Background: Salivary gland tumors (SGT) are one of the most complex human neoplasms, demonstrating variations in their clinicopathological profile related to racial and geographic differences. Few studies with large samples have been reported in Iran. We here investigated a large group of patients in southern Iran. Materials and Methods: In this retrospective study, all cases of primary epithelial salivary gland tumors, which had been recorded in a 5 years period from 2005-2009, were enrolled. Clinical data such as histopathologic type and site of the lesion as well as patients' age and gender were analyzed. Results: Data of 366 cases of SGTs were recorded. Pleomorphic adenoma (80.2%) and adenoid cystic carcinoma (46.6%) were the most common benign and malignant neoplasms. Male to female ratio (M/F) and the mean age of patients were 1:1.05 and 37.7 for benign tumors while they were 1.2:1 and 50.6 for malignant tumors, respectively. Parotid and minor salivary glands were involved more frequently. Conclusions: Although the salivary gland tumours encountered were similar in most of their characteristics to those reported in other countries, some differences such as relative frequency and age and gender prevalence were discovered. These findings should help surgeons and pathologists for more accurate diagnosis, management and treatment.
The authors reviewed 95 cases(46 men and 49 women) of the orbital tumors diagnosed histopathologically at the Department of Ophthalmology, Yeungnam University Hospital from March 1984 through August 1994. Seventy-five cases of benign tumors were found evenly in all decades, but twenty cases of malignant tumors were noticed more frequently in 1st, 6th, and 7th decades. The frequency of benign orbital tumors was in this order; dermolipoma(21%, 20 cases), dermoid cyst(11.6%, 11 cases), mucocele(8.4%, 8 cases), lipoma(7.4%, 7 cases) and pleomorphic adenoma(5.3%, 5 cases). Of malignant orbital tumors, the frequency was in this order; retinoblastoma(3 cases), malignant melanoma(2 cases), sebaceous carcinoma(2 cases), and maxillary sinus carcinoma(2 cases). Malignant orbital tumors of 8 expired patients were revealed as secondary or metastatic tumors.
H$\"{u}$rthle cell neoplasm of the thyroid gland is an uncommon, but potentially malignant lesion. However, in many instances, the malignant potential of the H$\"{u}$rthle cell neoplasm is very difficult to judge histologically. For this reason, the biologic behavior of this tumor and its optimal treatment have come under considerable debate in recent years. In order to review the clinicopathologic features of the H$\"{u}$rthle cell neoplasm and to determine its optimal treatment modalities, we studied 26 patients with path logical proof of H$\"{u}$rthle cell tumor from January 1987 to September 1997. We also performed an immunohistochemical study using the monoclonal antibodies against antigen CD34 for the angiogenic activity of this tumor and evaluated the differences of microvessel density(MVD) between benign and malignant tumors. The age of the patients ranged from 1 to 71 years with a mean of 44.2 years. There were 6 males and 20 females(M : F= 1 : 3.3). The accuracies of fine needle aspiration biopsy and frozen section were very low; 6.3% and 34.8%, respectively. There were 20 benign tumors and 6 malignant tumors(23.1%). All the malignant tumors were microinvasive(intermediate) type which had minimal capsular invasion and most of them(5 cases) were diagnosed postoperatively. Any specific clinicopathologic differences were not seen between benign and intermediate groups. Most of the cases had conservative surgeries(15 ipsilateral lobectomy-isthmusectomy, 7 subtotal thyroidectomy) while total thyroidectomy was performed in 4 cases. Of the cases with malignant tumor, 2 had ipsilateral lobectomy-isthmusectomy, 3 had subtotal thyroidectomy and the remaining 1 had total thyroidectomy. Mean size of the tumors was 3.0 cm(0.1- 8.5 cm) in the greatest diameter and multiple tumors were seen in 6 cases(23.1 %). During the follow-up period, only one recurrence(3.8%) of benign tumor occurred but distant metastasis or cause-specific death was seen in the benign or intermediate groups. Mean MVDs of the benign(n=13) and intermediate(n=6) groups were $121.7{\pm}35.3$ and $114.3{\pm}31.7$, respectively and there was no statistical significance between them. In conclusion, because of the low accuracies of fine needle aspiration biopsy and frozen section for the H$\"{u}$rthle cell neoplasm, the extent of surgery could be individualized based on permanent pathologic examination; Conservative surgery would be adequate for patients with benign or intermediate H$\"{u}$rthle cell neoplasm and total or near-total thyroidectomy for those with definite malignancy.
Background: Tumors of the heart are uncommon. The aim of this study is to review our clinical experience and outcome of surgical treatment of cardiac neoplasm. Material and Method: From March 1990 to December 2005, 35 patients(14 males and 21 females) with mean age of 52.4 years underwent surgical treatment of cardiac neoplasm. The clinical and pathologic data were analyzed retrospectively. Surgical treatment consisted in complete resection of the tumor in all cases but 1 patient who was left ventricular fibroma received biopsy only. Result: Thirty cases were benign and five cases were malignant tumor. Benign tumors were myxoma(29 cases) and fibroma(1 case). Five malignant tumors were osteosarcoma, hepatocellular carcinoma, renal cell cancer, yolk sac tumor, and unclassified myxoid spindle cell type sarcoma. There were no operative mortality in benign cases and twenty seven cases of myxoma were followed up for 8 months to 15 years without recurrence. But four patients of malignant tumor were expired within six months after operation. Conclusion: Left atrial myxomas are most common benign neoplasm. Surgical treatment is effective for the benign cardiac tumors but prognosis is poor in patients with malignant cardiac tumors.
Objectives: The salivary gland tumor shows heterogeneity in histologic patterns and biological behavior. The aim of this study is to elucidate the relationships between apoptosis and expressions of apoptosis-related factors(bcl-2, bax, M30), p53 and MIB-1 in the salivary gland tumors. Methods: Immunohistochemical stains for apoptosis-related factors, p53 and MIB-1 and TUNEL study for apoptosis were performed in 46 cases of salivary gland tumors 02 benign and 34 malignant). Results: Twenty(43.5%) of 46 cases showed positive reaction for apoptosis, and the expression rates of bcl-2, bax, M30, p53 and MIB-1 were 85.3%, 68.8%, 65.9%, 39.1% and 26.1%, respectively. A significant difference between benign and malignant tumors was only noted in MIB-1 expression(p=0.0167). In malignant tumors, apoptosis showed no significant relationships to expressions of apoptosis-related factors. There were inverse relationships between p53 and bcl-2 expression(p=0.0375), and between M30 and MIB-1 expressions(p=0.0379). No significant differences of apoptosis, bcl-2, bax, M30, p53 and MIB-1 expression rates according to the tumor size, lymph node status, recurrence and survival were found. Conclusion: In the development of benign and malignant salivary gland tumors, apoptosis might be associated, however, apoptosis and expressions of apoptosis-related factors seemed to be not reliable prognostic factors in malignant salivary gland tumors.
Many varieties of space-occupying lesions can occur in the mediastinum. During the twenty-three-year period 1966-1988, we had experienced the 151 cases of primary mediastinal tumors and cysts at the Thoracic k Cardiovascular Dept., CUMC. The most common histologic types were thymomas and germ cell tumors, followed by neurogenic tumors and lymphomas. Most non-neoplastic lesions were granulomatous lesions and cysts. The mediastinal tumors were often asymptomatic, the malignant tumors in 35.4%, the benign tumors and cysts in 72.8% of the cases. The most common symptom was pain, which occurred in one-fourth of the patients. The most useful diagnostic method was X-ray examination of the chest. However, a final diagnosis could usually be made only at operation. Nine malignant tumors[30.0%] were excised radically, 17[56.7%] palliatively and 4[13.3%] were only biopsied. Almost all benign tumors except tuberculous lymphadenitis were radically excised. Fourteen patients received postoperative radiation therapy and 17 received chemotherapy. The postoperative complications were developed in 15[11.7%] and hospital mortality was 0.8%.
Fifty-five patients with primary mediastinal tumors and cysts which were seen at T.S. Dept., HYUH, were analyzed clinically, histologically, in an incidence of anatomic location and therapeutic results. The results were summarized as follows; 1] The ages of the patients in this series ranged from 15 months to 79 years with the highest incidence in the age group of third decade, and there were no sex distribution[M:F=0.96]. 2] The most frequently encountered tumors were teratodermoid tumors[29%] followed by neurogenic tumors[22%], thymomas[15%] and benign cysts[11%] in decreasing order of frequency. 3] Based on the subdivision of the mediastinum, 62% of the tumors were in the antero-superior mediastinum, 7% in the middle mediastinum and 31% in the posterior mediastinum. 4] The most frequent symptom was chest pain and others were dyspnea, cough, chest tightness and dysphagia. Asymptomatic patients were 29%. 5] Benign tumors and cysts were 71% and malignant tumors were 29%. 6] The successful removal was possible in all the benign mediastinal tumors and cysts[39 cases] and partial removal or biopsy was performed in the 12 cases among 16 cases of malignant mediastinal tumors. 7] Postoperative complications were bleeding, chylothorax, vocal cord paralysis, wound infection and hypothyroidism. 8] The most frequent mediastinal tumor in the West is neurogenic tumor but is teratoma in Korea.
Purpose: We evaluated the standard uptake value (SUV) of F-18 FDG at PET/CT for differentiation of benign from malignant tumor in primary musculoskeletal tumors. Materials and Methods: Forty-six tumors (11 benign and 12 malignant soft tissue tumors, 9 benign and 14 malignant bone tumors) were examined with F-18 FDG PET/CT (Discovery ST, GE) prior to tissue diagnosis. The maxSUV(maximum value of SUV) were calculated and compared between benign and malignant lesions. The lesion analysis was based on the transverse whole body image. The maxSUV with cutoff of 4.1 was used in distinguishing benign from malignant soft tissue tumor and 3.05 was used in bone tumor by ROC curve. Results: There was a statistically significant difference in maxSUV between benign (n=11; maxSUV $3.4{\pm}3.2$) and malignant (n=12; maxSUV $14.8{\pm}12.2$) lesions in soft tissue tumor (p=0.001). Between benign bone tumor (n=9; maxSUV $5.4{\pm}4.0$) and malignant bone tumor (n=14; maxSUV $7.3{\pm}3.2$), there was not a significant difference in maxSUV. The sensitivity and specificity for differentiating malignant from benign soft tissue tumor was 83% and 91%, respectively. There were four false positive malignant bone tumor cases to include fibrous dysplasia, Langerhans-cell histiocytosis (n=2) and osteoid osteoma. Also, one false positive case of malignant soft tissue tumor was nodular fasciitis. Conclusion: The maxSUV was useful for differentiation of benign from malignant lesion in primary soft tissue tumors. In bone tumor, the low maxSUV correlated well with benign lesions but high maxSUV did not always mean malignancy.
For the purpose of evaluation of clinical characteristics and histopathological properties in mediastinal tumors, 130 patients with mediastinal tumors treated during the period from 1958 to 1982 were reviewed. Cancers of unidentified primary site of origin, nonneoplastic lesions and the cases with clinical diagnosis only were excluded from this report. There were 69 males and 61 females, and their ages ranged from 2 months to 66 years, with the average age of 31.3 years. 19.2% of patients were younger than 15 years of age. The most frequently encountered tumors were teratodermoids, followed by neurogenic tumors, thymic tumors, benign cysts, malignant lymphomas and miscellaneous tumors in order of decreasing frequency. 75.4% of the mediastinal tumors were benign and 24.6% were malignant. 16.6% of the patients were asymptomatic at admission. There were 2 postoperative deaths with 1.7% of operative mortality rate, and 2 other deaths who were not operated upon, showing overall hospital mortality rate of 3.1%. Postoperative follow up was possible in 84 cases, and there were 4 late deaths.
Thoracoscopy, mediastinoscopy and/or mediastinotomy and explo-thoracotomy were performed and confirmed diagnosis of the 26 cases of the mediastinal tumors, those were admitted and treated at the Department of Thoracic and Cardiovascular surgery, Chonbuk National University Hospital, from June,1976 to September, 1986. We experienced 7 thoracoscopies, 7 mediastinoscopies &/or mediastinotomies, and 12 explo-thoracotomies. The most common histologic type was teratoma and lymphoma [6 cases]. Of the 26 cases of the histologically confirmed mediastinal tumor, 19 were benign tumors and 7 were malignants. 19 cases of benign tumors were surgically resected with good result but one case of the surgically resected malignant thymoma was recurred 4 months later. 6 cases of malignant lymphoma and one case of recurred malignant thymoma were treated with chemo-and radiation therapy. Thoracoscopy and mediastinoscopy with tissue biopsy were simple in procedure and had a tissue diagnosis with obtaining the pathologic tissue in a acceptable complication rate and reduced exploratory thoracotomy.
