보건진료 전담공무원의 심폐소생술에 대한 지식, 태도, 자신감과 수행경험 (Knowledge, attitude, confidence, and experiences of community health practitioner regarding cardiopulmonary resuscitation)

  • Purpose : The purpose of the study is to investigate the knowledge, attitude, confidence, and experiences of community health practitioner(CHP) regarding cardiopulmonary resuscitation(CPR). Methods : The subjects were 333 CHP in G Province, C Province, and I Metropolitan City. A structured self-reported questionnaire was filled out by 209 participants from July 29, 2013 to September 30, 2013. Results : Confidence level toward CPR was intermediate, and knowledge and attitude level was very low. Seventeen participants performed CPR to cardiac arrest victim. Eleven patients among 25 patients had spontaneous recovery of circulation (ROSC). So the rate was very high. Conclusion : It is necessary to perform the efficient CPR in CHP. In order to improve the positive attitude and confidence in CHP, the renewal system must maintain every two years.

간호대학생의 북한이탈주민에 대한 태도와 영향요인 (Factors affecting the attitudes of nursing college students toward North Korean Refugees)

  • Purpose: This descriptive study was conducted to identify the attitudes of nursing college students to North Korean refugees (NKR) and to examine factors influencing these attitudes. Methods: A total of 195 participants completed a structured questionnaire between September and October 2016. Data were analyzed using the SPSS 21.0 program. Results: The results of this study were as follows: attitudes toward NKR was 2.74 out of 4 points; cultural receptivity was 3.25 out of 5 points; nationalism was 2.46 out of 4 points. Cultural receptivity (r=.26, p<.001) and nationalism (r=-.18, p=.036) were significantly related to attitudes toward NKR. Unification attitude (${\beta}=0.27$, p <.001) and cultural receptivity (${\beta}=0.20$, p=.005) explained 13.7% of the variance in attitudes toward NKR. Conclusion: The findings of this study provide fundamental data for the development of a nursing education program on multicultural perspectives. Furthermore, these findings might highlight the need for a nursing education program that could enhance understanding of patients from different backgrounds and recognize their differences, going beyond a national point of view as commonly found in Korea. The nursing education program should also promote an open and receptive attitude.

중소규모 사업장 고혈압환자의 치료순응과 관련요인 (Factors Related to Therapeutic Compliance of Hypertensive Patients in Small and Medium Scale Industry)

  • In order to evaluate the factors related to therapeutic compliance of hypertensive patients in small and medium scale industry, the questionnaire survey and the blood pressure measurement were performed on 138 hypertensive patients who were reported to have C or $D_2$ result of hypertension at the workers' periodic health examination from March to November 2001. The contents of questionnaire included the informations of factors related to therapeutic compliance of hypertensive patients such as, age, sex, marital status, income, education levels, scale, occupation, work duration, smoking and drinking habits, exercise, family history, stress and A type personality, employer's concern, organizational culture, health status, awareness, knowledge and attitude toward the needs of hypertension treatment, and experience of health education. The results of the study were as follows: 1. The proportion of those who were compliant to the treatment of hypertension was 41.3% of subjects. Among small scale industry less than 50 employees, the rate of therapeutic compliance group was 27.5% and therapeutic noncompliance group was 72.5%. Among medium scale industry more than 50 employees, the rate of therapeutic compliance group was 60.3% and therapeutic noncompliance group was 39.7%. 2. Among therapeutic noncompliance group, 95.1% of patients were aware of the fact that workers have hypertension, and 77.8% of patients got 6-10 marks of hypertension related knowledge. For health education, 27.2% were experienced and 34.6% said no intention to participate. And for hypertension treatment, 9.9% said no need to get the treatment and 44.4% said have no idea whether get treatment or not. 3. The significant factors related to therapeutic compliance of hypertensive patients in small scale industry were work duration, A type personality(anger), health status, attitude toward the needs of hypertension treatment, and experience of health education. 4. The significant factors related to therapeutic compliance of hypertensive patients in medium scale industry were age, occupation, subjective symptom, attitude toward the needs of hypertension treatment, and experience of health education. In consideration of above findings, it was suggested that in order to improve the therapeutic compliance in small and medium scale industry hypertensives, it be necessary to change attitude, perception, knowledge and treatment of hypertension by various methods such as effective health education and individually consulting programs by occupational health professional.

일종합병원의 지역 사회보건 간호사업에 대한 지역 사회 간호써비스 수혜자 및 의뢰자의 태도 조사 연구 (A Study on the Attitude of the Consumers and the ″Referees″ toward the Community Health Nursing Service of a General Hospital in Rural ]Korea)

  • The change of society has brought about various health problems which have become to demand comprehensive health services not only for individuals but also for families and communities as the unit. To meet the societal needs W Hospital in a rural Korea established a Department of Community Health Nursing Services and provided nursing services to patients under a early discharge program. This study is to evaluate the community health nursing services carried out the department in general, and has following specific objectives : 1. To learn attitude of the consumers and the "referees" toward early discharge program. 2. To find out attitude toward home care and cure services. 3. To find out the consumers and the "Referees" attitude toward the 15 selected home cue and cure activities by public health nurse. 4. To investigate possibility of charging fee for the 15 selected home care and cure activities by public health nurse. Three different study population were chosen: namely the consumer, the "referee" I and II. Excluding families moved out and not able to be contacted, the total families (77) referred to the department during the study period of September 1974 - December 1975 are defined as the consumer. Thirty seven nurses among 81 nurses who have been working in W Hospital since the inception of the Community Health Nursing Service Program were randomly selected. Thirty two nurses were defined as the "referee"I, because 5 questionnaires were not able to be collected. Twenty four doctors out of 37 who have been working since the Community Health Nursing Service Program started, and who were able to contact were called as the "referee" II. Data collection method employed for the consumers was direct interviewing with preposed interview schedules, and for the "referees"questionnaire method was utilized. x$^2$test and 100-percentage were employed in analyzing the data. The study findings are follows: 1. Attitude toward early discharge: Above 85% of the consumers and the "referees" approved the early discharge: Above 85% of the consumers and the "referees" approved the early discharge program. 2. Attitude toward the home care and cure services: Above 75% of the consumers and the "referees" showed positive attitude toward the home care and cure services by public health nurses. 3. Attitude toward 15 selected home care and curative activities by public health nurses: On the average more than 50% of the consumers and the "referees" expressed an affirmative attitude to perform 15 selected home care and cure services by public health nurses. 4. Attitude toward charging fee for 15 selected home care and curative activities by public health nurses: Above 85% of the consumers thought that they would pay for the services. Only 12.8% of the consumers and 5% of the "referees" what to have the services for nothing. Based upon the findings of this study; That is, the home care and cure services to be given by public health nurse were positively responded by the consumers and the pro-fissionable, one can conclude that the services provided by the Department of Community Health Nursing Service of W Hospital have been well accepted, and that early discharge program of hospital is believed to be a means to lessen the expenses of patients from hospitalization and to promote recovery from illness. It is recommended that hospital centered cure services should be reconsidered and scrutinized to meet community health needs. NOTE : "referees" are nurses and doctors who refer the early discharged Patient to the Department of Community Health Nursing Service.

신경계 병동에서 억제대를 적용 중인 환자가족의 억제대 적용에 대한 인식과 태도 (Families' Perception and Attitude toward Applied Physical Restraints in General Neurological Wards)

  • 본 연구는 신경계병동에서 억제대를 적용 중인 환자의 가족을 대상으로 억제대 적용에 대한 인식과 태도를 파악할 목적으로 시행된 서술적 조사연구이다. 2014년 6월 1일부터 2014년 9월 30일까지 부산시 소재 일개상급종합병원의 신경계병동에서 억제대를 적용 중인 환자의 가족 70명을 대상으로 설문조사했고 환자자료는 의무기록으로 조사했으며 수집된 자료는 서술통계, t-test, ANOVA, $Scheff\acute{e}$ tests로 분석했다. 환자가족은 여자가 68.6%, 평균 47.9세, 환자의 배우자나 자녀가 70% 이상이었고 일일 평균 15.4시간 동안 환자를 돌보고 있었다. 억제대 적용에 대한 인식의 평균점수는 4.2/5점이었고, 치료장치 유지와 관련된 인식점수가 높았고(평균 4.6-4.9/5점) 억제대 적용부위에 따라 양손이 한손보다 더 중요하다고 인식했다. 억제대 적용태도에 있어서는 환자와 가족에게 적용과 제거에 대한 결정권이 있다와 관련된 문항에 동의하는 가족이 많았다. 본 연구의 결과를 토대로 환자가족에게 억제대 관련 정보제공과 지속적인 교육을 통한 지원이 요구된다.

Factors that Influence Attitudes toward Advance Directives among Hemodialysis Patients

  • Purpose: Advance directives (ADs) are legal documents that outline a person's preferences or decisions regarding end-of-life care ahead of time. In Korea, there is insufficient awareness and knowledge about ADs among patients undergoing hemodialysis. This study explored the relationship between perceptions of a good death, knowledge about ADs, and attitudes toward ADs in this patient population. Methods: This cross-sectional survey enrolled 119 hemodialysis patients from a secondary hospital in 2021. The participants completed a self-administered questionnaire, and the data were analyzed using the t-test, analysis of variance, Pearson correlation coefficients, Spearman rank correlation coefficients, and multiple regression analysis. Results: The average score for perceptions of a good death among hemodialysis patients was 2.80 out of 4, with clinical symptoms identified as the most critical factor. The average scores for knowledge about ADs and attitudes toward ADs were 5.69 out of 9 and 2.79 out of 4, respectively. There was a positive correlation between perceptions of a good death and attitudes toward ADs (r=0.34, P<0.001), as well as between knowledge about Ads and attitudes toward ADs (r=0.19, P=0.037). Factors influencing attitudes toward Ads included employment status (β=0.22, P=0.011), education level (β=0.22, P=0.013), and perceptions of a good death (β=0.29, P=0.001), which accounted for 24.8% of the variance in attitudes toward ADs. Conclusion: A positive perception of a good death among patients undergoing hemodialysis was associated with a positive attitude toward ADs. Educational programs are needed to improve individuals' understanding of a good death and encourage the development of end-of-life care plans.

농촌지역 주민의 고혈압 치료순응도와 관련요인 (Therapeutic Compliance and Its Related Factors of Patients with Hypertension in Rural Area)

  • Objective : The purpose of this study was to examine the therapeutic compliance and its related factors in the rural hypertensives. Method : A questionnaire survey and blood pressure measurement were performed to 3,876 residents of a rural area, and 660 hypertensives were selected as subjects of study. The study employed a hypothetical model which was composed of constructs from the health belief model and KAP model. The analysis techniques employed included contingency table analysis and structural equation modeling. Result : The proportion of those who were compliant to the treatment of hypertension was 44.2% of subjects. As the result of structural equation modeling, when patients had more favorable attitude toward treatment, higher perceived benefit, or lower perceived barriers to treatment, the therapeutic compliance was significantly higher(T>2.0). When patients had more knowledge about hypertension, or higher perceived severity of hypertension, the attitude toward the treatment of hypertension was more favorable significantly(T>2.0). And when patients had the support for treatment from family or neighbor, the attitude toward treatment was more favorable(T>2.0). When patients had experience of health education, they had more knowledge, higher perceived susceptibility of complication, perceived severity for hypertension, and perceived benefit of treatment, compare to patients without health education(T>2.0). Conclusion : In consideration of above findings, in order to improve the therapeutic compliance in the rural hypertensives, it would be necessary to change attitude, perception, knowledge about hypertension and its treatment, by various methods such as effective health education and programs for maintaining the supportive environment for hypertension treatment.

Physical Therapists' Awareness of Dementia and Attitude

  • Purpose: The purpose of this study was to find out the personal characteristics of physical therapists, dementia awareness and dementia attitude, and to find out what relationship is there between personal characteristics and dementia awareness and dementia attitude. Methods: Participants in this study surveyed physical therapists who are members of the Association of Korean Physical Therapists on their awareness of dementia, and conducted online surveys from January 28 to February 27, 2021. The survey questions used in the survey consisted of 29 questions in total, including 9 general characteristics of the participant, 10 questions on perception of dementia, and 10 attitudes toward dementia. All 104 participants were surveyed, and 100 surveys were analyzed, excluding 4 surveys with insufficient responses. Results: In this study, the correct answer rate for all items in the dementia awareness sub-item was 65%, and the dementia attitude-related sub-items were generally positive. However, there was no significant correlation between personal characteristics such as gender, age, educational background, treatment target, treatment experience and dementia awareness, and no correlation with dementia attitude was significant. Conclusion: Regardless of personal characteristics such as gender, age, treatment target, and treatment experience, a positive attitude and correct recognition of dementia can improve the quality of treatment with dementia patients and increase the reliability of patients and caregivers.

지역사회 거주 남성노인을 위한 치매 돌봄 프로그램 효과 (The Development and Effect of a Dementia Care Support Program among Community Dwelling Older Adult Men)

  • Purpose: We developed and tested the effects of a care program for dementia patients among community- dwelling older adult men. Methods: This study used a non-equivalent control group pre-and post-test design. To verify the effectiveness of the care program for dementia patients, 26 community dwelling older adult men participated in this study. We used multiple intervention strategies including improving understanding of dementia through education, activities for dementia prevention, and promoting psychological change. In particular, based on the current understanding of the care of the men older adults, we used strategies to promote motivation and reinforce strengths. Results: After completing the 6-week intervention program, when compared with the control group, older adult men in the care program intervention group showed significant differences in scores for the following: cognitive functions (p=.035), attitude toward dementia (p=.026), preventive behavior (p=.007), geriatric depression (p=.013), caring confidence (p=.018), and self-esteem (p=.013). Conclusion: These results indicate that the care program for dementia patients has positive effects on increasing their cognitive function, attitude toward dementia, preventive behavior, caring confidence, self-efficacy, and on decreasing depression rates in this population. Based on this, we can recommend this program to men caregivers for the improved care of dementia in community centers.

수술환자의 통증자가조절기 관리교육 효과 (Effect of Patient Controlled Analgesia (PCA) Education for Postoperative Patients)

  • Purpose: The study examined the effects of preoperative patient controlled analgesia (PCA) education on postoperative pain, knowledge of PCA, and attitude toward pain medication. Methods: The participants were patients who underwent surgery in H University Hospital,and were assigned to PCA group (experiment group, n=30) or the control group (n=30). Results: Knowledge of PCA and positive attitude toward pain medication were higher in the experiment group than in the control group. Postoperative pain scores in the experiment group were significantly lower than those in the control group at 48 and 72 hr after surgery, but there was no significant difference at 24 hr after surgery. The experiment group had more positive attitudes on the medication than the control group. Conclusions: Preoperative PCA education could be an effective nursing intervention for pain management of patients after surgery.
