  • BACKGROUND/OBJECTIVES: This study was conducted to compare the overall changes in dietary intake and metabolic syndrome risk parameters in Vietnamese marriage-based female immigrants over time. SUBJECTS/METHODS: The subjects of this study were 581 Vietnamese marriage-based female immigrants, who were recruited from local clinical centers in Korea. Baseline data were collected from 2006-2011 and the follow-up data were collected from 2012-2014. Dietary food intake was assessed by a 1-day 24-hour recall method. RESULTS: Compared to the baseline, the mean body weight, body mass index, waist circumference, high density lipoprotein (HDL)-cholesterol, systolic blood pressure and diastolic blood pressure increased while the fasting blood sugar, triglycerides and low density lipoprotein-cholesterol decreased at follow-up. The total consumption of foods such as vegetables/fruits/seaweeds/mushrooms, oil and fat, and eggs decreased during the follow-up period, whereas the consumption of sugars, beverages and meats increased. Partial correlation between the rate of change [(Follow-up - Baseline) / Baseline ${\times}100$] in metabolic syndrome risk parameters and food intake after controlling for confounding factors showed that the waist circumference was positively correlated with either the total plant food intake (r = 0.1042, P = 0.0129) or the total food intake (r = 0.0880, P = 0.0359). The plasma levels of total cholesterol (r = -0.1918, P = 0.0289) and HDL-cholesterol (r = -0.1424, P = 0.0007) were negatively correlated with the percentage of total intake from plant food, and HDL-cholesterol levels were positively correlated with the intake of total animal food (r = 0.0980, P = 0.0217). The serum C-reactive protein levels were positively correlated with the total intake of animal food (r = 0.2374, P < 0.0001) or the percentage of total intake from animal food (r = 0.1346, P = 0.0016). CONCLUSIONS: These results suggest that the change rate of dietary intake such as total plant food or animal food is associated with the change rates of metabolic syndrome risk parameters.

  • The purpose of this study was to investigate the relationship between dietary fat intake, anthropometric data, blood lipids, C-reactive protein, and adiponectin in Korean male college students. Forty-eight subjects were divided into 2 groups based on dietary fat intake: UERF (under 30% of energy ratio for fat source), AERF (above 30% of energy ratio for fat souce). We collected dietary intake data using 24-hour dietary recall for 3 days. Anthropometric and biochemical parameters were measured by using standard methods. Segmental body composition analysis was carried out using an 8-electrode multifrequency bioelectrical impedance method of body fat estimation. There was no significant difference in anthropometric data and serum lipid profile between UERF and AERF group. Serum C-reactive protein level was significantly higher in the AERF group compared to the UERF group. Although there was no significant difference in serum adiponectin level between UERF and AERF groups, subjects had lower adiponectin levels. Correlation data show that serum adiponectin level was positively correlated with vegetable intake (p < 0.05). In addition, dietary fat intake had a positive correlation with meat (p < 0.01), whereas a negative correlation with grain (p < 0.01), vegetables (p < 0.05), and fish (p < 0.05). These results suggest that the increased fat intake of non-obese Korean male college students is associated with their increased serum C-reactive protein concentration. Therefore, proper guidelines on fat intake and nutrition education are necessary for the prevention and management of metabolic syndromes.

  • Purpose: This study aimed to evaluate the utility of acanthosis nigricans (AN) severity as an index for predicting insulin resistance in obese children. Methods: The subjects comprised 74 obese pediatric patients who attended the Department of Pediatrics at Chosun University Hospital between January 2013 and March 2016. Waist circumference; body mass index; blood pressure; fasting glucose and fasting insulin levels; lipid profile; aspartate transaminase, alanine transaminase, glycated hemoglobin, C-peptide, and uric acid levels; and homeostatic model assessment insulin resistance (HOMA-IR) and quantitative insulin check sensitivity index (QUICKI) scores were compared between subjects with AN and those without AN. Receiver operating characteristic curves were used to investigate the utility of the AN score in predicting insulin resistance. HOMA-IR and QUICKI were compared according to AN severity. Results: The With AN group had higher fasting insulin levels ($24.1{\pm}21.0\;mU/L$ vs. $9.8{\pm}3.6\;mU/L$, p<0.001) and HOMA-IR score ($5.74{\pm}4.71$ vs. $2.14{\pm}0.86$, p<0.001) than the Without AN group. The AN score used to predict insulin resistance was 3 points or more (sensitivity 56.8%, specificity 83.9%). HOMA-IR scores increased with AN severity, from the Without AN group (mean, 2.15; 95% confidence interval [CI], 1.72-2.57) to the Mild AN (mean, 4.15; 95% CI, 3.04-5.25) and Severe AN groups (mean, 7.22; 95% CI, 5.08-9.35; p<0.001). Conclusion: Insulin resistance worsens with increasing AN severity, and patients with Severe AN (AN score ${\geq}3$) are at increased risk of insulin resistance.

  • Purpose: The prevalence of obesity has significantly increased among children and adolescents worldwide and is becoming an important health care problem in parallel with the increased prevalence of obesity pediatric non-alcoholic fatty liver disease. Betatrophin is a newly define hormone that is commonly secreted by liver and plays role in glucose tolerance. This study aimed to investigate the relationship between serum betatrophin levels and non-alcoholic fatty liver disease in obese children. Methods: The study included 40 obese children with a body mass index (BMI) above 95th centile, and 35 non-obese subjects with a BMI 3-85th centile, whose age and gender were similar to those of the patient group. For the evaluation of metabolic parameters fasting serum glucose, insulin, alanine aminotransferase, aspartate aminotransferase, lipid profile and serum betatrophin levels were measured. Total cholesterol: high-density lipoprotein cholesterol and low-density lipoprotein cholesterol: high-density lipoprotein cholesterol ratios were calculated as "atherogenic indices." Results: Serum betatrophin levels of the obese subjects were similar to that of non-obese subjects (p=0.90). Betatrophin levels were not correlated with the metabolic parameters. Conclusion: In the present study, levels of betatrophin are not different between obese and insulin resistant children and non-obese subjects, and they are not correlated with atherogenic indices. To elucidate the exact role of betatrophin in obesity, further studies are required to identify the betatrophin receptor and/or other possible cofactors.

  • Objectives: This study aims to investigate trends of cardiovascular disease (CVD) risk factor profiles over 17 years in percutaneous coronary intervention (PCI) patients at the Mayo Clinic. Methods: We performed a time-trend analysis within the Mayo Clinic PCI Registry from 1994 to 2010. Results were the incidence and prevalence of CVD risk factors as estimate by the Framingham risk score. Results: Between 1994 and 2010, 25 519 patients underwent a PCI. During the time assessed, the mean age at PCI became older, but the gender distribution did not change. A significant trend towards higher body mass index and more prevalent hypercholesterolemia, hypertension, and diabetes was found over time. The prevalence of current smokers remained unchanged. The prevalence of ever-smokers decreased among males, but increased among females. However, overall CVD risk according to the Framingham risk score (FRS) and 10-year CVD risk significantly decreased. The use of most of medications elevated from 1994 to 2010, except for ${\beta}$-blockers and angiotensin converting enzyme inhibitors decreased after 2007 and 2006 in both baseline and discharge, respectively. Conclusions: Most of the major risk factors improved and the FRS and 10-year CVD risk declined in this population of PCI patients. However, obesity, history of hypercholesterolemia, hypertension, diabetes, and medication use increased substantially. Improvements to blood pressure and lipid profile management because of medication use may have influenced the positive trends.

  • Chitosan, hydroxycitrate and L-carnitine have been known to be antiobesity components. The purpose of this study was to evaluate the combined effects of chitosan, hydroxycitrate and L-carnitine mixture as a potential antiobesity supplement in overweight women. Pre-menopausal healthy females who were overweight (percent ideal body weight > 110) were included in this study. Forty-nine subjects randomly received a placebo (n = 25) or antiobesity-supplement (n = 24), which was a mixture of chitosan, hydroxycitrate, and L-carnitine. Before and after the eight-week experimental period, anthropometric parameters, blood components and computerized tomography were measured. At baseline, the two groups were well matched in terms of age, body mass index and lipid profile. After the eight weeks of potential antiobesity supplementation, the subjects' body fat percent had decreased significantly (p < 0.001) by 5.6% (39.1 $\pm$ 1 vs 36.9 $\pm$ 1%) while lean body mass increased (p < 0.01). Vsceral fat area at the L4 vertebra decreased significantly (p < 0.01) by 8.6% in the supplemented group and the total fat area at the L4 vertebra showed a tendency to decrease (p = 0.051) by 2.4%. Also, in the group given the antiobesity-supplement rather than the placebo, the fasting triglyceride level decreased significantly (p < 0.05) by 10.0%. In addition, serum total cholesterol levels in the antiobesity-supplement group showed a tendency to decrease (p=0.159) by 2.7% (194 $\pm$ 6 vs 189 $\pm$ 6 mg/dl). No side effects were found in either group during the intervention. In conclusion, the present study demonstrated that taking a mixture of chitosan, hydroxycitrate, and L-carnitine as a potential antiobesity supplement for eight weeks produced advantageous changes in the weight and visceral fat accumulation of overweight women without any side effects. • Objective: To evaluate several metabolic changes in patients with differentiated thyroid carcinoma (DTC ) which enhance cardiovascular risk in the western region of Nepal. Materials and Methods: This hospital based case control study was carried out using data retrieved from the register maintained in the Department of Biochemistry of the Manipal Teaching Hospital, Pokhara, Nepal between $1^{st}$ January, 2009 and $31^{st}$ December, 2011. The variables collected were age, gender, BMI, glucose, insulin, HbA1C, CRP, fibrinogen, total cholesterol, triglycerides, HDL, LDL, VLDL, f-T3, f-T4, TSH. One way ANOVA was used to examine statistical significance of differences between groups, along with the Post Hoc test LSD for comparison of means. Results: fT3 values were markedly raised in DTC cases ($5.7{\pm}SD1.4$) when compared to controls ($2.2{\pm}SD0.9$). Similarly, fT4 values were also moderately raised in cases of DTC ($4.9{\pm}SD1.3$ and $1.7{\pm}SD0.9$). In contrast, TSH values were lowered in DTC cases ($0.39{\pm}SD0.4$) when compared to controls ($4.2{\pm}SD1.4$). Mean blood glucose levels were decreased while insulin was increased and HDL reduced ($39.5{\pm}SD4.7$ as compared to the control $43.1{\pm}SD2.2$). Conclusion: Cardiovascular risk may be aggravated by insulin resistance, a hypercoagulable state, and an atherogenic lipid profile in patients with differentiated thyroid cancer.

  • In the biological world, there are a number of ecological fights for survival between each organism such as plants, animals and microorganism In such events, an organism can use its natural bioactive products as defence agent against other organisms. Furthermore, natural bioactive products can be utilized for medicine or functional food. Recently, we investigate the effect of Monascus pigment extracted from a fungus, Monascus anke, on the alcohol metabolism and blood lipid profile. In the present study, it is observed that Monascus pigment supplemented dietary may have a hepatoprotective effect on rat's liver damage induced with $CCl_4$ . By treatment with $CCl_4$(3 times, I.P), liver damage was reduced more in the rats fed 2% Monascus pigment extract supplemented diet than those fed standard diet, based on the serum levels of alanine aminotransferase, microsomal glucose-6-phosphatse activity and hepaic malondialdehyde content. On the other hand, oxygen free radical generating enzymes, hepatic P-450 dependent aniline hydroxylase, xanthine oxidase, and oxygen free radical scavenging enzymes, hepatic glutathione S-transferase, catalase, superoxide dismutase activities were generally higher both in $CCl_4$ treated group and control fed 2% Monascus pigment extract supplemented diet than those fed standard diet. In conclusion, the rats fed 2% Monascus pigment extract supplemented diet showed more reduced liver damage than those fed standard diet, which may be due to the acceleration of oxygen free radical metabolism.

  • Background: Intrauterine environment plays a pivotal role in the origin of fatal diseases such as the metabolic syndrome. Diabetes is associated with low-grade inflammatory state and dysregulated adipokines production. The aim of this study is to investigate the effect of maternal diabetes on adipocytokines (adiponectin, leptin and TNF-${\alpha}$) production in F1 offspring in rats. Methods: The offspring groups were as follows: F1 offspring of control mothers under control diet (CD) (CF1-CD), F1 offspring of control mothers under high caloric diet (HCD) (CF1-HCD), F1 offspring of diabetic mothers under CD (DF1-CD), and F1 offspring of diabetic mothers under HCD (DF1-HCD). Every 5 weeks post-natal, 10 pups of each subgroup were culled to obtain blood samples for biochemical analysis. Results: The results indicate that DF1-CD and DF1-HCD groups exhibited hyperinsulinemia, dyslipidemia, insulin resistance and impaired glucose homeostasis compared to CF1-CD (p > 0.05). DF1-CD and DF1-HCD groups had high hepatic and muscular depositions of TGs. The significant elevated NEFA level only appeared in offspring of diabetic mothers that was fed HCD. DF1-CD and DF1-HCD groups demonstrated low serum levels of adiponectin, high levels of leptin, and elevated levels of TNF-${\alpha}$ compared to CF1-CD (p > 0.05). These results reveal the disturbed metabolic lipid profile of offspring of diabetic mothers and could guide further characterization of the mechanisms involved. Conclusion: Dysregulated adipocytokines production could be a possible mechanism for the transgenerational transmittance of diabetes, especially following a postnatal diabetogenic environment. Moreover, the exacerbating effects of postnatal HCD on NEFA in rats might be prone to adipcytokine dysregulation. Furthermore, dysregulation of serum adipokines is a prevalent consequence of maternal diabetes and could guide further investigations to predict the development of metabolic disturbances.
