A Clinical Study of Pediatric Inguinal Hernia Before and After Using Ultrasonography for Diagnosis (소아 서혜부탈장 진단에 초음파검사 실시 후 임상 양상의 변화)

  • The purpose of this retrospective study was to evaluate the effects of diagnostic sonography in pediatric patients with inguinal hernias. The patients were classified into two groups. Group A included the patients who had been operated upon for inguinal hernia in 1980's, when diagnostic sonography was not available. Group B included the patients, operated upon for inguinal hernia from 2001 to 2002, when inguinal sonography was employed to detect potential bilateral hernias. The age distribution, sex ratio, laterality, bilaterality, and concomitant symptoms were compared between group A and group B. There were 296 cases in group A and 377 cases in group B. The prevalent age group was from 1 to 5 years. There was no difference in age group distribution between both groups. The male to female ratio was 5.3:1 in group A and 3.5:1 in group B. The ratio of unilateral to bilateral hernia was 5:1 in group A and 3:1 in group B. In cases with a unilateral hernia, the ratio of right to left was 1.5:1 in group A and 1.8:1 in group B. In cases with bilateral hernia, the simultaneous bilateral hernia was 33 cases (67.4 %) in group A and 75 cases (80.6 %) in group B. The sequential bilateral hernia was 16 cases (32.7 %) in group A and 18 cases (19.4 %) in group B. Although the ratio of bilateral hernia was increased in group B, the portion of the sequential bilateral hernia was significantly decreased in group B. In conclusion, there were no differences in the age distribution and the laterality between group A and B. The ratio of female patients and the incidence of bilateral hernia were increased in group B even though the portion of the sequential bilateral hernia was decreased. This result shows that the preoperative inguinal sonography in unilateral hernia with potential bilateral hernia is useful in early detection of the sequential contralateral hernia.

  • The purpose of this study was to investigate the effect of Isotonic Training and Isotonic isometric complex Training to Knee joint Isokinetic muscle function in the high-school Taekwondo athlete. This subjects were randomly divided into two groups; a Isotonic Training group (n=7) Isotonic isometric complex Training group (n=7), Taekwondo athletes share each, 12 weeks between executes a motion. peak torque, H/Q ratio, bilateral muscular ratio muscular endurance of knee joint measured. For this study, two-way ANOVA with repeated measure were used. The result of this study presents between groups was not plain difference expressed. H/Q ratio, bilateral muscular ratio, muscular endurance was plain difference expressed after 12 weeks point of time (p>0.05) and peak torque was plain difference expressed (p<0.05). Conclusion each group did not appear all but one (peak torque), but point of view exercise was effect H/Q ratio, bilateral muscular ratio, muscular endurance, peak torque plain difference expressed.

  • Since the telemanipulator consisting of high reduction ratio gear joints has a slow dynamics comparing with the master manipulator, its control signal is likely to be saturated resulting in poor tracking performance and deteriorated stability. This paper proposes a bilateral control scheme of a telemanipulator having high reduction ratio, which can compensate the control input saturation. The experimental results show that the proposed control scheme has excellent performances.

  • In this paper, frame rate up conversion (FRUC) algorithm considering the direction and magnitude of identical motion vectors is proposed. extended bilateral motion estimation (EBME) has higher complexity than bilateral motion estimation (BME). By using average magnitude of motion vector with x and y direction respectively, dynamic frame and static frame are decided. We reduce complexity to decide EBME. also, After we compare the direction and magnitude of identical motion vectors, We reduce complexity to decide motion vector smoothing(MVS). Experimental results show that this proposed algorithm has fast computation and better peak singnal to noise ratio(PSNR) results compared with EBME.

  • A heavy duty power manipulator consisting of high reduction ratio joints is usually used in heavy duty tasks. When the heavy duty power manipulator is used as the slave manipulator in the teleoperated bilateral control system, the position control performance of the slave manipulator and the system stability tend to deteriorate due to the windup phenomenon caused by actuator saturation. Bilateral Vocal Cord Paralysis : A 10-year Review of 42 Patients (양측성대마비의 10년간 경험 분석)

  • The clinical investigation was done in the 42 patients with bilateral vocal cord palsy who visited the otorhinolaryngologic department of the Yongdong Severance Hospital during the 10-year period between August 1986 to August 1996. On the sex and age distribution, the ratio of male to female patients was 2.8 : 1 and the age was evenly distributed and average was 46 years old. Of their chief complaints, dyspnea was the most common symptom. Among the position of the paralyzed vocal cords, paramedian position was most common. The most common causes of the bilateral vocal cord palsy was idiopathic Other causes include iatrogenic, prolonged intubation, head & neck trauma, brain tumor, Myasthenia Gravis, and mediastinitis. Our treament results were as follows. Recovery rate of idiopathic bilateral vocal cord palsy was 77.7% and recovery period after bilateral vocal cord palsy was shortened remarkedly after use with steroid. We performed laser arytenoidectomy in patients with irreversible idiopathic vocal cord palsy, neural injury, and cricoarytenoid joint fixation. Decannualtion was possible to be carried out in 86% of the patients and none of complication except for 1 case of aspiration developed. Thus we concluded that it was meaningful surgical treatment of bilateral vocal cord palsy.

CLINICAL EVALUATION OF BILATERAL INTERNAL DERANGEMENT OF TEMPOROMANDIBULAR JOINT BY MAGNETIC RESONANCE IMAGING (MRI를 이용한 양측성 약관절 내장증의 임상적 평가)

  • This is a retrospective study of 31 patients, who were diagnosed as bilateral TMJ internal derangement in consequence of magnetic resonance imaging (MRI) of the temporomandibular joint (TMJ) at Department of Dentistry, Ajou University Hospital, and who were compared clinical findings with radiological findings. The results obtained were as follows. 1. The average of the patients of bilateral TMJ internal derangement was 27.3 years with a range from 12 to 74 years, and 93.6% of the patients were yonger than 40 years of age. The male-female ratio was 1 : 6.8, and the females of $20{\sim}29$ years were most common in 35.5%. 2. In the patients of bilateral TMJ internal derangement, the patients who complained clinical symptoms bilaterally were 38.7% and the patients who complained clinical symptoms untilaterally were 61.3%. 3. The most common disc position of bilateral TMJ internal derangement was anterior disc displacement without reduction on both sides in 38.7%. The most common disc configuration of bilateral TMJ internal derangement was biconcave on both sides and amorphous on both sides in closed mouth, and was amorphous on both sides in open mouth. 4. In the patients of bilateral TMJ internal derangement, the joints which really showed clinical symptoms were most common in anterior displacement without reduction of 48.9%.

  • Bilateral Variable-Ratio chopper system performs Step-down chopper and Step-up chopper operations automatically every operating period of the chopper. The Ratio between Step-down chopper and Step-up chopper operations is determined by a Tine- ratio and the circuit elements. In this paper, we suggest that this chopper system can operate variable speed control of the electric motorcar when it is applied to the electric motorcar.

A Bilateral Teleoperation Control Scheme for 2-DOF Manipulators with High Reduction Ratio Joints

  • Since the dynamics of the slave manipulator with high reduction ratio joints is likely to be much slower than that of the master manipulator, the control input the slave manipulator is so frequently saturated. This paper proposes a bilateral teleoperation control scheme for 2-DOF manipulators with high reduction ratio joints, which can effectively compensate the control input saturation. In the proposed scheme, the controllers of the slave manipulator are designed with an anti-windup feature and forces caused by the saturation are reflected to the operator holding the operating handle of the master manipulator. When the control input of the slave manipulator is saturated, the master manipulator moves slowly file to tile reflected forces. In this way, the position tracking performance of the slave manipulator with high reduction ratio joints can be enhanced regardless of saturation. The proposed scheme is shown to give excellent position tracking performance through a series of experiments.

A Clinical Study of Bilateral Spontaneous Pneumothorax (양측성 자연기흉의 임상적 고찰)

  • In this study, 26 cases of bilateral spontaneous pneumothorax experienced at the department of thoracic and cardiovascular surgery, Chungnam National University Hospital during from 1985 to August 1989 were analyzed retrospectively. The results were as follows; 1. The incidence of bilateral spontaneous pneumothorax was 13.5 % and sex ratio was 7.7:1 with male preponderance. 2. Among the nonsimultaneously occurring cases of bilateral pneumothorax, 12 patients [75 %] were developed at contralateral side within a year. 3. The most patients [65.4%] belonged to the age group between 15 and 25 year-old, and among 57 male patients suffered spontaneous pneumothorax in the same age group, 16 cases[28%] developed bilaterally. 4. The etiologic factors were as follows; blebs or bullae; 65.4 %, tuberculosis; 15.4 %, unknown; 19.2 %. 5. In the method of treatment, 15 patients were treated by closed tube thoracotomy and underwater-seal drainage only, 10 patients were treated by open thoracotomy. One patient died of respiratory failure due to severe destructed lung.
