Although blunt traumatic cardiac rupture is an uncommon injury, it can be associated with a high mortality rate. Two cases of cardiac rupture in blunt trauma patients are described herein. In those cases, applications of mechanical support devices such as ECMO (extracorporeal membrane oxygenation) and early surgery for exploration under cardiopulmonary bypass may be helpful for treating blunt chest trauma patients.

The rupture of tracheobronchial tree caused by non penetrating blunt trauma is being increased in incidence though it is still rarely occurred on now Because this type of injury is uncommon, a high index of suspicion should be maintained in all crush injuries involving the chest. Early diagnosis and primary repair not only restore normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair. We experienced a case of tracheobronchial rupture caused by non penetrating blunt chest trauma without rib fracture. The patient was a 16 year old male who was a high school student. He was compressed on anterior chest by hand ball goal post being failed down on the morning of admission day. After this accident, he was suffered from progressively developing dyspnea and subcutaneous emphysema on face, neck and anterior chest. The diagnosis, tracheal rupture, was made by chest CT and bronchoscopy. After right thoracotomy, the ruptured site was directly closed by using interrupted suture. Post-operative course was uneventful. Thus we report this case of traumatic tracheal rupture with review of literature.

Traumatic chylothorax after blunt chest trauma alone is considered rare. Our patient was a 27-year-old female who was in a motorcycle accident and sustained blunt thoracic and traumatic thoracic aortic injuries with T1-T2 vertebral subluxation. She underwent thoracic endovascular aortic repair from T4 to T9 without any thoracic or spinal surgery. On postoperative day 7, the drainage from her left chest turned into a milky-white fluid indicative of chyle leakage. The patient was treated conservatively for 2 weeks and then the chest drain was safely removed. The results show that traumatic chylothorax can be successfully managed with conservative treatment.

Rupture of a bronchus is an unusual result of blunt trauma to the chest. We experienced a case of bronchial rupture caused by blunt chest trauma without external wound or rib fracture. This case was a 13 years old male who was elementary school student. He was compressed on anterior chest by basket ball goal post being failed down on the afternoon of admission day. After this accident, he was suffered from progressive dyspnea and chest pain. The bronchial rupture was confirmed by bronchogram, The operation was performed through standard posterolateral thoracotomy incision along the 5th rib course. The right upper lobe bronchus and intermediate bronchus was completely transected. The right upper lobectomy was done. The ruptured intermediate bronchus was connected direct simply. Post-operative course was uneventful. Thus we report this case of traumatic bronchial rupture with review of literature.

Diaphragmatic hernias have been reported in 0.8%-1.6% of patients who experience blunt chest trauma. The hernia is assumed to form as a result of direct diaphragmatic violation or significant intraabdominal or intrathoracic pressure caused by the trauma. Some reports have described cases of delayed diaphragmatic hernia and subsequent stomach perforation that occurred a few days to several years after an accident. We report an extremely rare case of diaphragmatic herniation in which the process from initial blunt trauma to visceral organ perforation took only 2 days, without any evidence of herniation on the initial X-ray or computed tomography. Delayed diaphragmatic herniation and subsequent visceral organ perforation should not be missed during the period immediately after blunt chest trauma.

비관통성 흉부손상에 의한 기관-기관지 파열의 높은 사망률과 이환율은 조기 진단과 수술로 이를 낮출 수 있으므로 초기 임상증상과 단순흉부방사선상 기관-기관지 파열이 의심되는 소견이 있으면 바로 기관지경검사로 기관-기관지 파열을 진단하고 적절한 치료를 해야한다. 저자들은 교통사고후 비관통성 흉부손상을 받고 호흡곤란을 주소로 내원한 환자에서 기관지경검사로 기관 파열을 조기 진단하여 수술적 치료로 회복시킨 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Ascending aortic injury after blunt chest trauma is an emergency condition that requires urgent diagnosis and treatment. The authors report the case of a patient with traumatic ascending aortic injury who received ascending aorta replacement under cardiopulmonary bypass after failure of primary repair.

We have experienced a patient, 16 year-old male, with ventricular septal defect with tricuspid recur- gitation due to blunt chest trauma. He suffered from congestive heart failure after the trauma. Echocardiogram and cardiac catheterization revealed left to right shunt at the ventricular level (muscu- far portion of interventricular septum) and tricuspid regurgitation. At the time of the operation, marked systolic thrill was palpable over the rlght ventricle near the apex and a chorda tendina was seen sharply ruptured just near the medial papillary muscle. We repaired the ventricular septal defect with a Dacron patch and chordal reconstruction with autologous pericardium. The postoperative course was uneventful and the patient was discharged in good condition.

흉부 둔상으로 인한 무명동맥 손상은 드문 것으로 보고되고 있다. 본원에서는 교통사고로 인한 흉부 둔상으로 발생한 40세 남자 환자의 무명동맥 박리 및 가성 동맥류를 흉부 전산화 단층촬영으로 진단하여 상행대동맥과 무명동맥 간에 인조혈관 우회로 조성술로 성공적으로 치료하였다. 환자는 술 후 별다른 합병증 없이 1주일만에 퇴원할 수 있었으며 현재 외래 추적 중이다.

27세 남자 환자가 오토바이 사고로 인한 급성전벽 심근경색증으로 내원하였다. 흉부에 손상의 흔적은 없었으며 심초음파검사상 전벽의 이상운동증과 미미한 심낭삼출액이 보였다. 관상동맥조영술상 좌전하행지 근위부에 박리가 있었고, 좌심실조영술상 심첨부에 좌심실류 및 혈전이 관찰되어 관상동맥우회술을 시행하였다.
