남성 간경변 환자의 성기능, 수면의 질, 우울에 관한 연구 (A study of Sexual Function, Sleep Quality and Depression in Male Patients with Liver Cirrhosis)

  • 본 연구는 남성 간경변 환자의 성기능, 수면의 질, 우울 간의 관계를 확인하기 위한 서술적 연구이다. 대상자는 간경변을 진단 받고 외래 추적 관찰 중인 환자 144명으로, 자료 수집은 구조화된 설문지를 이용하여 2017년 8월 16일부터 2018년 1월 30일까지 실시하였다. 성기능은 Rosen 등이 개발한 International Index of Erectile Function의 한국어 번안한 것을 이용하여 측정하였고, 수면의 질은 Buysse 등이 피츠버그 수면센터에서 개발한 Pittsburgh Sleep Quality Index의 한국어 버전을 사용하여 측정하였다. 우울은 Beck Depression Inventory-II를 한국어로 번안한 한국어판 BDI-II 도구를 사용하였다. 분석은 t검정, 분산분석, 상관관계분석을 이용하였다. 연구 결과 남성 간경변 환자들의 성기능이 낮을수록 수면의 질이 낮고(r=-.232, p=.005) 우울 점수가 높았으며(r=-.381, p<.001), 수면의 질이 낮을수록 우울 점수가 높게(r=.561, p<.001) 나타났다. 대상자들의 성기능, 수면의 질, 우울은 통계적으로 유의한 상관관계가 있는 것으로 나타났다. 본 연구 결과는 남성 간경변 환자를 위한 간호 중재 개발의 기초 자료로 활용 될 수 있을 것이다.

뇌졸중 환자의 지역사회중심재활서비스: 사례연구 (The Service of Community-Based Rehabilitation in Stroke Patient: A Case Study)

  • 목적 : 지역사회에 거주하는 뇌졸중 환자를 대상으로 사례연구를 통해 지역사회중심재활서비스에 대한 소개와 중재 결과를 제시하고자 하였다. 연구방법 : 지난 2009년 7월 처음 내소한 63세 여성 뇌졸중 환자 1명을 대상으로 지역사회중심 재활서비스를 제공하였다. 대상자의 일상생활동작수준을 평가하기 위해 수정바델지수(Modified Barthel Index; MBI)를 이용하였고, 우울정도를 측정하기 위해 Beck 우울척도(Beck Depression Inventory; BDI)를 사용하였다. 중재 기간은 2011년 4월 27일부터 2011년 11월 30일까지로 세계보건기구에서 제안한 지역사회중심재활서비스와 일반적 작업치료를 제공하였다. 결과 : 환자의 MBI는 35점에서 55점으로 일상생활수행능력이 증가하였고, BDI는 26점에서 14점으로 우울증세가 큰 폭으로 감소한 것을 확인하였다. 초기 5~10분이던 서기 훈련이 30분 이상으로 늘어난 것으로 보아 근력 및 근지구력도 증가하였다. 치료시간에 환자의 기분변화가 심했던 이전과 달리 안정되고 웃는 모습을 자주 볼 수 있었다. 보호자 역시 환자가 심리적으로 안정된 상태를 체감할 수 있었다. 결론 : 지역사회로 복귀한 만성 뇌졸중 환자에게 지역사회중심재활서비스를 적용하여 기능수준 및 삶의 만족도가 향상된 것을 알 수 있었다.

갱년기 과체중과 비만여성의 신체활동량에 따른 스트레스, 우울, 자존감의 차이 연구 (Difference between Stress, Depression, and Self-esteem along with Physical Activity Levels in Overweight and Obese Perimenopausal Women)

  • Objectives : Physical activity is known as beneficial to mental health like decreasing depression and stress. In this study, we tried to find out the relationship between body composition, stress, depression, and self-esteem with physical activity level. Methods : International Physical Activity Questionnaire(IPAQ)-short form, SRRS(Social readjustment rating scale), SRI(Stress response index), and BDI(Beck's depression index), SES(Self-esteem scale) were given to peri-menopausal women aged 45-55 and BMI$\geq$23kg/m2. They were given written consent and this study is performed under the permission of institutional review board of Kyung Hee East-west Neo Medical Center. Results : The mean of physical activity of total subjects was 2406MET-min/week(n=42). The physical activity of overweight(23kg/m2$\leq$BMI<25kg/m2) and obese(BMI$\geq$25kg/m2) permimenopausal women was 1428MET-min/week and 2970MET-min/week(p<0.05) respectively. When three levels of physical activity pattern(inactive, minimally active, and HEPA(Health-enhancing physical activity) active) was analyzed, all of them were heavily relied on the activity intensities of walking. The scores of BDI, SRRS, and SRI were lower and that of SES were higher in HEPA active group, not showing statical significance. Anger(subscale of SRI) measures were lower and SES measures were higher in HEPA active group than others among overweight women(p<0.05). Conclusions : The level of physical activity of peri-menopausal women was on the average, but the vigorous activity is thought to be necessary. The HEPA active group showed lower depression, stress and higher self-esteem than minimally active and inactive group.

In-hospital malnutrition among adult patients in a national referral hospital in Indonesia

  • BACKGROUND/OBJECTIVES: Malnutrition during hospitalization is linked to increased morbidity and mortality, but there are insufficient studies observing clinical factors contributing to weight loss during hospitalization in Indonesia. This study was therefore undertaken to determine the rate of weight loss during hospitalization and the contributing factors. SUBJECTS/METHODS: This was a prospective study involving hospitalized adult patients aged 18-59 yrs, conducted between July and September 2019. Body weight measurement was taken at the time of admission and on the last day of hospitalization. The factors studied were malnutrition at admission (body mass index < 18.5 kg/m2), immobilization, depression (Beck Depression Inventory-II Indonesia), polypharmacy, inflammatory status (neutrophil-lymphocytes ratio; NLR), comorbidity status (Charlson Comorbidity Index; CCI), and length of stay. RESULTS: Totally, 55 patients were included in the final analysis, with a median age of 39 (18-59 yrs) yrs. Of these, 27% had malnutrition at admission, 31% had a CCI score > 2, and 26% had an NLR value of ≥ 9. In all, 62% presented with gastrointestinal symptoms, and depression was documented in one-third of the subjects at admission. Overall, we recorded a mean weight loss of 0.41 kg (P = 0.038) during hospitalization, with significant weight loss observed among patients hospitalized for 7 days or more (P = 0.009). The bivariate analysis revealed that inflammatory status (P = 0.016) was associated with in-hospital weight loss, while the multivariate analysis determined that the contributing factors were length of stay (P < 0.001) and depression (P = 0.019). CONCLUSIONS: We found that inflammatory status of the patient might influence the incidence of weight loss during hospitalization, while depression and length of stay were independent predictors of weight loss during hospitalization.

다낭난소증후군 환자에서의 우울, 불안, 스트레스를 유발하는 위험 인자 (Risk Factors for Depression, Anxiety, and Stress in Patients with Polycystic Ovary Syndrome)

  • 본 연구는 다낭난소증후군 여성의 불안, 우울, 스트레스를 평가하고, 발병 위험인자에 대하여 살펴보고자, 다낭난소증후군 여성을 대상으로 Beck's 우울증(BDI) 및 우울 불안 스트레스 척도(DASS) 설문을 시행하였고, 항뮐러리안호르몬(AMH), 총 테스토스테론, 황체형성호르몬(LH), 난포자극호르몬(FSH), 에스트라디올(E2), 지질대사검사 및 당부하검사(OGTT)를 시행 후 분석하였다. 설문지를 완전히 작성하여 회수된 52명의 환자군과 29명의 건강한 가임기 여성 대조군을 분석하였다. 다낭난소증후군 환자 중 BDI>13점, DASS>10점인 우울증 38.5 %, DASS>8점인 불안 23.1 %, DASS>15점인 스트레스 30.8 %로 대조군에 비해 유의하게 높았다. 총 테스토스테론, LH 및 AMH는 우울증 및 스트레스와 유의한 상관관계를 보였고, 체중 및 체질량지수도 우울증과 유의한 상관관계를 보였다. 당뇨병과 고지혈증으로 진단된 여성에서 우울증과 스트레스가 높았다. 다낭난소증후군 여성은 우울증, 불안 및 스트레스에 더 취약한 것으로 나타나, 조기진단과 치료가 중요하다.

일 대학병원 소아청소년 정신건강클리닉에 내원한 청소년에서 인터넷게임중독과 정신과적 증상의 관계 (Relationship between Internet Game Addiction and Psychiatric Symptoms in a Child and Adolescent Psychiatric Clinic)

  • Objectives : The aim of this study was to examine the relationship between internet game addiction and psychiatric symptoms. Methods : A total of 447 adolescents who were recruited from a child and adolescent psychiatric clinic of university hospital completed a self-report questionnaire consisting of Korean Internet Addiction Self-scale (K-scale), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Korean-Attention-Deficit Hyperactivity Disorder Rating Scale (K-ARS), and Adolescent Happiness Index (AHI). They were classified into three internet user groups, non-addicted group (below or equal to 94), potential risk group (95 to 107) and high risk group (above or equal to 108) according to K-scale total score. Results : Significant mean differences in BDI, BAI, K-ARS, and AHI scores were observed among the three groups. The BDI, BAI and K-ARS scores showed positive correlation with K-score. The AHI score showed negative correlation with K-score. Conclusion : Findings of this study suggest that depression, anxiety, inattention, hyperactivity/impulsivity, and happiness are associated with internet game addiction.

침구치료로 개선된 장애인의 정신건강 증례보고 (A Case Report on Improvement Mental Health of the Disabled by Acupuncture and Moxibustion)

  • The purpose of this study is to report the case mental health of people with disabilities has improved through visit treatment at public health centers. From October 26, 2021 to January 04, 2022, 6 visits were conducted and Patient Health Questionnaire-9 (PHQ-9), Beck Depression Inventory (BDI), Korean version of Center for Epidemiological Studies-Depression Scale (K-CES-D), Visual Analog Scale (VAS), Euro Qol-Visual Analogue Scale (EQ-VAS), Modified Barthel Index (MBI) were used to check changes. After Korean Medicine treatments such as acupuncture, electroacupuncture, electromoxibustion, PHQ-9 decreased from 18 to 8, BDI decreased from 22 to 15, K-CES-D decreased from 42 to 21. And VAS decreased from 85 to 50, EQ-VAS increased from 30 to 50. But There was no change in the MBI. Confirmation of improvement in mental health-related indicators of disabled patients through Korean Medicine treatments.

The Effect of Breathing Training on the Physical Function and Psychological Problems in Patients with Chronic Stroke

  • Purpose: To identify the effect of breathing training on the physical function and psychological problems in patients with chronic stroke. Methods: In total, 26 patients with chronic stroke were randomly assigned to one of two groups: the experimental group, who underwent breathing training and neurodevelopmental treatment, and the control group, who underwent neurodevelopmental treatment (13 patients per group). Physical function was evaluated using the balance and Activities of Daily Living (ADL). Psychological problems were assessed using the Depression. Balance was measured using the Berg Balance Scale (BBS). ADLs were measured using the Modified Barthel Index (MBI). Depression was measured using the Beck Depression Inventory (BDI). The BBS, MBI, and BDI were recorded before and after the 4-week treatment. Results: As a result of comparison within groups, the experimental and control group showed significant difference for BBS, MBI and BDI after the experiment (p<0.05). In comparison between two groups, the experimental group showed more significant difference in BBS, MBI and BDI than the control group (p<0.05). Conclusion: This study showed that breathing training was effective improvement physical function, and psychological problems in patients with chronic stroke.

The Effects of Injury and Accidents on Self-rated Depression in Male Municipal Firefighters

  • Objectives: The present study aims to determine the causal relationship between self-rated depression and experiences of injury and accidents in municipal firefighters. Methods: A panel survey of 186 municipal firefighters measured with depressive symptoms according to the Beck's depression index (BDI) was conducted. The effects of job-related injuries and accidents were evaluated using self-administered questionnaires that were taken once in a 12-month period from 2005 to 2006. Firefighters were classified into the Depression Group or Control Group based on follow-up BDI results with a cutoff level that was set to having "over mild depression." Results: The depression Group was comprised of 17 (9.1%) workers, including 9 firefighters who met had sufficient BDI scores twice in the 2-year test period and newly sufficient BDI scores in the follow-up test. A significantly higher number of subjects in the Depression Group experienced injuries and accidents in the 2-year test period as compared to the Control Group (15.4% vs. 1.5%, p=0.04). Firefighters who experienced injuries and accidents in the 2-year test period had a 7.4 times higher risk of being in the Depression Group than those who had not. As compared to accidents, near-miss accidents revealed stronger risks related to being classified as in the Depression group (adjusted odds ratio [OR] = 4.58, 95% confidence interval [CI] = 1.15-18.18 vs. Adjusted OR = 4.22, 95% CI = 1.08-16.58). Conclusion: The above results suggest that we should establish an effective program to promote mental health for groups at high risk for self-rated depression, including persons who have experienced consecutive injuries and accidents as well as near-miss injuries.

  • Objectives : Poststroke depression is a frequent and specific entity that impaires the rehabilliation and functional recovery of patients with hemiplegia. The author evaluated the effect of Banhahubak-tang(Banxiahoupotang) in patients with poststroke depression. Methods : 38 patients suffering from poststroke depression(determined by Diagnostic and Statistical Manual of Mental Disorders, revised. 3rd edition. and Beck Depression Inventory[BDI] cutoff $point{\geqq}10$) in Kyunghee Oriental hospital were randomized into two groups; treatment group(n=19) and control group(n=19). The treatment group was prescribed with Banhahubak-tang(Banxiahoupotang) three times a day fur a week. Control troop was prescribed with other herbal medicines used for stroke Patients three times a day for a week. Patients were evaluated by use of BDI scale, Modified Barthel Index, Depression of Ki score, Yin syndrome score, and Yang syndrome score. Among 38 patients, 24 patients got BDI scores above 21, which is the cut-off score for depression in Korean. The same procedures and assessments described above were applied. Results : Treatment group did not significantly improve compared with control group. Results yielded only slight significance (P=0.086). Especially. patients with poststroke depression as yin syndrome improved more significantly on BDI than those classified as yang syndrome. When BDI cutoff point for depression was defined as being ${\geq}\;21$, treatment group did not significantly improve compared with control group(P=0.114). However, patients with poststroke depression classified as yin syndrome were also significantly improved on BDI than those classified as yang syndrome. Conclusions : This study suggests that Banhahubak-tang(Banxiahoupotang) is significantly effective in patients with poststroke depression classified as yin syndrome.
