Sleeve Fracture of the Superior Pole of Patella in an Adolescent (청소년기에서 발생한 슬개골 상단부 소매형 골절)

  • A patella sleeve fracture is a type of avulsion fracture in which a small osseous fragment gets, along with a sleeve of periosteum and cartilage, is pulled off from the patella. The avulsed sleeve of cartilage and periosteum is not apparent on plain radiographs, which can result in a missed diagnosis or an underestimate of the extent of injury. An avulsion or sleeve fracture of the patella in a child can occur at the superior or the inferior pole of the patella. However, most reported cases of sleeve fractures involve the inferior patellar pole, with fractures involving the superior patellar pole being very rare. The authors report a case of a sleeve fracture of the superior pole of the patella in an adolescent; the fracture was diagnosed with magnetic resonance imaging (MRI) and was treated with surgery after early diagnosis. The course was uneventful, and the outcome was excellent.

Operative Treatment for Avulsion Fracture of Base of the Fifth Metatarsal (제5 중족골 기저부 견열 골절의 수술적 치료)

  • Purpose: To analyze the clinical and radiological results of the operative treatment in the avulsion fracture on the base of the fifth metatarsal. Materials and Methods: We studied retrospectively, 11 patients of avulsion fracture on the base of the fifth metatarsal operated and followed over 1 year, from February 2000 to May 2002. There were eight men and three women and the average age was 39 years old. The mean follow up period was 14 months. Ten cases were slip-down and one case was fall from a height injuries. We used the modified Foot Score by Wiener for the clinical evaluation, and analyzed the time of union and state of reduction radiologically. Results: In clinical results, we had ten excellent and one good cases by the modified Foot Score at last follow-up. In radiologic results, the complete bony union was achieved in all cases and the duration of the bony union was 37 days in average. Conclusion: We had good result for the avulsion fractures on the base of the fifth metatarsal treated by the operation. This study shows the operation was recommended for the moderate and severe displacement of the avulsion fractures on the base of the fifth metatarsal.

  • Avulsion fractures of the intercondylar eminence of the tibia are uncommon. This kind of fracture is most commonly found in adolescents, with an increasing incidence for those involved in competitive athletics. Recently, an arthroscopic procedure is preferred to reduce the operation-related morbidity. Screws, K-wires, Suture anchor have been described as fixation devices. Cannulated screw fixation is a simple method, but this instrument has the risk of causing damage to the growth plate. The authors report good results using cannulated screw in arthroscopic treatment for pediatric anterior cruciate ligament avulsion fracture.

  • The avulsion fracture of the calcaneal tuberosity is rare injury. Usually, it occurs from indirect trauma in old patients with osteoporosis or in patients with diabetic neuropathy. Especially, the bone and joint damage occurred in active patient with severe sensory loss or arthropathy related to nerve damage regardless of the cause is referred to neuropathic arthropathy. Generally, a patient with nondisplacement or minimally displacement is treated by conservative therapy and a patient with severe displacement is treated by open reduction and internal fixation. We experienced a 33 years-old woman with diabetes mellitus who had the displaced avulsion fracture of the calcaneal tuberosity without significant trauma and did not treat. We report upon this case at the 2 years follow-up.

  • In clinical practice, recurrent Achilles ruptures have been noted to occurr at the original ruptured site. However, reports of new developed fresh rupture of the Achilles tendon in other sites are is extremely rare. Our report is about one uncommon case of a traumatic calcaneal tuberosity avulsion fracture following augmented repair, which was performed using the Krackow locking loop technique. We performed open reduction and intra-osseous fixation using a suture anchor. This procedure was done through the primary longitudinal incision for the calcaneal avulsion fracture fragment. After 6 months of follow-up, our patient has achieved a complete functional recovery and he can normally perform daily and work-related tasks without pain.

Effects of Pharmacopuncture and Danggwisu-powder for Lateral Malleolus Avulsion Fracture: a Case Report (족외과 견열골절 환자에서 약침치료, 당귀수산을 통해 호전된 1례 증례보고)

  • Objectives : The purpose of this study is to report the clinical effect of Korean medical treatment, specifically Homnis Placenta pharmacopuncture, bee venom injection and Danggwisu-powder, on a patient suffering from a lateral malleolus avulsion fracture. Methods : A 63-year-old female patient suffering from right ankle pain due to lateral malleolus avulsion fracture was treated with Homnis Placenta pharmacopuncture, bee venom injection, and herbal medicine(Danggwisu-powder). from May 20th to June $16^{th}$ of 2015. The change in patientcondition was measured with the American Orthopedic Foot and Ankle Society Ankle-Hindfoot Scale, the verbal numerical rating scale, and via the ankle's range of motion. Results : After treatment, all obtained results showed improvement. The American Orthopedic Foot and Ankle Society Ankle-Hindfoot Scale increased from 18 to 71, the verbal numerical rating scale changed from 8 to 3, and the ankle's range of motion improved. Conclusions : This study suggests that Korean medical care focused on Homnis Placenta pharmacopuncture, bee venom injection and Danggwisu-powder can be effective to treat lateral malleolus avulsion fracture.

Successful Surgical Treatment of Severe Lower Lip Avulsion in Two Cats

  • Two Korean short-haired cats were admitted to the Veterinary Medical Teaching Hospital, Chonnam National University, with severe lower lip avulsion. In the first case, the treatment was performed using the tension-free suture technique with rubber tube stents. The second case was treated using the tension-free suture technique with rubber tube stents for lip avulsion and using the cerclage wiring technique for alveolar fracture. The teeth around the alveolar fracture were extracted and bone graft and collagen membrane were applied at the alveolar fracture site to stabilize the mandibular alveolar fracture. Thereafter, the cerclage wire was placed circumferentially around the mandible. In both cases, normal function of the oral cavity was successfully recovered by repairing the anatomic abnormality. In conclusion, tension-free suture technique can be a treatment option for bilateral lower lip avulsion in cats.

The usefulness of CT for the diagnosis and the fragment fixation of anteroinferior tibiofibular ligament avulsion fracture in ankle fracture (족관절 골절에서 전하 경비 인대 견열 골절의 진단과 골편 고정술을 위한 전산화 단층 촬영의 유용성)

  • Purpose: To evaluate the usefulness of computerized tomography (CT) for the diagnosis and the fragment fixation of anteroinferior tibiofibular ligament avulsion fracture in ankle fracture. Materials and Methods: We retrospectively studied 108 patients with an ankle fracture who had been checked with plain radiographs and CT from July 2006 to July 2010. They were divided into two groups; patients with (19 patients) and without (89 patients) an avulsion fracture of anteroinferior tibiofibular ligament. The two groups were evaluated with Lauge-Hansen classification, the energy of trauma, and the radiologic indices for syndesmotic injury, and were compared each other. Average follow up periods of two groups were 25 and 23 months each. Those who were unstable at stress test during surgery were divided into fragment fixation of anteroinferior tibiofibular ligament avulsion fracture group (8 patients) and transfixation one (11 patients) according to treatment method. Clinical and radiological results at last follow up were also compared. Results: Fourteen avulsion fractures of anteroinferior tibiofibular ligament were diagnosed by CT only. Incidences of pronation-external rotation injury, high energy trauma, positive radiologic indices for syndesmotic injury were significantly higher in patients with an avulsion fracture of anteroinferior tibiofibular ligament than those without it. Clinical and radiological results were satisfactory in both groups at last follow up, and were not significantly different between them. Conclusion: In patients who have an ankle fracture by pronation-external rotation injury, high energy trauma, or with positive radiologic indices for syndesmotic injury, CT is useful for diagnosis of an avulsion fracture of anteroinferior tibiofibular ligament. Fragment fixation of anteroinferior tibiofibular ligament avulsion fracture is a useful treatment option for syndesmotic injury.

Avulsion Fracture of Navicular (주상골의 견열 골절)

  • The purpose of this article is to evaluate and analyze diagnosis and the clinical result of avulsion fracture of navicular. Eight patients operated from Jan. 1991 to June 2002 were evaluated retrospectively. Clinical tolerability, AOFAS score, patient's satisfaction were evaluated. Preoperative AOFAS score is $59.88{\pm}10.90$. Postoperative AOFAS score is $94.75{\pm}6.11$. All patient satisfied result of operation, and 5 patient return to training wihin 3 month. Navicula avulsion fracure is relatively rare. The small dorsal bone triangular fragment is best seen in lateral view radiographs and bone scan and/or computed tomography help corfirm diagnosis. Operative treatment is the method of choice because of shorter recovery time. Simple excision is enough to get a good clincal outcome.
