  • Background: Vascular occlusive event is one of the etiologies of sudden sensorineural hearing loss (SNHL). Stellate ganglion block (SGB) induces dramatic and intense vasodilatation in head and neck. Based on this principle, SGB has used as one of the treatment modalities in SNHL. This study was performed to evaluate immediate response of SGB on pure tone audiogram (PTA) in SNHL. Methods: Forty patients were studied. Each patient received daily ipsilateral SGB in paratracheal approach using 0.2% bupivacaine for 2 weeks. On first, third, and fifth day of treatment, we checked their PTA twice 1 hour before (Pre-PTA) and after (Post-PTA) SGB. Pre- and Post-PTA were compared. Several factors were analyzed as a prognostic factor of therapeutic results. Results: Eleven of 40 patients revealed decreased PTA after SGB. Degree of decreased PTA were insignificant ($2.5{\pm}1.6$ dB). Initial and final PTA results was $76.2{\pm}22.5$ and $49.8{\pm}28.3$ dB, respectively. Thirty-one of 40 patients were improved their PTA over 10 dB. The recovery was mainly influenced by the severity of initial hearing loss (P<0.001) and slightly by age (P<0.05). However, the change of PTA after SGB, time interval to receive SGB, sex, site, and number of SGB were not correlated to therapeutic outcome. Conclusions: These results suggest that vasodilatation by SGB has no immediate improvement in SNHL. Therefore, we question whether SGB is beneficial to all patients with SNHL as a therapeutic modality.

  • Objectives: It is expected that the combined cisplatin and radiation therapy for nasopharyngeal carcinoma produces more sensory neural hearing losses compared to radiation therapy alone. The purpose of this study was to evaluate the incidence of sensory neural hearing losses after concurrent cisplatin and radiation therapy for nasopharyngeal carcinoma. Materials and Methods: From Jun. 1994 to Mar. 1997, 10 patients were available for this study with the following eligibility criteria: 1) The patients received concurrent cisplatin and radiation therapy for nasopharyngeal carcinoma. 2) There was no pre-existing auditory disease except serous otitis media due to nasopharyngeal carcinoma. 3) They had normal sensorineural hearing function on the pretreatment pure tone audiogram. 4) Pure tone audiograms were performed at least one time after treatment between 6months to 1 year follow-up without local recurrence. Results: At 1 year follow-up, 3 patients complained of decreased hearing capacity. Sensorineural hearing losses were developed in 15 ears out of 20(75%) and were more frequent and severe in high frequency area on pure tone audiogram. Conclusion: The concurrent cisplatin and radiation therapy for nasopharyngeal carcinoma may produce sensorineural hearing losses more frequently compared to historical data of radiation therapy alone and close evaluations with regular audiograms are necessary even in patients without symptoms and signs of hearing impairment.

  • Idiopathic sudden sensorineural hearing loss(SNHL) is defined as a sensorineural hearing loss which develops over a period of hours or few days without a definitive causes. The etiology, prognosis, and treatment of SNHL are uncertain. We reviewed records of 76 patients to investigate the efficacy of the stellate ganglion block(SGB)in SNHL. The control group(40 patients) was managed with the medication such as hypaque, heparin, steroid, nicotinic acid, and vitamine. The SGB group(27 patients) was managed with the same medication and SGB (1~2 times/day). The efficacy of treatment was evaluated by mean dB of 500, 1000, 2000 Hz on follow up pure tone audiogram. Recovery rate above 15dB was not different between SGB and the control group (52.4 v.s. 52.0%), though SGB group has much poor prognostic factors. Recovery rate was higher in a case that was treated early. Especially, the recovery rate was 100% when SGB begins within 2 weeks from the onset. Headache and profound type of audiogram were poor prognostic factors. In conclusion, SGB within 2 weeks from the onset is recommanded to improve the recovery rate.

  • Auditory thresholds were determined by means of a conditioned response to sound stimuli at frequencies 80, 100, 200, 300, 500 and 800 Hz for a 10 black rock fish, Sebastes schlegeli. The conditioned response was a change of a cardiac rhythm. It was established through an electric shock as unconditioned stimulus, and could be monitored on an oscilloscope. A stable acoustic condition was obtained by suspending the fish in a small cage at fixed position in the test tank. The sensitive frequencies ranged from 80 Hz to about 800 Hz, showing the best frequency around 100 Hz where the mean threshold value was 90.5 dB. A gradual rise below 300 Hz and a relatively sharp turn above 500 Hz were indicated in the audiogram. The method presented here seemed to be useful for a rapid determination of the audiogram of fishes.

  • DSP chip parameters of a digital hearing aid (HA) should be optimally selected or fitted for hearing impaired persons. The more precise parameter fitting guarantees the better compensation of the hearing loss (HL). Digital HAs adopt DSP chips for more precise fitting of various HL threshold curve patterns. A specific DSP chip such as Gennum GB3211 was designed and manufactured in order to match up to about 4.7 billion different possible HL cases with combination of 7 limited parameters. This paper deals with a digital HA fitting program which is developed for optimal fitting of GB3211 DSP chip parameters. The fitting program has completed feature from audiogram input to DSP chip interface. The compensation effects of the microphone and the receiver are also included. The paper shows some application examples.

  • In this paper, we propose an efficient auto-fitting system for digital hearing-aids which automatically adjusts the fitting parameters according to the auditory characteristics of hearing handicapped person. The fitting parameters are extracted from audiogram of hearing handicapped and are applied to digital hearing-aid purposed GM3036 chip. The characteristics of each parameter are compared with those from theoretical 2cc graph. The purposed system has applied to 50 patients and their satisfaction ratios show to the very high. As results, it shows effectiveness of proposed system.

  • There are over 60 types of hereditary hearing loss in man and most of these are distinguished from one another by outstanding associated defects caused the same gene producing hearing loss. However, there are at least 12 types of hereditary hearing loss with no known associated defects. The authors experienced two similar families with hereditary hearing loss. The affected offsprings had inevitably an affected parent in type of transmissin (autosomal dominant) and the hearing loss developed in childhood and progressed. Audiologic findings showed bilateral symmetrical moderate to severe sensori-neural hearing loss with flattened configuration on the audiogram. Vestibular function tests showed no abnormal findings and the associated anomalies could'nt be observed in history or clinical examinations.

  • In this paper, the analysis of the relation between noise induced hearing loss and noise exposure is described for the A-weighted noise levels and exposure duration. The hearing loss and sensitivity threshold shift is investigated by changing the various parameters such as the effects of aging and noise exposure. Total hearing loss is proportional to a function of exposure level based upon the integral of pressure with time. If the noise exposure term is large so that the presbycusis may be neglected, the hearing loss due to aging and noise exposure becomes the noise induced hearing loss. It is shown that exposure to excessive noise can cause temporary loss of hearing that may become permanent if the exposure is prolonged or intense. An audiogram taken from a person suffering from noise induced hearing loss will usually show the greatest loss of hearing sensitivity in the 4kHz region, which is typically the region most sensitive to damage resulting from many types of industrial noise.

  • The combination of congenital stapes fixation and proximal symphalangism has been described in several kindreds. It exhibits autosomal dominant inheritance. A patient, 11-year-old female, visited department of otolaryngology of SNUH in December, 1980. Her complaint was bilateral hearing loss since birth. Physical examination revealed fusion of proximal interphalangeal joints of both 4th and 5th fingers. Tympanic membrane was normal. Temporal bone X-ray was normal. Pure tone audiogram revealed about 60 dB conductive loss with horizontal curve. On exploratory tympanotomy of right side, bony fixation of stapedial footplate was found. So, stapedectomy with connective tissue wire prosthesis was performed. As a result, marked improvement of hearing was obtained with 5 dB A-B gap on operated side.
