Study on the Origin, Description and Composition of Sokmyeung-tang(續命湯) (속명탕(續命湯)의 출처(出處), 종류(種類) 및 조성(造成)에 대한 고찰(考察))

  • Sokmyeungtang was the representative prescription for Apoplexy under the rule of Tang and Song dynasty of which the cultures were thriving in the history of China. However, the clinical use of Sokmyeungtang has been gradually reduced since Geumwon dynasty of China because it was misunderstood that the dryness heat drugs of pungent in flavor and warm in property such as Ephedra, Pubescent Angelica Root, Chinese Cassia Tree-Bark, Divaricate Saposhnikovia Root, Prepared Aconite Root, Fresh Ginger, and Wildginger Herb included in the presciption for Apoplexy supplemented heat as damaging Yin flood. In fact, the drugs pungent in flavor and warm in property activate exterior and interior circulation, circulate channels and collaterals, promote blood circulation, and remove blood stasis with the side effect of relieving exterior syndrome with drugs warming channels. When treating Apoplexy with Sokmyeungtang, the cold drugs such as Gypsum, Baikai Skullcap Root, and Pueraria Root are prescribed to suppress fire of pungent dryness and to control excessive heat of people with Apoplexy as reducing the effects of hot drugs causing impairment of Yin. For treatment of Apoplexy, the above drugs accelerate blood and Qi circulation in channels and collaterals and then in necrotic tissue of human body as removing blood stasis. Consequently, these drugs improve disorders of capillary tube circulation. If Sokmyeungtang, an old prescription, is properly understood, it will be substantially helpful to all kinds of treatments in clinical cases

Effects of Induced Microcurrent Shoes on Change of Blood Circulation to patients with Chronic Plantar (미세전류발생 신발 착용이 만성 족저근막염 환자의 혈류량 변화에 미치는 영향)

  • Purpose: The purpose of this study was to investigate how induced microcurrent shoes influenced changes of the blood circulation in patients with Plantar Fasciitis. Methods: Initially, the subjects were comprised of 5 males and 5 females, who agreed with this research and are more than fifty years old, but 4 of those were dropped during the experiment. They all have plantar fasciitis and pain on their feet. Subjects wore the induced microcurrent shoes for more than 4 hours everyday during 4 weeks. When they wore those shoes, they also wore the specially produced shocks made of silver-mixed thread and they were asked to avoid intense exercise. Assessments were carried out before and after walking on a treadmill and we measured changes between the test before and after 4 weeks. In the examination of the before test, general shoes were used, and in the examination of the after test, induced microcurrent shoes were used. Temperature difference was measured by thermograpy DOREX spectrum 9000MB(USA). Subjects walked total 20 minutes and during gait, the walking rate on a treadmill was increased from 2Km/h to 3Km/h after 10 minutes. We measured plantar temperature by thermograpy especially both heel, 1st, and 5th metatarsal areas. Results: Firstly, in comparison of the blood circulation on the left and right foot at 0 week and 4 weeks, it tended toward increasing blood circulation but there was. no statistically significant difference (p>0.05). Secondly, in comparison of the blood circulation before and after treadmill with the induced microcurrent shoes, the blood circulation of the heel and the 5th area on the right foot was increased to all subjects(p<0.05). Conclusion: The results of this study revealed that subjects showed tendency to increase blood circulation in both right and left feet after wearing microcurrent shoes and specially after walking treadmill at 4 weeks. Therefore induced microcurrent shoes are useful to improve blood circulation for patients with plantar fasciitis.

Effect of Self-Foot Reflexology on Peripheral Blood Circulation and Peripheral Neuropathy in patients with Diabetes Mellitus (자가 발반사마사지가 당뇨병 환자의 말초순환과 말초신경증에 미치는 효과)

  • Purpose: This study was done to investigate the effect of self-foot reflexology on peripheral blood circulation, peripheral neuropathy and to determine the feasibility of self-foot reflexology as a nursing intervention. Method: This was nonequivalent control pretest-posttest study with 76 patients with type 2 diabetes mellitus (ages between 40-79) recruited from public health centers in Busan city. Intervention was a 6 week self-foot reflexology, and outcome variables were peripheral blood circulation and peripheral neuropathy(tactile response to monofilament, intensity of symptoms of peripheral neuropathy). ANCOVA was used to do the statistical analysis. A.05 significance level was set for evaluating the effects of self-foot reflexology. Results: The self-foot reflexology was relatively effective not only in reducing peripheral neuropathy(especially tingling sensation and pain) but also in improving ability to sense the 10-g force monofilament. Conclusion: Even though self-foot reflexology was not effective in improving peripheral circulation, it had good effect on improving peripheral neuropathy. Therefore self-foot reflexology can be used as a nursing intervention program for promoting foot care for patients with DM patients.

Analysis of High School Students' Conceptual Change in Model-Based Instruction for Blood Circulation (혈액 순환 모형 기반 수업에서 고등학생들의 개념 변화 분석)

  • The purpose of this article is to analyze the conceptual change of nine 11th graders after implementing the model-based instruction of blood circulation by multidimensional framework, and to find some implications about teaching strategies for improving conceptual understanding. The model-based instruction consisted of 4 periods: (1) introduction for inducing students' interests using an episode in the science history of blood circulation, (2) vivisectional experiment on rats, (3) visual-linguistic model instruction using the videotape of heartbeat, and (4) modeling activity on the path of blood flow. Based on the data from pre-test, post-test and interviews, we classified students' models on the path of blood flow, and investigated their ontological features and the conceptual status of blood circulation. Most students could describe the path of blood flow and the changes of substances in blood precisely after the instructions. However, the modeling activity were not sufficient to improve students' understanding of the mechanisms of the blood distribution throughout various organs and the material exchanges between blood and tissues. From the interview of 9 students, we acquired informative results about conceptual status elements that were helpful to, preventing from, or not used for students' understanding. It was also found that conceptual status of students depended on the ontological categories into which students' conceptions of blood circulation fell. The results of this study can help design the effective teaching strategy for the understanding of concept of the equilibrium category.

Application of the Pulsatile Cardiopulmonary Bypass in Animal Model (이중 박동성 인공심폐기의 동물 실험)

  • Background: Currently, the cardiopulmonary machine with non-pulsatile pumps, which are low in internal circuit pressure and cause little damage to blood cells, is widely used. However, a great number of experimental studies shows that pulsatile perfusions are more useful than non-pulsatile counterparts in many areas, such as homodynamic, metabolism, organ functions, and micro-circulation. Yet, many concerns relating to pulsatile cardiopulmonary machines, such as high internal circuit pressure and blood cell damage, have long hindered the development of pulsatile cardiopulmonary machines. Against this backdrop, this study focuses on the safety and effectiveness of the pulsatile cardiopulmonary machines developed by a domestic research lab. Material and Method: The dual-pulsatile cardiopulmonary bypass experiment with total extracorporeal circulation was conducted on six calves, Extracorporeal circulation was provided between superior/inferior vena cava and aorta. The membrane oxygenator, which was placed between the left and right pumps, was used for blood oxygenation. Circulation took four hours. Arterial blood gas analysis and blood tests were also conducted. Plasma hemoglobin levels were calculated, while pulse pressure and internal circuit pressure were carefully observed. Measurement was taken five times; once before the operation of the cardiopulmonary bypass, and after its operation it was taken every hour for four hours. Result: Through the arterial blood gas analysis, PCO2 and pH remained within normal levels. PO2 in arterial blood showed enough oxygenation of over 100 mmHg. The level of plasma hemoglobin, which had total cardiopulmonary circulation, steadily increased to 15.87 $\pm$ 5.63 after four hours passed, but remained below 20 mg/㎗. There was no obvious abnormal findings in blood test. Systolic blood pressure which was at 97.5$\pm$5.7 mmHg during the pre-circulation contraction period, was maintained over 100 mmHg as time passed. Moreover, diastolic blood pressure was 72.2 $\pm$ 7.7 mmHg during the expansion period and well kept at the appropriate level with time passing by. Average blood pressure which was 83$\pm$9.2 mmHg before circulation, increased as time passed, while pump flow was maintained over 3.3 L/min. Blood pressure fluctuation during total extracorporeal circulation showed a similar level of arterial blood pressure of pre-circulation heart. Conclusion: In the experiment mentioned above, pulsatile cardiopulmonary machines using the doual-pulsatile structure provided effective pulsatile blood flow with little damage in blood cells, showing excellence in the aspects of hematology and hemodynamic. Therefore, it is expected that the pulsatile cardiopulmonary machine, if it becomes a standard cardiopulmonary machine in all heart operations, will provide stable blood flow to end-organs.

Numerical analysis of the blood flow in coronary artery combining CFD method with the vascular system modeling (혈관계 시스템 모델과 CFD의 결합을 통한 관상동맥 내 혈류의 수치적 해석)

  • For the simulation of the blood flow in coronary artery, the system modeling of coronary hemodynamics is combined with CFD technique. The blood flow in coronary artery interacts with the global coronary circulation. Especially in case of the coronary artery with stenosis, the interaction plays an important role in the hemodynamics of the circulation. In this study we present a combined numerical approach using both the CFD technique for flow simulation and the global system model of coronary circulation. We use a lumped parameter model for the global simulation of coronary circulation whereas the finite element method is employed to compute the viscous flow field in stenosed coronary artery, The time variation of the pressure drop due to stenosis is obtained from the proposed numerical method. Numerical results shows that the flow resistance and pressure drop due to stenosis has a relatively large value in systole.

Two Cases of Insomnia in Stroke Patients, Treated with Enhanced Blood Circulation (활혈거어법(活血祛瘀法)으로 호전된 중풍환자(中風患者)의 불면(不眠) 2례)

  • Insomnia is defined as an experience of inadequate or poor-quality sleeping habit, coupled with more than one of the following symptoms : having difficulty sleeping, waking up too early in the morning or not having sound sleep. Insomnia is one of the most common complications that stroke patients experience, which interfere the patients' recovery. The authors observed two improved cases of insomnia in stroke patients by administrating traditional Korean medicine. The patients suffered from insomnia with nocturnal irritation, constipation, petechia of the tongue, thready or weak pulse. These symptoms, were diagnosed as blood stasis and we administered Hyulboochucke-tang three times a day, to improve blood circulation to remove blood stasis. After the administration, nocturnal irritation disappeared and the insomnia improved. Based on this experience, it is considered that enhancing blood circulation to remove blood stasis can be applied to the treatment of insomnia that stroke patients suffer.

The Concept of Artificial Liver Support by Using the Extracorporeal Circulation System

  • In this study, a basic research on artificial liver was performed for its application to people on the waiting list of liver transplant or patients with hepatic insufficiency. Artificial livers are generally classified into mechanic type, bioartificial type, and hybrid type. An extracorporeal circulation device was examined herein, which is indispensable in the application of an artificial liver, for its effectiveness in supporting the recovery of liver functions. Extracorporeal circulation system is a treatment and life-support system which sends out the patient's blood, removes toxicity by various methods, and then sends the blood back to the interior of the body. This study used an extracorporeal circulation system which enables the Plasma Perfusion by CVVH method, and applied the program of Bioateco corp. Animals with acute hepatic insufficiency were produced to apply the extracorporeal circulation device. As a result, their ammonia, bilirubin, SGOT, SGPT, and bile acid levels rose, confirming the liver function restoration in the experimental animals.

The Effect of Woohwangcheongsim-won on Circulatory Disturbance in Diabetes (우황청심원이 당뇨병 Rat의 혈액순환장애에 미치는 영향)

  • Object: Death rate due to hypertension, atherosclerosis, ischemic heart disease and cerebral infarction induced by Westernized diet and increased average life span is on the rise. Decrease in blood circulation, activation of thrombus generation and intravascular lipid accumulation, cited as the principal causes of the above mentioned diseases in recent studies, result in circulatory disturbance and blood vessel obstruction leading to ischemic cell death of heart, brain and peripheral vessels. Method: We investigated the biochemical changes in microvascular permeability, aggregation of platelet and the intravascular lipid accumulation in induced-diabetic rat using Streptozotocin. We also studied the effects of Woohwangcheongsirn-won after oral administration on blood circulation, platelet function and lipid metabolism. The results are as follows: I. Woohwangcheongsim-won increased blood circulation in microvessels. 2. Woohwangcheongsim-won increased the reduced erythrocyte deformability in diabetes. 3. Woohwangcheongsim-won induced the reduction of contents of 2, 3-DPG, but failed to affect the reduced contents of ATP in erythrocyte in diabetes. 4. Woohwangcheongsim-won reduced the activity of Ca/sup 2+/-ATPase in the membrane of erythrocyte. 5. Woohwangcheongsim-won reduced the platelet aggregation evoked by platelet agglutinin factor. 6. Woohwangcheongsim-won reduced the production of platelet-derived granules. 7. Woohwangcheongsim-won reduced the production of metabolites of arachidonic acid in diabetes, and also reduced the production of increased thromboxane B2. 8. Woohwangcheongsim-won reduced the synthesis of oxidized LDL-cholesterol. In conclusion, Woohwangcheongsim-won enhanced blood circulation in microvesseles, erythrocyte deformability and inhibited the increased platelet aggregation and the synthesis of oxidized LDL-cholesterol in diabetes. Therefore Woohwangcheongsim-won is believed to positively affect blood circulation (J Korean Oriental Med 2002;23(2):164-179)

  • This experiment was performed in order to research the effects of moxibustion on peripheral circulation. The results were as follows : 1. Distribustion of sex: male 5 cases, female 15 cases 2. Distribustion of age: 20s case, 30s 1 case, 40s 4 cases, 50s 7cases, 60s 6 cases, 70s 1 case. 3. After moxibustion, there were no changes in 4cases but 16cases were improved in peripheral blood circulation.
