Effect and Safety of Replacement Therapy with Insumon in Type 2 Diabetes Mellitus (제2형 당뇨병 환자에 대한 대체 요법 [인슈몬]의 효과 및 안전성)

  • This research was concentrated to Investigate the effects of Insumon as a newly developed dietary supplement in patients with type 2 diabetes. The average glycated hemoglobin level was dropped from 7.0${\pm}$1.1% to 6.7${\pm}$0.9% after 8 weeks with statistic significance (p=0.0202). For patients of HbA1c over 7%, the average HbA1c significantly decreased from 8.1${\pm}$0.8% to 7.5${\pm}$0.8% (p=0.0171). The average fasting blood glucose level was decreased significantly from 9.5${\pm}$1.2 mmol/L (171.6${\pm}$22.3 mg/dL) to 8.5${\pm}$1.5 mmol/L (152.3${\pm}$26.3 mg/dL) (p=0.0262) after 4 weeks and to 8.8${\pm}$1.5 mmol/L (158.3${\pm}$24.1 mg/dL) (p=0.0445) after 8 weeks in the group of patients of FBG of over 7.8 mmol/L (140 mg/dL). The average HDL cholesterol level was increased significantly after 8 weeks from 1.2${\pm}$0.2 mmol/L (46.5${\pm}$8.6 mg/dL) to 1.3${\pm}$0.3 mmol/L (51.6${\pm}$10.9 mg/dL) (p=0.0007). Thus, these results suggested that patients with type 2 diabetes might benefit by Insumon as a herbal dietary supplement without any serious side effects.

Accuracy Evaluation of Bi-medium Deep Body Thermometer Based on Finite Element Simulation (유한 요소 시뮬레이션을 이용한 이중 매질 심부 체온계의 정확도 평가)

  • Continuous body temperature monitoring is useful and essential in diverse medical procedures such as infection onset detection, therapeutic hypothermia, circadian rhythm monitoring, sleep disorder assessment, and gynecological research. However, the existing thermometers are too invasive or intrusive to be applied to long-term body temperature monitoring. In our previous study, we invented the bi-medium deep body thermometer which can noninvasively and continuously monitor deep tissue temperature. And the ratio of thermal resistances expressed as K-value should be obtained to estimate body temperature with the thermometer and it can be different under various measurement environments. Although the device was proven to be useful through preliminary simulation test and small group of human study, the experimental environment was restrictive in our previous approach. In this study, a finite element simulation was executed to obtain the K-value and evaluate the accuracy of bi-medium thermometer under various measurement environments. In addition, K-value estimation equation was developed by analyzing the influence of 5 measurement environmental factors (medium length, medium height, tissue depth, blood perfusion rate, and ambient temperature) on K-value. The results revealed that the estimation accuracy of bi-medium deep body thermometer based on computer simulation was very high (RMSE < $0.003^{\circ}C$) in various measurement environments. Also, bi-medium deep body thermometer based on K-value estimation equation showed relatively accurate results (RMSE < $0.3^{\circ}C$) except for one case. Although the K-value estimation technology should be improved for more accurate body temperature estimation, the results of finite element simulation showed that bi-medium deep body thermometer could accurately measure various tissue temperatures under diverse environments.

Posterolateral Thoracotomy versus Muscle-sparing Vertical Thoracotomy (후측개흉술과 근육보존수직개흉술의 임상고찰)

  • Although posterolateral thoracotomy(PLT) has been a standard thoracic incision in resection surgery of the lung for surgeons to achieve a good surgical field, there remains concern about severing a group of thoracic muscles remains. Muscle-sparing vertical thoracotomy (MVT) is an alternative to PLT, which gives cosmetic result and may preserve motion of the shoulder girdle as well as respiratory function of the patient in the early postoperative period. However, surgeons tend not to perfer it because of limited surgical field from the vertical wound made on the lateral thoracic wall. The purpose of this study is to compare the surgical outcomes of PLT versus MVT. We retrospectively reviewed 29 patients(15 who had PLT and 14 who had MVT, organized into those two groups) who had undergone lung resection surgery in our institute. There were no clinical differences between the two groups in terms of operation time, estimated amount of blood loss during the operation, amount of chest drainage on the first and the second postoperative day, duration of chest tube placement, incidence and amount of transfusion, and postoperative complications. We conclude that, from our limited experience, MVT can be applied to lung resection surgery as safely as PLT and that it may have a beneficial role for the patient with compromised lung function in addition to cosmetic effect.

Effects of Gamigukihwandong-hwan on Renal Function, Oxidative Stress and Polyol Pathway in Diabetic Nephropathy Rats (Streptozotocin으로 유발된 흰쥐의 당뇨병성 신증에서 가미구기환동환(加味枸杞還童丸)이 Oxidative Stress 및 Polyol Pathway에 미치는 영향)

  • Diabetes is a disease in which the body does not produce or properly use insulin. Etiological studies of diabetes and its complications showed that oxidative stress might play a major role. Therefore, many efforts have been tried to regulate free oxygen radicals for treating diabetes and its complications. Gamigukihwandong-hwan has been known to be effective for the treatment of diabetes. The present study was carried out to investigate the effect of Gamigukihwandong-hwan on renal function, peroxynitrite (ONOO$^-$) scavenging activity and polyol pathway in streptozotocin-induced diabetic rats. The crushed Gamigukihwandong-hwan was extracted 3 times, each time with 3 volumes of methyl alcohol at 60$^{\circ}C$ for 24 h. The extract was filtered and evaporated under a reduced pressure using a rotary evaporator to yield 74.95 g. Gamigukihwandong-hwan extract was oral-administered 100 mg per 1 kg of body weight for 20 days to the diabetic rats induced by streptozotocin (60 mg/kg). The effects of Gamigukihwandong-hwan extract on the streptozotocin-induced diabetic rats were observed by measuring the serum level of glucose, insulin, lipid components, creatinine and BUN, and also the kidney levels of superoxide anion radical (${\cdot}O_2^-$), nitric oxide (NO) and ONOO$^-$, and also the enzyme activities involved in polyol pathway. The Effects of Gamigukihwandong-hwan on the streptozotocin-induced diabetic rats with regards to body weight, blood glucose and insulin levels, creatinine and BUN levels, total cholesterol and triglyceride levels, and HDL-cholesterol levels were all shown to be good enough to cure and prevent the diabetes and its complications. Gamigukihwandong-hwan inhibited the generation of ${\cdot}O_2^-$, NO and ONOO$^-$ in the kidney of streptozotocin-induced diabetic rats. Renal aldose reductase and sorbitol dehydrogenase activities were increased in the streptozotocin-induced diabetic rats, whereas the ones in the Gamigukihwandong-hwan administered group among the streptozotocin-induced diabetic rats were reversed toward the natural activities. Gamigukihwandong-hwan might inhibit the development of diabetic nephropathy by scavenging reactive oxygen and nitrogen species, thereby by reducing oxidative stresses and also by regulating the activities of polyol pathway enzymes, all of which could help to recover the function of kidney.

Therapeutic Benefits of Mesenchymal Stromal Cells in a Rat Model of Hemoglobin-Induced Hypertensive Intracerebral Hemorrhage

  • Previous studies have shown that bone marrow mesenchymal stromal cell (MSC) transplantation significantly improves the recovery of neurological function in a rat model of intracerebral hemorrhage. Potential repair mechanisms involve anti-inflammation, anti-apoptosis and angiogenesis. However, few studies have focused on the effects of MSCs on inducible nitric oxide synthase (iNOS) expression and subsequent peroxynitrite formation after hypertensive intracerebral hemorrhage (HICH). In this study, MSCs were transplanted intracerebrally into rats 6 hours after HICH. The modified neurological severity score and the modified limb placing test were used to measure behavioral outcomes. Blood-brain barrier disruption and neuronal loss were measured by zonula occludens-1 (ZO-1) and neuronal nucleus (NeuN) expression, respectively. Concomitant edema formation was evaluated by H&E staining and brain water content. The effect of MSCs treatment on neuroinflammation was analyzed by immunohistochemical analysis or polymerase chain reaction of CD68, Iba1, iNOS expression and subsequent peroxynitrite formation, and by an enzyme-linked immunosorbent assay of pro-inflammatory factors (IL-$1{\beta}$ and TNF-${\alpha}$). The MSCs-treated HICH group showed better performance on behavioral scores and lower brain water content compared to controls. Moreover, the MSC injection increased NeuN and ZO-1 expression measured by immunochemistry/immunofluorescence. Furthermore, MSCs reduced not only levels of CD68, Iba1 and pro-inflammatory factors, but it also inhibited iNOS expression and peroxynitrite formation in perihematomal regions. The results suggest that intracerebral administration of MSCs accelerates neurological function recovery in HICH rats. This may result from the ability of MSCs to suppress inflammation, at least in part, by inhibiting iNOS expression and subsequent peroxynitrite formation.

An Experimental Study on the Effect of Subperiosteal Transplantation of Fracture Site Hematoma: Focus on the Scintigraphic Detection (골절부위에 생긴 혈종의 골막하 이식이 골형성에 미치는 영향에 관한 실험적 연구 - 골스캔 소견을 중심으로 -)

  • It has been reported that hematoma is one of the most crucial factors in fracture healing since callus formation is disturbed by washing out the hematoma near a fracture site. However, it is not clear why the hamatoma is important and how it plays a role during the fracture healing. In order to investigate the role of hematoma in the process of fracture healing, the osteogenic potential by subperiosteal transplantation have been studied. Experimental fractures by operation were made at the mid-shaft of the tibia in New Zealand white rabbits. Removal of hematoma at the fracture site was done after 2 and 3 days from experimental fracture, and the removed hematoma was transplanted into the subperiosteal area at the mid-shaft of the ulna of each rabbit. As control groups, we have performed 3 different procedures 1) the hematoma was transplanted into the muscular layers at the thigh and forearm; 2) autologous blood clots were transplanted into the subperiosteal area of the ulna; and 3) sham operation without a transplantation into the subperiosteal area. After transplantation, serial bone scintigraphy and simple radiography were performed at 4 days, 1 week, and 2 weeks to detect an abnormality. The results of bone scintigraphy were positive in 5 of 6 experimental group. However, all in three control groups were negative. Histological observation of the positive bone revealed new bone formation with trabeculation. These results suggest the hematoma in fracture site has osteogenic potential in the subperiosteal area which can be demonstrable by bone scintigraphy and histologic findings. Therefore, it is considered that hematoma of the fracture site plays an important role in the process of fracture healing. Further biochemical investigation using various experimental models is mandatory to apply this preliminary result to the treatment of clinical delayed union or nonunion.

Effects of Forage Feeding Levels on the Udder Volume, Serum Hormone Level and Lactation Characteristics in Dairy Cows: From Growing to Lactating Phase (Holstein 젖소에서 조사료 급여 수준이 유방크기, 호르몬 및 산유특성에 미치는 영향: 육성기부터 비유기까지)

  • In the present study, we investigated the effects of high forage diets on the volume of udder, hormone level in blood, and lactation characteristics in the Holstein dairy cow. We divided into two groups; high forage diet [HF, concentrate: forage=4:6 n=41] and low forage diet [LF, 6:4 n=21]. Five cows were selected from each group based on their age for measuring the udder volume and the serum levels of estradiol and progesterone. Lactation characteristics were compared between HF and LF. The udder volume was 2.5 fold larger in HF at early gestation (p<0.01), but no difference was noted afterward. For the hormone levels, no significant difference was found between the groups. In HF, milk yield was significantly increased and maintained high longer, while somatic cell count was approximately 50% lower. Meanwhile, the milk fat content was significantly lower in HF during early lactating phase (p<0.001), but there was no difference thereafter. For milk protein and solid content, and MUN, no differences were found between the groups during lactation. Our results indicated that feeding high forage diets to dairy cows can increase milk yield and quality without notable changes in the udder volume and hormone level.

Hepatoprotective Effects of Lycium chinense Mill Fruit Extracts and Fresh Fruit Juice (구기자 추출물과 생과즙의 간독성 보호효과)

  • Hepatoprotective effects of Bulro Kugi (Lycium chinense Mill) fruit extracts on $CCl_4-administered$ rats were investigated in vivo. Administration of $CCl_4$ increased plasma glutamic oxalacetic transaminase (GOT), glutamic pyruvic transaminase (GPT), and lactate dehydrogenase (LDH) activities, induced lipid peroxidation as measured by malondialdehyde (MDA) content of rat liver, and significantly increased liver weight. Feeding of B. Kugi (Lycium chinense Mill) slightly increased body weight gain, although not significantly different from normal group. B. Kugi (Lycium chinense Mill) fruit extracts reduced blood cholesterol level and inhibited $CCl_4-induced$ increases of plasma GPT, GOT, and LDH activities, whereas increased contents of MDA and cytochrome P-450, and GST activity in liver tissue of $CCl_4-administered$ rats. Roasted B. Kugi (Lycium chinense Mill) fruit extract showed highest hepatoprotective effect among samples tested. These results suggest water extracts of B. Kugi (Lycium chinense Mill) fruit possess promising hepatoprotective activity against $CCl_4-induced$ hepatic damage in rats.

The Antidiabetic Effect of Gamiyookmijihwangtang on Diabetes-prone BB Rats (Diabetes-prone BB Rat을 이용한 가미육미지황탕의 항당뇨 효과)

  • This study was conducted to know whether Gamiyookmijihwangtang(GY) which is Yookmijihwang added with Liriopis tuber, Anemarrhenae rhizoma and Phellodendri cortex can remedy the overt diabetes in diabetes-prone BB(BBDP) rats. The rats were given GY through the mother from the fetal stage until birth. After birth they received GY through breast feeding until 20 days old. From 21 days old which is the beginning of the weaning period 60 BB rats(30 males and 30 females) were divided into 2 experimental groups(BBDP and BBDP-GY) and placed individually in metabolic cages. BBDP was the control group which didn't receive any GY and BBDP-GY received 16 mL/㎏ B.W./day of GY until 120 days old. The antidiabetic effects of GY were characterized by the clinical features such as polyurea, polydipsia, hyperglycaemia and the rapid loss of body weight. Body weight, water consumption, urine volume and blood glucose level showed no signs of impending diabetes but after onset there were big changes in those parameters. The onset of diabetes was delayed and the incidence of diabetes was also much decreased with GY but after onset there were no beneficial effects from it.

Studies on Effect of Dietary Zine on Tissue Trace Elements in the Rat (식이아연(食餌亞鉛)이 흰쥐의 조직중(組織中) 미양금속(微量金屬)에 미치는 영향(影響))

  • Zinc is one of the essential trace elements in the living organism for growth and health. The first identified metalloenzyme, carbonic anhydrase, is a zinc compound and several others have been described since. Among zinc deficiency syndromes in animals porcine parakeratosis has been successfully treated with zinc supplements, and in man a syndrome of anemia, hypogonadism, hepatosplenomegaly, and dwarfism, prevalent in parts of Iran and Egypt, has been ascribed to lack of zinc in the diet. Dietary zinc excess in the rat is manifested by a hypochromic, microcytic anemia, poor growth, reduction in liver catalase and cytochrome oxidase. The present study is an attempt to delineate the changes of tissue contents of trace elements, especially of iron, copper and zinc in liver and kidneys of the rats. Weanling albino rats, weighing 60 to 80gm. were used in this experiments. The rats were housed in cages with aluminum floors and received feed and distilled water ad libitum. Animals were divided into three groups, control, low zinc diet and high zinc diet groups. The high zinc diet group was subdivided into 0.5% Zn and 0.7%Zn groups. The supplementary copper or iron was added to the high dietary zinc groups. The animals were sacrificed and the tissues were washed several times with deionized water. The wet digested samples were analyzed by Hitachi Model 207 atomic absorption spectro-photometer for the determination of iron, copper and zinc in the liver and kidney. Hemoglobin level in the blood was measured by cyanmethemoglobin method. The results of this study are as follows: 1) All rats fed high zinc diets and low zinc diets gained less weight than control. Weight gain was not improved by the supplementary copper or iron and both. 2) Hemoglobin concentration was decreased significantly in the rats fed high zinc diets and less in the low zinc diet. Supplementary copper and iron to the higher zinc diet appeared to give some improvement of anemia. 3) The iron contents of the liver and kidneys were significantly decreased in the high zinc groups and the reduction was more significantly in the rats receiving higher zinc diet (0.7%). The supplementary copper caused a further depression of liver iron. On the other hand, the iron, added to the high zinc diet lessoned the severity of the decrease in liver iron and caused kidney iron to be maintained almost at the level found in the rats fed by zinc and supplementary copper diet. 4) High zinc diets did not change copper content of the liver and kidney. Supplementary copper elevated the concentration in the liver and kidney and added iron had no effect on the accumulation of copper in the liver and kidneys. 5) The high zinc diets caused marked increases of zinc content in the liver and kidney. Supplementary iron to the high zinc diet caused increases of zinc contents of liver and kidneys.
