물리치료사의 직무 및 조직특성이 전문직업성에 미치는 영향에 관한 연구 (A Study on the Relation Between Physical Therapist Professionalism and Organizational and Job Characteristics)

  • Purpose: This study was designed to measure the level of professional autonomy regarding physical therapists and to determine the association of organizational and occupational characteristics of the profession with its professional autonomy. Methods: We utilized a structured questionnaire survey of physical therapists (280 persons) in Jeju province. Data were collected from June 25 to July 14 in 2012. An additional 173 (63%) of them were used in the final analysis. Using PASW 18.0, descriptive and Hierarchical Linear Model were performed. Results: Regression analysis Result of Factors influencing Physiotherapist Professionalism, refresher training course (t=4.27), formalization (t=3.13), task significance (t=3.39), and autonomy (t=4.17) had a positive effect. Autonomy (${\beta}$=0.33) and formalization (${\beta}$=0.33) exerted the greatest influence. Conclusion: The survey regarding organizational and job characteristics showeds that occupational reeducation, formalization, and self- regulation constitute a positive part of what the professional autonomy is to be upheld. Results of the survey imply that in order to establish professional autonomy for the profession of physical therapist, the organization must make various efforts to beef up the exclusive knowledge and technology, and professional norms of the occupation that are considered essential for specific problem solving.

The Influence of Professionalism to Job Satisfaction and Job Performance of the Physical Therapist

  • Purpose: This study was conducted in order to understand the influence of professionalism on job satisfaction and job performance of the physical therapist. Methods: Subjects were 148 physical therapists. We conducted a questionnaire survey on professionalism, job satisfaction, and job performance. Professionalism questions were 'Compliance of professional organization', 'Beliefs about public service', 'Autonomy', 'Beliefs about self-regulation', and 'Vocation awareness of the profession'. A regression analysis was then performed. Results: The subjects' average score for professionalism was $3.18{\pm}0.31$ points. The lowest score for satisfaction was observed for 'Compliance of professional organization'. Professionalism had an effect on job satisfaction. 'Vocation awareness of the profession (${\beta}=0.44$, p<0.001)', 'Beliefs about public service (${\beta}=0.25$, p<0.01)', and 'Autonomy (${\beta}=0.17$, p<0.05)' had an effect on job satisfaction. R square=0.46 (adjusted R square=0.45). Professionalism had an effect on job performance. 'Autonomy (${\beta}=0.29$), p<0.001)', 'Vocation awareness of the profession (${\beta}=0.22$, p<0.05)', and 'Beliefs about self-regulation (${\beta}=0.18$, p<0.05)' had an effect on job performance. R square=0.25 (adjusted R square=0.22). Conclusion: The results of this study indicate that in order to improve job satisfaction and job performance, it is necessary to improve professionalism. Therefore physical therapists should improve their professionalism ('Compliance of professional organization', 'Beliefs about public service', 'Autonomy', 'Beliefs about self-regulation', and 'Vocation awareness of the profession') in order to improve their job satisfaction and job performance. In addition, research to improve professionalism of the physical therapist should be continued.

간호전문직 자율성(Professional Nurse Autonomy)의 개념분석 (Concept Analysis of Professional Nurse Autonomy)

  • Professional nurse Autonomy is an essential attribute of a discipline striving for full professional status. Purpose: This study was to clarify the concept of professional nurse autonomy to provide basic data needed for development of professional autonomy enhancing strategies. Method: This study use the process of Walker & Avante's concept analysis based on Wade's research (1999), and field data of 21 nurses. Results: Professional nurse autonomy is defined as competency and creative performance of the professional nurse in practice, to decide independently or interdependently nursing activities and to be had accountable for results of decisions, that reflect advocacy and caring. It was identified that critical attributes include responsible discretionary decision making, collegial interdependence, initiative, creativity, and caring, advocacy, cooperative relationship with clients, receptive capacity to others, activeness, self confidence, and devotion and responsibility to their profession. Antecedents include personal characteristics, educational background, experience and structural characteristics that enhance professional nurse autonomy. Consequences of professional nurse autonomy are feelings of self-efficacy, empowerment, job satisfaction, reduction of intention to leave their job. Conclusion: According to these results, it is recommended that the curriculum provides an environment for learning professional nurse autonomy, and that is used as basic data to develope strategies to enhance professional autonomy of nurse in practice and it's effects

Enhancing the Autonomy of Physical Therapy in Korea and Its Significance for the National Healthcare System: Facing the Challenges of a Super-aging Society

  • Most advanced countries that are members of the World Physiotherapy have established a 4-year education system or specialized graduate school system for physical therapists based on national standards. They have also expanded their laws and systems to provide physical therapists with the autonomy and independence to offer services in their clinics. However, compared with developed countries in North America and Europe, there are issues with the autonomy and independence of physical therapists in Korea related to national regulations. Social status and recognition of the profession are also lagging. Korea is expected to become a super-aged society by 2025. To reduce the financial burden of healthcare and welfare on the government, it is necessary to extend the time spent by older adults on independent activities and minimize their time spent using medical services. To achieve this goal and maximize the active life of older adults, a plan to efficiently use licensed physical therapists in the country should be prepared. Korea should increase the license utilization rate of physical therapists to reduce waste at the national level and increase the professional hope of the younger generations of physical therapists. To create a healthcare policy focusing on the use of physical therapy personnel, similar to that in advanced countries, it is necessary to unify educational systems and produce excellent physical therapists. Providing professional autonomy can help physical therapists develop a sense of job satisfaction. Outstanding talent will choose physical therapy as a profession if they can see hope for their future careers, and if physical therapy services in Korea are similar to those delivered in advanced countries, physical therapy in Korea can develop into a healthcare service that people desire.

전문직과 국가의 관계 -한국 한의사 조직을 중심으로- (A Study of the Relations between the Profession and the State: Focusing on the Korean Oriental Medicine Doctors' Organization)

  • The degree of doctors' autonomy is decided by the relations between the profession and the state. The exclusive right required by doctors is based on knowledge, but it is most often decided by the relations between the profession and the state. The critical factor affecting the status of the medical profession is the span of its control on medical related areas. Therefore, the status of doctors depends on the degree of institutionalization and the span of their control. The status of Korean Oriental Medicine doctors has been largely affected by the governments medical policies. The dramatic change of the status of Korean Oriental Medicine in modern Korea dearly shows the effects of medical policy on the status of a medical system. Under the plural medical system every medical group lobbies for more favorable medical policies. Korean Oriental Medicine doctors have secured an enhanced status in government and expanded the range of medical insurance coverage. Also, they have accomplished the standardization of knowledge in their efforts to expand it. These achievements are the most important aspects of the professionalization of Korean Oriental Medicine. Therefore, we can conclude that the conflicts between Korean Oriental Medicine doctors and government representatives have had significantly positive effects on the professionalization of Korean Oriental Medicine.

바우처 도입에 따른 사회복지전문직 정체성의 변화와 그 의미 (A Study on the Change and Meaning of Identity in Social Work Profession Following the Introduction of Voucher in Korea)

  • 이 연구는 사회서비스 전달에서 바우처의 도입이 사회복지전문직의 정체성에 어떤 변화를 가져오는지에 대한 궁금증에서 출발하였다. 연구방법은 바우처 실천을 수행하는 20명의 사회복지사들을 대상으로 심층 인터뷰를 실시하였다. 연구결과 사회복지실천에서 바우처 도입은 사회복지전문직 정체성에 혼란을 가져다주었는데, 사회복지사들의 역할을 중개적 관리자로, 클라이언트와의 관계를 시장적 관계로 변화시켰으며, 이는 관료적 통제의 증가와 영리화 기제를 통해 자율권을 축소시킴으로써 이루어졌다. 사회복지전문직에서의 이러한 변화는 단순한 변화가 아닌 '재구조화'로 볼 수 있고, 사회복지실천에서 관리주의의 압력이 가시화되고, 유사 사적실천을 형성하는 의미를 갖고 있는 것으로 드러났다.

미국 치과위생사의 전문직화 경향 (The trend toward professionalization of American dental hygienists)

  • Objectives: This study presents the basic data necessary to explore the methods used of Korean dental hygienists to resolve oral health inequalities in Korea via reviewing the professionalization trend of American dental hygienists. Methods: The supervision levels of American dental hygienists, and system of dental therapists were analyzed using the data published by the relevant associations. Results: In America, dental hygienists help address oral health inequalities. However, due to limitations in the supervision levels and scope of practice of dental hygienists, the autonomy of dental hygienists was expanded, and mid-level practitioner were employed. The autonomy for dental hygiene practice was higher in public than in private in America. Some states have introduced dental therapists as mid-level practitioners. Their practice settings have limitations such as serving low-income, uninsured, and underserved patients or serving in a dental health professional shortage area. Conclusions: It is necessary to expand the autonomy of dental hygienists based on their profession. In particular, it is suggested that they start with the low-risk level practices. Further, it is necessary to introduce a dental hygiene specialist system specialized for fields with high social demands.

  • This paper discussed motivation, some theories of motivation and motivation of library personnel. The most important aspect of any organization is its workforce. Motivation arouses, energizes, influences and sustains behaviour and performance of employees. Motivating factors include extrinsic factors which pertain to the conditions under which a job is performed and intrinsic motivators which contribute to job satisfaction and subsequently increase productivity. Two levels of motivation were identified in the library: the need-factor motivation and the profession-related motivation. Need-factor motivation is well-pronounced in the lower cadre of library staff such as library assistants, clerical staff, and porters who set their priority on economic needs. The profession-related motivation occurs among professionally trained librarians whose motivation hinges self-fulfillment derived from professional growth and development. Other motivating factor in the library is job design which includes job rotation, job enlargement, and job enrichment. Job design specifies the contents or methods of any job in such a way that various requirements of a job can be effectively satisfied. Adequate motivation of library employees lies with the managers because they generally control work distribution, freedom and autonomy, work challenge Remove, staff development and training, and advancement opportunities. The managers are urged to create enabling environment to ensure improved commitment, satisfaction, and productivity of employees in the library.

한의과 대학 직업윤리 교육 현황을 통해 바라본 의료인 윤리교육 방법론 연구 (Study on Bioethical Education of Oriental Medical Doctor, Based on Vocational Ethics of Medical Personnel)

  • The medical profession has the problem of lack of bioethics, due to the expansion of capitalism and mannerism after modernization. Therefore, the need of education of bioethics is increasing, however, the cramming system of education is insufficient for promoting personal morals. So the author studied ancient and present bioethics and searched for the cause of current bioethics absence and the method of overcoming it. Especially, studying the vocational features of oriental medical doctor as profession and the problems of education of bioethics in oriental medical college, the author searched for the directions of educations of bioethics. The conclusions are as follows. The current medical profession have ethical problems because of social moral hazard, evils of capitalism, change of doctor-patient relationship due to expansion of consumerism, limitation of autonomy due to commercialize of hospitals, decrease of knowledge monopolism of professionals, moral indifference and frailty, and a missdeed preference. The education of bioethics needs "Rest's 4 components" but the current education of bioethics in oriental medicine college lacks of time and is composed of the cramming system of education. So it needs various types of education system. Morals are subjective and discretionary personal character. Therefore, informational education is insufficient for enhancing morals and complex education for various personal attainments (various social indirect experience, philosophical speculation, mental serenity) is needed. It has to be done on the basis of educational method by experience not lecture, long term expectation, and basic understanding of bioethics.

한의학과 중의학에 대한 국가정책 비교연구 (Comparative Study of the National Policies for Korean Oriental Medicine and Traditional Chinese Medicine)

  • The structure of medical profession is composed of multiple relations among state, patients, and medical professions. There are conflicts between the nation and medical professions because the nation controls the monopoly of medical professions through medical policies. Patients make relations with medical profession as medical consumers. And medical professions compete each other in order to gain the control of the medical market. This paper attempts to review the dynamic relations between the nation and medical professions. The medical professions and the nation are in conflict about the control of the autonomy of medical professions. The medical professions want to exercise the monopoly rights in their own area and, on the other had, the nation wants to prevent problems that might result from the monopoly by regulations and to have the control over the national operation. Given this, the common view of medical sociology is that the nation and the medical professions are in constant conflict. The arguments that the present medical sociology has on the relationship between the nation and medical professions can be summarized like these: first, the nation is the authenticator of medical system; second, the nation is a medical provider and consumer; and, third, the nation is a mediator of regulations and conflicts. Based on the above mentioned relations between the nation and medical professions, this paper attempts to see how the nation, which is one component of the medical structure, make influences on Korean Oriental doctors and Traditional Chinese doctors. So as to do this, the changes in medical policies and promoting policies for Korean Oriental medicine and Traditional Chinese medicine are analyzed. Finally, the differences in national policies of Korean Oriental medicine and Traditional Chinese medicine are compared.
