Arthroscopic Management for the Treatment of Ankle Gutter Syndrome (족 관절 구 증후군에 시행한 관절경적 치료)

  • Purpose : The purpose of this study was to evaluate the gutter pathologies and the clinical results of arthroscopic treatment for ankle gutter syndrome. Materials and Methods : Seventy six patients(77 ankles) who had lesions in the ankle gutter were treated by arthroscopy. The gutter pathology could be divided into 3 categories; hypertrophic scar tissue, loose bodies and bone spurs. Fifty two patients were evaluated subjectively and functionally with authors' criteria. The follow-up period averaged 44 months(range, 2.9 year-8.5 year). Results : The incidence of the gutter lesion was $21\%$(77 ankles) among 366 ankles undergoing arthroscopy. The most common pathology was hypertrophic scar tissue. In 31 ankles$(40\%)$, the lesions were found only in gutter, and 46 ankles$(60\%)$ were associated with pathologies in other areas. All of parameters for subjective and functional evaluation were improved with statistical significance(p<0.001). Seventeen patients$(33\%)$ could returned to competitive or contact sports activity with same level of premorbid period. Conclusion : Ankle gutter is an important site of pathologies which cause chronic ankle pain, and should be explored in ankle arthroscopy. The ankle arthroscopy is a very effective treatment method fur the removal of pathologies in ankle gutter syndrome.

Estimation of the Range of the Suspended Solid from the Nakdong River using Satellite Imageries and Numerical Model (위성영상 및 수치모델을 이용한 낙동강유출 부유토사 확산범위 추정)

  • We were trying to understand indirectly the range of the discharge from the Nakdong with the dispersion of suspended solid(SS) related to the amount of discharge from river in this study. The range of dispersion of SS from the Nakdong was estimated using satellite remote sensing and numerical modeling. The stream field with two dimensional and numerical model using the condition of integrated depth was calculated. According to the results, the streamline flowed from Busan to the Jinhae Bay and Geojae Island. at the flood. The situation at the ebb was totally changed. The streamline flowed out Busan from the Bay. The velocity in offshore was faster than one at coastal water of the Nackdong. Residual current which was averaged during 12hours dominantly appeared the dominant direction from the southwestern part of the Nackdong to the northeastern part of it. The eastward current appeared at the eastern coast of Gaduck Is. Base on the results of the velocity field, the quantifying of the dispersion of SS was estimated by the method of numerical tracer related to the Lagrangian method. The significant range of the dispersion of the SS from the Nackdong was from the eastern coast of Gaduck Is, to the coastal areas of Busan, Korea. The estimated range of the dispersion of the SS using the SeaWiFS and Landsat satellite data was similar to the estimated results using the numerical model.

Entropy-based Discrimination of Hand and Elbow Movements Using ECoG Signals (엔트로피 기반 ECoG 신호를 이용한 손과 팔꿈치 움직임 추론)

  • In this paper, a method of estimating hand and elbow movements using electrocorticogram (ECoG) signals is proposed. Using multiple channels, surface electromyogram (EMG) signals and ECoG signals were obtained from patients simultaneously. The estimated movements were those to close and then open the hand and those to bend the elbow inward. The patients were encouraged to perform the movements in accordance with their free will instead of after being induced by external stimuli. Surface EMG signals were used to find movement time points, and ECoG signals were used to estimate the movements. To extract the characteristics of the individual movements, the ECoG signals were divided into a total of six bands (the entire band and the ${\delta}$, ${\Theta}$, ${\alpha}$, ${\beta}$, and ${\gamma}$ bands) to obtain the information entropy, and the maximum likelihood estimation method was used to estimate the movements. The results of the experiment showed the performance averaged 74% when the ECoG of the gamma band was used, which was higher than that when other bands were used, and higher estimation success rates were shown in the gamma band than in other bands. The time of the movements was divided into three time sections based on movement time points, and the "before" section, which included the readiness potential, was compared with the "onset" section. In the "before" section and the "onset" section, estimation success rates were 66% and 65%, respectively, and thus it was determined that the readiness potential could be used.

  • Even though a lot of advanced researches on analysis, design, and performance evaluation of reinforced concrete (RC) under seismic action have been carried out, there has been only a few study on seismic analysis of underground RC structures surrounding soil medium. Since the underground RC structures interact with surrounding soil medium, a path-dependent soil model which can predict the soil response is necessary for analyzing behavior of the structure inside soil medium. The behavior of interfacial zone between the RC structure and the surrounding medium should be also considered for more accurate seismic analysis of the RC structure. In this paper, an averaged constitutive model of concrete and reinforcing bars for RC structure and path-dependent Ohsaki's model for soil are applied, and an elasto-plastic interface model having thickness is proposed for seismic analysis of underground RC structures. A finite element analysis technique is developed by applying aforementioned constitutive equations and is verified by predicting both static and dynamic behaviors of RC structures. Then, failure mechanisms of underground RC structure under seismic action are numerically derived through seismic analysis of underground RC station structure under different seismic forces. Finally, the changes of failure mode and the damage level of the structures are also analytically derived for different design cases of underground RC structures.

Transilluminated Powered Phlebectomy for Varicose Vein (하지정맥류의 투시조명하 전동형 정맥류 수술법)

  • To determine the efficacy and safety of endoscopic resection and ablation of superficial varicosities using a powered vein resector, irrigated illuminator. Material and Method: 83 consecutive patients were involved in the study. 103 limbs in 83 patients were treated using a minimal invasive, powered, vein resecting devide with cutaneous transillumination and tumescent anesthesia technique. There were 51 women and 32 men. All patients were operated under general anesthesia or regional anesthesia. Operative time and patient satisfaction scores were recorded along with the number of incisions made. Result: 83 patients(51/61.4% female, 32/38.6% male, aged 25-78 years) had varicose vein. Average age at the time of operation was 45 years(range, 25 to 78 years). There were 63 unilateral procedures and 20 bilateral operations. Operative time ranged from 24 to 46 minutes (average 35.3 minutes) in the unilateral procedure. The number of incisions per limb averaged 2.7(range, 2 to 5). Postoperative complications occurred in 7(8.4%) patients. Patients were asked to describe their pain on an analog scale ranging form 1 to 10 with r representing no pain and 10 worst imaginable pain. Immediately postoperative pain score was 2.4 Postoperative pain score at 72 hours had a mean score of 2.0. Postoperative pain score at 1 months were 1. Conclusion: Varicose vein removal using Transilluminated Powered Phlebectomy(TIPP) is a safe and efficient procedure. The procedure saves time, is easy to perform, and gives direct visualization and a distinct endpoint of the removal of veins. It is also less tedious to perform and gives good cosmetic results with significant pain relief.

  • A rapid and very sensitive high-performance liquid chromatography/atmospheric-pressure chemical-ionization mass spectrometry method to detect ethylenethiourea (ETU) fungicide residues in fruits was developed. Methylene chloride was used as the surface extraction solvent for the target component. Recovery rates improved when cysteine hydrochloride and sodium carbonate were added to product prior to fortification. The limits of detection and quantification were approximately 0.006 and 0.02 mg/kg, respectively, from mandarin oranges. Recoveries from mandarin oranges, oranges, bananas, and pears, spiked in the range of 0.05-0.5 mg/kg, averaged 80-100%. The proposed method was used to monitor the presence of ETU in commercial fruits purchased from different markets in Seoul, Korea. ETU was found in four orange peels and in three mandarin orange peel samples. The highest ETU residue levels were $73.6{\mu}g/kg$ and $29.8{\mu}g/kg$.

Compensation Characteristics of Distorted Channels in 200 Gbps WDM Systems using Mid-Span Spectral Inversion Method (200 Gbps WDM 시스템에서 Mid-Span Spectral Inversion 기법을 이용한 채널 왜곡의 보상 특성)

  • In this paper, the characteristics of compensation for WDM channel signal distortion due to both chromatic dispersion and Ken effect in 1,000 km 200 Gbps(5${\times}$40 Gbps) WDM systems was investigated. The WDM system has a path-averaged intensity approximation(PAIA) mid-span spectral inversion(MSSI) as a compensation method. This system has a highly nonlinear dispersion shifted fiber(HNL-DSF) optical phase conjugator(OPC) in the mid-way of transmission line. In order to evaluate the degree of compensation, 1 dB eye opening penalty(EOP), bit error rate(BER) characteristics and power penalty of 10$\^$-9/ BER are used. It is confirmed that HNL-DSF is an useful nonlinear medium in OPC fur wideband WDM system with PAIA MSSI and that the optimal compensation for WDM channel distortion is achieved by the selection of pump light power of OPC, which equalize the conjugated light power into the second half fiber section with the input WDM signal light power depending on total transmission length, dispersion coefficient of fiber, OPC pump light wavelength, conversion efficiency of WDM channel in OPC.

Moment Magnitude Determination Using P wave of Broadband Data (광대역 지진자료의 P파를 이용한 모멘트 규모 결정)

  • A method to quickly estimate broadband moment magnitudes (Mwp) to warn regional and teleseismic tsunamigenic earthquakes is tested for application of the method to the different seismic observation environment. In this study, the Mwp is calculated by integrating far-field P-wave or pP-wave of vertical component of displacement seismograms in time domain from earthquakes, having magnitude greater than 5.0 and occurred in and around the Korean peninsula from 2000 to 2006. We carefully set up the size of the time window for the computations to exclude S wave phases and other phases following after the P wave phase. The P wave velocities and the densities from the averaged Korean crustal model are used in the computations. Instrumental correction was performed to remove dependency on the seismograph. The Mwp after the instrumental correction is about 0.1 greater than the Mwp before the correction. The comparison of our results to the those of foreign agencies such as JMA and Havard CMT catalogues shows a higher degree of similarity. Thus our results provide an effective tool to estimate the earthquake size, as well as to issue the necessary information to a tsunami warning system when the effective earthquake occurs around the peninsula.

Quantitative Analysis of Quadrupole Noise Sources upon Quick Opening The Throttle (쓰로틀밸브 급개방시 기류소음의 4극음원에 대한 정량적 해석)

  • In recent years, modularization of engine parts has increased the application of plastic products in air intake systems. Plastic intake manifolds provide many advantages including reduced weight, contracted cost, and lower intake air temperatures. These manifolds, however, have some weakness when compared with customary aluminium intake manifolds, in that they have low sound transmission loss because of their lower material density. This low transmission loss of plastic intake manifolds causes several problems related to flow noise, especially when the throttle is opened quickly. The physical processes, responsible for this flow noise, include turbulent fluid motion and relative motion of the throttle to the airflow. The former is generated by high-speed airflow in the splits between the throttle valve and the inner-surface of the throttle body and surge-tank, which can be categorized into the quadrupole source. The latter induces the unsteady force on the flow, which can be classified into the dipole source. In this paper, the mechanism of noise generation from the turbulence is only investigated as a preliminary study. Stochastic noise source synthesis method is adopted for the analysis of turbulence-induced, i.e. quadrupole noise by throttle at quick opening state. The method consists of three procedures. The first step corresponds to the preliminary time-averaged Navier-Stokes computation with a $k-\varepsilon$ turbulence model providing mean flow field characteristics. The second step is the synthesis of time-dependent turbulent velocity field associated with quadrupole noise sources. The final step is devoted to the determination of acoustic source terms associated with turbulent velocity. For the first step, we used market available analysis tools such as STAR-CD, the trade names of fluid analysis tools available on the market. The steady state flows at three open angle of throttle valve, i.e. 20, 35 and 60 degree, are numerically analyzed. Then, time-dependent turbulent velocity fields are produced by using the stochastic model and the flow analysis results. Using this turbulent velocity field, the turbulence-originated noise sources, i.e. the self-noise and shear-noise sources are synthesized. Based on these numerical results, it is found that the origin of the turbulent flow and noise might be attributed to the process of formulation and the interaction of two vortex lines formed in the downstream of the throttle valve. These vortex lines are produced by the non-uniform splits between the throttle valve and inner cylinder surface. Based on the analysis, we present the low-noise design of the inner geometry of throttle body.

A simple approach to refraction statics with the Generalized Reciprocal Method and the Refraction Convolution Section (GRM과 RCS 방법을 이용한 굴절파 정적 시간차를 구하는 간단한 방법)

  • We derive refraction statics for seismic data recorded in a hard rock terrain, in which there are large and rapid variations in the depth of weathering. The statics corrections range from less than 10 ms to more than 70 ms, often over distances as short as 12 receiver intervals. This study is another demonstration of the importance in obtaining accurate initial refraction models of the weathering in hard rock terrains in which automatic residual statics may fail. We show that the statics values computed with a simple model of the weathering using the Generalized Reciprocal Method (GRM) and the Refraction Convolution Section (RCS) are comparable in accuracy to those computed with a more complex model of the weathering, using least-mean-squares inversion with the conjugate gradient algorithm (Taner et al., 1998). The differences in statics values between the GRM model and that of Taner et al. (1998) systematically vary from an average of 2ms to 4ms over a distance of 8.8 km. The differences between these two refraction models and the final statics model, which includes the automatic residual values, are generally less than 5 ms. The residuals for the GRM model are frequently less than those for the model of Taner et al. (1998). The RCS statics are picked approximately 10 ms later, but their relative accuracy is comparable to that of the GRM statics. The residual statics values show a general correlation with the refraction statics values, and they can be reduced in magnitude by using a lower average seismic velocity in the weathering. These results suggest that inaccurate average seismic velocities in the weathered layer may often be a source of short-wavelength statics, rather than any shortcomings with the inversion algorithms in determining averaged delay times from the traveltimes.
