편측 인공와우 이식자의 보청기 사용 (Use of Hearing Aids in Unilateral Cochlear Implantee)

  • The cochlear implantation(CI) as an useful tool for aural rehabilitation in bilateral severe to profound hearing impairment. However, CI prefer to usually one ear in spite of bilateral hearing impaired. because of the various characteristics of hearing loss, the hearing conservation for the future possibility, and socioeconomic condition of hearing impaired person and their families. The unilateral CI has limitations such as a directional loss, a difficult speech understanding in noise and a neural plasticity. These limitations will be overcome by hearing aid(HA) which is familiar with hearing impairer. but HA fitting for bimodal-binaural hearing are difficult because the difference output characteristic of HA and CI. This study will be confirm realities of use of HA in unilateral cochlear implantee. For this goal, 25(m:f=10:15) child participated who are used to HA for 1 to 17 months. We had telephone interviews with their mother about use of HA, change of auditory performance and own voice. As the results, hearing threshold levels of unimplanted ear, the use of a appropriate HA, implanted and aided hearing threshold level(HTL) are must be considered for successful biomodal-binaural hearing. Especially, implanted and aided HTL should be very useful parameter for a prediction of HA effect and a criterion of selection for bilateral cochlear implantation.

인공와우 사용자의 심리음향적 음질평가 예비연구 (A preliminary study of sound quality evaluation of cochlear implant users)

  • 음질평가는 물리적 음향 자극에 대해 주관적으로 지각되는 음색을 평가한다는 측면에서 심리음향학적 측정과 관련이 있다. 본 논문은 인공와우 사용자의 음질평가를 위한 예비연구로서 바이모달 인공와우 사용자를 대상으로 음질평가를 시행하고 인공와우만 착용했을 때와 바이모달을 착용했을 때의 음질평가 차이를 파악하고자 하였다. 총 13명의 바이모달 인공와우 사용자와 7명의 편측 인공와우 사용자가 연구에 참여하였고 순음 및 어음 청력검사, 보청기 기능이득과 실이삽입이득을 측정하였다. 음질평가에서는 바이올린소리, 남자와 여자 노래소리, 냉장고 소음의 4개 음향자극을 방음실에서 들려주고 6개 음질평가 항목에 대해 0에서 10까지의 척도로 표시하도록 하였다. 검사결과 바이모달 사용자는 편측 인공와우만 착용했을 때보다 바이모달을 착용했을 때 음질평가 5개 항목에서 0.8점 높았고, 바이모달 사용자와 편측 인공와우 사용자 그룹간 비교에서는 두 그룹 사이에 차이를 보이지 않았다. 주관적 측면의 바이모달 혜택 측정을 위해서 좀 더 체계적인 음질평가 도구와 방법에 대한 후속 연구가 필요할 것으로 생각된다.

양이 동시 인공와우 사용자의 조기 언어발달 (Early Linguistic Developments of Simultaneous Bilateral Cochlear Implantees)

  • Background and Objectives The present study aimed to compare receptive and expressive language development in children who have undergone simultaneous bilateral cochlear implantation (SCI) and those who have undergone bimodal stimulation (unilateral CI+ hearing aid). Subjects and Method In a retrospective analysis of clinical data, 15 pediatric patients who have received SCI and nine patients who have received bimodal stimulation (BM group) were enrolled. CI was performed for all patients at 24 months of age. Category of Auditory Performance (CAP) scores, Infant-Toddler Meaningful Auditory Integration Scale (IT-MAIS) scores, and developmental quotients (DQ) for expressive and receptive language were compared between the groups at 12 month of follow-up. The Percentage of Consonants Correct (PCC) of children evaluated at 4 years old was also compared. Results At 12 months of follow-up, significantly greater improvements in CAP scores (${\Delta}4.25{\pm}0.5$) were noted in the SCI group compared to the BM group (${\Delta}3.56{\pm}0.88$, p=0.041). Significantly greater improvements in IT-MAIS scores were also noted in the SCI group (${\Delta}36.17{\pm}4.09$) than in the BM group (${\Delta}30.17{\pm}2.91$, p=0.004). The DQ of receptive language was higher in the SCI group than in the BM group ($87.6{\pm}15.4%$ vs. $75.5{\pm}12.0%$, p=0.023) at 12 months of follow-up. Moreover, early SCI was associated with better receptive language skills. PCC index of children at 4 years old was higher in the SCI group than in the BM group ($88.5{\pm}13.2%$ vs. $62{\pm}15.8%$, p=0.014). Earlier SCI was associated with even greater improvements. Conclusion Bilateral SCI is associated with significant improvements in language development when compared with bimodal stimulation. Earlier SCI was associated with better outcomes.
