Prevention of Occupational Diseases in Turkey: Deriving Lessons From Journey of Surveillance

  • Introduction: To prevent and manage the societal and economic burden of occupational diseases (ODs), countries should develop strong prevention policies, health surveillance and registry systems. This study aims to contribute to the improvement of OD surveillance at national level as well as to identify priority actions in Turkey. Methods: The history and current status of occupational health studies were considered from the perspective of OD surveillance. Interpretative research was done through literature review on occupational health at national, regional and international level. Analyses were focused on countries' experiences in policy development and practice, roles and responsibilities of institutions, multidisciplinary and intersectoral collaboration. OD surveillance models of Turkey, Belgium and the Netherlands were examined through exchange visits. Face-to-face interviews were conducted to explore the peculiarities of legislative and institutional structures, the best and worst practices, and approach principles. Results: Some countries are more focused on exploring OD trends through effective and cost-efficient researches, with particular attention to new and emerging ODs. Other countries try to reach every single case of OD for compensation and rehabilitation. Each practice has advantages and shortcomings, but they are not mutually exclusive, and thus an effective combination is possible. Conclusion: Effective surveillance and registry approaches play a key role in the prevention of ODs. A well-designed system enables monitoring and assessment of OD prevalence and trends, and adoption of preventive measures while improving the effectiveness of redressing and compensation. A robust surveillance does not only provide protection of workers' health but also advances prevention of economic losses.

Aristolochia manshuriensis, Aristolochia fang chi, which is the substitutions of Akebia quinata, Stephania tetrandra has being used in clinical. Aristolochia manshuriensis. Aristolochia fang chi were different with Akebia quinata. Stephania tetrandra in botany, and it have not been classified with medicines in Oriental medicine, That is, the aristolochic acid, not Herb medicines. causes CHN, So, Chinese Herb Nephropathy should be changed to Aristolochic acid Nephropathy.

Three-dimensional measurement of periodontal surface area for quantifying inflammatory burden

  • Purpose: Measurement of the root surface area (RSA) is important in periodontal treatment and for the evaluation of periodontal disease as a risk factor for systemic disease. The aim of this study was to measure the RSA at 6 mm below the cementoenamel junction (CEJ) using the Mimics software (Materialise, Leuven, Belgium). Methods: We obtained cone-beam computed tomography (CBCT) data from 33 patients who had visited the Department of Oral and Maxillofacial Radiology of Dankook University Dental Hospital. The patients comprised 17 men and 16 women aged from 20 to 35 years, with a mean age of 24.4 years. Only morphologically intact teeth were included in our data. Because the third molars of the maxilla and mandible have a high deformation rate and were absent in some participants, they were not included in our research material. Results: The CBCT data were reconstructed into 3-dimensional (3D) teeth models using the Mimics software, and the RSA at 6 mm below the CEJ was separated and measured using 3-Matic (Materialise). In total, 924 3D teeth models were created, and the area at 6 mm below the CEJ could be isolated in all the models. The area at 6 mm below the CEJ was measured in all teeth from the 33 patients and compared based on sex and position (maxilla vs. mandible). Conclusions: In this study, we demonstrated that it was feasible to generate 3D data and to evaluate RSA values using CBCT and the Mimics software. These results provide deeper insights into the relationship between periodontal inflammatory burden and systemic diseases.

  • The Korean spine geometry and property data for researchers were made by KISTI and Catholic Institute for Applied Anatomy. We took whole spine CT, X-Ray, BMD scan for making high resolution cross-sectional spine images using more 20 donated cadavers(60 - 80 years). Then we constructed 3-dimensional volume model using serial CT images by Mimics software. The major morphometric parameters of vertebrae were measured. Mechanical motion and property data were obtained by the same cadavers using the DEXA for BMD and the spine simulator. The Korean spine geometry and property data could be used for research and development of medical device.

Prevelance of Common YMDD Motif Mutations in Long Term Treated Chronic HBV Infections in a Turkish Population

  • In the current study we aimed to show the common YMDD motif mutations in viral polymerase gene in chronic hepatitis B patients during lamivudine and adefovir therapy. Forty-one serum samples obtained from chronic hepatitis B patients (24 male, 17 female; age range: 34-68 years) were included in the study. HBV-DNA was extracted from the peripheral blood of the patients using an extraction kit (Invisorb, Instant Spin DNA/RNA Virus Mini Kit, Germany). A line probe assay and direct sequencing analyses (INNO-LIPA HBV DR v2; INNOGENETICS N.V, Ghent, Belgium) were applied to determine target mutations of the viral polymerase gene in positive HBV-DNA samples. A total of 41 mutations located in 21 different codons were detected in the current results. In 17 (41.5%) patients various point mutations were detected leading to lamivudin, adefovir and/or combined drug resistance. Wild polymerase gene profiles were detected in 24 (58.5%) HBV positive patients of the current cohort. Eight of the 17 samples (19.5%) having rtM204V/I/A missense transition and/or transversion point mutations and resistance to lamivudin. Six of the the mutated samples (14.6%) having rtL180M missense transversion mutation and resistance to combined adefovir and lamivudin. Three of the mutated samples (7.5%) having rtG215H by the double base substituation and resistance to adefovir. Three of the mutated samples (7.5%) having codon rtL181W due to the missense transversion point mutations and showed resistance to combined adefovir and lamivudin. Unreported novel point mutations were detected in the different codons of polymerase gene region in the current HBV positive cohort fromTurkish population. The current results provide evidence that rtL180M and rtM204V/I/A mutations of HBV-DNA may be associated with a poor antiviral response and HBV chronicity during conventional therapy in Turkish patients.

Effects of Sulfate Ion Concentration in Nutrient Solution on the Growth and Quality of Artemisia mongolicar var. tenuifolia (배양액 내의 황산이온 농도가 참쑥의 생육과 품질에 미치는 영향)

  • This experiment was conducted to evaluate the effects of sulfate ion concentration in nutrient solution on the growth and qualify of Mongolian wormwood (Artemisia mongolica var. tenuifolia). Sulfate ion concentration was treated 0, 0.5, 1, 2 and 3mM using the modified nutrient solution composition for herb plants developed by European Vegetable R & D Center in Belgium. The growth of Mongolian wormwood was good at 3mM treatment and dry weight was best at 3mM treatment, Chlorophyll content increased with sulfate ion concentration. Mineral content did not show any significant difference among treatments. But Ca content in tissue markedly decreased at 3mM treatment. Sulfate ion uptake increased in proportion to sulfate ion concentration in nutreint solution, the higher sulfate ion concentration, the more uptake of sulfate ion by plant. At 1mM sulfate ion treatment, essential oil content was best, but the higher sulfate ion concentration resulted in decrease of essential oil content.

The Effect of Irrigation Concentration on the Growth and Fruit Quality of Sweet Pepper(Capsicum annuum L.) in Fertigation (관비재배에서 급액농도가 착색단고추의 생육과 품질에 미치는 영향)

  • Objective of this research was to investigate the effect of irrigation concentration on the growth and fruit quality of sweet pepper(Capsicum annuum L.) in fertigation. The sweet pepper was grown for 210 days with irrigation concentration of EC 0.5, 1.0, 1.5, 2.0, and 3.0 dSㆍ$m^{-1 }$ in fertigation nutrient solution developed by European Vegetable R & D Center, Belgium. The net $CO_2$ assimilation and transpiration rate were the highest in the treatment of 2.0 dSㆍ$m^{-1 }$. The pH in the soil was range of 5.63 ~6.03, the EC increased as the irrigation concentration was getting higher. The SPAD value also increased as the irrigation concentration was getting higher, N, P, K, Mg except Ca were highest in the treatment of EC 2.0 dSㆍ$m^{-1 }$. The growth was good in the treatment of EC 2.0 dSㆍm$m^{-1 }$. The fruit length, width, firmness, and pericarp thickness had no statistical differences among treatments, the fruit fresh weight and dry weight were good in the treatment of EC 2.0 dSㆍ$m^{-1 }$ the yield was good in the treatment of EC 1.5 dSㆍ$m^{-1 }$ and EC 2.0 dSㆍ$m^{-1 }$ The sugar contents was the highest in the treatment of EC 2.0 dSㆍ$m^{-1 }$ with 9.0$^{\circ}$Brix. In conclusion, the optimal irrigation concentration for sweet pepper fertigation was EC 2.0 dSㆍm$^{-1}$ .

A Trend Analysis of Contemporary Dance (컨템포러리 댄스 트렌드 분석에 관한 연구)

  • This study aims to examine the present (Western) modern dance by analyzing the works of three choreographers who are currently at the forefront of the dance scene to identify and understand the current choreographic trend. The three selected choreographers for this study are Justin Peck (USA), Sidi Larbi Cherkaoui (Belgium) and Zach Morris (USA). These choreographers were chosen on the basis of their significant identity within the current dance scene. For analysis, one noteworthy works of each choreographer were selected for video analysis. The result of the analysis illustrates that in the current era of promoting technology convergence in the performing arts industry, the three choreographers approach convergence differently by integrating dance and other mediums in their choreography. Peck converges between ballet and the ordinary, Cherkaoui converges between dance and other cultures and Morris converges between dance and venue.

2015 National Health Accounts and Current Health Expenditures in Korea (2015년 국민보건계정과 경상의료비)

  • Background: This paper aims to demonstrate current health expenditure (CHE) and National Health Accounts of the years 2015 constructed according to the SHA2011, which is a new manual of System of Health Accounts (SHA) that was published jointly by the Organization for Economic Cooperation and Development (OECD), Eurostat, and World Health Organization in 2011. Comparison is made with international trends by collecting and analysing health accounts of OECD member countries. Particularly, financing public-private mix is parsed in depth using SHA data of both HF as financing schemes as well as FS (financing source) as their revenue types. Methods: Data sources such as Health Insurance Review and Assessment Service's publications of both motor insurance and drugs are newly used to construct the 2015 National Health Accounts. In the case of private financing, an estimation of total expenditures for revenues by provider groups is made from the Economic Census data; and the household income and expenditure survey, Korean healthcare panel study, etc. are used to allocate those totals into functional classifications. Results: CHE was 115.2 trillion won in 2015, which accounts for 7.4 percent of Korea's gross domestic product. It was a big increase of 9.3 trillion won, 8.8 percent, from the previous year. Government and compulsory schemes's share (or public share) of 56.4% of the CHE in 2015 was much lower than the OECD average of 72.6%. 'Transfers from government domestic revenue' share of total revenue of HF was 17.8% in Korea, lower than the other contribution-based countries. When it comes to 'compulsory contributory health financing schemes,' 'Transfers from government domestic revenue' share of 14.9% was again much lower compared to Japan (44.7%) and Belgium (34.8%) as contribution-based countries. Conclusion: Considering relatively lower public financing share in the inpatient care as well as overall low public financing share of total CHE, priorities in health insurance coverage need to be repositioned among inpatient care, outpatient care and drugs.

Development of multiplex PCR for detection of vancomycin resistant enterococci(VRE) and epidemiological application in Korea

  • Vancomycin resistant enterococci (VRE) have emerged as an important nosocomial pathogen. Since 1989 the Center for Disease Control, United States, has reported a rapid increase in the incidence of enterococcal bacteremia and endocarditis infection by VRE. It was suggested that the use of avoparcin was associated with the appearance of VRE in animal husbandry. To date, several detection methods have been used based on conventional methods of culture and gene detection. However, these methods have some limitations such as time-consuming, laborious and additional differential needs. Therefore, In this study a multiplex PCR method was established to detect and differentiate resistance types of enterococci which specifically amplify the four van genes encoding vancomycin resistance elements. Using the method, we investigated the incidence rates and types of VRE from farms using or not using avoparcin. A total of 1091 animal fecal samples were collected from 70 pig and 32 poultry farms. A total of 425 of enterococci were isolated from samples. Of the 425 isolates, 11 of the them showed a pattern of high-level vancomycin resistance (MIC : $64{\sim}256{\mu}g/ml$) which was associated with the presence of the vanA or vanB gene. Fifty-seven isolates showed a pattern of low-level vancomycin resistance (MIC : $3{\sim}8{\mu}g/ml$) associated with the vanC-1 or vanC-2 gene. Interestingly, all isolates with high-level vancomycin resistance were from farms that have never used avoparcin. Moreover, the high-level VRE isolation rate in Korea (2.58%) was much lower than that of other countries (50% in England, 7% in Belgium) where avoparcin have been used. In conclusion, the multiplex PCR method established in this study could be applied for detection of VRE.
