A rare and unique experience of a blunt intrathoracic traumatic injury of the trachea and its management in South Africa: a case report

  • Blunt intrathoracic tracheal injuries are rare, even among blunt chest trauma patients. An early diagnosis based on a high index of suspicion allows for timely surgical management of potentially fatal airway trauma, thereby improving overall outcomes. Diagnosing these injuries can be difficult due to their nonspecific clinical features and the occasional difficulty in radiologic diagnosis. If a patient exhibits respiratory compromise with difficult ventilation and poor lung expansion, despite the insertion and management of an intercostal drain following high-energy blunt trauma, there should be a heightened suspicion of potential airway trauma. The aim of primary repair is to restore airway integrity and to minimize the loss of pulmonary parenchyma function. This case report discusses the rare clinical presentation of a patient with blunt trauma to the intrathoracic airway, the surgical management thereof, and his overall outcome. Although blunt traumatic injuries of the trachea are extremely rare and often fatal, early surgical intervention can potentially reduce the risk of mortality.

  • Duodenal injuries following a blunt or penetrating trauma are uncommon and account for just 3% to 5% of all abdominal injuries. About 22% of all duodenal injuries are caused by blunt trauma. An overlooked injury or delayed diagnosis of duodenal injury may lead to increased mortality and morbidity. We report two cases of a duodenal injury following blunt abdominal trauma.

  • Although blunt traumatic cardiac rupture is an uncommon injury, it can be associated with a high mortality rate. Two cases of cardiac rupture in blunt trauma patients are described herein. In those cases, applications of mechanical support devices such as ECMO (extracorporeal membrane oxygenation) and early surgery for exploration under cardiopulmonary bypass may be helpful for treating blunt chest trauma patients.

  • Ocular injury is one of the commonest causes of eye diseases, but its patterns and incidences are variable according to the environment. Blunt Ocular Trauma is one of causes of ocular injury. Visual disturbance by Blunt Ocular Trauma is rarely reported and we do not know the exact convalescence and healing method of that. We treated a patient suffered from visual disturbance by blunt ocular trauma with herbal medicine, acupuncture, cupping treatment. We diagnosed as Blood stasis and noxious water disease. We report a case of visual disturbance by blunt ocular trauma after treatments, we observed the improvement. Based on this study, it is considered that oriental medical treatment can be applied to the management of visual disturbance.

  • Esophageal perforation due to blunt trauma is rare. A 67-year-old male presented to Konyang University Hospital with painful neck swelling. His neck was injured by blunt trauma at work. Esophageal perforation was detected by neck CT and esophagography. We performed primary repair of cervical esophagus through the Lt. neck approach. The postoperative course was uneventful and the patient was discharged at postoperative day 15.

  • A tracheoesophageal fistula following blunt chest trauma is rare, with only slightly more than 40 cases having been reported since 1936. With the increased incidence of blunt chest trauma from traffic accidents, it may be anticipated that this complication will be seen more frequently in the future. This report describes successfully managed two cases with such lesion. The pathophysiology and management of such lesion are discussed with a review of the literatures.

  • Bladder rupture following blunt trauma is rare, and no neobladder rupture following blunt trauma has yet been reported. We present a case of neobladder rupture following blunt trauma. The patient was a 65-year-old male patient who had been treated for bladder cancer via a radical cystectomy with an orthotopic ileal neobladder four years prior to this admission, and who was admitted to our emergency department due to multiple trauma after a 1.5 m fall. Primary repair was performed for the neobladder rupture.

  • The aim of this work is to analyze high temperature flows around an axisymmetric blunt body taking into account chemical and vibrational non-equilibrium state for air mixture species. For this purpose, a finite volume methodology is employed to determine the supersonic flow parameters around the axisymmetric blunt body. This allows the capture of a shock wave before a blunt body placed in supersonic free stream. The numerical technique uses the flux vector splitting method of Van Leer. Here, adequate time stepping parameters, along with Courant, Friedrich, Lewis coefficient and mesh size level are selected to ensure numerical convergence, sought with an order of $10^{-8}$.

  • Major bleeding caused by vascular injuries of the abdominal aorta or its branches after blunt trauma often leads to mortality or major morbidity. We report a case that lumbar artery injury following blunt trauma was spontaneously resolved without any surgical or interventional treatment. Lumbar artery injury after blunt trauma could be treated conservatively without surgical or interventional treatment in a selected case. When an aortic or its branch injury was suspicious, diagnostic angiograms in the setting of interventional treatment may be helpful to decide an appropriate treatment modality.

  • Sternal fracture is relatively common after blunt chest trauma, and this usually resolves without complication. But acute extrapericardial tamponade caused by sternal fracture and injury to the internal mammary artery secondary to blunt chest trauma is very rare. We report here on two cases of acute extrapericardial tamponade that were caused by blunt chest trauma.
