysfunction increase both in the area of iodine deficiency and excess. In this study, we tried to discover the difference in iodine intake, anthropometric measurements, and blood parameters between male subjects with or without subclinical thyroid dysfunction. A total of 5,249 subjects (Euthyroid: 4706, SubHypo: 454, SubHyper: 89) were used in this study. There were no significant differences in BMI, body fat, visceral fat, waist circumference, SBP, DBP, total cholesterol, HDL-cholesterol, LDL-cholesterol, TG, fasting serum glucose, HbA1c, alcohol intake, however significant differences were noticed in both age and smoking status. Through a food frequency questionnaire (FFQ), iodine intake per day was estimated. The average iodine intake was similar (SubHypo $392.9{\pm}279.0{\mu}g$, Euthyroid $376.5{\pm}281.7{\mu}g$, SubHyper $357.3{\pm}253.8 {\mu}g$) among groups. The main source of iodine intake was eggs (52.8%, 54.2%, 52.4%) followed by milk (16.3%, 15.8%, 17.8%), then sea mustard & sea tangle (12.4%, 11.9%, 11.6%). The prevalence of subclinical hypothyroidism and subclinical hyperthyroidism was higher in subjects whose intake was higher than the recommended nutrient intake (RNI). These results suggest that the excess consumption of iodine intake may act as one of the risk factors regarding thyroid dysfunction in Korea. Therefore, an adequate amount of iodine intake is necessary in order to prevent subclinical thyroid dysfunction and clinical thyroid dysfunction.

Gender-specific cephalometric features related to obesity in sleep apnea patients: trilogy of soft palate-mandible-hyoid bone

  • Background: The aim of this study is to investigate the relationship between gender-specific and obesity-related airway anatomy in patients with obstructive sleep apnea (OSA) by using cephalometric analyses. Methods: We retrospectively evaluated 206 patients with suspected OSA undergoing polysomnography and anthropometric measurements such as body mass index, neck circumference, and waist-hip ratio. We checked lateral cephalometry to measure tissue landmarks including angle from A point to nasion to B point (ANB), soft palate length (SPL), soft palate thickness (SPT), retropalatal space (RPS), retrolingual space (RLS), and mandibular plane to hyoid (MPH). Results: Male with OSA showed significantly increased SPL (P = .006) compared with controls. SPL and MPH had significant correlation with apnea-hypopnea index (AHI) and central obesity. Female with OSA showed significantly increased ANB (P = .013) and SPT (P = .004) compared with controls. The receiver operating characteristic curves revealed that SPT in male and ANB and SPT in female were significant in model 1 (AHI ≥ 5) and model 2 (AHI ≥ 15). MPH was also significant for male in model 2. Conclusion: Male and female with OSA had distinct anatomic features of the upper airway and different interactions among soft palate, mandible, and hyoid bone.

Evaluation of Clinical Usefulness of Instant Nutritional Assessment Scale for the Nutritional Status and Prognosis of Hemodialysis Patients (만성질환자의 영양상태와 예후판정에 대한 간이영양평가 도구의 유용성 평가 -혈액투석환자를 중심으로-)

  • The prognosis of maintenance hemodialysis (HD) patients is closely related to their nutritional status. It is important to develop and use of a reliable, useful and easy method of nutritional assessment scale for evaluation of nutritional status and progression of the patients. This study was initiated to evaluate the clinical usefulness of Instant Nutritional Assessment Scale(INAS) by cross-sectional and longitudinal studies. One hundred HD patients entered a continuing nutritional study and followed for 1 year. The results were as follow ; 1. 24% of patients was normal to mild, 43% was moderate and 33% was severe deficit of nutritional status. 2. The mean INAS score of the patients was 8.00(S. D.=2.83), and there didn't reveal any differences in INAS score by general characteristics. The mean transferrin score was 1.98, whih was the highest of 5 nutirtional parameters of INAS. Only 7 patients had within nomal range of transferrin concentration. 3. Within one year since this study was initiated, 10 patients died. Six of them were with severe deficit and one of them was normal to mild deficit groups. The death rate in severe deficit group was higher than that of normal to mild deficit group (P=.0640). 4. Occurrence of acute complication during HD in severe deficit guoup was higher than that of normal to mild deficit group(P=.001). 5. The number of consultation to the doctor and hospital admission in severe deficit group was higher than that of normal to mild deficit group(P=.0001). 6. INAS score was significantly correlated with occurrence of acute complications during HD and the number of consultation to the doctor. In conclusion, INAS based on the levels of body mass index, midarm circumference, triceps skinfold thickness, transferrin concentration and total lymphocyte count seems to be a reliable predictive nutritional index for prognosis. So nurses are encouraged to adopt INAS in care of the chronically illed patients. Recommendations for further research was suggested.

Development of Basic Pattern of Wedding Dress I - Focused on Torso Pattern for Top Dresses -

  • This thesis intends to suggest a new wedding dress pattern through researches on history and changes of wedding dress, researches on silhouette and details and analyses of existing basic patterns. In this study, to develop torso patterns of top dresses selected as preferred designs through literature reviews and company surveys, dress form sizes suitable for standard sizes were selected and study basic patterns were made using draping techniques. The study finds characteristics of body type dimension that is changed when wearing brassiere (hereinafter, 'bra') for wedding dress through customer surveys for development of basic patterns of wedding dress. That is, wearing wedding bra was used for the purpose of increasing the bust size and we will evaluate later whether this increase helps completeness of aesthetic impression through wearing evaluation. As the result, it was found out that top torso pattern wearing wedding bra made wearers feel more comfortable and in the aspect of aesthetic impression, it provided impacts on women's beautiful silhouettes. The effects of whether a bra is used or not on changes in patterns were reviewed through the degree of polymerization of finished patterns. When a bra for dresses was worn, the waist front length increased by1.7cm, the bust circumference increased by 2.1cm, and the amount of dart increased by 1.5cm in the patterns. A new torso pattern for top dresses, which will solve problems evaluated in each of the above from the aspect of composition of pattern by a method through analysis of existing basic patterns and multi-dimensional cut patterns wearing wedding bra, will be suggested.

Research on Growing Progress of Foot Size to Improve Appellation of Shoe Sizes (신발 사이즈 호칭 개선을 위한 발치수 성장 추이 탐색)

  • This study aims to categorize age groups for shoes manufacturing including shoes size and boot tree development for Koreans. In order to carry out the research, the characteristics of different parts of foot in relation to size are analyzed according to age and sex, making use of human body measurement database. As a result of statistical analysis, it was found that the measurement items such as foot length, metatarsal tibiale, foot breadth and instep circumference of a person reached the range of standard deviation and average numerical figures of an adult group (18-24) from the age of 14 in men's case, and from 12 for women's case, which indicates the time of completion of their foot growth. Based on these findings where males of 14 years old and females of 12 are within similar measurement range to adults, it is necessary to categorize the age groups for shoe sizing system into the following four groups: males 13 years old or under, males 14 years old or over, females 11 years or under, and females 12 years or over. The proportional measurement produced by dividing each measurement item of foot parts by foot length was compared in an independent samples t-test, and there were meaningful differences according to different foot shapes of the two age groups of males 13 or under - males 14 or over, and females 11 or under - females 12 or over. Also, the independent samples t-test for the age groups of males 13 or under - females 11 or under, and males 14 or over - female 12 or over, showed similar meaningful differences, which indicates the differences in foot shapes of male and female during the period of growth.
