Effects of Carrier Wave on the Brain Stem Electric Response (BER) in Scala Tympanic Electrode Array

  • Using electronic cochlear implant system, we studied in cats the difference in the response of the brain stem evoked response (BER) during the stimulation with the acoustic signals and the electric signals. These brain stem electric responses were analyzed using the integral pulse frequency modulation method of the auditory nervous system. Animal experimental results and the analysis show that the carrier wave hasimprored the frequency specificity. of the electronic auditory prosthesis.

  • The special interest should be paid to the analysis of the influences of positive emotions in terms of their possible effects on the dyanmics of autonomic. recovery after the negative affective stimualtion. Taking into account emotion-specific autonomic response patterning and dissociation of parameters of autonomic arousal during experience of both positive and negative emotional states, this problem seems a challenging one. In present study several autonomic parameters were analyzed altogether, namely inedices of electrodermal activity, heart rate and respitation rate during consecutive combination or both IAPS-based visual affective and auditory stimulation. The aim of the study was analysis of patterns of electrodermal and cardiorespiratory responses during emotional states evoked by negative affective visual stimulation followed by positive or neutral auditory one with intention to identify if the latter is able to facilitate post-stress recovery and enhance restoration of pre-arousal levels. The main orientation was dirdcted towards the further application of experimentally induced comfort emotions for dampening the negative consequences of exposure to stressful stimuli.

편마비환자의 트레드밀 보행 훈련 중 빠른 템포 음향이 미치는 효과 (Effect of Fast-Tempo Auditory Stimulation during Treadmill Gait Training in Patients with Hemiplegia)

  • 본 연구는 편마비 환자의 트레드밀 보행 훈련 중 빠른 템포 음향을 가미했을 경우 보행속도 및 보행능력에 미치는 효과에 차이가 있는지 알아보고자 수행되었다. 22명의 만성 편마비 환자를 대상으로 빠른 템포 음향을 가미한 트레드밀보행 훈련군 11명과 일반 트레드밀보행 훈련군 11명으로 확률할당 한 후, 각각 30분씩 하루에 1회 6주간 훈련이 시행되었다. 프로그램 전후 10 m 보행검사와 20미터 걸음 수의 변화를 평가한 결과, 빠른 템포 음향을 가미한 트레드밀 보행 훈련군이 일반 트레드밀 보행 훈련군에 비해 10 m 보행검사는 28.53% point, 20미터 걸음 수는 16.93% point 더 크게 향상 되었다(p<.05). 그러나 보행 질 평가는 그 향상도가 유의하지 않았다(p>.05). 본 연구결과로 보아 빠른 템포 음향을 가미한 트레드밀 보행 훈련이 일반 트레드밀 보행 훈련보다 만성 편마비 환자의 보행 속도를 증가시켜 보행능력에 긍정적인 영향을 준 것으로 판단된다. 따라서 편마비 환자의 트레드밀 보행 훈련 시 빠른 템포 음향을 가미한 트레드밀 보행 훈련의 임상적용을 적극적으로 고려하여야 할 것으로 여겨진다.

Effects of Rhythmic Auditory Stimulation Using Music on Gait With Stroke Patients

  • This study aimed to determine the effects of Rhythmic Auditory Stimulation (RAS) using music and a metronome on the gait of stroke patients. 13 female and 15 male volunteers were randomly allocated to two groups: namely a group to receive RAS using music and a metronome group (the experimental group; $n_1=14$) and a group to receive RAS using a metronome only (the control group; $n_2=14$). The affected side was the left side in 15 subjects and the right side in 13 subjects. The mean age of the subjects was 56.6 years, and the mean onset duration of stroke was 8.6 months. Intervention was applied for 30 minutes per session, once a day, 5 times a week for 4 weeks. To measure the patients' gait improvement, we measured gait velocity, cadence, stride length, double limb support using GAITRite, body center sway angle using an accelerometer, and Timed Up-and-Go test. Functional Gait Assessment were conducted before and after the experiment. The paired t-test was used for comparisons before and after the interventions in each group. Analysis of covariance was used for comparisons between the groups after the interventions. Statistical significance was set at ${\alpha}=.05$. Within each of the two groups, significant differences in all of the dependent variables before and after the experiment (p<.05) were observed. However, in the comparison between the two groups, the experimental group showed more significant improvements in all dependent variables than the control group (p<.05). Our results also suggest that in applying RAS in stroke patients, the combination of music and a metronome is more effective than using a metronome alone in improving patients' gait.

산모의 심장소리가 미숙아의 체중, 생리적 반응 및 행동상태에 미치는 효과 (The Effects of Maternal Heart Sound on the Weight, Physiologic Responses and Behavioral States of Premature Infants)

  • Purpose: The study was done to measure the effects of maternal heart sound on body weight, physiologic reactions (heart rate [HR] and cortisol) and behavioral states of preterm infants. Methods: Thirty-five preterm infants were recruited from a neonatal intensive care unit at a university hospital. Institutional Review Board approval and informed consent were obtained. The infants were assigned to an experimental group (n=18) with an auditory stimulation for 7 days of life, a continuous delivery of maternal heart sound using MP3 attached inside the incubator, or to a control (n=17) without any auditory stimulation. The outcome variables, daily variations in weight, HR and behavioral states, and differences in cortisol were analyzed. Results: There were differences in variations of daily weights (F=3.431, p=.011) and in cortisol (t=3.184, p=.006) between groups, but no difference in variations of daily HR (F=0.331, p=.933) and behavioral states (F=1.842, p=.323). Conclusion: The findings support the safety of continuous maternal heart sound as no changes in HR and behavioral states occurred, and the efficacy as weight increased and cortisol decreased. This auditory simulation may lead to more efficient utilization of energy in preterm infants by consistently providing familiar sounds from intrauterine life and blocking noxious sounds from NICU environments.

임상 체성감각 유발전위 검사 (Clinical Somatosensory Evoked Potential)

  • Evoked potentials(EP) are defined as electric responses of the nerves system to sensory stimulation. EPs are used mainly to test conduction in the visual, auditory, and somatosensory systems, especially in the central parts of these systems. Somatosensory evoked potentials (SEP) are the potentials elicited by stimulation of peripheral nerves and recorded at various sites along the sensory pathway. SEPs types consist mainly of SEPs to electric stimulation of arm or leg nerves. SEPs to arm stimulation are usually recorded simultaneously from clavicular, cervical, and scalp electrodes; SEPs to leg stimulation are recorded from lumbar, low thoracic, and scalp electrodes. Subject variables that have practical impotance are age, limb length, body height, and temperature. General clinical interpretation of abnormal SEPs wave decreases of peripheral conduction time, and abolition of SEPs recorded from different levels to identify lesions of peripheral nerves, plexus, nerve root, spinal cord, cauda equina, hemispheric brainstem, and cerebral parts of the somatosensory pathway.

Reduced Gray Matter Volume of Auditory Cortical and Subcortical Areas in Congenitally Deaf Adolescents: A Voxel-Based Morphometric Study

  • Purpose: Several morphometric studies have been performed to investigate brain abnormalities in congenitally deaf people. But no report exists concerning structural brain abnormalities in congenitally deaf adolescents. We evaluated the regional volume changes in gray matter (GM) using voxel-based morphometry (VBM) in congenitally deaf adolescents. Materials and Methods: A VBM8 methodology was applied to the T1-weighted magnetic resonance imaging (MRI) scans of eight congenitally deaf adolescents (mean age, 15.6 years) and nine adolescents with normal hearing. All MRI scans were normalized to a template and then segmented, modulated, and smoothed. Smoothed GM data were tested statistically using analysis of covariance (controlled for age, gender, and intracranial cavity volume). Results: The mean values of age, gender, total volumes of GM, and total intracranial volume did not differ between the two groups. In the auditory centers, the left anterior Heschl's gyrus and both inferior colliculi showed decreased regional GM volume in the congenitally deaf adolescents. The GM volumes of the lingual gyri, nuclei accumbens, and left posterior thalamic reticular nucleus in the midbrain were also decreased. Conclusions: The results of the present study suggest that early deprivation of auditory stimulation in congenitally deaf adolescents might have caused significant underdevelopment of the auditory cortex (left Heschl's gyrus), subcortical auditory structures (inferior colliculi), auditory gain controllers (nucleus accumbens and thalamic reticular nucleus), and multisensory integration areas (inferior colliculi and lingual gyri). These defects might be related to the absence of general auditory perception, the auditory gating system of thalamocortical transmission, and failure in the maturation of the auditory-to-limbic connection and the auditorysomatosensory-visual interconnection.

Effects of Action Observation Training Combied with Auditory Cueing on Gait Ability in Patients with Stroke: a Preliminary Pilot Study

  • Purpose: New therapeutic approaches have emerged to improve gait ability in patients with brain damage, such as action observation learning (AOT), auditory cueing, motor imagery etc. We attempted to investigate the effects of AOT with auditory cueing (AOTAC) on gait function in patients with stroke. Methods: The eighteen stroke patients with a unilateral hemiparesis were randomly divided into three groups; the AOTAC, AOT, and control groups. The AOTAC group (n=8) received training via observing a video that showed normal gait with sound of footsteps as an auditory cue; the AOT group (n=6) receive action observation without auditory stimulation; the control group (n=5) observed the landscape video image. Intervention time of three groups was 30 minutes per day, five times a week, for four weeks. Gait parameters, such as cadence, velocity, stride length, stance phase, and swing phase were collected in all patients before and after each training session. Results: Significant differences were observed among the three groups with respect to the parameters, such as cadence, velocity, stride length, and stance/swing phase. Post-hoc analysis indicated that the AOTAC group had a greater significant change in all of parameters, compared with the AOT and control groups. Conclusion: Our findings suggest that AOTAC may be an effective therapeutic approach to improve gait symmetry and function in patients with stroke. We believe that this effect is attributable to the change of cortical excitability on motor related to cortical areas.

A Comparative Study of the Brainstem Auditory-Evoked Response during Medetomidine, Propofol and Propofol-Isoflurane Anesthesia in Dogs

  • Specialized hearing tests for pets are currently in demand. A brainstem auditory evoked response (BAER) test is an objective, non-invasive, and practical electrophysiological method that records electric signals from the peripheral auditory system to the brainstem when an auditory stimulation is provided. In veterinary medicine, sedation or anesthesia is essential for a successful examination. In human medicine, research has established the indications for various sedatives, anesthetics, and drugs according to the depth of anesthesia required. However, in veterinary medicine, there are very few comparative studies on propofol or isoflurane, which are the most common anesthetics used. Therefore, the present study aimed to analyze the difference in BAER test results between sedation with medetomidine, anesthesia using propofol, and inhalation anesthesia with isoflurane after propofol administration. The test was conducted on four healthy adult dogs. There was no statistically significant difference in latency, interpeak latency, or amplitude between the various drugs. The results suggest that a sedative or anesthetic for the administration of a BAER test can be selected according to the patient's needs.

가상현실 점포의 특성에 관한 연구 -현대백화점 VR 스토어와 eBay VR 백화점 사례를 중심으로- (A Research on the Characteristics of Virtual Reality Stores -Focused on Hyundai VR Store and eBay VR Department Store-)

  • This study investigates the characteristics of VR stores that emerged as new fashion communication media. Two case studies on Hyundai and eBay VR Department stores were conducted along with a discussion of the function and meaning of the fashion VR store. The results showed that both stores provide novel shopping experiences; however, the two were differentiated in terms of production method and technology implementation level. Functional aspects such as providing shopping efficiency and purchasing service was insufficient in both stores. Instead, they were complementing by means of product rotation, recommendation system, voice guidance, or linkage with an online shopping mall. In experiential aspects, both stores provided a strong sense of immersion. Hyundai VR store enhanced immersion with a high resolution image of a real offline store; however, it lacked in the ability to provide multisensory stimulation such as kinetic sense or auditory stimulation. The eBay VR Department store intensified the immersion experience by providing auditory stimulation as well as visual stimulation that enhanced the speed and distance sense through the utilization of animation. However, the extent of experience was limited in terms of agency and transformation because of the low interactivity found in both store systems.
