A Case of Double Primary Cancer (Gallbladder cancer and pancreatic cancer) in Patient with Anomalous Union of Pancreaticobiliary Duct

  • Anomalous union of the pancreaticobiliary duct (AUPBD) has been shown to predispose to carcinomas of the biliary tract (bile duct and gallbladder) and pancreatic cancer because of chronic recurrent inflammatory reaction due to pancreatic or bile juice refluxes. However, pancreatic duct would be less affected by the bile because of the relatively higher intraductal pressure of the pancreatic duct. We report a case of metachronous pancreatic cancer in AUPBD patient without choledochal cyst who underwent cholecystectomy because of gallbladder cancer.

Komi type 2 pancreaticobiliary maljunction: Minimal access surgical treatment (with video)

  • Pancreaticobiliary maljunction (PBM) is associated with the development of neoplasms of bile ducts. Cholecystectomy with diversion of the biliary-pancreatic flow is considered the treatment of choice. To describe the surgical treatment employed for a patient with Komi's type 2 PBM and its long-term results. Laparoscopic common bile duct exploration, intraoperative cholangioscopy, and Roux-en-Y hepatico-jejunostomy were performed. Postoperative evolution was satisfactory. The patient was discharge 72 hours after the surgery. There was no associated morbidity. At 62-month follow-up, clinical examination, laboratory tests, and imaging studies confirmed an adequate patency of bilio-enteric anastomosis. The surgical approach employed was effective and safe, with satisfactory long-term results.

배양된 흰쥐 담관 섬유모세포에 대한 간흡충 분비배설 물질의 영향 (Effect of Clonorchis sinensis Excretory-secretory Product on the Cultured SD Rat Bile Duct Fibroblast)

  • 간흡충(Clonorchis sinensis)은 우리나라에서 높은 감염률(2.9%)을 나타내는 기생충으로(KAHP, 2004), 간흡충에 감염된 담관은 간흡충의 흡반에 의한 물리적 자극과 대사산물 및 분비물 등의 화학적 자극에 의해 담관염이 일어나고, 간흡충이 성장할 때 충체 주위의 담관 상피세포의 증식, 탈락, 담관 주위의 염증 및 섬유화가 일어난다. 담관 점막에 분포하는 섬유모세포는 결합조직을 구성하는 세포의 한 종류로서 세포질돌기들이 잘 발달된 형태적 특징이 있으며, 세포질 내에 세포의 형태 유지, 신호전달, 인접세포와의 연접 등에 관여한다. 또한 조직을 발달시키고, 조직이 손상된 부위에서 콜라겐 층을 형성하여 손상된 조직이 복구되도록 하기도 한다. 상처의 반흔(scar) 형성과 지방축적, 염증(inflammation) 발생 과정에서 섬유모세포의 작용이 제대로 조절되지 못하면 섬유증(fibrosis)이 발달하게 된다는 연구 보고도 있다. 본 연구에서는 정상 흰쥐에서 분리된 담관 섬유모세포와 간흡충 감염 흰쥐에서 분리된 담관 섬유모세포를 배양하고, 각각의 실험군에 간흡충 분비배설 물질(Cs excretory-secretory product, ESP)을 첨가하여 배양하였다. 배양된 섬유모세포의 미세구조 변화와 세포 표면에 존재하는 sialic acid 및 actin의 분포를 전자현미경으로 관찰하여, 간흡충 감염에 따른 섬유모세포의 변화 및 간흡충 분비배설 물질의 자극에 따른 섬유모세포의 변화를 관찰하여 흰쥐의 담관 섬유모세포와 간흡충 감염과의 연관성을 알아보고자 하였다. 정상 흰쥐 담관에서 분리한 섬유모세포(G1)에 비하여 간흡충에 감염된 흰쥐 담관에서 분리한 섬유모세포(G2)와 간흡충 분비배설 물질을 첨가하여 배양한 섬유모세포(G1-1, G2-1)의 증식속도가 느린 것이 확인되었다. 세포질돌기의 수는 간흡충에 감염된 흰쥐의 담관으로부터 분리된 섬유모세포(G2)에서 가장 많은 수가 관찰되었고, 배양배지에 분비배설 물질을 첨가하면 정상 담관의 섬유모세포에서도 세포질돌기가 증가하였다. 따라서 간흡충 대사물질은 섬유모세포의 세포질돌기형성을 촉진시키는 것으로 생각된다. 간흡충에 감염된 흰쥐 담관에서 분리한 섬유모세포(G2)의 소포체는 정상 담관에서 분리된 섬유모세포(G1)의 것에 비하여 감소하는 양상을 나타내었고, 여기에 분비배설 물질을 첨가하면 섬유모세포의 소포체가 증가하는 것이 관찰되었다. 그리고 세포표면에 분비되는 sialic acid는 주로 세포질의 소포낭 주변에서 관찰되었으며, 정상 섬유모세포(G1, G1-1)보다 감염된 섬유모세포(G2, G2-1)에서 증가하였다. Actin은 세포표면과 세포질돌기에서 주로 관찰되었으며, 정상 섬유모세포(G1, G1-1)보다 감염된 섬유모세포(G2, G2-1)에서 반응이 증가하였고, 간흡충 분비배설 물질을 첨가하면 G1-1은 반응이 증가하고, G2-1은 반응이 감소하는 것이 관찰되었다. 이상의 결과로 간흡충에 감염된 흰쥐 담관의 섬유모세포는 세포질돌기들이 매우 발달하며, actin단백과 sialic acid가 증가하여 세포변형을 초래하게 된다. 또한 간흡충 감염으로 손상된 담관의 섬유모세포로 구성된 결합조직은 정상으로 회복되지 않으며, 세포질 부피 및 세포질돌기의 증가는 이루어지지만 간흡충 대사물질의 영향으로 섬유모세포의 분열 및 성장 속도가 억제되는 것으로 확인되었다. 이 결과 간흡충 감염으로 손상된 숙주의 담관 결합조직과 섬유모세포들은 간흡충 대사물질에 의하여 변형을 일으키고, 세포 활성 및 증식이 저하되므로 팽대된 담관은 간흡충이 사멸된다 하더라도 원상회복이 불가능할 것으로 생각된다.

총담관결찰후 집토끼 Paneth세포의 변화에 대한 면역전자현미경적 연구 (Immunoelectron Microscopic Study on the Paneth Cell of Rabbit after the Common Bile Duct Ligation)

  • Lysozyme has been reported to be present in the secretory granules of the Paneth cell, and lysozyme immunoreactivity has been detected by immunogold method in Paneth cells of the intestine of human, mouse and rat. The present study was aimed at clarifying the intracellular distribution and changes of the lysozyme immunoreactivity in rabbit Paneth cell after common bile duct ligation of rabbit, using the electron microscope immunogold technique. Healthy adult rabbits weighing about 2kg body weight were divided into normal and bile duct ligated groups. Common bile duct ligation was performed aseptically under ether anesthesia. Experimental animals were sacrificed on the 1st, the 3rd, the 5th, the 7th and the 14th day after the operation. Mucosal specimens from the intestinal gland of ileum were fixed in 2.5% glutaraldehyde-1.5% paraformaldehyde, followed by 1% osmium tetroxide, embedded in araldite mixture, cut with LKB-V ultratome. Ultrathin sections were placed on parlodion coated nickel grids (200mesh). The section-bearing grids were floated upside down on the added substance in a moist chamber at room temperature except for the primary antibody step, which was at $4^{\circ}C$. Sections were etched with a saturated solution of sodium m-periodate for 60min. After etching, sections were pretreated with 0.02M tris buffered saline (TBS), pH 8.4, with 1% bovine serum albumin (BSA, Sigma) for 60min, then treated polyclonal rabbit anti-human lysozyme (Dakipatts) diluted 1 : 50 in TBS with 0.1% BSA for 20hr. Subsequently, grids were incubated 60min in biotinylated goat anti rabbit IgG (Amersham) diluted 1 : 100 in TBS with 0.1% BSA. After this, sections were incubated 60min on streptavidin gold G10 (Amersham) diluted 1 : 50 in TBS with 0.1% BSA. After each step, the grids were briefly rinsed with TBS with 0.1% BSA. After the strepavidin gold step, the sections were jet washed with distilled water. Counterstain of the sections performed by uranyl acetate and lead citrate, and observed with JEM 100 CX II electron microscope. Observed results were as follow; 1. Secretory granules of mouse Paneth cells have a lysozyme immunoreactivity and also eosinophil leucocyte of rabbit applied for the positive-control stain, are well labeld with gold particles. 2. Normal rabbit Paneth cells have a lysozyme immunoreactivity restricted on the secretory granules. 3. Amount lysosomes containing myelin figures in the Paneth cells were significantly increased from 5th day after the common bile duct ligation. 4. Immunoreactivity of Paneth cell secretory granules were more activated on the 3rd day after the common bile duct ligation as compared with those of the normal animal. But the lysozyme immunoreactivity were decreased from the 5th day after the common bile duct ligation. 5. Considering the above finding, lysozyme contained Paneth cell are affected following of common bile duct ligation, whereas lysosomes containing myelin-figure do not exhibit any immunoreactive relationship with those of secretory granules.

Tc-99m DISIDA 간담도 신티그라피에서 간 실질의 분절형 배설지연의 임상적 의의 (Clinical Significance of Segmental Parenchymal Excretion Delay on Tc-99m DISIDA Hepatobiliary Scan)

  • 목적: Tc-99m DISIDA 신티그라피에서 간 실질에 분절형으로 배설지연을 보이는 소견은 분절형 간내담관폐쇄의 소견으로 보고되고 있으나, 이러한 소견의 분절형 간내 담관폐쇄에서의 진단적 가치는 알려져 있지 않다. 본 연구에서는 간 실질 분절형 배설지연 소견의 분절형 간내 담관폐쇄 진단에서의 양성예측도와, 방사선학적 검사와 비교한 진단적 가치를 알고자하였다. 대상 및 방법: Tc-99m DISIDA 신티그라피에서 간 실질에 분절형 배설지연 소견을 보인 43명의 환자(48개 영상)를 대상으로하여 그 원인을 분석하고, 1개월 이내에 시행된 CT와 US와 비교하였다. 결과: 31예는 ERC나 PTC에 의해, 13예는 임상적 경과 관찰에 의해 분절형 담도 폐쇄가 확진되었고, 4예는 원인이 규명되지 않았다. 분절형 담관폐쇄에 대한 Tc-99m DISIDA 간담도 신티그라피의 양성예측도는 92% (44/48)이었다. US의 28%(5/ 18), CT의 13% (5/38)는 정상 소견이고, US의 17% (3/18), CT의 18% (7/38)에서는 담관 확장 소견만이 있었고, 원인이 되는 병변을 알 수 없었다. 결론: Tc-99m DISIDA 신티그라피에서 간 실질에 분절형 배설지연을 보이는 소견은 분절형 간내 담관폐쇄 진단에 높은 양성 예측도를 가진다. Tc-99m DISIDA 신티그라피는 특히 US나 CT가 정상인 경우에 분절형 담관폐쇄 진단에 선별검사로서 이용되어질 수 있을 것으로 생각된다. Tc-99m DISIDA 신티그라피의 분절형 담도관쇄 진단에서의 진단적 가치에 대한 전향적인 연구가 요구된다.

닭 쓸개 methanol 추출물의 생리기능적 특성에 관한 연구 (Studies on Physiological and Functional Properties of Methanol Extract from Chicken Bile)

  • 닭 쓸개의 생리기능적 특성을 연구하기 위하여 TLC 및 HPLC를 이용하여 쓸개내 성분을 정성 및 정량 분석을 하였고, 항균활성, 전자공여능, 아질산염 소거능 및 ACE 저해능을 비교 검토하였다. TLC 및 HPLC에 의해서 분석한 결과 닭 쓸개의 담즙산 중에는 taurochenodeoxycholic acid와 taurocholic acid로 추정되는 성분이 검출되었으며, 이들의 함량은 taurochenodeoxycholic acid 가 5,893mg/100g, taurocholic acid는 385mg/100g이었다. 이들 추출물은 Bacillus cereus에 대하여 강한 항균활성을 보였으며 Salmonella typhimurium, Staphylococcus acreus 및 Vibrio parahaemolyticus에 대해서도 항균 활성을 나타내었다. 그러나 Escherichia coli에 대해서는 항균활성을 보이지 않았다. 쓸개의 냉동 건조물 및 methanol 추출물의 전자공여능은 시료의 농도가 증가할수록 유의적으로 증가하였다. 또한 pH 1,2에서 측정한 아질산염 소거작용에 있어서도 농도가 높을수록 유의적으로 증가하였으며, methanol 추출물이 냉동 건조물 보다 높게 나타났다. Angiotensin I-converting enzyme(ACE) 저해 작용에서도 methanol 추출물과 냉동건조물 모두에서 나타났으며, 첨가량을 증가시켰을 때 methanol 추출물의 작용이 냉동건조물의 경우보다 상승폭이 큰 것으로 확인되었다. 이상의 결과로부터 닭 쓸개는 항균활성, 전자공여능 및 아질산염 소거능 그리고 ACE 저해능 등의 다양한 생리기능적 특성을 지니고 있어 기능성 식품 등의 소재로서의 활용을 기대할 수 있을 것으로 사료된다.

Successful Endoscopic Treatment of Hepatic Duct Confluence Injury after Blunt Abdominal Trauma: Case Report

  • Hepatic duct confluence injury, which is developed by blunt abdominal trauma, is rare. Conventionally, bile duct injury was treated by surgical intervention. In recent decades, however, there had been an increase in radiologic or endoscopic intervention to treat bile duct injury. In a hemodynamically stable patient, endoscopic intervention is considered as the first-line treatment for bile duct injury. A 40 year-old man was transferred to the emergency department of ${\bigcirc}{\bigcirc}$ trauma center after multiple blunt injuries. Contrast-enhanced abdominal computed tomography performed in another hospital showed a liver laceration with active arterial bleeding, fracture of the sacrum and left inferior pubic ramus, and intraperitoneal bladder rupture. The patient presented with hemorrhagic shock because of intra-peritoneal hemorrhage. After resuscitation, angiographic intervention was performed. After angiographic embolization of the liver laceration, emergency laparotomy was performed to repair the bladder injury. However, there was no evidence of bile duct injury on initial laparotomy. On post-trauma day (PTD) 4, the color of intra-abdominal drainage of the patient changed to a greenish hue; bile leakage was revealed on magnetic resonance cholangiopancreatography and endoscopic retrograde cholangiopancreatography (ERCP). Bile leakage was detected near the hepatic duct confluence; therefore, a biliary stent was placed into the left hepatic duct. On PTD 37, contrast leakage was still detected but both hepatic ducts were delineated on the second ERCP. Stents were placed into the right and left hepatic ducts. On PTD 71, a third ERCP revealed no contrast leakage; therefore, all stents were removed after 2 weeks (PTD 85). ERCP and biliary stenting could be effective treatment options for hemodynamically stable patients after blunt trauma.

간외 담도암 고선량률 관내근접방사선치료 시 몬테카를로 시뮬레이션을 통한 주변장기의 선량평가 연구 (Study of Radiation dose Evaluation using Monte Carlo Simulation while Treating Extrahepatic Bile Duct Cancer with High Dose Rate Intraluminal Brachytherapy)

  • MCNPX를 통하여 계산한 상대선량과 고체팬텀과 전리함을 이용하여 측정한 상대선량을 비교하여 몬테카를로 시뮬레이션의 정확성을 평가하였다. 그리고 간외 담도암 관내근접방사선치료를 몬테카를로 시뮬레이션에 적용하기 위해 192Ir 밀봉방사성선원을 모사하였고, 한국 성인남성 표준인을 기초로 하는 KMIRD형 팬텀을 이용하여 담도 및 주변 장기를 제작하였다. 간외 담도암 관내근접방사선치료를 MCNPX를 이용하여 담도 주변 정상장기의 비유효에너지와 초기방사능을 1 Ci로 설정하여 흡수선량을 산정하였다. 몬테카를로 시뮬레이션의 정확성 평가에서 상대선량 차이가 가장 많은 지점이 1.96%로 MCNPX에서 제시한 상대오차 2%를 만족하는 것으로 나타났다. 또한, 담도 주변 정상장기의 비유효에너지 및 흡수선량은 담도와비교적 인접한 위치에 있는 우측신장, 간, 췌장, 횡행결장, 척수, 위장, 소장이 높았고, 담도와의 거리가 떨어져 있는 장기들인 좌측신장, 비장, 상행결장, 하행결장, S상결장이 낮게 나타났다.

  • That different mechanisms are involved in the secretory processes by the liver and the kidney of various dyes has been indicated by Sporter (1959), Kim and Hong (1963). Andrews (1958). suggested that a striking difference in the dye-secretory mechanism existed even in the same organ from species to species. Hence, the attempt has been made to study in the rabbit the secretory processes by the live. and the kidney of either phenol red (PSP), bromsulfalein (BSP) or green in the presence of Na-acetate, Na-taurocholate, P-Aminohippurate (PAH) or Benemid. In 37 rabbits, weighing about 2kg., anesthetized with ether, a dye was administered in such 8 manner that the plasma concentration was kept at a relatively constant level throughout the whole experimental period. Hepatic bile sad urine samples were quantitatively collected through the canulae which were previously inserted into the common bile duct (with the cystic duct ligated) and the urinary bladder, respectively, while arterial samples were taken from a femoral artery. After 50 min from the onset of dye administration, these samples were obtained every 10 mit for a period of 40 min. This was followed by the administration of either Na-acetate, Na-tauro-cholate, PAH or Benemid with a repetition of the same sample collecting procedures just stated. The results may be summarized as follows: 1) Na·acetate augmented urinary clearance of PSP by nearly 300 per cent, but lowered urinary BSP clearance by about 50 per cent. It enhanced biliary BSP clearance by 40% and had no effect on biliary psp clearance. 2) Na-taurocholate lowered biliary and urinary clearance of PSP by 10 per cent and 30 per cent respectively, and had no effect on both biliary and urinary clearance of BSP. 3) PAH lowered both biliary and urinary excretion of BSP and PSP, while it lowered the biliary excretion of indocyanine green which was excreted only in the bile. 4) Benemid suppressed BSP excretion by the liver and the kidney. 5) raper chromatographic analysis of PSP and of BSP in the bile and urine samples gave the following results: a) PSP Ivas excreted in the urine and bile only in free forms, and no modification in the excretory pattern was brought about by Na-taurocholate. b) BSP was excreted in the urine in 4 different conjugated froms and in the bile in both 3 different conjugated forms and in a free form. Na-taurocholate modified the excretory pattern of the urinary BSP.

간내 담도가 확장된 담관낭의 수술 후 간내 담도의 변화 (Postoperative Change of Intrahepatic Bile Duct Dilatation in Choledochal Cyst)

  • Choledochal cyst is a congenital dilatation of the bile duct. Intrahepatic bile duct dilatation of type IVa by Todani's classification at the time of diagnosis resolved spontaneously after cyst excision and hepaticojejunostomy in many cases. It should be distinguished from the true cystic dilatation of the intrahepatic ducts, which tends to persist, albeit after some regression. We therefore studied postoperative intrahepatic duct dilatation changes in choledochal cyst. A total of seventy-six choledochal cysts were managed at the Division of Pediatric Surgery, Department of Surgery, Samsung Medical Center from May 1995 to December 2005. The ratio of males to females was 1:2.8. Preoperative radiologic diagnosis by Todani's classification was Type I (n=52, 68.4 %), II (n=1, 1.3 %), IVa (n=23, 30.3 %). Among fifty-five patients with intrahepatic bile duct dilatation we were able to follow up forty-eight by ultrasonography. Twenty-two patients were type IVa, and twenty-six patients were type I and showed intrahepatic duct dilatation. Mean follow-up duration was 35.3 months (9~105 months). Complete regression of dilated intrahepatic duct was observed in fifteen patients of type IVa and twenty-four patients of type I. Incomplete regression of dilated intrahepatic duct was observed in six patients in type IVa and two patients in type I. Only one patient in type IVa showed no change in ductal dilatation during a follow-up period of 15 months. We conclude that true type IVa is much less frequent than what was diagnosed preoperatively by imaging study. Therefore in type IVa patients who are diagnosed preoperatively the decision to perform liver resection should be carefully considered. Postoperative long term follow up of choledochal cyst with intrahepatic bile duct dilation is needed.
