A case of intramuscular xanthelasma palpebrarum found during blepharoplasty

  • Xanthelasma palpebrarum is the most common cutaneous xanthoma found on the medial side of the eyelid. The typical lesion is usually a flat and yellowish plaque on the skin. However, we report on a unique case of intramuscular xanthoma found during blepharoplasty for the correction of ptosis. A 53-year-old male patient visited our department with a complaint of a ptotic eyelid. He was concerned about the cosmetic appearance and the uncomfortable feeling while opening his eyes, and wanted these problems to be solved. A yellowish plaque of about $0.3{\times}0.3cm$ in size was found in the orbicularis oculi muscle during the surgery. The lesion was excised and xanthelasma was confirmed with biopsy. We have found this specific case of xanthelasma palpebrarum in the only muscle. Therefore, a careful approach to clinical and histologic examination and imaging is required for patients with these lesions.

동양인에서의 노인성 하안검의 유형별 분류 및 수술법의 선택 (The Classification of Aging Lower Eyelid and Selection of the Operation Options in Asians)

  • Purpose: It is generally accepted that anatomical structures of the soft tissue in Asian faces are quite different from those in Caucasian. It is presumed that these differences are due to collagen rich thick dermis and durable superficial musculo-apponeurotic system (SMAS) in Asian. We classified the aging lower eyelids and reviewed the operative procedures according to the types of aging lower eyelids in Asian. Methods: We compared preoperative and postoperative photos of 117 patients over 30 years of age, who underwent lower blepharoplasty at the Kyunghee Medical Center from January 2001 to April 2006. We classified the patients based on the degree of skin laxity, presence of nasojugal groove and malar bag, the extent of aging process. We also reviewed the operative procedures according to each type of classification. Results: We classified our patients into four types as following. Type I patients showed minimal skin-muscle excess confined to lower eyelids regardless of the facial line. For these patients, we performed only transcutaneous or transconjunctival blepharoplasty. In type II patients, nasojugal grooves were shown and skin- muscle laxity was limited to the medial side of imaginary vertical line at lateral margin of pupil. In these cases, we performed free fat graft or fat repositioning on nasojugal groove or fat removal and septal duplication confined to medial side. Type III patients displayed more advanced medial bulging and remarkable laxity over the lateral side, the same operation methods as those of type II were applied at the lateral side of the line. Type IV patients demonstrated extensive midfacial aging changes including malar bags and underwent superficial subciliary cheek lift. Conclusion: The lower eyelid aging of Asian is different from those of Caucasian. We think that our classification is useful in selection of appropriate operative procedure to address specific problems for Asian patients.

Anthropometric analysis of the eyeball exposure area in Koreans

  • Background Eyelids vary in morphology and structure depending on age, sex, and race. Usually, one-dimensional measurements are used to evaluate blepharoplasty outcomes. However, in recent years, three- or four- directional enlargements of the eyelid have been used for blepharoplasty in Asian patients, and the outcomes of techniques such as lateral canthoplasty or canthotomy and medial epicanthoplasty with increased scleral area cannot be analyzed using marginal reflex distance 1 or the area of corneal exposure. Therefore, we describe the use of a measurement method based on the eyeball exposure area (EEA). Methods To obtain normal EEA values by age and sex, 320 individuals were recruited. The participants were divided into eight age groups (n=40 in each group: female subjects, n=20; male subjects, n=20): 10-19, 20-29, 30-39, 40-49, 50-59, 60-69, 70-79, and 80-89 years. The EEA value was obtained by dividing the exposed area by the total eyeball area. The normal EEA values and differences in EEA according to age and sex were calculated. Results The mean EEA values for male and female subjects were 66.86% ±9.20% and 71.48%±10.48%, respectively. In each age group, the EEA values of females were higher than those of males. EEA decreased significantly with age in both male and female subjects (P<0.05). Conclusions We determined normal EEA values according to age and sex. EEA can be used as an indicator of the outcome of blepharoplasty.

Y needle을 이용한 간단한 이중검 수술 (SIMPLE DOUBLE EYELID OPERATION (ORIENTAL BLEPHAROPLASTY) USING Y NEEDLE)

  • 본 방법을 통해 수술할 경우 몇 가지 장점이 있다. Y needle의 경우 5mm간격으로 고안되어 있어 3부위에서 일정량의 조직결찰이 가능하고 고안에 따라 여러 가지 간격으로 만들어 질 수 있어 환자의 상태에 따라 적용가능하다. 또한 피부부위에서 결막으로 들어가는 부위가 수직이어서 원하는바 정확한 이중검 위치를 만들어 줄 수 있다. 초심자가 시행하더라도 관통부위를 eyelash 경계와 비교적 평행하게 만들어 줄 수 있다. Needle의 삽입이 2개가 한꺼번에 삽입되므로 환자가 느끼는 통증이 감소되며 술자의 편의도도 증가하리라 생각된다. 부분절개법을 사용하여 잉여지방 및 조직을 제거하였고 매몰하여 흉터와 부작용을 최소화하였고 회복기간과 수술시간을 감소시킬 수 있었다. 본 방법은 모든 경우의 이중검수술에 이용될 수는 없겠으나 고찰에서 기술한 비절개식이나 부분절개식 이중검수술의 적응증에 해당되는 경우에 이용할 경우 아주 간단하고 임상적으로 표준화 될수 있는 방법으로 응용될 수 있을 것으로 사료되어 문헌고찰과 함께 보고하는 바이다.

제거한 하안검 지방의 이식을 이용한 눈물받이 고랑의 교정 (Correction of the Tear Trough Using Orbital Fat Graft from the Lower Lid)

  • Lower palpebral bags and tear trough grooves are the common signs of aging. Weakness of the orbital septum, atrophy of the orbicularis oculi muscle, slackness and laxity of the lower lid skin are the main causes of these aging changes. Tear trough groove with skeletonization of the lower orbital rim can not be corrected easily by the conventional lower blepharoplasty, but the augmentation of the volume, can be an effective method. Lower lids provide a well vascularized recipient site, which is vital for proper grafting of fat to occur. We treated tear trough groove with free fat graft of surgically removed orbital fat. Surgical excision of the fat pad and septal reinforcement with free fat graft below the levator labii superioris alaeque nasi muscle were performed to correct tear trough groove. This technique was performed in 29 consecutive individuals from October, 2001 to January, 2005. Palpebral bags and tear trough groove were corrected concomitantly and obtained a youthful attractive midface with no secondary hollowness of lower lid and irregularity. The overall results obtained were satisfactory without any complications. Recurrence of tear trough deformity due to grafted fat absorption was not noted during the 7 months of the follow-up period. The authors conclude that the procedure is safe and effective in selected patients.

안와하벽재건술 후 발생한 지연성 눈뒤출혈: 증례보고 (Delayed Retrobulbar Hemorrhage after Orbital Floor Reconstruction)

  • Purpose: Retrobulbar hemorrhage is a rare complication followed by blepharoplasty, trauma, orbital reconstruction, and so on. Most of the cases occur within 24 hours, half of them in the first 6 hours. Some authors have reported delayed retrobulbar hemorrhage after blepharoplasty and trauma within 1 day to 9 days. However, there have been few reports of delayed retrobulbar hemorrhage resulting from the complication of orbital reconstruction. Methods: A 22-year-old male underwent orbital floor reconstruction due to the orbital floor fracture. In 84 hours after the surgery, he complained sudden onset orbital pain and decreased visual acuity immediately after defecation. Intraocular pressure was unmeasurable due to the swelling at that time. Emergency computed tomography was performed. Results: Computed tomography revealed subperiosteal hematoma on inferior orbital wall extended to the apex. Emergency decompressive surgery was performed within 1 hour. After evacuation of hematoma, orbital symptom was improved and visual acuity was restored. Conclusion: Delayed retrobulbar hemorrhage is rare but vision-threatening. Therefore early diagnosis and treatment of delayed retrobulbar hemorrhage is thought to be crucial. The cause of delayed hemorrhage was not clear, however, valsalva maneuver might be the cause of hemorrhage.
